Periodontal Disease of Pregnant Women and Low Weight Newborn in Senegal: A Case-Control Study

The aim of this work was to study the connection between periodontal diseas of women during their pregnancy and the weight of newborn infants the women gave birth to. Method: It was a case-control study and carried out on the outskirts of Dakar (Senegal). The sample consisted of 129 mothers with infants of weight < 2500 g (case) and 258 mothers with infants of weight ≥ 2500 g (control). The socio-demographic variables, patterns of life, history and outcome of pregnancy were collected. The variables relating to periodontal status included the plaque index, inflammation, bleeding, loss of clinical attachment and pocket depth. Data were analyzed by the R software. Logistic regression had identified associations on the threshold of 5%. Results: The proportions of mothers with periodontitis were 70.6% among cases versus 33% in controls. Periodontitis was significantly associated with low weight (OR = 4 [2.3 - 5.7] 4) adjusted on age, BMI and other periodontal indices. Conclusion: periodontal disease appears to be involved in the occurrence of low birth weight in the African context. The introduction of an oral component in prenatal consultations is therefore necessary for the health of mothers and their children.

Hemodynamic instability secondary to left atrial (LA) compression by an aortic aneurysm is a rare entity. We report?the case of a 43-year old woman with no previous diagnosis of congestive heart failure who was admitted for an initial diagnosis of pulmonary embolism (PE) based on shortness of breath, hypotension and D-Dimers?elevation. The electrocardiogram and blood counts were within normal limits. The chest X-ray revealed widening of the mediastinum. Transthoracic echocardiography demonstrated LA compression by a large descending thoracic aortic aneurysm. Left and right ventricle systolic functions were preserved. Chest angiography showed LA and left pulmonary artery (LPA) compression by a descending aortic aneurysm and an intramural hematoma with no evidence of PE evidence. Emergency surgery could not be done because of her financial status. She was treated medically and was discharged 1?week later with significant improvement. However she remained hypotensive.

Sudden cardiac death (SCD) generally refers to sudden cardiovascular death of a person with or without pre-existing heart disease. This is a retrospective study conducted at the Department of Anatomy and Pathological Cytology at Aristide Le Dantec Hospital in Senegal. This study covers a period of 7 years from January 1st, 2000 to December 31st, 2006. During this 7-year period we collected 235 cases of sudden cardiovascular death out of a total of 3717 forensic autopsies, representing a frequency of 6.32%. Sudden cardiovascular deaths accounted for 75.3% of all 312 sudden deaths. 96.1% were black compared to 3.9% Caucasians. The average age of the patients was 46.86 years with extremes ranging from 17 to 86 years. The most affected age group was between 50 - 59 years old, representing 23.4% of the cases. The sex ratio was 4.3 in favour of men. Men aged 50 - 59 were the most affected, while women were more affected in the 20 - 29 age group. Cardiomyopathies constituted 54.9% with 90% of dilated cardiomyopathies of which 75% were male and 10% of hypertrophic cardiomyopathies more frequent also in men. Valvulopathies came in 3rd position with 3.4% of the cases and in 75% of the cases they were poly-valvulopathies. Atherosclerosis was incriminated in 2.5% of the cases and aortic localization was the most represented. Aortic dissection was implicated in 6 cases out of 235 or 2.5%. It was isolated in 4 cases (without other cardiac conditions), and in the other 2 cases it was accompanied by other cardiac lesions including hypertrophy, pericarditis and endocarditis. The other causes found were interauricular communication (n = 1) and pericarditis (n = 2). Sudden cardiovascular death is a major global public health problem. The lack of epidemiological data on sudden death in Africa motivated our work, which led us to note that 75.3% of all sudden deaths in adults were of cardiovascular origin.

Secondary metabolites produced by plants can be used in popular medicine, as well they can interact with plants and other organisms, in which case they are called allelochemicals and influence the neighboring ecosystem. This work aimed to evaluate the allelopathic effects of Schinus molle L. species widely used in reforestation and urban afforestation. Therefore, leaves of S. Molle were collected from different populations located at Alfenas and Nepomuceno in Minas Gerais, Brazil. Cypselae of lettuce （Lactuca sativa L.） were germinated in biochemical oxygen demand （BOD） chamber at 25 ℃ with a photoperiod of 12 h under different extracts concentrations. The experiment was performed in factorial design （2 × 4） with two kinds of extracts （aqueous and ethanol） and four concentrations （2.5, 5, 10 and 20 mg/mL） in randomized blocks. The variables analyzed were germination （%）, germination speed index （GS1）, % of normal seedlings, root length, fresh biomass and dry biomass. Changes in the cell cycle in meristematic cells of the used model were also evaluated. Concentration-dependent effect on all parameters was observed, with the exception of dry biomass exposed to ethanol extract. The root elongation parameter was different between the extracts even in lower concentrations, thus indicating that this is the most sensitive parameter of this species. Toxic effect from S. rnolle extracts was observed in all parameters regardless the kind of pruning management or method of extraction （water or ethanol）.

Objective: To report the experience of surgical caravans for urogenital fistulas care. Methodology: This was a retrospective study covering the period from January 2014 to December 2014 and which took place on 7 sites of fistula care. The epidemiological, anatomoclinical, therapeutic and evolutionary aspects were studied. Results: 346 patients were operated during 14 caravans. Their average age was 33.11 years (range: 12 to 70 years). Most of these patients were without remunerative activities (80%) and without education (63.3%). The average duration of fistula progression was 6.08 years (range: 0 to 42 years). Obstetrical etiology was predominant (87.9%). According to the classification of Kees Waaldijk, fistulas were divided into type I (67.4%), type II (21.1%), and type III (11.5%). The most common surgical approach was the transvaginal route (82.1%). The vesicovaginal splitting with separated suture of the bladder and the vagina was the basic technique (94.7%). The therapeutic results were judged after a follow-up of 1 month and 3 months. Across the cohort, 80 patients (23.1%) were lost of sight for follow-up at 1 month and 245 (70%) at 3 months. The success rates evaluated in patients reviewed at 1 month and 3 months were respectively 70% and 64%. Conclusion: The incidence of urogenital fistulas is still high in Cote d'Ivoire. Various anatomoclinical varieties have been identified and treated with satisfactory results in poorly equipped local structures.

Perceptions of Actors on the Community-Based Maternal and Neonatal Health Services Project at the Kolda and Sedhiou Regions

Introduction: The health of mothers and children remains a major concern in developing countries. Infant and maternal mortality rates, although declining, are still high. Access to health services is a major obstacle to reducing maternal and infant mortality. To support the Ministry of Health and Social Action in the implementation of its Strategic Plan for Reproductive Health (2012-2015), the NGO Micronutriment Initiative has developed a project based on community-based maternal and neonatal health services (CBMNHS) in the Kolda region. The general objective of this study is to collect the perceptions of the actors on the Community-Based Maternal and Neonatal Health Services project in the Kolda and Sedhiou regions. Method: This was a qualitative study consisting of an evaluation of the activities carried out within the framework of the CBMNHS project at the level of the intervention zone. The study focused on support groups, community actors, post nurses and implementers of the intervention. Sampling was comprehensive for community actors and nurse heads. Focus groups and in-depth individual interviews were used to assess the perceptions of women and support groups, as well as to better understand the dynamics of the intervention. The content analysis was performed to exploit the qualitative data. Results: All local and national health authorities interviewed are unanimous about the relevance of the project. The project’s strategies are essentially community-oriented. Thus, a large number of committees have been set up. A total of 1258 support groups were set up during the project, distributed as follows: 260 grandmother groups (Grandmothers Strategy);266 groups of pregnant women (Pregnant Women’s Solidarity Circles);248 Care Group;468 community watch and alert circles and 16 Future Fathers’ Groups (Future Fathers’ Solidarity Circles).
