Internal fixator vs external fixator in the management of unstable pelvic ring injuries:A prospective comparative cohort study

BACKGROUND Reconstruction of the pelvic ring anatomy in unstable anterior pelvic ring injuries is a significant step to reduce the mortality rate associated with these injuries efficiently.There is a debate on using either an anterior subcutaneous pelvis internal fixator(INFIX)or an anterior supra-acetabular external fixator(EXFIX)to manage an unstable anterior pelvic ring fracture.AIM To compare the functional and radiological outcomes and complications of INFIX vs EXFIX in managing unstable pelvic ring injuries.METHODS A prospective cohort study included 54 patients with unstable pelvic ring fractures.The patients were divided into two groups;the INFIX group,in which 30 cases were fixed by INFIX,and the EXFIX group,in which 24 patients were treated by EXFIX.The average age in the EXFIX group was 31.17 years(16-57 years),while in the INFIX group,it was 34.5 years(17-53 years).The study included 20(66.7%)males and 10(33.3%)females in the INFIX group and 10(41.7%)males and 14(58.3%)females in the EXFIX group.The radiological outcomes were evaluated using Matta and Tornetta's score,and the functional outcomes using the Majeed score.RESULTS The results revealed a statistically significant difference between both groups(P=0.013)regarding radiological outcomes,according to Matta and Tornetta's score in favor of the INFIX group.Sitting,standing,and walking abilities were measured at a 3-mo follow-up visit using Majeed score modules.It was significantly better among the INFIX group than the EXFIX group in all three modules.At the final follow-up,both groups had no statistically significant difference according to the Majeed score;92.35 in the INFIX group and 90.99 in the EXFIX group(P=0.513).A lower surgical site infection rate was noticed in the INFIX group(P=0.007).CONCLUSION Anterior subcutaneous pelvis INFIX is associated with better radiological outcomes and a lower infection rate than anterior supra-acetabular EXFIX in managing patients with unstable anterior pelvic ring fractures.

As the extensive use of cloud computing raises questions about the security of any personal data stored there,cryptography is being used more frequently as a security tool to protect data confidentiality and privacy in the cloud environment.A hypervisor is a virtualization software used in cloud hosting to divide and allocate resources on various pieces of hardware.The choice of hypervisor can significantly impact the performance of cryptographic operations in the cloud environment.An important issue that must be carefully examined is that no hypervisor is completely superior in terms of performance;Each hypervisor should be examined to meet specific needs.The main objective of this study is to provide accurate results to compare the performance of Hyper-V and Kernel-based Virtual Machine(KVM)while implementing different cryptographic algorithms to guide cloud service providers and end users in choosing the most suitable hypervisor for their cryptographic needs.This study evaluated the efficiency of two hypervisors,Hyper-V and KVM,in implementing six cryptographic algorithms:Rivest,Shamir,Adleman(RSA),Advanced Encryption Standard(AES),Triple Data Encryption Standard(TripleDES),Carlisle Adams and Stafford Tavares(CAST-128),BLOWFISH,and TwoFish.The study’s findings show that KVM outperforms Hyper-V,with 12.2%less Central Processing Unit(CPU)use and 12.95%less time overall for encryption and decryption operations with various file sizes.The study’s findings emphasize how crucial it is to pick a hypervisor that is appropriate for cryptographic needs in a cloud environment,which could assist both cloud service providers and end users.Future research may focus more on how various hypervisors perform while handling cryptographic workloads.

The use of carbon nanomaterials in membrane distillation membranes:a review

Membrane distillation(MD)is a thermal-based separation technique with the potential to treat a wide range of water types for various applications and industries.Certain challenges remain however,which prevent it from becoming commercially widespread including moderate permeate flux,decline in separation performance over time due to pore wetting and high thermal energy requirements.Nevertheless,its attractive characteristics such as high rejection(ca.100%)of nonvolatile species,its ability to treat highly saline solutions under low operating pressures(typically atmospheric)as well as its ability to operate at low temperatures,enabling waste-heat integration,continue to drive research interests globally.Of particular interest is the class of carbon-based nanomaterials which includes graphene and carbon nanotubes,whose wide range of properties have been exploited in an attempt to overcome the technical challenges that MD faces.These low dimensional materials exhibit properties such as high specific surface area,high strength,tuneable hydrophobicity,enhanced vapour transport,high thermal and electrical conductivity and others.Their use in MD has resulted in improved membrane performance characteristics like increased permeability and reduced fouling propensity.They have also enabled novel membrane capabilities such as in-situ fouling detection and localised heat generation.In this review we provide a brief introduction to MD and describe key membrane characteristics and fabrication methods.We then give an account of the various uses of carbon nanomaterials for MD applications,focussing on polymeric membrane systems.Future research directions based on the findings are also suggested.

Pancreatic injury in patients with septic shock: A literature review

Sepsis and septic shock are life threatening condition associated with high mortality rate in critically-ill patients. This high mortality is mainly related to the inadequacy between oxygen delivery and cellular demand leading to the onset of multiorgan dysfunction. Whether this multiorgan failure affect the pancreas is not fully investigated. In fact, pancreatic injury may occur because of ischemia, overwhelming inflammatory response, oxidative stress, cellular apoptosis and/or metabolic derangement. Increased serum amylase and/or lipase levels are common in patients with septic shock. However, imaging test rarely reveal significant pancreatic damage. Whether pancreatic dysfunction does affect the prognosis of patients with septic shock or not is still a matter of debate. In fact, only few studies with limited sample size assessed the clinical relevance of the pancreatic injury in this group of patients. In this review, we aimed to describe the epidemiology and the physiopathology of pancreatic injury in septic shock patients, to clarify whether it requires specific management and to assess its prognostic value. Our main finding is that pancreatic injury does not significantly affect the outcome in septic shock patients. Hence, increased serum pancreatic enzymes without clinical features of acute pancreatitis do not require further imaging investigations and specific therapeutic intervention.

Geochemistry of island arc assemblage in the Eastern Desert of Egypt and the role of Pan-African magmatism in crustal growth of the Arabian–Nubian Shield:A review

Neoproterozoic island arc assemblage of the Arabian–Nubian Shield(ANS)in the Eastern Desert(ED)of Egypt comprises juvenile suites of metavolcanics(MV),large amounts of meta-sedimentary rocks(MS),and voluminous metagabbros-diorites(MGD)and syn-tectonic intrusions of older granitoids(OG).We report here the updates of these four rock units in terms of classification,distribution,chemical characteristics,geodynamic evolution,metamorphism,and ages.In addition,we discuss these integrated data to elucidate a reasonable and reliable model for crustal evolution in the ANS.The main features of these rock units indicate their relation to each other and the geodynamic environment dominated by early immature oceanic island arcs to primitive continental arcs.Integrated information of the island arc metavolcanic and plutonic rocks(gabbros,diorites,tonalites,and granodiorites)furnish evidence of the genetic relationships.These include proximity and a coeval nature in the field;all protolith magmas are subalkaline in nature following calc-alkaline series with minor tholeiitic affinities;common geochemical signature of the arc rocks and subduction-related magmatism;their similar enrichment in LREEs;and similar major element compositions with mafic melts derived from metasomatized mantle wedge.The volcano-sedimentary and the OG rocks underwent multiphase deformation events whereas the MGD complexes deformed slightly.Based on the magmatic,sedimentological,and metamorphic evolutions constrained by geochronological data as well as the progressive evolutionary trend from extensional to compressional regimes,a possible gradual decrease in the subducted slab dip angle is the most infl uential in any geodynamic model for arc assemblage in the ED of Egypt.

Impacts of Climate Change on Seasonal Residential Electricity Consumption by 2050 and Potential Adaptation Options in Alexandria Egypt

Climate change and associated more frequent, hot or cold, extreme/weather events, as well as increasing temperature may increase future residential demand for electricity for heating and cooling purposes. The paper in hand intends to assess potential impacts of increasing temperature attributed to climate change on seasonal residential electricity consumption in Alexandria city, Egypt. Additionally, it aims to identify and discuss potential soft and hard adaptation options to such impacts. For this purpose, seasonal changes in electricity consumption were investigated. For this purpose, data on monthly residential electricity consumption, population size and income levels at district level as well as maximum monthly temperature in Alexandria city, Egypt over the period 2007-2016 were collected. This is followed by developing a panel-data model to simulate influence of temperature on residential electricity consumption. It was found that there is a significant growth trend over the study period as well as considerable seasonal variation with summer season experienced significant increase in consumption. It was found that increasing temperature, under RCP 2.6 and RCP 8.5, may contribute to significant increase in residential summer electricity consumption by 2050. Different adaptation options to such an increase in consumption, both soft and hard, have been identified and assessed.

Petrology and Geochemistry of Some Ophiolitic Metaperidotites from the Eastern Desert of Egypt:Insights into Geodynamic Evolution and Metasomatic Processes

Ophiolitic peridotites exposed in the Eastern Desert(ED)of Egypt record multiple stages of evolution,including different degrees of partial melting and melt extraction,serpentinization,carbonatization and metamorphism.The present study deals with metaperidotites at two selected localities in the central and southern ED,namely Wadi El-Nabáand Wadi Ghadir,respectively.They represent residual mantle sections of a Neoproterozoic dismembered ophiolite that tectonically emplaced over a volcano-sedimentary succession that represents island–arc assemblages.The studied metaperidotites are serpentinized,with the development of talc-carbonate and quartz-carbonate rocks,especially along shear and fault planes.Fresh relics of primary minerals(olivine,orthopyroxene and Cr-spinel)are preserved in a few samples of partiallyserpentinized peridotite.Most of the Cr-spinel crystals have fresh cores followed by outer zones of ferritchromite and Crmagnetite,which indicates that melt extraction from the mantle protolith took place under oxidizing conditions.The protoliths of the studied metaperidotites were dominated by harzburgites,which is supported by the abundance of mesh and bastite textures in addition to some evidence from mineral and whole-rock chemical compositions.The high Cr#(0.62–0.69;Av.0.66)and low TiO_(2)(<0.3 wt%)contents of the fresh Cr-spinels,the higher Fo(89–92;Av.91)and NiO(0.24–0.54 wt%,Av.0.40)contents of the primary olivine relics,together with the high Mg#(0.91–0.93;Av.91)and low CaO,Al2 O3 and TiO_(2)of the orthopyroxene relics,are all comparable with depleted to highly depleted forearc harzburgite from a suprasubduction zone setting.The investigated peridotites have suffered subsequent phases of metasomatism,from oceanfloor hydrothermal alteration(serpentinization)to magmatic hydrothermal alteration.The enrichment of the studied samples in light rare earth elements(LREEs)relative to the heavy ones(HREEs)is attributed to most probably be due to the contamination of their mantle source with granitic source hydr

Histopathological,histochemical and biochemical postmortem changes in induced fatal hypothermia in rats

Reaching a postmortem diagnosis of hypothermia is challenging in forensic practice.Therefore,this study was conducted to detect the histopathological,histochemical and biochemical changes that occur in adult albino rats following exposure to induced fatal hypothermia.Twenty-four adult albino rats were divided into the negative control,moderate hypothermia,severe hypothermia and hypoxia groups.Rats in the control group were euthanized when those in the moderate hypothermic group died.Blood samples were collected via heart puncture,and the cerebrum,heart,suprarenal gland,kidney,liver and skeletal muscle were removed to investigate the biochemical,histochemical and histopathological changes.Postmortem assessment depicted significant changes in lipid peroxidation,represented by increased malondialdehyde levels in the studied organs of the rats in hypothermic and hypoxia groups.Histopathological examination of the rats’organs revealed degeneration and necrosis in the hypothermia and hypoxia groups.Sections taken from the severe hypothermic rats revealed a loss of normal cardiac tissue architecture,necrotic changes in the pyramidal cells in the cerebral cortex,and massive necrosis,mainly in the tubules of the renal cortex and medulla.These findings suggest that histological changes might be used as biochemical markers for postmortem diagnosing of fatal hypothermia,particularly in severe hypothermic conditions.
