Objective: To determine if insemination at standing heat results in a similar or higher pregnancy rate compared with fixed time artificial insemination, and to study some factors affecting the pregnancy rate. Methods: A total of 8944 inseminations were included in this study, from which 6823 inseminations were done in Holstein cows and 2121 inseminations were performed in Simmental cows. All cows were subjected to a Presynch-Ovsynch protocol. Cows detected in estrus (n=7424) were artificially inseminated, whereas cows not observed in estrus (n=1520) were submitted to fixed time artificial insemination. Results: The overall pregnancy rate of cows inseminated on the basis of the detected standing heat was comparable to that recorded for cows receiving fixed time artificial insemination. A higher pregnancy rate was recorded for cows during cold months than that recorded during hot months (P=0.000). A higher pregnancy rate was recorded for Simmental compared with that recorded for Holstein cows (P=0.001). Regarding parity, significant differences in the pregnancy rate were detected between primiparous and multiparous cows (P=0.040). In addition, artificial insemination technicians had no sigificant effect on pregnancy rate (P>0.05). Meanwhile, the used artificial insemination sires significantly (P=0.000) impacted the pregnancy rate. Conclusions: Insemination of cows detected in standing heat prior to predetermined fixed time results in similar pregnancy outcome and decreases days to the first service compared with insemination at the scheduled fixed time at the end of the Presynch-Ovsynch synchronization program. The overall pregnancy outcome is not affected by the breeding program, but it is highly impacted by cow's breed, parity, artificial insemination sire and breeding season.

Background: Prolonged pressure on the skin known as pressure sores, can cause frequent injuries to the skin and underlying tissues. However, several prevention approaches, including non-surgical and surgical management,are available. In flap surgery for pressure sores, a variety of flap types may be used; each offers certain benefits and some incidences of complications and recurrence. This study evaluated the versatility of the perforatorenhanced rhomboid flap technique for ischial sore ***: This prospective study was conducted in the Plastic Surgery Department of Fayoum University *** perforator-enhanced rhomboid flap technique was employed in 20 patients with ischial pressure sores who failed to respond to conservative treatment. A proper history was obtained and a preoperative arterial duplex was performed to determine the site of the prominent perforators for their preservation at the base of the *** follow-ups and evaluations were ***: The mean age of the study group was 43.7 ± 13.1, ranging from 19 to 65 years. Moreover, 80% of the participants were males while 20% were female. The main etiology of the sores was paraplegia in 45% of *** mean surface area of sores was 24.8 ± 6.3 cm2. In 75% of cases, the color match of the flap was rated as excellent. Similarly, 70% of the patients rated flap thickness as excellent. In 55% of the cases, the scar appearance was deemed excellent. Overall satisfaction was reported as excellent by 40% of the patients, while only 5% reported poor satisfaction. Background: The myometrium at the location of the CS (caesarean section) scars, also known as residual myometrium thickness (RMT), is larger after a double-layer uterine closure procedure than following a single-layer one. It may lessen the formation of a niche that is the myometrium’s disruption at the location of the scar of the uterus. Gynecological manifestations, obstetric problems in a future pregnancy and birth, and maybe subfertility are linked to thin RMT and a niche. Objective: To ascertain if double-layer unlocked closure of the uterus is better than single-layer one in terms of post-menstrual spotting and niche development following a first CS. Patients and Methods: In this randomized clinical study, 287 patients were evaluated for qualifying. Of all eligible individuals, 57 patients were excluded from the study based on the inclusion criteria. Results: The variation in ages, gestational age, body mass index (BMI), and cesarean section indications between the two assigned groups is statistically insignificant. However, postmenstrual spotting was statistically significantly more common in single-layer group compared to in double-group. The current study revealed ultrasound findings suggestive of niche formation was statistically significantly more common in single-layer group compared to in double-layer group. Conclusion: As evident from the current study, it demonstrates the advantages of double-layer unlocked closure of the uterus over single-layer one in terms of post-menstrual spotting and niche development following first-time cs. Thus, we deduced that fewer niches are formed, and fewer menstrual spotting occurs in the presence of double unlocked layers closure. To ascertain the impact of uterus closure method on post-operative niche development and the risk of obstetrics and gynaecological problems, further prospective trials with extended follow-up periods are required.

Background: Improving Water, Sanitation and Hygiene in health care settings is a critical prerequisite for achieving national health goals and Sustainable Development Goals (SDGs). The World Health Organization (WHO) has set a target for each United Nations member state to reach by 2030. Each member state is required to reach by 2022, 2025 and 2030 at least 60%, 80% and 100%, respectively of basic level of service of the five elements which are water, sanitation, hygiene, waste management and environmental cleaning. Methods: This study aimed to evaluate and document the current state of basic water, sanitation, and hygiene services in all lower-level health care facilities in the Dar es Salaam region of Tanzania as of July 2022. A cross-sectional study was conducted in 99 public dispensaries in the Dar es Salaam region’s five councils: Ubungo, Kigamboni, Kinondoni and Temeke Municipalities, and Ilala City. The interviewee form and observational checklists were both digitalized using the Kobo tool software. The respondents were health care facility in-charges or nurse in-charges. Data were downloaded, validated, and imported to Stata version 15 for analysis. Results: The basic WASH level per JMP is far below the target in 2022. Each member state by 2022 is required to reach at least 60% of the basic level of service of each element. We found a low coverage of basic WASH in the 99 dispensaries included in this study. The basic WASH coverage was met in only 10 (10.1%) of the dispensaries, while the remaining 89 (89.9%) dispensaries fall on limited WASH services. Conclusion: This study revealed lower coverage of basic WASH services in dispensaries. An urgent need is required to improve the status of WASH in all the dispensaries and facilitate the provision of quality health care services, patient safety and reduce health care associated infections.

As the Internet of Things(IoT)and mobile devices have rapidly proliferated,their computationally intensive applications have developed into complex,concurrent IoT-based workflows involving multiple interdependent *** exploiting its low latency and high bandwidth,mobile edge computing(MEC)has emerged to achieve the high-performance computation offloading of these applications to satisfy the quality-of-service requirements of workflows and *** this study,we propose an offloading strategy for IoT-based workflows in a high-performance MEC *** proposed task-based offloading strategy consists of an optimization problem that includes task dependency,communication costs,workflow constraints,device energy consumption,and the heterogeneous characteristics of the edge *** addition,the optimal placement of workflow tasks is optimized using a discrete teaching learning-based optimization(DTLBO)*** experimental evaluations demonstrate that the proposed offloading strategy is effective at minimizing the energy consumption of mobile devices and reducing the execution times of workflows compared to offloading strategies using different metaheuristics,including particle swarm optimization and ant colony optimization.

Efficient Channel Estimation Techniques for MIMO Systems with 1-Bit ADC

With a low resolution 1-bit ADC on its receiver(RX) side, MIMO with 1-bit ADC took a considerable step in the fulfillment of the hardware complexity constrains of the internet of things(IoT) PHY layer design. However, applying 1-bit ADC at MIMO RX results in severe nonlinear quantization error. By which, almost all received signal amplitude information is completely distorted. Thus, MIMO channel estimation is considered as a major barrier towards practical realization of 1-bit ADC MIMO system. In this paper, two efficient sparsity-based channel estimation techniques are proposed for 1-bit ADC MIMO systems, namely the low complexity sparsity-based channel estimation(LCSCE), and the iterative adaptive sparsity channel estimation(IASCE). In these techniques, the sparsity of the 1-bit ADC MIMO channel is exploited to propose a new adaptive and iterative compressive sensing(CS) recovery algorithm to handle the 1-bit ADC quantization effect. The proposed algorithms are tested with the state-of-the-art 1-bit ADC MIMO constant envelope modulation(MIMO-CEM). The 1-bit ADC MIMO-CEM system is chosen as it fulfills both energy and hardware complexity constraints of the IoT PHY layer. Simulation results reveal the high effectiveness of the proposed algorithms in terms of spectral efficiency(SE) and computational complexity. The proposed LCSCE reduces the computational complexity of the 1-bit ADC MIMO-CEM channel estimation by 86%, while the IASCE reduces it by 96% compared to the recent techniques of MIMO-CEM channel estimation. Moreover, the proposed LCSCE and IASCE improve the spectrum efficiency by 76 % and 73 %, respectively, compared to the recent techniques.
