Early Morbidity and Mortality in Neonates of Mothers with Sickle Cell Disease at the Borgou/Alibori Center Departmental Teaching Hospital in Benin

Introduction: Association of sickle cell disease and pregnancy is a risky situation for both the mother and neonate. Objective: To determine the early morbidity and mortality among neonates of mothers with sickle cell disease at Borgou/Alibori Center Departmental Teaching Hospital (CHUD B/A) in Benin. Patients and Methods: This was a descriptive and analytical observational study conducted at CHUD-B/A from January 1, 2015, to August 31, 2019. It included pregnant women with sickle cell disease who gave birth to a liveborn neonate at the term of at least 28 weeks of amenorrhea. Variables studied were sociodemographic, clinical, and evolutionary. Analysis of the factors associated with early death was also carried out with a significance threshold set at p Results: Out of a total of 119 pregnant women, 95 neonates were recorded. Main morbidities were: prematurity and intrauterine growth restriction (49.5%);respiratory distress (40%), bacterial infection (30.5%), and perinatal asphyxia (21.1%). Early mortality rate was 8.4%. In bivariate analysis, the factors associated with early death were: parity (p < 0.001), the severity of maternal anemia (p < 0.008), birth weight under 1500 g (p < 0.002), the birth term under 32 weeks of amenorrhea (p < 0.001), resuscitation for at least 5 minutes (p = 0.001). In multivariate analysis, resuscitation for at least 5 minutes (p = 0.007) was mainly associated with early death. Conclusion: One out of two neonates of mothers with sickle cell disease has a low birth weight. Early mortality is high due to perinatal asphyxia. Hence the multidisciplinary care of these mothers.

Associated with Knowledge, Attitude, Practice of Women of Childbearing Age towards Gynecological and Breast Cancer in Glazoue, Benin in 2021

Introduction: Gynaecological and breast cancers (GBC) are the most frequent and most serious cancers of women. They are frequent in Benin with an average of 108.5 cases per year. Objective: To identify factors associated with women’s knowledge, attitudes and practices regarding GBC. Methods: This was a cross-sectional, prospective, descriptive and analytical study. Women of childbearing age residing in the commune of Glazoué in Benin for at least six months were included. The WHO cluster sampling technique was used. The Bruno De Finetti postulate and the Likert scale were used to study women’s knowledge, attitudes and practices. Data were collected and analysed using Epi Data 3.1 and Epi info 7.***.2.6 software. Results: A total of 540 women were registered. The mean age was 27.1 ± 8.84 years with extremes of 15 and 49 years. They were married (61.1%), had secondary education (40.2%) and lived in urban areas (61.9%). They had a low level of knowledge (75.4%), an adapted attitude (93.0%) and poor practice (88.9%). Occupation (p = 0.000), level of education (p = 0.007) and place of residence (p = 0.001) significantly influenced knowledge of GBC. The attitude was adequate (93%) and the practice was poor (88.9%). Inappropriate attitude was significantly associated with the level of education (p = 0.006). Conclusion: Women in the commune of Glazoué in Benin have a low level of knowledge of GBC with an adapted attitude and poor practice. Several factors are associated, hence the need to take them into account to facilitate GBC screening.

Emergency Obstetric Care in Parakou: Availability in Maternity Hospitals, Knowledge, Attitudes and Practices of Providers

Objective: To evaluate the availability of emergency obstetric care (EmOC) in maternity hospitals in Parakou as well as the knowledge, attitudes and practices of EmOC providers in 2017. Material and method: This was a descriptive, cross-sectional analytical study conducted in the maternity wards of public and private health facilities in Parakou and the health care providers of the maternity wards. Epi Data software version 3.1, Epi Info version 7.2 and EXCEL 2007 were used for data processing. Results: A total of 17 maternity homes and 91 providers participated in the study. All 7 basic functions (BEmOC) were available in 23.53% of these maternities. Providers’ knowledge of EmOC was good at 6.59%, average at 15.38%, insufficient at 24.18% and bad at 53.85%. As for the attitudes, they were just in 3.30%, approximate in 36.26%, wrong in 26.37% and harmful in 34.07%. Regarding the practices of these providers, they were adequate in 6.59%, inadequate in 38.46% and harmful in 54.95%. The global knowledge, attitudes and practices (CAP) score is acceptable in the majority of providers (57.14%). It is poor in 38.46% and good in 4.40%. Conclusion: In Parakou, the availability of emergency obstetric care (EmOC) contrasts with the level of knowledge, attitudes and practices of providers who require training in EmOC.

Background: Dysmenorrhea is a common symptom of gynecology that causes social and economic disruption in the lives of women who suffer from it. But many of them consider it inevitable and do not consult a health worker. Knowing the factors associated with this condition could help reduce its frequency and impact. Objective: To determine the prevalence and factors associated with dysmenorrhea in Benin in 2017. Method: This was a cross-sectional, descriptive and analytical study from 17 April to 17 May 2017. It involved 822 women aged 15 to 44 selected by a cluster sampling technique. Information on socio-demographic characteristics, functional signs, and medical care was collected using questionnaires. Introduction: The use of oxytocic plants (OPs) to facilitate childbirth is old and it is a common practice in the world, particularly in northern Benin. Objective: To study the effect of oxytocic plants on the progress of labour. Materials and Method: This was a cross-sectional and analytical study with prospective data collection conducted in the public maternity hospitals of Parakou from 15 July 2022 to 15 August 2022. All the parturients admitted to the maternity units were included. Data were entered and processed using Epi Data 3.1fr, Epi info 7.***.3.3 and Excel 2010. The difference was statistically significant for a p-value of less than 0.05. Results: Of 297 parturients surveyed, 109 had used OPs, representing a frequency of use of 36.7%. The average age was 27.5 ± 8.9 years, with extremes of 17 and 45 years. They were in common-law unions (54.1%), housewives (32.1%), from Muslim religion (53.2%), from Bariba ethnic group and related (30.3%) and from a rural area (60.5%). These parturients were pauciparous (37.6%) and primiparous (44.9%). Six OPs were used by the parturients. These were Hybanthus enneaspermus (40.7%), Schrankia leptocarpa (12.0%), Ceratotheca sesamoides (35.2%), Cassytha filiformis (5.6%), Selaginella lepidophylla (5.6%) and Newbouldia leavis (18.5%). The parturients used the leaves (90.6%) and decoction (97.2%). The administration was oral (99.0%) without precise dosage (85.3%) in the third trimester of pregnancy (84.41%) and facilitated delivery (99.1%). Fon and related ethnic groups (p = 0.014), Yoruba and related ethnic groups (p = 0.031), rural background (p = 0.032) and multiparity (p = 0.04) were the factors associated with OPs use. These parturients had a higher risk of caesarean delivery (p = 0.001;OR = 1.8 [1.23 - 2.65]), dynamic dystocia (p = 0.001;OR = 1.3 [1.12 - 1.49]), burnout syndrome (p = 0.002;OR = 1.1 [1.02 - 1.17]), uterine rupture (p = 0.032;OR = 1.1 [0.97 - 1.17]), rapid labour (p = 0.001;OR = 1.6 [1.38 - 2.00]), and risk

Introduction: Uterine fibroids are the most common gynecological tumors in women of childbearing age and have multiple clinical manifestations. Objective: To determine factors associated with uterine fibroids in the Borgou department of Benin in 2022. Method: Descriptive, analytical cross-sectional study from August 22 to September 22, 2022. The sample consisted of women consulting for gynecological problems in 6 hospitals in the Borgou department in the North of Benin. Results: Of the 744 women surveyed, 10.1% were diagnosed with uterine fibroids. Women with uterine fibroids were often married, paucigesture, and nulliparous. Their main reasons for consultation were the desire for motherhood (50.7%), pelvic pain (46.7%), and genital hemorrhage (14.7%). Age, history of miscarriage, high blood pressure, and diabetes were significantly associated with the presence of uterine fibroids (Conclusion: Uterine fibroids are common in hospitals in northern Benin. There were factors associated with them that should be taken into account in management strategies.

Epidemiological and Therapeutic Aspects of Obstetric Fistula in 2021: A Review of 97 Cases at the Departmental University Hospital Centre of Borgou and Alibori in Benin

Background: Obstetric Fistula (OF) is a breach of the birth canal due to obstructed labor. It is a major public health problem in developing countries. Objective: Study the epidemiology and treatment of obstetric fistulas at the Departmental University Hospital Centre of Borgou and Alibori (CHUD-B/A) in Parakou, Benin. Study Method: This was a descriptive and analytical cross-sectional study with prospective data collection on women treated during OF care missions from February 2020 to April 2021 in CHUD-B/A in Parakou, Benin. The data concerned their socio-demographic characteristics, obstetric history, the clinic and therapeutic characteristics of their obstetric fistulas. Results: In total 97 patients with OF were treated during the 14-month period. The average age of the patients was 36.42 ± 11.75 years old. The patients were married (51.54%), without professional occupation (54.64%) and had no formal education (73.20%). The vesico-vaginal variety was predominating (54.64%). The fistulas were of Type I (46.39%), Type II (20.62%) and Type III (32.99%) according to the classification of Waaldjik Kees. The patients were operated on under spinal anesthesia (94.79%), and vaginally (55.67%) according to the principle of Chassar Moir (68, 60%). The associated procedures were lengthening urethroplasty (17.53%), and interposition of the Martius flap (1.03%). The overall success rate without Stress Urinary Incontinence was 71.13%. The factors associated with the failure of the fistula repair were: the nutritional status of the patients (p = 0.004), the previous repair failure (p = 0.001), the high size of the fistula (p = 0.007), the fistula severity (p and a urethral reconstruction (p Conclusion: OF is a relatively frequent pathology in Benin. Vesico-vaginal fistula is the most common form. The success rate of the surgical treatment is satisfactory, but depends on some factors.

Epidemiology and Clinical Signs of Gynecological Cancers in an African Country South of the Sahara: Case of the Republic of Benin in 2022

Introduction: Gynaecological cancers are the deadliest of the women’s cancers in the Republic of Benin. Late diagnosis is the most common reason. Objective: This paper aims to describe the epidemiological characteristics, and clinical and pathological signs of gynaecological cancers treated in the Republic of Benin between 2018 and 2022. Patients and Methods: This was a cross-sectional, descriptive, retrospectively collected study of patient data treated between 2018 and 2022 in two university gynaecology departments in Cotonou. All gynaecological cancers that have histological evidence were included. The epidemiological, clinical and pathological characteristics of the cancers were assessed. Results: Cervical, endometrial and ovarian cancers were the most common in the proportions of 62.0%, 24.1%, 12.0% and 1.8% respectively. The mean age at diagnosis was 54 years. The victims were uneducated and had low economic power in 81% and 85% of cases, respectively. The consultation was late in 82.1% of cases. Metrorrhagia, postmenopausal metrorrhagia and pelvic cluster headache were the common reasons for consultation for cervical, endometrial and ovarian cancer, respectively. Diagnosis was late in 66.7% (n = 71). The most common histological types were squamous cell carcinoma, endometrioid adenocarcinoma, and serous cystadenocarcinoma for cervical, endometrial, and ovarian cancers, respectively. Conclusion: Gynaecological cancers were common and their consultation time was delayed. The diagnosis was made at the advanced stage and there were several reasons for this.

Introduction: Hypertensive disorders of pregnancy (HDP) are often accompanied by cardiovascular sequelae. The objective of this study was to describe the prognosis in the postpartum period of patients with HDP in Departmental University Hospital of Borgou CHUD-B from Parakou in 2020. Methods: The study was prospective with a descriptive and analytical design and was conducted from January 2020 to September 2020. Patients with HDP were recruited from the gynecology-obstetrics department and each followed for 3 months in the cardiology department. HDP was classified according to the International Society for the Study of Hypertension in Pregnancy, and blood pressure (BP) was taken according to WHO recommendations. Self-measurement of BP at home was performed to assess blood pressure control outside the hospital. Epidata 3.1 and SPSS 21 software were used for data processing and analysis. P values Results: During the study period, the hospital frequency of HDP was 15.6%. The frequency of maternal complications in the postpartum period was 28% (severe hypertension: 23.2%;eclampsia: 3.6%;puerperal psychosis: 1.2%). At the end of the three-month follow-up, blood pressure returned to normal in 73.2% of cases;it persisted in 26.8% of women.
