Evaluation of clinical competence and its related factors among ICU nurses in Kermanshah-Iran: A cross-sectional study

Objectives:The present study was conducted to evaluate the clinical competence and its related demographic factors among critical care nurses in Kermanshah,***:In this cross-sectional study,155 Iranian nurses were selected by stratified random *** data collection tools included a personal information form and the"Nurse Competence Scale".Data were analyzed using descriptive and analytical ***:The mean score of nurses'clinical competence was equal to 76.14±1.59 out of 100,which was at a"very good level".The mean score of using clinical competence in practice was equal to 70.38±15.25 out of 100,which was at a"good level".Among the subscales of clinical competence,the highest mean score was related to"managing situation".The mean score of"using clinical competence in practice"was related to the subscale of"therapeutic interventions".There was no statistically significant difference among the score of clinical competence of nurses varying with different gender,age,academic degree,and work ***:The clinical competence of critical care nurses in Kermanshah was at a"very good"level,and the use of clinical competence in practice was at a"good level."Given the importance of clinical competencies in practice,nurses'clinical competence should be evaluated objectively and positive measures should be taken to promote the application of their clinical competence.

Cochlear Synaptopathy Following Noise Exposure in Guinea Pigs: Its Electrophysiological and Histological Assessments

Exposure to high level of noise,may cause the permanent cochlear synaptic *** present study,a model of noise induced cochlear synaptopathy was established and the electrophysiological and histological metrics for its assessment was designed.6 guinea pigs were subjected to a synaptopathic noise(octave band of 4 kHz at 104 dB SPL,for 2-h).The amplitude growth curve of Auditory Brainstem Response(ABR)wave-I and wave-III latency shift in presence of noise were *** indexes were considered in pre-exposure,1 day post exposure(1DPE),1 week post exposure(1WPE)and 1 month post exposure(1MPE)to ***,the samples were histologically *** wave-I amplitude was different between pre and 1DPE(p-value≤0.05).However,at 1WPE,it was recovered at low intensities but at 70 dB SPL and above,the differences persisted even till *** masked ABR,the latency shift of wave-III was different between pre and 3 post exposure assessments(p-value≤0.05).Electro-microscopic analysis confirmed the synaptic degeneration,as the ribbons were larger than normal,hollow inside,and spherical and irregular in shape,also,the post synaptic density was abnormally thick and missed its flat *** data revealed that noise at level below that can produce permanent hearing loss,can incur synaptic ***,noise is considered to be more damaging than previously ***,designing tools for clinical assessment of synaptopathy is beneficial in comprehensive auditory evaluation of those with history of noise exposure and also in hearing protection planning.

Investigation of the Cause of Precocious Puberty in an 8-Year-Old Girl Ended up in Juvenile Granulosa and Theca Cell Tumor of the Ovary

Isosexual precocious puberty in girls has several etiologies. Juvenile granulose cell tumor is one of the rarest causes that only stands for 1.5% of ovarian cancers. This tumor mostly encounters in first 2 decades of life. This paper is a report of an 8-year-old girl with precocious puberty that within five months developed breast enlargement followed by menarche. Works which are done to find the underlying cause of precocious puberty revealed juvenile granulosa cell tumor in her left ovary. She then under went laparoscopic surgery and 3 courses of chemotherapy. She did not experience any vaginal bleeding after that and the serum level of estradiol lay among its normal ranges, but after that the tumor relapsed and presented as abdominal pain and a huge mass which under went resection of all afflicted tissues. After 2 courses of chemotherapy, her status deteriorated and unfortunately she died after 6 months from the time of diagnosis. Treatment for this disease is consists of resection surgery and chemotherapy. If this tumor is diagnosed in its early stages, it will be curable, but in its advanced stages, up to 80% of patients die from recurrent tumors. The reported patient was diagnosed at stage IIIC that had poor prognosis.
