In recent years, the popularity of medicinal plants as a remedy has been increased manifold due to having minimal adverse effects. The current study aimed to compare the hypoglycemic, hypolipidemic and hepatoprotective effects of the ethanolic extract of Asparagus racemosus (EEAR) Linn. alone and combinedly with conventional antidiabetic agents (gliclazide and pioglitazone) in alloxan-induced diabetic rats. Diabetes was induced in male Wister albino rats by the administration of single intra-peritoneal injection of alloxan monohydrate (120 mg/kg b.w.). Effect of oral administration of two different doses of EEAR (200 and 400 mg/kg b.w.), gliclazide (10 mg/kg b.w.) and pioglitazone (10 mg/70kg/b.w.) alone for 2 weeks and a combination of EEAR (200 mg/kg b.w.) with either gliclazide (10 mg/kg b.w.) or pioglitazone (10 mg/70kg/b.w.) for 2 weeks were examined on hypoglycemic activity on 0th, 5th, 10th and 14th day of treatment. After 2 weeks of treatment, hypolipidemic and hepatoprotective effects were estimated by serum biochemical markers such as total cholesterol (TC), triglyceride (TG), low density lipoprotein (LDL), very low density lipoprotein (VLDL), high density lipoprotein (HDL), serum glutamate oxaloacetate transaminases (SGOT), serum glutamate pyruvate trans-aminases (SGPT) and total protein (TP) with the help of commercially available kits. The survival rate, body weight and organ weight were also measured. Alloxan treatment resulted in persistent hyperglycemia, hyperlipidemia and liver dysfunction in rats. Treatment with EEAR at different doses improved hyperglycemia significantly (p th and 14th day of treatment in a dose-dependent mood when compared to the disease control rats, gliclazide treated rats and pioglitazone treated rats. Acute Lymphoblastic Leukemia Presenting as Systemic Juvenile Idiopathic Arthritis: Experience from Bangladesh

Background: Acute lymphoblastic leukemia (ALL), the most common paediatric malignancy, is a heterogeneous hematologic disease. ALL patients may present with isolated and persistent osteo-articular complaints, lower incidence of hepatomegaly, splenomegaly or lymphadenopathy without clear laboratory features, and misdiagnosed as systemic juvenile idiopathic arthritis (sJIA). Methods: This was a single center cross sectional study over a period of 4 years. Clinic laboratory profiles of 39 ALL children were compared with 39 age and sex-matched sJIA cases. Result: Among 39 ALL patients 89.7% were initially misdiagnosed as sJIA upon clinical presentation. Majority (66.7%) of ALL patients had oligo-articular joint involvement. In sJIA, small joints of the hands were most commonly involved. The total WBC count was significantly higher in ALL patients (p-value 0.0065). CRP and LDH values between the two groups showed significant differences (p-value 0.00006 and 0.00001 respectively). Conclusion: The presentation of leukemia with arthralgia or arthritis makes the diagnosis difficult for the physicians. The diagnosis of sJIA must be made with caution keeping the possibility of haematological malignancy in mind.

Serum Zinc and Copper Level in Juvenile Idiopathic Arthritis (JIA) Patients and Its Correlation with Disease Duration-A Tertiary Hospital Study

Background: Juvenile Idiopathic Arthritis (JIA) is the most prevalent rheumatic disease in children. It is associated with abnormal levels of serum zinc (Zn) and copper (Cu) as during inflammation serum copper concentration increases and zinc decreases. Objective: To assess the serum Zn and Cu levels in different sub-types of JIA patients and their correlation with the disease duration. Methods: This cross-sectional study was conducted over twelve months at the Pediatric Rheumatology Division, Department of Paediatrics, Bangabandhu Sheikh Mujib Medical University. Sixty-nine JIA cases that fulfilled the International League of Association for Rheumatology (ILAR) criteria were taken as cases and age and sex-matched healthy children were considered as controls. The serum Zn and Cu tests were done using the spectrophotometric method with INDIKO PLUS Drug Analyzer. Data were recorded in a pre-designed questionnaire. Data were checked, verified and analyzed manually where continuous variables were analyzed using unpaired t-test and categorical variables using the ANOVA test. Pearson’s correlation coefficient test was used to see the correlation of serum zinc and copper levels with disease duration. Results: Boys were predominant in both case and control groups, with the majority within the 10 to 16-year-age group. Enthesitis-related arthritis (ERA) was the most common subtype followed by sJIA, Oligo JIA, Poly JIA (RF-) and unclassified subtypes. Disease duration was found less than 12 months in 30.4% of JIA patients. Serum analysis revealed a statistically significant reduction in mean zinc levels and increased copper levels in JIA patients compared to controls. This study observed a negative correlation between serum zinc levels and disease duration, whereas serum copper levels exhibited a positive correlation with disease duration. Frequency of Uveitis among Juvenile Idiopathic Arthritis Patients in a Tertiary Care Hospital of Bangladesh: A Retrospective Study

Background: Juvenile idiopathic arthritis (JIA) is the most frequently encountered pediatric rheumatologic disorder with an unknown etiology. At present there is no published data regarding the frequency of uveitis in patients with JIA in Bangladesh. This study aimed to observe the frequency of JIA-associated uveitis (JIAU) and distribution of uveitis among different sub-categories of JIA at the Pediatric Rheumatology division, both outdoor and indoor patients, Department of Pediatrics, Bangabandhu Sheikh Mujib Medical University (BSMMU) in Bangladesh. Methods: This was a retrospective study of 1784 JIA patients at the Pediatric Rheumatology division, BSMMU from July 2010 to March 2023. Results: Among the 1784 enrolled JIA patients, we observed that 0.73% of cases had uveitis. Here, 61.5% of JIAU cases were male. Most of the cases (92.3%) had bilateral uveitis and only 7.7% cases had unilateral uveitis. Among JIAU patients, the majority were Oligo JIA (53.8%), followed by ERA 30.8%, Poly JIA RF(−) 7.7% and Systemic JIA 7.7% cases respectively. This study also revealed that 15.4% of JIAU patients had ANA positivity and 23% had HLA B-27 positivity. Here we also found ocular complications associated with uveitis such as band keratopathy (23.1%), posterior synechiae (15.4%) and cataract (15.4%). Conclusions: In this study, we observed only 0.73% of patients of JIA had developed uveitis which is lower than the frequency observed in other European studies. This study also showed various ocular complications amongst JIA-associated uveitis patients which signifies the importance of adherence to periodic ophthalmological follow-up to prevent these ocular complications.

Geochemical characteristics of Holocene sediments from Chuadanga district, Bangladesh: Implications for weathering, climate, redox conditions, provenance and tectonic setting

The present research deals with the geochemical characteristics of the Holocene sediments from Alamdanga area, Chuadanga district, Bangladesh. Main goals of the study are to delineate source rock characteristics, degree of chemical weathering and sorting processes and behavior of redox conditions during deposition of the sediments. Geochemical characteristics of the sediments show comparatively a wide variation in accordance with stratigraphy in their major element contents(e.g. Si O2 69.46–82.13, Al2O3 2.28–8.88 in wt%), reflecting the distinctive provenance and in part an unstable period in terms of tectonic activity. Geochemical classification of the sediments shows mostly sub-arkose with few sub-litharenites. Some major and trace elements display comprehensible correlation with Al2O3 confirming their possible hydraulic fractionation. The chemical index of alteration(CIA*), W* index, index of compositional variability(ICV), plagioclase index of alteration(PIA*) values and the ratio of Si O2/Al2O3, suggest low degrees of chemical weathering in the source areas as well as immature to moderately mature the sediments. The sediments suggest semi-arid climatic trends within oxic deltaic depositional conditions during the Holocene, at 3–12 ka. Whole rock geochemistry and discrimination diagrams demonstrate the continental signature derivatives, which might have been derived from the felsic to intermediate igneous rocks(granitic plutonic rocks) as well as from quartzose sedimentary/metamorphic provenance. These typical sources are present in a vast region of the Himalayan belt and catchment areas of Ganges. The tectonic setting of the sediments demarcates typically passive margin with slightly continental arc system.

A Computational Investigation on the Control of Cavity-Induced Pressure Oscillations Using Sub-Cavity

A computational investigation has been conducted to determine the effectiveness of a passive control technique of attenuating cavity-induced pressure oscillations in a confined two-dimensional supersonic flow. The passive control technique is achieved by fitting two flat plates near the front wall of a square cavity at Mach number 1.83 at the cavity entrance. The results showed that the flat plates attached near the front wall of the cavity, discouraged the formation of feedback loop which is widely believed to be the reason of cavity resonance. The resultant amount of attenuation of pressure oscillations was also dependent on the length of the flat plate used as an oscillation suppressor.

GIS-Based Analysis to Identify the Distribution and Accessibility of Urban Green Space in Dhaka Metropolitan City, Bangladesh

Urban Green Space (UGS) plays an important role in minimizing the negative effects of urbanization on city dwellers, which is predominantly factored into the accessibility to UGS. This study elucidated the distribution and accessibility of UGS in Dhaka, Bangladesh considering the paucity of such important scientific studies in the given area. The methodological structure of this study employed a multispectral Landsat satellite image from 2020 for identifying the logically defined UGSs, as well as a minimum proxy distance being the parameter to estimate accessibility to the UGS through a primary survey scheme and literature review. Considering UGS as a public area, we adopted a hybrid (combination of supervised and unsupervised classification) method followed by post-classification for UGS distribution assessment. The unsupervised classification identified the overall distribution of green spaces, whereas the anomalies of generated classes were rectified during the post-classification. Following the study findings, UGS in Dhaka metropolitan city accounts for only 602 ha or 1.9% of the total geographical space, with Ramna thana ranked as the highest contributor of 111 ha. However, in terms of accessibility to the UGS by city residents, 19.9% of the buildings in Dhaka metropolitan city were computed to be within the determined proxy distance of 500 m around the UGS. Also, parallel to UGS distribution, green space in Ramna thana (111 ha) exhibited the highest accessibility rate (64%) to the UGS compared with other thanas in Dhaka metropolitan city. The baseline findings will contribute to the long-term sustainable urban planning and development of more accessible green spaces in the study area.

The Effect of Non-Equilibrium Condensation on Supersonic Air Flows over Rectangular Cavities

Numerical simulations have been carded out for a supersonic two-dimensional flow over open, rectangular cavities （length-to-depth ratios are L/D = 1.0 and 3.0） in order to investigate the effect of non-equilibrium condensation of moist air on supersonic internal flows around the cavity for the flow Mach number 1.83 at the cavity entrance. In the present computational investigation, a condensing flow was produced by an expansion of moist air in a Laval nozzle. The computational results showed that the upstream traveling compression waves become weaker than those without the condensation. Consequently, the weaker compression waves cannot excite the shear layer strongly and amplitudes of oscillation in the cavity became smaller than those without the condensation. The occurrence of the non-equilibrium condensation in case of L/D = 1.0 affected strongly the amplitude and frequency of oscillation in the cavity compared with L/D = 3.0.
