This research paper aims to perform dynamics analysis,3E assessment including energy,exergy,exergoeconomic,and the multiobjective evolutionary optimization on a novel solar Li-Br absorption refrigeration cycle.The research is time-dependent,owing to solar radiation variability during different timelines.Theoretically,all the necessary thermodynamic,energy,and exergy equations are applied initially.This is followed by the thermoeconomic analysis,which takes place after defining the designing variables during the thermoeconomic optimization process and is presented together with the economic relations of the system and its thermoeconomic characteristics.Furthermore,the sensitivity analysis is undertaken,the source of system inefficiency is determined,the multi-objective evolutionary optimization of the whole system is carried out,and the optimal values are compared with the primary stage.Engineering Equation Solver(EES)software has been used to accomplish comprehensive analyses.As part of the validation process,the results of the research are compared with those published previously and are found to be relatively consistent.

In this work, the effects of superthermal and trapped electrons on the oblique propa- gation of nonlinear dust-acoustic waves in a magnetized dusty （complex） plasma are investigated. The dynamic of electrons is simulated by the generalized Lorentzian （k） distribution function （DF）. The dust grains are cold and their dynamics are simulated by hydrodynamic equations. Using the standard reductive perturbation technique （RPT） a nonlinear modified Korteweg-de Vries （mKdV） equation is derived. Two types of solitary waves; fast and slow dust acoustic solitons, exist in this plasma. Calculations reveal that compressive solitary structures are likely to propagate in this plasma where dust grains are negatively （or positively） charged. The properties of dust acoustic solitons （DASs） are also investigated numerically.

The wireless distributed acquisition system for near infrared spectrosecopy(WDA-NIRS)is a portable,ultra-compact,contimuous wave(CW)NIRS system.Its main advantage is that it allows continuous synchronized multi-site hemodynarnic monitoring.The WDA-NIRS syster calculates online changes in hemoglobin concentration based on modifed Beer-Lambert law and the tissue oxy genation index based on the spatial resolved spectroscopy method.It consists of up to seven signal acquisition units,sufficiently small to be easily attached to any part of the body.These units are remotely synchronized by a PC base station for independent acquisition of NIRS signals.Each acquisition module can be freely adapted to individual requirements such as local skin properties and the microcirculation of interest,e.g,different muscles,brain,skin,etc.For this purpose,the light emitted by each LED can be individually,interactively or automatically adjusted to local needs.Furthermore,the user can freely create an emitter time multiplexing protocol and choose the detector sensitivity most suitable to a particular situation.The potential diagnostic value of this advanced device is demonstrated by three typical applications.

Bicanalicular versus monocanalicular intubation after failed probing in congenital nasolacrimal duct obstruction

AIM：To investigate the clinical outcomes of different intubation techniques in the cases of failed primary probing.METHODS： This retrospective study was performed on338 patients with the diagnosis of congenital nasolacrimal duct obstruction with age 1-4y that had failed primary probing.Intubation was performed under light sedation in operating room and the stent was left 3mo in place.Clinical outcome was investigated 3mo after tube removal.RESULTS： Bicanalicular intubation method had higher complete and relative success rates compared to monocanalicular intubation（P =0.00）.In addition,Monoka intubation had better outcomes compared to Masterka technique（P =0.046）.No difference was found between genders but the higher the age,the better the outcomes with bicanalicular technique rather than monocanalicular.CONCLUSION： Overall success rate of bicanalicular intubation is superior to monocanalicular technique especially in older ages.Also,based upon our clinical outcomes,Masterka intubation is not recommended in cases of failed probing.

AIM:To report the clinical outcomes of utilizing a three-layer flap and graft in reconstruction of the lower lid in one session.METHODS:Seventeen patients with total or near total lower eyelid defect were included.The defects were reconstructed in three layers.Posterior lamella was reconstructed by using tarsoconjunctival free graft from the ipsilateral upper lid and periosteal flap from lateral orbital rim.Mobilization of residual orbicularis muscle provided a rich blood supply;and the anterior lamella was reconstructed by skin flap prepared from upper lid blepharoplasty as a one-pedicular or bipedicular bucket handle flap.RESULTS:The cause of lower eyelid defect was basal cell carcinoma in 15 patients and trauma in two of them.No intraoperative and postoperative complication occurred.Patients were followed from 10 to 15mo postoperatively.Cosmetic results were favorable in all patients and we had acceptable functional results.Thickness of the reconstructed tissue was a concern in early postoperative period.CONCLUSION:Three-layer lower lid reconstruction in one session is an effective technique for total lower lid reconstruction with minimal complications and acceptable functional and aesthetic outcomes and can be considered as a safe alternative for the preexisting techniques.

Modified Widman flap procedure: With or without periodontal dressing?

Aim: The purpose of this study was to evaluate the postoperative pain experience and gingival indexes with and without the use of periodontal dressing after Modified Widman flap procedure. Materials and Methods: Twenty patients requiring comparable bilateral flap procedures were selected. One quadrant of each jaw randomly received periodontal dressing after the surgery while the other one didn’t. Plaque Index (PI), Sulcus Bleeding Index (SBI) and Probing Depth (PD) were measured prior to the surgery, one week and 2 weeks after the surgery. Postoperative pain experience also was assessed at the conclusion of study. Results: Results showed no significant differences between dressed and undressed segments regarding changes in probing depth, plaque index or sulcus bleeding index. However patients reported significantly less pain postoperatively when the dressing was used Conclusion: Pain is reduced by periodontal dressing but No significant differences between dressed and undressed segments regarding changes in probing depth, plaque index or sulcus bleeding index.

Introduction: People with diabetes often develop diverse microvascular, macrovascular, and neuropathic complications. Diabetes named high disease because of its high prevalence, high incidence, chronicity and high costs. Accurate projections of diabetes burden are essential for health planning so the aim of this study was to calculate the burden of diabetes in Razavikhorasan population, in Iran. Methodology: We used Disability Adjusted Life Years (DALYs) index using computer model (DisMod) for assessment of burden of diabetes and complications in Razavikhorasan province in Iran in year 2010-2011. Results: Burden of diabetes in Razavikhorasan population was 64,733 DALY in year 2010-2011. The total Years Life Lost (YLL) from diabetes mellitus was 62,883 (34,216 in men and 28,667 in women) due to premature death and the total Years Life Disability (YLD) was 1850. Conclusion: Diabetes mellitus and lesser forms of glucose intolerance can now be found in almost every population and epidemiological evidences suggest that, without effective prevention and control programs, diabetes will continue to increase and therefore advances in all levels of prevention may help reduce the number of people with diabetes.

Electrical stimulation via invasive microelectrodes is commonly used to treat a wide range of neurological and psychiatric conditions.Despite its remarkable success,the stimulation performance is not sustainable since the electrodes become encapsulated by gliosis due to foreign body reactions.Magnetic stimulation overcomes these limitations by eliminating the need for a metal-electrode contact.Here,we demonstrate a novel microfabricated solenoid inductor(80µm×40µm)with a magnetic core that can activate neuronal tissue.The characterization and proof-of-concept of the device raise the possibility that micromagnetic stimulation solenoids that are small enough to be implanted within the brain may prove to be an effective alternative to existing electrode-based stimulation devices for chronic neural interfacing applications.
