Tunnel construction opportunities involving shallow overburdens under difficult(e.g.,soft,unconsolidated)grounds have been increasing in *** auxiliary methods for excavating mountain tunnels have been developed and can satisfy stringent construction *** ground improvement method,which is one of the auxiliary methods for shallow overburden tunnels,has demonstrated its ability to effectively control the amount of settlement under soft ***,the mechanism of the ground improvement method has not been clarified,nor has a suitable design code been established for ***,because the strength of the improved ground and the suitable length and width of the improved area have not been fully understood,an empirical design has been applied in every *** this paper,the mechanical behavior during the excavation,including that of the stabilized ground,is evaluated through trapdoor experiments and numerical *** addition,the enhancement of tunnel stability resulting from the application of the ground improvement method is discussed.

To mitigate the damage caused by debris flows resulting from heavy precipitation and to aid in evacuation plan preparation, areas at risk should be mapped on a scale appropriate for affected individuals and communities. We tested the effectiveness of simply identifying debris-flow hazards through automated derivation of surface curvatures using LiDAR digital elevation models. We achieved useful correspondence between plan curvatures and areas of existing debris-flow damage in two localities in Japan using the analysis of digital elevation models(DEMs). We found that plan curvatures derived from 10 m DEMs may be useful to indicate areas that are susceptible to debris flow in mountainous areas. In residential areas located on gentle sloping debris flow fans, the greatest damage to houses was found to be located in the elongated depressions that are connected to mountain stream valleys. Plan curvaturederived from 5 m DEM was the most sensitive indicators for susceptibility to debris flows.

Androgens, the levels of which decrease with ageing, play many physiological roles in various organs. Testosterone deficiency syndrome （TDS） has received widespread attention in the last several years. First-line treatment for TDS should be testosterone replacement therapy （TRT）, which is reported to improve several TDS symptoms. Recently, a clinical practice manual for TDS was written and published by a collaborative team from the Japanese Urological Association and the Japanese Society for the Study of the Aging Male to recommend standard procedures for the diagnosis, treatment, prevention and monitoring of adverse reactions to TRT and for post-treatment assessment. In this manual, intramuscular injection of testosterone enanthate or human chorionic gonadotropin and the testosterone gel ＇Glowmin＇ were recommended as TRT. Currently, two topics related to TDS are being focused on in Japan： the relationship between TDS and metabolic syndrome and treatment options for eugonadal patients with TDS symptoms. In this review, the possibility of TRT for metabolic syndrome as well as the relationship between testosterone and adiponectin, which is a key molecule in metabolic syndrome, is discussed. Finally, the possibility of herbal medicines as a treatment option for patients with TDS is addressed, especially for eugonadal patients, because eugonadal men with TDS symptoms account for approximately 30% of the general population. The increase in the levels of several cytokines, such as IL-8, IL-13, interferon.y and tumor necrosis factor-α, after herbal medicine treatment may be the reason for this efficacy.

Safety and Efficacy of Once-Daily Administration of Mycophenolate Mofetil in Kidney Transplant Patients—A New Treatment Option for Non-Adherence

Non-adherence to immunosuppressive therapy is associated with reduced graft survival. Fifteen long-term follow up kidney transplant patients with stable post-transplant clinical courses were enrolled in the study. All patients were prescribed to tacrolimus prolonged release, mycophenolate mofetil (MMF) and corticosteroid at the time of enrolment. Twice-daily administration of MMF was then converted to a single daily dose. As a result, all immunosuppressive agents were administered simultaneously at one time in the morning. The daily total doses of the three immunosuppressants were identical during the study period. No acute rejection or adverse event was observed during the study period. Blood urea nitrogen and estimated glomerular filtration rate did not change significantly after conversion to once-daily administration. The blood tacrolimus trough level and the Mycophenolic acid (MPA) trough level also did not change significantly after conversion to once-daily administration. Interestingly, the meanmedian MPA trough level remained >3 μg/ml even after conversion to once-daily administration. A sufficient plasma MPA level was maintained after conversion to once-daily administration, and no acute rejection was observed during the study period. To our knowledge, this study is the first to report that the plasma MPA concentration can be maintained after once-daily administration of MMF in long-term kidney transplant patients. Once-daily administration of immunosuppressive agents may improve long-term graft survival because of better treatment adherence due to the reduced dosing frequency. The safety or efficacy of conversion to once-daily administration of MMF should be evaluated in a future randomized controlled large-scale clinical study.

TO THE EDITORHepatocellular carcinoma (HCC) is thought to develop through a multistep process[1]. A long history of viral hepatitis or prolonged exposure to environmental toxins predisposes liver cells to mutations of the genes critical in the control of hepatocyte growth. In fact, both activation of cellular oncogenes and inactivation of tumorsuppressor genes are involved in the development of HCC. Activation of oncogenes by hepatitis virus integration has been shown in the woodchuck animal model[2],although the significance of this finding in human hepatocarcinogenesis is still under investigation.

目的设计合成新的大流量钙激活钾离子通道开放剂。方法以他莫希芬为先导化合物,采取分子间McMurry偶联反应合成15个新化合物,采用荧光分析法,利用高表达鼠的α和β1亚基的HEK293细胞对目标化合物进行活性筛选。结果与结论目标化合物结构经质谱、高分辨质谱及氢谱确证,与对照药他莫希芬相比,具有类似的大流量钙激活钾离子通道开放活性。
