Risk factors for postoperative stoma outlet obstruction in ulcerativecolitis

BACKGROUND Current medical treatments can achieve remission of ulcerative colitis (UC).Surgery is required when potent drug treatment is ineffective or when coloncancer or high-grade dysplasia develops. The standard procedure is restorativeproctocolectomy (RPC) with ileal pouch-anal anastomosis, commonly performedas two- or three-stage RPC with diverting ileostomy. Postoperative stoma outletobstruction (SOO) is frequent, but the causes are not well *** To identify the risk factors for SOO after stoma surgery in patients with *** We retrospectively reviewed the files of 148 consecutive UC patients whounderwent surgery with stoma construction. SOO was defined as small bowelobstruction symptoms and intestinal dilatation just below the penetrating part ofthe stoma on computed tomography. Patients were divided into two groups:Those who developed SOO within 30 d after surgery and those who did *** characteristics, intraoperative parameters, the stoma site, and rectusabdominis muscle thickness were collected. Moreover, we identified the patientswho repeatedly developed SOO. Univariate and multivariate analyses wereperformed to identify risk factors for SOO and recurring *** Eighty-nine patients who underwent two-stage RPC were included betweenJanuary 2008 and March 2020. Postoperatively, SOO occurred in 25 (16.9%)patients after a median time of 9 d (range 2-26). Compared to patients withoutSOO, patients with SOO had a significantly higher rate of malignant tumors ordysplasia (36.0% vs 17.1%, P = 0.032), lower total glucocorticoid dose one monthbefore surgery (0 mg vs 0 mg, P = 0.026), higher preoperative total protein level(6.8 g/dL vs 6.3 g/dL, P = 0.048), higher rate of loop ileostomy (88.0% vs 55.3%, P= 0.002), and higher maximum stoma drainage volume (2300 mL vs 1690 mL, P =0.004). Hydrodynamic performance of spouted beds with different types of draft tubes

Hydrodynamic performance of spouted beds with four different types of draft tubes has been investigated experimentally. In this work, irregular and angular particles were used as the spouting solids and the results were compared with those of the previous work in which regular and round particles had been used. It was found that the conical-cylindrical porous draft tube was the best option for solid-gas contact and solids circulation as in the case of round particles. Moreover, by using conical-cylindrical porous draft tube, the gas flow rate through the annulus reached 65-70%, equal to the level of a conventional spouted bed.

A Blowdown Cryogenic Cavitation Tunnel and CFD Treatment for Flow Visualization around a Foil

Cavitation is one of the major problems in the development of rocket engines. There have been few experimental studies to visualize cryogenic foil cavitation. Therefore a new cryogenic cavitation tunnel of blowdown type was built. The foil shape is “piano-convex”. This profile was chosen because of simplicity, but also of being similar to the one for a rocket inducer impeller. Working fluids were water at room temperature, hot water and liquid nitrogen. In case of Angle of Attack （AOA）=8°, periodical cavity departure was observed in the experiments of both water at 90℃ and nitrogen at -190℃ under the same velocity 10 m/sec and the same cavitation number 0.7. The frequencies were observed to be 110 and 90 Hz, respectively, and almost coincided with those of vortex shedding from the foil. Temperature depression due to the thermodynamic effect was confirmed in both experiment and simulation especially in the cryogenic cavitation.

An Experimental Study on Transverse Hydrogen Gas Injection into Mach 1.8 Airnow Channel-The lst report:Single Circular Injector

Experimental results from a series of injection tests of pressurized H2, N2 gases into Mach 1.8 airfiows between parallel channel walls through a flush-mounted circular sonic opening have been *** pictures revealed complex interaction flow features including the occurrence of bow/separation shock waves due to the injection as well as the barrel shock/Mach disc structure inside the injected gas stream. The injectant penetration measured by the Mach disc height against the injection pressure showed a good agreement with the correlation curve based upon the "effective back pressure" concept. The reversed flow region beneath the separation shock wave, the injectant wake and its associated flow entraimment were also visualized by the oil paint method. Wall static pressure distributions around the injector were measured in detail, which corresponded very well to the above results of flow *** samplings were also undertaken by using the pressure taps to confirm the presence of H2 gas in the separation region ahead of the injector. Traversing of total pressure and H2 gas concentration at the exit of the test channel showed monotonous increase of the loss while its profile was kept very similar with the injection pressure. The area indicating the loss and the presence of H2 gas almost coincided with each other, which remained to be small to indicate very slow gas mixing/diffusion with the main air flow. With the increase of airflow total temperature to 1200 K, a bulk flame was first observed at the exit section. Further increase up to 1460 K observed an ignition flame at the injector. However,the reflection of the bow shock wave was found to be a more likely trigger of the bulk flame ignition within the test section.

Pancreatic Carcinoma with Prominent Mucin Production: A Clinicopathologic and Immunohistochemical Study of 9 Cases

Pancreatic carcinoma occasionally associated with prominent mucin production and this type of tumor designated as PCM (pancreatic carcinoma with prominent mucin production) was diagnosed depends on subjective estimation of the amount of mucous area, and there has been no report on a quantitative evaluation of the amount of mucinous area in the tumor. To examine the feature of PCM, we analyzed 9 cases of PCM among 243 cases of pancreas carcinoma and evaluated the amount of mucin by imaging analysis. Morphologically, 5 cases were classified as intradactal papillary mucinous neoplasms (IPMN)-derived PCM and 4 cases were as ductal adenocarcinoma (DA)-derived PCM. Mucous composition was found to be more than 50% in all IPMN-derived PCM cases, and that was 40% - 50% in DA-derived PCM cases with one exception. IPMN-derived PCM cases showed expansive growth with pancreatic duct dilatation filled with mucin, while DA-derived PCM cases possessed mucin infiltration into interstitial tissue. Immunohisto-chemically, three of 4 DA-derived PCM cases were MUC1(–)/MUC2(+), and the results of expressions for p16 and Dpc4 suggesting that DA-derived PCM was similar to IPMN-derived PCM rather than ordinary DA. Survival rate of DA-derived PCM cases was lower than that of IPMN-derived PCM cases. We advocate that DA-derived PCM may constitute a borderline group between IPMN and ordinary DA.

Gastroduodenal ulcer bleeding in elderly patients on low dose aspirin therapy

AIM To determine the clinical characteristics of elderly patients of hemorrhagic gastroduodenal ulcer on low-dose aspirin(LDA)*** A total of 1105 patients with hemorrhagic gastroduodenal ulcer treated in our hospital between January 2000 and March 2016 were grouped by age and drugs used,and these groups were compared in several *** groups were compared in terms of length of hospital stay,presence/absence of hemoglobin(Hb)decrease,presence/absence of blood transfusion,Forrest Ⅰ,percentage of Helicobacter pylori infection,presence/absence of underlying disease,and percentage of severe *** The percentage of blood transfusion(62.6%vs 47.7%,P<0.001),Hb decrease(53.8% vs 40.8%,P<0.001),and the length of hospital stay(23.5 d vs 16.7 d,P<0.001)were significantly greater in those on drug *** percentage of blood transfusion(65.3%vs 47.8%,P<0.001),Hb decrease(54.2%vs 42.1%,P<0.001),and length of hospital stay(23.3 d vs 17.5 d,P<0.001)were significantly greater in the *** comparison with the LDA monotherapy group,the percentage of severe cases was significantly higher in the LDA combination therapy group when elderly patients were concerned(16.1%vs34.0%,P=0.030).Meanwhile,among those on LDA monotherapy,there was no significant difference between elderly and non-elderly(16.1%vs 16.0%,P=0.985).CONCLUSION A combination of LDA with antithrombotic drugs or nonsteroidal anti-inflammatory drugs(NSAIDs)contributes to *** advanced age is not an aggravating factor when LDA monotherapy is used.

Analysis of Serum Antibody Level after COVID-19 Vaccine and Side Effects after Vaccine

Background: COVID-19 has caused a large number of deaths, in the elderly. In this situation, vaccines have been considered the main strategy to combat COVID-19 and were developed worldwide in 2020. One problem with COVID-19 vaccination is side effects after vaccination. In this report, we identify the relationship between side effects and elevation of antibodies as determined by monitoring several kinds of antibodies against COVID-19. Objective: We investigated whether the antibody level changed depending on side effects after COVID-19 vaccination. Methods: Healthy volunteers, who received two vaccinations between 10 March and 7 May 2021, were collected for this study. Information including age, gender, smoking history, medical history including allergies and side reactions after vaccination was obtained by questionnaire. Serum levels of antibodies of IgG and IgM against each S1, SP, and NP antigens of COVID-19 were evaluated frequently for 3 - 4 months after the first vaccination. Result: Ten employees working at Iwate Medical University were evaluated in this study. Side effects were observed in 7 of 10 patients, and grade 2 side effects in all 3 patients with a history of allergic disease. Serum S1 and SP IgG were elevated sufficiently in all patients. In all patients, IgG antibody titers fell below the cutoff point in approximately 3 months. No cases had elevated NP antibodies. SP IgM was elevated in three cases;all three cases with elevated IgM had allergic disease and the degree of side effects was relatively higher. Subjects with long-lasting elevated SPIgM were observed. Conclusion: S1 IgG and SP IgG exceeded the cutoff in all subjects after vaccination but decreased below the cutoff in all subjects within 4 months, regardless of side effects or allergic history. On the other hand, elevated SPIgM was suspected to be related to side effects and history of allergies, and cases with persistent elevation of SPIgM were observed.
