Industry 4.0, or the Fourth Industrial Revolution, is based on digitized the manufacturing process and makes use of all digital tools so its combination of various digital technologies computers, ERP software, IoT, machine learning and AI techniques, Manufacturing Execution Systems (MES), and big data analytics to create a new, fully digitized manufacturing system. The Critical Success Factors (CSFs) of MES adoption are both a quantitative and qualitative measurement. We use the case of ready-made garments to improve each of the three Overall Equipment Efficiency (OEE) factors: Availability, Performance, and Quality. In this study, we adopt real-time management of production activities on the shop floor from order receipt to finished products, then measure the improvement.

Objective: Nerve blockade is commonly used in oncosurgical procedures as an alternative to general anesthesia for older patients and those with significant medical problems. We report a single tertiary oncology center experience in performing various lip resections and reconstructive techniques using this technique. Patients and methods: sixty patients with lower lip tumors were enrolled into this study with exclusion of refusing, non-compliant and critically ill patients. A solution of 4 ml 2% Lidocaine and epinephrine was put in a 5 ml syringe. Half the solution (2 ml) was injected into each side with a 23 gauge needle and after 5 minutes the surgical resection was carried out after testing for anesthesia. Results: The mean age was 68 ± 6.2 years. The mean ASA score was 3 ± 0.75. There were 4 cases who expressed painful sensation and their operations were completed with fentanyl increments. Mean hospital stay was 1 ± 0.75 days. There were three cases of wound gaping who were treated with secondary closure in an outpatient basis. One patient expressed postoperative acute ischemic heart pain that was managed with anti-ischemic measurements. Conclusion: Bilateral mental nerve block is a safe and effective alternative to general anesthesia in lower lip tumors’ resection especially in older patients and those with poor tolerance for general anesthesia provided that there is no need for cervical nodal dissection.

Distributed generators now is widely used in electrical power networks, in some cases it works seasonally, and some types works at special weather conditions like photo voltaic systems and wind energy, and due to this continuous changes in generation condition, the fault current level in network will be affected, this changes in fault current level will affect in the coordination between protection relays and to keep the coordination at right way, an adaptive protection system is required that can adaptive its setting according to generation changes, the fault current level in each case is evaluated using ETAP software, and the required relay setting in each case is also evaluated using Grey Wolf Optimizer (GWO) algorithm, and to select suitable setting which required in each condition, to select the active setting group of protection relay according to generation capacity, central protection unite can be used, and to improve protection stability and minimizing relays tripping time, a proposed method for selecting suitable backup relay is used, which leads to decrease relays tripping time and increase system stability, output settings for relays in all cases achieved our constrains.

With the rapid development in business transactions,especially in recent years,it has become necessary to develop different mechanisms to trace business user records in web server log in an efficient *** business transactions have increased,especially when the user or customer cannot obtain the required *** example,with the spread of the epidemic Coronavirus(COVID-19)throughout the world,there is a dire need to rely more on online business *** order to improve the efficiency and performance of E-business structure,a web server log must be well utilized to have the ability to trace and record infinite user *** paper proposes an event stream mechanism based on formula patterns to enhance business processes and record all user activities in a structured log *** user activity is recorded with a set of tracing parameters that can predict the behavior of the user in business *** experimental results are conducted by applying clustering-based classification algorithms on two different datasets;namely,Online Shoppers Purchasing Intention and Instacart Market Basket *** clustering process is used to group related objects into the same cluster,then the classification process measures the predicted classes of clustered *** experimental results record provable accuracy in predicting user preferences on both datasets.

With an increasing number of services connected to the internet,including cloud computing and Internet of Things(IoT)systems,the prevention of cyberattacks has become more challenging due to the high dimensionality of the network traffic data and access ***,researchers have suggested deep learning(DL)algorithms to define intrusion features through training empirical data and learning anomaly patterns of ***,due to the high dynamics and imbalanced nature of the data,the existing DL classifiers are not completely effective at distinguishing between abnormal and normal behavior line connections for modern ***,it is important to design a self-adaptive model for an intrusion detection system(IDS)to improve the detection of ***,in this paper,a novel hybrid weighted deep belief network(HW-DBN)algorithm is proposed for building an efficient and reliable IDS(***)model to detect existing and novel *** HW-DBN algorithm integrates an improved Gaussian–Bernoulli restricted Boltzmann machine(Deep GB-RBM)feature learning operator with a weighted deep neural networks(WDNN)*** CICIDS2017 dataset is selected to evaluate the *** model as it contains multiple types of attacks,complex data patterns,noise values,and imbalanced *** have compared the performance of the *** model with three recent *** results show the *** model outperforms the three other models by achieving a higher detection accuracy of 99.38%and 99.99%for web attack and bot attack scenarios,***,it can detect the occurrence of low-frequency attacks that are undetectable by other models.

Background and Aims:Occult HCV infections(OCIs)include IgG antibody seronegative cryptogenic(COCIs),as well as seropositive secondary na(i)ve(SNOCIs)and experienced(SEOCIs)*** used peripheral-blood-mononuclear-cell(PBMC)-PCR to evaluate COCIs and SNOCIs prevalence,serum HCV spontaneous disappearance(SCSD)in na(i)ve cirrhotics and non-cirrhotics,intra-PBMC HCV-RNA strands in relation to cirrhosis density in na(i)ve non-viremia cases,and HCV-RNA seroconversion after 1 year of solitary naive intra-PBMC ***:The anti-HCV IgG antibodypositive na(i)ve-patients(n=785)were classified into viremic(n=673)and non-viremic[n=112,including non-cirrhotics(n=55)and cirrhotics(n=57)],and 62 controls without evidence of *** and post-HCV non-viremia cases(n=62+112=174)were submitted to hepatic FibroscanElastography *** subjects(n=847)were screened for intra-PBMC HCV-RNA sense and antisense strands by ***:Na(i)ve-OCI cases(4.84%)that were diagnosed by PBMC-PCR significantly raised the total numbers of HCV-infection to 714(p=0.01).The percent positivity of SNOCIs(34.82%)was significantly higher than for asymptomatic-COCIs(3.125%,p=0.0001).Comparing PBMC-PCR with single-step-reverse-transcription(SRT)-PCR for identification of SCSD in na(i)ve IgG antibody-positive nonviremia patients(n=112)revealed a decline in SCSD prevalence by PBMC-PCR(from 14.27%to 9.3%),regardless of presence of hepatic cirrhosis(p=0.03).SCSD was found to be higher by PBMC-PCR in non-cirrhotics compared to cirrhotics(p=0.0001),with an insignificant difference when using SRT-PCR(p=0.45).Intra-PBMC HCV-RNA infection was significantly more frequent in cirrhotics compared to both non-cirrhotics and controls(p<0.0005).An increased hepatic fibrosis density was recognized in intra-PBMC HCV-RNA infection with sense(p=0.0001)or antisense strand(p=0.003).HCV-RNA seroconversion was associated with intra-PBMC infection when both sense and antisense strands were dete

Background and Aims:Sustained virologic response is evaluated by single-step reverse transcription(SRT)PCR as-say,which assesses hepatitis C virus(HCV)clearance from plasma but not from tissues such as peripheral blood mono-nuclear cells(PBMCs).Persistence of HCV RNA in PBMCs beyond end of treatment(EOT)is associated with *** goal was to measure intra-PBMC HCV RNA levels during oral antiviral therapy according to the HCV therapy follow-up fractionation(CTF2)***:Compen-sated chronic HCV patients(n=278 SRT-PCR positive)were scheduled to receive oral antiviral *** were followed-up by SRT and intra-PBMCs HCV RNA PCR at the end of the 2nd,6th,10th,14th,18th and 24th weeks to evaluate virus clearance from plasma and PBMCs,*** CTF2 protocol evaluated SRT and PBMC PCR status at each follow-up point for determining therapy continuation or inter-ruption to address cost ***:All patients tested negative by SRT PCR after therapy for 2 ***-cation of the CTF2 protocol revealed:a)increasing HCV clear-ance rate from 75.9%at the end of 10th week to 90.3%at the end of 24th week(p<0.00001);b)faster clearance of HCV from plasma compared to PBMCs at each point of follow-up until the 18th week(p<0.05);c)higher viral elimination rates diagnosed by PBMC HCV RNA PCR(?)compared to PBMC HCV RNA PCR(+)from the 6th to 24th week of treatment(p<0.0001);d)higher over-time increase curve of combined plasma and PBMC HCV RNA determined negativity compared to the decline in positivity curves by PBMC PCR at the 6th-18th week compared to the 24th week(p<0.01)—these results validated treatment continuation;and e)solitary evaluation of EOT sustained HCV infection and relapses by PBMC HCV RNA(p<0.001).Conclusions:Early elimination of serum and tissue(PBMC)HCV infection by oral antiviral therapy can be achieved and evaluated during a cost-effective CTF2 pro-tocol application.

Objective： To study the timing of Bevacizumab （BVC） in the overall treatment strategy of advanced metastatic colorectal cancer - early use （first-line） or later use. Methods： 41 patients with progressive metastatic colorectal carcinoma were included. Patients were randomized to receive chemotherapy with or without BVC. Primary end point was objective response. Secondary end points were median survival, time to tumor progression, and toxicity. Results： Partial response with second-line BVC group constituted 25% and 18.8% in patients with first-line chemotherapy and BVC-based regimen respectively, compared to 11.8% and 5.9% with second-line chemotherapy. Median time to progression was 3.1 vs. 2.3 months for cases with first-line chemotherapy and BVC-based regimens respectively. Median survival was 8.2 vs. 4 months in both groups respectively （P = 0.019）. Conclusion： Second-line chemotherapy combined BVC had higher disease control rate （partial response and stable disease）, median time to progression and median survival in BVC-naive patients compared to patients with first-line BVC-based therapy. BVC should be maintained in the second- and third-line settings, as cases with BVC discontinuation had significantly lower median time to disease progression and median survival. Selection of patients for use of BVC was recommended with taking into consideration the cost-benefit value and that the discontinuation of BVC would increase tumor progression.

Evaluation of Acute Toxicity and Dosimetric Parameters in High Risk Prostate Cancer Patients Treated by High Radiation Doses

For high risk prostate cancer, the treatment volumes and even dose levels are still a controversial issue. The aim of this study is to evaluate the dosemetric parameters and acute toxicity of dose-escalated whole pelvis (WP) Intensity Modulated Radiation Therapy (IMRT) and volumetric modulated arc therapy (VMAT) prostate boost following neoadjuvant and concomitant with androgen deprivation therapy in high-risk prostate cancer patients. This analysis included 73 high-risk prostate cancer patients treated with WP-IMRT followed by boost to the prostate by VMAT to total dose of 80 Gy;between January 2014 and October 2016. Androgen deprivation therapy (ADT) was given for all patients before and during radiation therapy. Drawing the dose volume histograms (DVHs) was done for planning target volumes (PTVs), including Prostate PTV & nodal PTV, and organs at risk including rectum, bladder, femoral heads, and bowel bag for the plans. Acute radiation toxicities were reported during the radiation course and the following 3 months. The DVH analysis showed good coverage of PTVs and organs at risk doses were acceptable. No recorded acute Grade ≥ 3 toxicity. Acute grade 1 toxicity for Gastrointestinal (GI) and Genitourinary (GU) were 65% and 35% respectively, while Grade 2 toxicity was 30% for both. The Proctitis and frequency were the commonest acute toxicity and were maximal during the 5th week of radiation therapy. Dose escalation in two phases utilizing Simultaneous integrated boost (SIB) combined with ADT in high risk prostate cancer patient is feasible and associated with acceptable acute GI and GU toxicity.
