Pancreatic pseudocyst or a cystic tumor of the pancreas?

Pancreatic pseudocysts are the most common cystic lesions of the pancreas and may complicate acute pancreatitis, chronic pancreatitis, or pancreatic trauma. While the majority of acute pseudocysts resolve spontaneously, few may require drainage. On the other hand, pancreatic cystic tumors, which usually require extirpation, may disguise as pseudocysts. Hence, the distinction between the two entities is crucial for a successful outcome. We conducted this study to highlight the fundamental differences between pancreatic pseudocysts and cystic tumors so that relevant management plans can be devised. We reviewed the data of patients with pancreatic cystic lesions that underwent intervention between June 2007 and December 2010 in our hospital. We identified 9 patients(5 males and 4 females) with a median age of 40 years(range, 30–70 years). Five patients had pseudocysts, 2 had cystic tumors, and 2 had diseases of undetermined pathology. Pancreatic pseudocysts were treated by pseudocystogastrostomy in 2 cases and percutaneous drainage in 3 cases. One case recurred after percutaneous drainage and required pseudocystogastrostomy. The true pancreatic cysts were serous cystadenoma, which was treated by distal pancreatectomy, and mucinous cystadenocarcinoma, which was initially treated by drainage, like a pseudocyst, and then by distal pancreatectomy when its true nature was revealed. We conclude that every effort should be exerted to distinguish between pancreatic pseudocysts and cystic tumors of the pancreas to avoid the serious misjudgement of draining rather than extirpating a pancreatic cystic tumor. Additionally, percutaneous drainage of a pancreatic pseudocyst is a useful adjunct that may substitute for surgical drainage.

Hydrogen has been regarded as a promising renewable and green energy source to meet energy needs and attain net-zero carbon emissions.The electrolysis of seawater to make hydrogen is one of the fascinating developments of the twenty-first century.This method uses abundant and relatively inexpensive seawater,as opposed to freshwater,which is rare and can be prohibitively expensive.In recent years,significant research and advancements have been made in direct seawater electrolysis technology for hydrogen production.However,producing highly effective and efficient electrocatalysts with long-term viability under harsh corrosive conditions remains a challenging and severe topic for large-scale seawater electrolysis technology.There is still a large accomplishment gap in understanding how to improve seawater electrolysis to increase hydrogen yields and prolong stability.It is,therefore,crucial to have a condensed knowledge of the tunable and inherent interactions between various electrocatalysts,covering electrolyzer types and paying particular attention to those with high efficiency,chemical stability,and conductivity.The extensive discussion is structured into a progression from noble metals to base metal compounds such as oxides,alloys,phosphides,chalcogenides,hydroxides,and nitrides,MXene-based complexes with a concise examination of hybrid electrocatalysts.In addition,proton exchange membranes,anion exchange membranes,alkaline water electrolyzers,and high-temperature water electrolyzers were potential contributors to seawater’s electrolysis.An extensive assessment of the techno-feasibility,economic insights,and future suggestions was done to commercialize the most efficient electrocatalytic systems for hydrogen production.This review is anticipated to provide academics,environmentalists,and industrial researchers with valuable ideas for constructing and modifying seawater-based electrocatalysts.

Injury severities from heavy vehicle accidents:An exploratory empirical analysis

Despite the importance of heavy vehicles in Australia’s transportation system,little is known on the factors influencing injury severity from accidents involving a single heavy vehicle.Heavy vehicular crashes have been one of the main causes of fatal injuries in Australia,and this raises safety concerns for transport authorities,insurance companies,and emergency services.Although there have been several potential attempts to identify the factors contributing to heavy vehicle crashes and injury severity,it is still necessary to reduce the number of traffic crashes and lower the fatality rate involving heavy vehicles.The aims of this study were investigating the effects of heavy trucks’presence in accidents on the injury severity level sustained by the vehicle driver and detecting the contributing factors that lead to specific injury severity levels.Fixed-and random-parameter ordered probit and logit models were applied for predicting the likelihood of three injury severity categories severe,moderate,and no injury based on data from crashes caused by heavy trucks in Victoria,Australia in 2012-2017.The results showed that the random-parameter ordered probit model performed better than the other models did.Twenty variables(i.e.,factors)were found to be significant,and 12 of them were found to have random parameters that were normally distributed.Since some of the investigated factors had different effects on the type of injury severity in Australia,this paper does not recommend generalizing the findings from other case studies.Based on the findings,Victoria state authorities can have insight and enhanced understanding of the specific factors that lead to various types of injury severity involving heavy trucks.Consequently,the safety of all road users,including heavy vehicle drivers,can be enhanced.

Wireless Sensor Networks(WSNs)can be termed as an autoconfigured and infrastructure-less wireless networks to monitor physical or environmental conditions,such as temperature,sound,vibration,pressure and motion etc.WSNs may comprise thousands of Internet of Things(IoT)devices to sense and collect data from its surrounding,process the data and take an automated and mechanized decision.On the other side the proliferation of these devices will soon cause radio spectrum shortage.So,to facilitate these networks,we integrate Cognitive Radio(CR)functionality in these networks.CR can sense the unutilized spectrum of licensed users and then use these empty bands when required.In order to keep the IoT nodes functional all time,continuous energy is required.For this reason the energy harvested techniques are preferred in IoT networks.Mainly it is preferred to harvest Radio Frequency(RF)energy in the network.In this paper a region based multi-channel architecture is proposed.In which the coverage area of primary node is divided as Energy Harvesting Region and Communication Region.The Secondary User(SU)that are the licensed user is IoT enabled with Cognitive Radio(CR)techniques so we call it CR-enabled IoT node/device and is encouraged to harvest energy by utilizing radio frequency energy.To harvest energy efficiently and to reduce the energy consumption during sensing,the concept of overlapping region is given that supports to sense multiple channels simultaneously and help the SU to find best channel for transmitting data or to harvest energy from the ideal channel.From the experimental analysis,it is proved that SU can harvest more energy in overlapping region and this architecture proves to consume less energy during data transmission as compared to single channel.We also show that channel load can be highly reduced and channel utilization is proved to be more proficient.Thus,this proves the proposed architecture cost-effective and energy-efficient.

The generation of electricity,considering environmental and eco-nomic factors is one of the most important challenges of recent years.In this article,a thermoelectric generator(TEG)is proposed to use the thermal energy of an electric water heater(EWH)to generate electricity independently.To improve the energy conversion efficiency of the TEG,a fuzzy logic con-troller(FLC)-based perturb&observe(P&O)type maximum power point tracking(MPPT)control algorithm is used in this study.An EWH is one of the major electricity consuming household appliances which causes a higher electricity price for consumers.Also,a significant amount of thermal energy generated by EWH is wasted every day,especially during the winter season.In recent years,TEGs have been widely developed to convert surplus or unused thermal energy into usable electricity.In this context,the proposed model is designed to use the thermal energy stored in the EWH to generate electricity.In addition,the generated electricity can be easily stored in a battery storage system to supply electricity to various household appliances with low-power-consumption.The proposed MPPT control algorithm helps the system to quickly reach the optimal point corresponding to the maximum power output and maintains the system operating point at the maximum power output level.To validate the usefulness of the proposed scheme,a study model was developed in the MATLAB Simulink environment and its performance was investigated by simulation under steady state and transient conditions.The results of the study confirmed that the system is capable of generating adequate power from the available thermal energy of EWH.It was also found that the output power and efficiency of the system can be improved by maintaining a higher temperature difference at the input terminals of the TEG.Moreover,the real-time temperature data of Abha city in Saudi Arabia is considered to analyze the feasibility of the proposed system for practical implementation.

Organosilicone thin films have been deposited by plasma polymerization(pp)in a plasma enhanced chemical vapor deposition(PECVD)system using hexamethyldisilazane(HMDSN:C_(6)H_(19)Si_(2)N)as a monomer precursor,at different biases of the stainless-steel substrate holder.The substrate bias affected film thickness,surface morphology,chemical composition and photoluminescence(PL)emission.For a negatively biased substrate,it is found that the film thickness is the minimum,while the porosity and PL emission are the maximum.For a positively biased substrate,the thickness and the ratio of Si/N are the maximum which correspond to a blue shift of the PL emission in comparison with the case of non-biased grounded substrate.In addition,the characterization of the plasma using a single cylindrical Langmuir probe has been performed to obtain information about both the electron density and the positive ion energy,where it can be concluded that the ion energy plays a major role in determining film thickness.

In the contemporary era,the abundant availability of health information through internet and mobile technology raises concerns.Safeguarding and maintaining the confidentiality of patients’medical data becomes paramount when sharing such information with authorized healthcare providers.Although electronic patient records and the internet have facilitated the exchange of medical information among healthcare providers,concerns persist regarding the security of the data.The security of Electronic Health Record Systems(EHRS)can be improved by employing the Cuckoo Search Algorithm(CS),the SHA-256 algorithm,and the Elliptic Curve Cryptography(ECC),as proposed in this study.The suggested approach involves usingCS to generate the ECCprivate key,thereby enhancing the security of data storage in EHR.The study evaluates the proposed design by comparing encoding and decoding times with alternative techniques like ECC-GA-SHA-256.The research findings indicate that the proposed design achieves faster encoding and decoding times,completing 125 and 175 iterations,respectively.Furthermore,the proposed design surpasses other encoding techniques by exhibiting encoding and decoding times that are more than 15.17%faster.These results imply that the proposed design can significantly enhance the security and performance of EHRs.Through the utilization of CS,SHA-256,and ECC,this study presents promising methods for addressing the security challenges associated with EHRs.

Current evidence of concept analyses recommending nanotechnology for biomedical uses abounds in recent research. The area of biotechnology interfaces with nanostructures, reconfigures their composition, and alters their characteristics;which influences the dispersion of the particles, the biotransformation they cause, and their potential toxic effect. It is vital to link the idea of the lifecycle of nanostructures to the biological impacts and use methodologies to identify, estimate, and track the gradual bioprocessing of nanostructures in vivo, from a body-wide level to a nanoscopic size. This is necessary because understanding how nanostructures processing, degradation, persistence, and recycling predict potential exposure risks. The safe implementation of nanotechnology-based products in biomedical applications necessitates an extensive understanding of the recycling and transformations of nanomaterials in a living organism. Long-term fate in the body is crucial, as it governs potential environmental risks to human health. Strategies may be used to manage the long-term outcome of nanostructures in an organism since, in addition to composition, their design also affects how long they last and how easily they degrade. The lifespan of nanoparticles, a flexible and biocompatible category of nanostructures that have made it into clinical trials, is the subject of this article. This review explained the safety of nanoscale materials, biotransformation, and the multifunctional recycling mechanism of nanostructures.

传统的建筑技术由于环境危害问题严重,经济损失大,一直受相关方面的到审查。预制装配式技术由于具有相对较少的环境危害性、经济效益好,对熟练劳动力的要求较低等优势,已经被视为传统建筑技术的可行替代方案。除此之外,在研究过程中,有限元分析技术也被充分证明比实验更节省时间和资源。因此本研究利用有限元分析技术评估了带有顶部和底部L形连接件的装配式端板连接节点的抗震性能。该节点连接属于装配式连接,端板在工厂与梁翼缘用螺栓连接,整个连接在现场用螺栓组装。这种类型的钢结构连接技术是一种新型的连接技术,有可能用于高层建筑。(1)使用ABAQUS软件建立了 H型柱-端板连接节点的1:1有限元分析模型,并在梁端施加了低周往复循环荷载。通过比较有限元分析结果和实验结果,验证了该模型的正确性。从有限元分析结果中获得了滞后曲线、骨架曲线和延性系数,并与实验结果进行了比较。结果与实验结果一致,因此证明了有限元分析模型的有效性。(2)对节点进行了参数化扩展分析,考虑了不同的连接几何参数,如垂直和水平肋的厚度、L形连接件的水平肢的长度、L型加劲肋厚度和数量以及L形连接件水平和垂直肢不同长度的组合。使用有限元分析对总共13个连接进行了建模,并对其施加了低的往复循环载荷。观察了不同的几何参数对节点抗震性能的影响。通过有限元分析,获得了节点的力学性能,如滞后曲线、骨架曲线、延性系数、刚度退化、承载能力退化和接头的能量耗散能力。提出了具有最理想的抗震性能的节点形式。(3)增加L型角钢水平肢长度对连接节点的抗震性能有显著的积极影响。通过增加L型连接件水平肢长度,节点承载能力提高了近16.2%,能量耗散能力也得到了相应提高。而增加L型加劲肋数量则对节点性能没有影响或影响很小。增加L型角钢的水平和垂直肢的厚度对节点性能没有明显的改善效果。另一方面,减少厚度对节点的承载能力和能量耗散能力有不利影响。(4)理论上,连接节点被假定为理想的铰接或刚性连接。然而,在实践中,钢结构连接的行为有一定的差异。在这项研究中,利用有限元分析对测试的连接进行了进一步的分类,根据欧洲标准EN1993-1-8,对连接的扭矩承载力和刚度进行了分类。这些连接被划分为全强度半刚性连接。
