Influence of Psychiatric Problems on Clinical Outcomes during the First 12 Months after Primary Total Knee Arthroplasty

Background: Total knee arthroplasty (TKA) is a useful treatment option for advanced knee osteoarthritis. Excellent clinical outcomes after TKA have been widely recognized, but the influence of psychiatric problems on them has not been focused on until quite recently. This study aimed to assess the influence of psychiatric problems on clinical outcomes after TKA in Japanese patients using two assessment scales developed in Japan because the Japanese cultural lifestyle is specifically characterized by bending to the floor and standing up. Methods: Clinical outcomes and psychiatric problems were evaluated using the Japanese Knee Osteoarthritis Measure (JKOM) and Brief Scale for Psychiatric Problems in Orthopaedic Patients (BS-POP), respectively. A total of 115 TKA patients were evaluated preoperatively and at 3, 6, and 12 months after TKA. The patients were classified into four groups (groups A-D) based on the BS-POP score. The JKOM scores were then compared between the two groups (groups A and D) with the worst and least psychiatric problems. The JKOM improvement rate between pre- and postoperative status in both groups A and D was also calculated. Results: The total JKOM score was significantly poorer in group A than in group D preoperatively and at 3, 6, and 12 months after TKA. The improvement rate showed no significant difference between groups A and D. Conclusion: Psychiatric problems influence both the poorer post- and preoperative clinical outcomes. However, a similar improvement rate in both groups A and D has indicated that TKA can be an effective treatment even for patients with psychiatric problems.

Pro-resolving lipid mediator reduces amyloid-β42–induced gene expression in human monocyte–derived microglia

Specialized pro-resolving lipid mediators including maresin 1 mediate resolution but the levels of these are reduced in Alzheimer's disease brain, suggesting that they constitute a novel target for the treatment of Alzheimer's disease to prevent/stop inflammation and combat disease pathology. Therefore, it is important to clarify whether they counteract the expression of genes and proteins induced by amyloid-β. With this objective, we analyzed the relevance of human monocyte–derived microglia for in vitro modeling of neuroinflammation and its resolution in the context of Alzheimer's disease and investigated the pro-resolving bioactivity of maresin 1 on amyloid-β42–induced Alzheimer's disease–like inflammation. Analysis of RNA-sequencing data and secreted proteins in supernatants from the monocyte-derived microglia showed that the monocyte-derived microglia resembled Alzheimer's disease–like neuroinflammation in human brain microglia after incubation with amyloid-β42. Maresin 1 restored homeostasis by down-regulating inflammatory pathway related gene expression induced by amyloid-β42 in monocyte-derived microglia, protection of maresin 1 against the effects of amyloid-β42 is mediated by a re-balancing of inflammatory transcriptional networks in which modulation of gene transcription in the nuclear factor-kappa B pathway plays a major part. We pinpointed molecular targets that are associated with both neuroinflammation in Alzheimer's disease and therapeutic targets by maresin 1. In conclusion, monocyte-derived microglia represent a relevant in vitro microglial model for studies on Alzheimer's disease-like inflammation and drug response for individual patients. Maresin 1 ameliorates amyloid-β42–induced changes in several genes of importance in Alzheimer's disease, highlighting its potential as a therapeutic target for Alzheimer's disease.

Effect of weld line positions on the tensile deformation of two-component metal injection moulding

Knowledge of the mechanical properties of two-component parts is critical for engineering functionally graded components. In this study, mono-and two-component tensile test specimens were metal injection moulded. Three different weld line positions were generated in the two-component specimens. Linear shrinkage of the two-component specimens was greater than that of the mono-component specimens because the incompatibility of sintering shrinkage of both materials causes biaxial stresses and enhances sintering. The mechanical properties of 316L stainless steel were affected by the addition of a coloured pigment used to identify the weld line position after injection moulding. For the two-component specimens, the yield stress and ultimate tensile stress were similar to those of 316L stainless steel. Because 316L and 630(also known as 17-4PH) stainless steels were well-sintered at the interface, the mechanical properties of the weaker material(316L stainless steel) were dominant. However, the elongations of the two-component specimens were lower than those of the mono-component specimens. An interfacial zone with a microstructure that differed from those of the mono-material specimens was observed; its different microstructure was attributed to the gradual diffusion of nickel and copper.

Unfinished Revolution： A Paradox of Mourning Subjectivity in Su Manshu＇s The Lone Swan

Su Manshu＇s 苏曼殊（1884-1918） The Lone Swan （Duanhong lingyanji 断鸿零雁记, 1911, 1912） is best known for a sustained use of subjective voice and a thematic emphasis on tragic love. Critics have often credited the novella＇s intensely tragic narrative for spearheading a new kind of literary subjectivity that became a cornerstone of modern Chinese literature as heralded by the May Fourth critics in the late 1910s and the 1920s. However, very few have examined this new subjectivity as an effect of Su＇s critical engagement with a late Qing nationalist narrative. Su＇s novella was an appropriation of the anti-Manchu revolutionary narrative of a nation, which hinged on a paradoxical mode of envisaging a new China through a temporal return to the past and by means of a tragic sacrifice of the individual. Following a brief analysis of Su＇s early piece published in The People＇s Journal （Minbao 民报）, this article demonstrates how The Lone Swan elaborated on an excess of individual sacrifice, while developing the new, mourning subjectivity as a witness to the unfinished revolutionary enterprise of forging a powerful nation. Su＇s narrative of cultural devastation resonates with Lu Xun＇s （1881-1936） late Qing work, but, in the May Fourth period that immediately followed, this sense of despair would become an unequivocal obiect for overcoming.

Propagation of Weak Pressure Waves against Two Parallel Subsonic Streams

In this paper, the characteristics of a pressure wave propagating against two parallel subsonic streams in a constant-area straight duct are investigated by one-dimensional analysis, two-dimensional numerical simulation, and experiments. Computations have been carried out by the two-dimensional Euler Equations using the Chakravarthy-Osher-type TVD scheme. Optical observations by the schlieren method as well as wall pressure measurements have been performed to clarify both the structure and the propagation velocity of pressure waves. The results show that the pressure wave propagating against the streams changes into a bifurcated pressure wave and the bifurcation occurs in the low speed streams. It is also found that the propagation velocity of the pressure wave obtained by the analysis and computation agrees well with the present experimental data.

Influence of Changes in Solar Radiation on Changes of Surface Temperature in China

The long-term trends of total surface solar radiation （SSR）, surface diffuse radiation, and surface air temperature were analyzed in this study based on updated 48-yr data from 55 observational stations in China, and then the correlation between SSR and the diurnal temperature range （DTR） was studied. The effect of total solar radiation on surface air temperature in China was investigated on the basis of the above analyses. A strong correlation between SSR and DTR was found for the period 1961-2008 in China. The highest correlation and steepest regression line slope occurred in winter, indicating that the solar radiation effect on DTR was the largest in this season. Clouds and water vapor have strong influences on both SSR and DTR, and hence on their relationship. The largest correlations between SSR and DTR occurred in wintertime in northern China, regardless of all-day （including clear days and cloudy days） or clear-day cases. Our results also showed that radiation arriving at the surface in China decreased significantly during 1961-1989 （dimming period）, but began to increase during 1990 2008 （brightening period）, in agreement with previous global studies. The reduction of total SSR offset partially the greenhouse warming during 1961-1989. However, with the increase of SSR after 1990, this offsetting effect vanished; on the contrary, it even made a contribution to the accelerated warming. Nonetheless, the greenhouse warming still played a controlling role because of the increasing of minimum and mean surface temperatures in the whole study period of 1961-2008. We estimated that the greenhouse gases alone may have caused surface temperatures to rise by 0.31-0.46℃ （10 yr）^-1 during 1961-2008, which is higher than previously estimated. Analysis of the corresponding changes in total solar radiation, diffuse radiation, and total cloud cover indicated that the dimming and brightening phenomena in China were likely attributable to increases in absorptive and scattering

Cell concentration,viability and culture composition of airborne bacteria during a dust event in Beijing

Airborne bacteria were measured when a dust storm passed Beijing in spring 2012 with a focus on cell concentration, viability and TSA- and R2A-cultured strain composition. The concentration varied at an order of 10^7 cells/m3 with dust loading （demonstrated with PM10） and they had a very close correlation （RT2 = 0.91, p 〈 0.01）. At the time of highest PM10 of 652 μg/m3, the bacterial concentration reached 1.4 × 10^8 cells/m3, which was larger than that before and after the dust event by one order. Bacterial viability, the ratio of number concentration of viable cells to total cells, was 32%-64% and smaller in the dust plume than that before the dust arrival. Bacterial strains from the culture ranged between 2.5 × 10^4 and 4.6 × 10^5 CFU/m3 and no correlation with PM10 was determined. Their composition was different before and after the dust arrival according to 16S rRNA gene sequences and strains belong to Actinomycetes and Firmicutes were the majority in the dust samples.

Effect of forest thinning on hydrologic nitrate exports from a Nsaturated plantation

Although several studies have assessed the effects of thinning on water quality,including nitrogen(N)exports,few have examined N-saturated *** study assessed the short-term effect of thinning on N exports from a N-saturated plantation forest in northern Kyushu,western Japan,that was thinned(43%of basal area)during January-March *** levels at the gauging station were continually measured,and streamwater baseflow and stormflow samples were collected before(2011)and after(2013)forest *** precipitation before(2469 mm)and after(2418 mm)forest thinning differed little,and annual water discharge after thinning(1641 mm)was similar to that before thinning(1609 mm).However,direct flow during stormflow periods was higher after thinning(260 mm)than before(153 mm).The concentrations of major ions in stream water did not differ before and after *** a result,the high annual dissolved inorganic N(DIN)exports differed little before(35.8 kg N ha^-1 a^-1)and after(36.5 kg N ha^-1 a^-1)forest *** exports during stormflow periods were slightly higher after(12.2 kg N ha^-1 a^-1)than before thinning(10.5 kg N ha^-1 a^-1)and were proportional to the increased direct flow after thinning(561 mm after vs.470 mm before thinning).We concluded that thinning does not affect annual N exports from the N-saturated plantation forest,but it can increase the proportion of N exported during stormflow periods in proportion to increased water volume of direct flow after thinning.

Extracts of 16 natural medicine powders {Galla chinensis,Malloti cortex,Cassiae semen,Sophorae radix,Myricae cortex,Crataegi fructus,Gambit,Mume fructus,Geranii herba,Phellodendri cortex,Coptidis rhizoma,Swertiae herba,and Cinnamomi cortex） were assayed for reactive oxygen concentrations using the peroxyoxalate chemiluminescent detection *** luminescence intensity was observed in Galla chinensis,Geranii herba,Malloti cortex,Myricae cortex,and Cinnamomi *** experiments identified the reactive oxygen species as hydrogen *** chinensis generated 2.4 × 10^（-4） mol/L hydrogen peroxide from a 1 mg/mL *** bacterial growth tests,Galla chinensis extract had antibacterial activity against Escherichia *** aureus,Bacteroides thetaiotaomicron,Campylobacter sputorum biovar *** salivarius thermophilus,Lactobacillus casei,and Bifidobacterium longum *** antibacterial activity was decreased by the addition of *** revealed that hydrogen peroxide which Galla chinensis produced participated in antibacterial activity.

Clinical features in proven and probable invasive fungal disease in children and adolescents at a pediatric referral center: a 5-year experience

Background There is limited information concerning the overall epidemiology of invasive fungal disease (IFD) in *** aim of this study was to clarify the clinical features of IFD in a tertiary pediatric care *** Patients diagnosed with proven or probable IFD at our hospital between 2011 and 2015 were retrospectively *** and probable IFD were defined according to the European Organization for Research and Treatment of Cancer/Cancer/Invasive Fungal Infections Cooperative Group and the National Institute of Allergy and Infectious Diseases Mycoses Study Group *** with possible IFD were *** The incidence of proven or probable IFD was 26 of 20,079 hospitalized patients (0.13%).The predominant underlying disease was malignancy (54%) and congenital anomaly (27%).The most common diagnosis was candidemia among the patients with proven IFD (8 of 13,62%).All the isolated pathogens in the candidemia patients were non-albicans Candida *** most common site of infection was the lungs in patients with probable IFD (11 of 13 patients,85%).In probable IFD episodes,positive β-D-glucan and galactomannan were found in 12 of 13 (92%) and 5 of 13 (38%) patients,*** but one patient (96%) received empirical antifungal *** patients underwent surgical resection of residual *** overall mortality was 23% and the attributable mortality of IFD was 12%.Conclusion Our results suggest the emergence of non-albicans Candida species as important pathogens in childhood IFD.
