Groundwater discharge drives water quality and greenhouse gas emissions in a tidal wetland

Wetlands play an important role in the global carbon cycle as they can be sources or sinks for greenhouse *** discharge into wetlands can affect the water chemistry and act as a source of dissolved greenhouse gases,including CO_(2)and CH_(4).In this study,surface water quality parameters and CO_(2)and CH_(4)concentrations were evaluated in a tidal wetland(Hunter Wetlands National Park,Australia)using time series ***(^(222)Rn),a natural ground water tracer,was used to investigate the role of groundwater as a pathway for transporting dissolved CO_(2)and CH_(4)into the *** addition,water-to-air CO_(2)and CH_(4)fluxes from the wetland were also *** results showed a high concentration of radon in wetland surface water,indicating the occurrence of ground water *** concentration had a strong negative relationship with water depth with a determination coefficient(R^(2))of 0.7,indicating that tidal pumping was the main driver of groundwater discharge to the *** concentration also showed a positive relationship with CO_(2)and CH_(4)concentrations(R^(2)=0.4 and 0.5,respectively),while the time series data revealed that radon,CO_(2),and CH_(4)concentrations peaked concurrently during low *** implied that groundwater discharge was a source of CO_(2)and CH_(4)to the *** wetland had an average water-to-air CO_(2)flux of 99.1 mmol/(m^(2)·d),twice higher than the global average CO_(2)flux from *** average CH_(4)flux from the wetland was estimated to be 0.3 mmol/(m^(2)d),which is at the higher end of the global CH_(4)flux range for *** results showed that groundwater discharge could be an important,yet unaccounted source of CO_(2)and CH_(4)to tidal *** work has implications for tidal wetland carbon budgets and emphasizes the role of groundwater as a subsurface pathway for carbon transport.

Sheathless and fluoroscopy-free retrograde intrarenal surgery:An attractive way of renal stone management in high-volume stone centers

Objective:To evaluate the efficacy and safety of sheathless and fluoroscopy-free flexible ureterorenoscopic laser lithotripsy(FURSL)for treating renal ***:Between May 2015 and May 2017,135 patients with renal stones treated with sheathless and fluoroscopy-free FURSL were prospectively *** technique involved a semirigid ureteroscopic assessment of the ureter,and the guidewire was left in situ to railroad the flexible ureteroscope.A holmium laser was used to fragment and dust the stones;fragments were neither grasped nor ***:The study population consisted of 135 patients including 85 males(62.96%)and 50 females(37.04%)with a mean age of 40.65 years(range:3-70 years)were *** mean stone size was 17.23 mm(range:8-41 mm).Complete stone-free status was achieved in 122(90.37%)patients and clinically insignificant residual fragments(CIRF)in two(1.48%),while residual stones were still present in 11(8.15%)*** complications occurred in 23(17.4%)cases and were mostly minor,including fever in 17(12.6%),pyelonephritis in four(3.0%),subcapsular hematoma in one(0.7%)and steinstrasse in one(0.7%).These complications were Clavien Ⅰ-Ⅱ,GⅠ in 17(12.6%)patients,GⅡ in five(3.7%),and Clavien Ⅲb in one(0.7%).No major complications were *** size2 cm,operative time ≥30 min,and lasing time ≥20 min were significantly associated with a higher rate of complications and lower stone-free rates upon univariate analysis(p<0.05).Conclusion:Sheathless and fluoroscopy-free FURSL are effective and safe for renal stone management,especially for stones under 2 cm in *** process is a feasible option for avoiding sheath complications,which can protect surgeons from the negative effects of radiation.

Long-Term Results of Balloon Angioplasty for Native Coarctation of the Aorta in the Surgical Specialty Teaching Hospital/Cardiac Center/Hawler

Background: Coarctation of aorta is a common congenital cardiac malformation;controversy has surrounded the use of balloon angioplasty for native coarctation of aorta as the primary treatment for patients with various ages with coarctation. Aim: This study aimed to assess the long term outcome of balloon angioplasty especially concerning aneurysm formation. Patients and Methods: A case series (case follow-up) study was carried out on a total of 50 patients (31 male and 19 female) with native coarctation of aorta aged 1 - 21 years of age who were treated with balloon angioplasty for more than 1 year at the time of follow up evaluation in surgical specialty hospital/cardiac center in Erbil Iraq. They were recalled and subjected to detail clinical examination, body weight, height, blood pressure measurements. Radiographic, Echo Doppler data, CT angiography were obtained. CT angiography was done for 34 patients. Full echocardiographic evaluation was done in follow up visits. Results: There was a significant reduction in the peak instantaneous pressure gradient across the coarctated area by Doppler echocardiogram gradient before balloon angioplasty and at the follow up visit from 56.92 ± 14.6 mmHg to 30.68 ± 16.89, P = 0.00. Six cases of total 50 patients had evidence of diastolic runoff pattern by Doppler Echcardiogram. While the only 6 of total 34 cases had the report of CT angiography that documented aneurysm formation (12%). Conclusions: Balloon angioplasty of native aortic coarctation can be performed safely and effectively with good immediate outcome. Furthermore, it offers satisfactory long-term results with low incidence of persisting restenosis, hypertension and aneurysm formation.

Ureteroscopic Management of Ureteral Calculi: Pneumatic versus Holmium: YAG Laser Lithotripsy

Background: Among various intracorporeal lithotripters, pneumatic lithotripter has become the widely used tool for the treatment of urinary stones. Recently the holmium: YAG laser has been used with a wide range of potential urological applications, including intracorporeal lithotripsy of urinary calculi. Purpose: Compare the effectiveness and complications of treatment for ureteric stones between holmium laser lithotripsy and pneumatic lithotripsy. Material and Methods: Comparison of 100 patients presented with ureteric stones, group one (50) of whom were treated with pneumatic lithotripsy and group two (50) with holmium laser was done and the effectiveness and complications of both were analyzed. Results: There was no difference in patient age, sex, stone size and location of stones between the two groups. The immediate stone free rates were 88% in the holmium: YAG group and 66% in the pneumatic lithotripsy group (p < 0.05). The four weeks stone free rates were 98% and 94% respectively (p = 0.07). The mean ± SD operative time in the holmium: YAG group (40 ± 26 min) was shorter than those with pneumatic lithotripsy group (60 ± 40 min). Postoperative stay in hospital was less than 24 hours in holmium: YAG group (70.4%) and shorter than those for pneumatic group (29.6%) (p < 0.002). Post treatment complications such as ureteral perforation were encountered in only two patients who underwent pneumatic lithotripsy. Other complications, such as mucosal injury, ureteral perforation and postoperative fever;there was no statistically significant difference between the two groups. While light hematuria was found more frequently in the pneumatic group (14%) in comparison to laser group (12%) (p = 0.02). On the other hand, the overall complication rates between the two groups was statistically significant (8%) laser group vs. (32%) pneumatic group (p = 0.003). The Additional Value of Cystoscopy with Urodynamic Study in the Assessment of Patients with Urinary Incontinence

Background: Urinary incontinence (UI) is generally defined as the involuntary loss of urine from the bladder through the urethral meatus. Filling cystometry is the method by which the pressure/ volume relationship of the bladder is measured during bladder filling. Purpose: To determine the value of diagnostic cystoscopy in addition to Urodynamic study (UDS) in patients with primary urinary incontinence. Material and Methods: 200 patients with primary incontinence studied prospectively from January 2013 to June 2014. Their age ranges from (14 - 93 years), 86.5% were female, and 13.5% of them were male. In addition to physical, neurological examination and bio-chemical investigations, urine analysis and urine culture with Ultra-sound and Post void residual volume (PVRV), all patients underwent diagnosticflexible cystoscopy under local anesthesia, and urodynamic study. Result: 43.5% of patients age were between (34 - 53 years), (39%) between (54 - 73 years), (9%) between (14 - 33 years) and (8.5%) were between (74 - 93 years). Atonic bladder on UDS were (40.5%), Detrusor over activity (29%), patients with normal UDS were (22%) and patients that had DSD (Detrusor Sphincter Dyssynergia) were (8.5%). Eighty-four cases (42%) were found to have normal cystoscopy, those with grade-I-II bladder wall trabeculations were (49%) and patients with grade-III were (8.5%). Sixty-one patients (30.5%) with normal diagnostic cystoscopy have abnormal UDS (Atonic bladder, over-active bladder and DSD) while twenty-one (10.5%) patients with normal UDS had bladder wall trabeculations (grade-I-III) on diagnostic cystoscopy. Conclusion: Diagnostic cystoscopy in addition to urodynamic study will put in further knowledge in the assessment of patients with urinary incontinence. To some extent, it can predict the diagnosis.

采用完全随机实验设计了4个重复,研究干旱胁迫对6个油菜栽培种的不同生长时期（种子萌发和秧苗生长期）的影响。干旱胁迫处理在四个时期（正常灌溉、花期灌溉、结实期灌溉、种子灌浆期灌溉）和六个冬油菜栽培种（SLM046,Licord,Opera,Zarfam,Orient,Okapi）中进行。冷处理是对100粒种子四次重复,每次把样品种子放在湿润的滤纸上,5°C处理5天,然后转移到22°C处理5天,进行萌芽。5天后测秧苗的根、芽长,根芽比、鲜干重,正常和不正常秧苗,MGT,MDG和发芽速度。方差分析表明干旱胁迫下根茎长度,根/茎比,秧苗的干鲜重（p∠0.01）和不正常的秧苗（p小于0.05）之间和栽培种×干旱胁迫下栽培种不正常的秧苗（p小于0.01）和FGP,MTG及秧苗干重（p小于0.05）之间有显著的差别,并在栽培种×干旱胁迫互作下根/茎比和秧苗鲜重也存在显著差异。比较数据的平均值,表明正常灌溉条件下有最大值的茎长和鲜重。在结荚和灌浆后进行干旱胁迫增加了非正常秧苗和苗干重。Licord有最大值的FGP,MTG,Zarfam和最大值的苗干重。在结荚期和灌浆期经干旱胁迫后Licord和Opera有最大的根芽比。在正常灌溉和开花期灌溉时,SLMO46和Zarfam有最大值的鲜重。
