Post-Operative Mortality after Ileal Perforation at a Teaching Hospital in Dhaka City

Background: Ileal perforation is a very critical condition. Objectives: The purpose of the present study was to see the post-operative mortality after ileal perforation. Methodology: This prospective cohort study was conducted in the Department of Surgery at Dhaka Medical College Hospital, Dhaka, Bangladesh from September 2000 to December 2002 for period of 2 years and 3 months. All the patients presented with ileal perforation at any age with both sexes were included in this study. Patients were selected consecutively and the patients who showed ileal perforation at laparotomy were included in this study. Preoperative diagnosis was based on detailed history, complete physical examinations supported by plain x-ray abdomen in erect posture including both domes of diaphragm. After immediate resuscitation surgical treatment was undertaken as soon as possible following admission in all cases. The patients were followed up and the mortality profiles were recorded after surgical intervention. Result: Out of 53 patients having postoperative complications 38 patients were survived and 15 patients were expired. So total survivors were 85% and non-survivors 15%. In this study most of the mortality (12%) was attributed to septicemia and mode of death was multiple organ failure. Respiratory complications caused 2% mortality one patient died of ARDS and another elderly patient with preexisting bronchial asthma developed respiratory failure and was unresponsive to treatment. One death was related to faecal fistula followed by severe fluid, electrolyte and acid-base imbalance with peritonitis and gross sepsis. Conclusion: In conclusion, the mortality is significantly high due to septicemia, ARDS and faecal fistula.

Background: CT-scan is a very useful diagnostic tool for the detection of hepatic mass. Objective: The present study was undertaken to determine the CT-scan findings of benign and malignant hepatic mass patients. Methodology: This was a cross sectional study conducted in Radiology and Imaging Department at Mymensingh Medical College Hospital (MMCH), Mymensingh;Dhaka Medical College Hospital (DMCH), Dhaka and Banghabandhu Sheikh Mujib Medical University (BSMMU), Dhaka with the collaboration of Pathology Department of the same institute for histopathological confirmation. This study was carried out from January 2006 to December 2007 for a period of 2 years. The patients who were clinically suspected of having hepatic mass attended in the Radiology and Imaging Department in the above mentioned institutes were included as study population. All the CT-scan findings were recorded. Result: A total number of 50 patients were enrolled for this study. CT-scan was done among 40 males and 10 females with a mean age of 51.28 years old. Hypodensity was found in 17 (60.7%) and 18 (81.8%) cases in malignant and benign hepatic lesions respectively. Ill-defined margin was detected in 12 (42.9%) and 6 (27.3%) cases respectively. Calcification was present on 11 (39.3%) malignant lesion and 6 (27.3%) benign lesions. Pressure effect on biliary apparatus was found in 11 (39.3%) malignant lesions and 1 (4.5%) benign lesions (p < 0.05). Lymphadenopathy was found in 10 (35.7%) malignant lesions and 1 (4.5%) benign lesions (p < 0.05). Conclusion: In conclusion, CT-scan findings of malignant and benign hepatic mass show hypodensity with more contrast enhancement in malignant lesions with more calcification in malignant lesion;however, significant difference is detected in pressure effect on biliary apparatus and lymphadenopathy.

Social media is an essential component of our personal and professional lives. We use it extensively to share various things, including our opinions on daily topics and feelings about different subjects. This sharing of posts provides insights into someone’s current emotions. In artificial intelligence (AI) and deep learning (DL), researchers emphasize opinion mining and analysis of sentiment, particularly on social media platforms such as Twitter (currently known as X), which has a global user base. This research work revolves explicitly around a comparison between two popular approaches: Lexicon-based and Deep learning-based Approaches. To conduct this study, this study has used a Twitter dataset called sentiment140, which contains over 1.5 million data points. The primary focus was the Long Short-Term Memory (LSTM) deep learning sequence model. In the beginning, we used particular techniques to preprocess the data. The dataset is divided into training and test data. We evaluated the performance of our model using the test data. Simultaneously, we have applied the lexicon-based approach to the same test data and recorded the outputs. Finally, we compared the two approaches by creating confusion matrices based on their respective outputs. This allows us to assess their precision, recall, and F1-Score, enabling us to determine which approach yields better accuracy. This research achieved 98% model accuracy for deep learning algorithms and 95% model accuracy for the lexicon-based approach.

Impact of Nitrogen, Phosphorus and Potassium on Brown Planthopper and Tolerance of Its Host Rice Plants

The brown planthopper(BPH),Nilaparvata lugens(St?l),appeared as a devastating pest of rice in Asia. Experiments were conducted to study the effects of three nutrients,nitrogen(N),phosphorus(P) and potassium(K),on BPH and its host rice plants. Biochemical constituents of BPH and rice plants with varying nutrient levels at different growth stages,and changes in relative water content(RWC) of rice plants were determined in the laboratory. Feeding of BPH and the tolerance of rice plants to BPH with different nutrient levels were determined in the nethouse. Concentrations of N and P were found much higher in the BPH body than in its host rice plants,and this elemental mismatch is an inherent constraint on meeting nutritional requirements of BPH. Nitrogen was found as a more limiting element for BPH than other nutrients in rice plants. Application of N fertilizers to the rice plants increased the N concentrations both in rice plants and BPH while application of P and K fertilizers increased their concentrations in plant tissues only but not in BPH. Nitrogen application also increased the level of soluble proteins and decreased silicon content in rice plants,which resulted in increased feeding of BPH with sharp reduction of RWC in rice plants ultimately caused susceptible to the pest. P fertilization increased the concentration of P in rice plant tissues but not changed N,K,Si,free sugar and soluble protein contents,which indicated little importance of P to the feeding of BPH and tolerance of plant against BPH. K fertilization increased K content but reduced N,Si,free sugar and soluble protein contents in the plant tissues which resulted in the minimum reduction of RWC in rice plants after BPH feeding,thereby contributed to higher tolerance of rice plants to brown planthopper.

Introduction:The brown planthopper(BPH),Nilaparvata lugens(Stål)(Hemiptera:Delphacidae),is a notorious insect pest of *** supplementation can alter plant biochemical compositions which may indirectly affect on ecological fitness parameters of its insect ***,few findings are available on nutrient-induced ecological fitness of BPH and relationship of BPH fitness parameters with its host rice plant biochemical ***:We studied the main and interaction effects of nitrogen(N),phosphorus(P),and potassium(K)inputs on BPH fitness,as measured by the following variables:nymph survival,nymphal duration,adult body weight,and its *** planthopper fitness parameters were regressed as function of rice plant(Oryza sativa)biochemical composition.A completely randomized design with four replications in a factorial scheme was used considering N,P,and K levels as ***:Nitrogen application to the host rice plants provided greater survival and enhanced body weight of *** nymphal duration was found to decrease with N subsidy that caused shorter generation time of *** compounds N and soluble protein(SP),total free sugar(TFS),and silicon(Si)content in rice plant tissue correlated strongly with all fitness traits of ***,SP,and TFS associated positively with BPH survival and body weight,while negatively with nymphal *** contrast,Si had negative correlation with BPH survival and body weight,but positive with nymphal *** supplementation to host plant contributed to increase BPH body *** of P with all parameters of BPH were weak except body weight which was highly significant and *** between N and P inputs demonstrated significant effect on BPH body *** of K had no significant effect on BPH survival and *** analysis did not detect significant relationship of BPH fitness parameters with plant tissue content of ***:The res

AIM:To investigate a new modification of pancreaticoduodenectomy(PD)-a mesh-like running suturing of the pancreatic remnant and Braun's ***:Two hundred and three patients underwent PD from 2009 to 2014 and were classified into two groups:Group A(98 patients),who received PD with a mesh-like running suturing for the pancreatic remnant,and Braun's enteroenterostomy; and Group B(105 patients),who received standard *** data,intraoperative findings,postoperative morbidity and perioperative mortality between the two groups were compared by univariate and multivariate ***:Demographic characteristics between Group A and Group B were *** were no significant differences between the two groups concerning perioperative mortality,and operative blood loss,as well as the incidence of the postoperative morbidity,including reoperation,bile leakage,intraabdominal fluid collection or infection,and postoperative *** relevant postoperative pancreatic fistula(POPF) and delayed gastric emptying(DGE) were identified more frequently in Group B than in Group *** A(PD with a mesh-like running suturing of the pancreatic remnant and Braun's enteroenterostomy) was independently associated with decreased clinicallyrelevant POPF and DGE,with an odds ratio of 0.266(95%CI:0.109-0.654,P =0.004) for clinically relevant POPF and 0.073(95%CI:0.010-0.578,P =0.013) for clinically relevant ***:An additional mesh-like running suturing of the pancreatic remnant and Braun's enteroenterostomy during PD decreases the incidence of postoperative complications and is beneficial for patients.

Changes of Nutritional Value of Three Marine Dry Fishes (Johnius dussumieri, Harpodon nehereusand Lepturacanthus savala) during Storage

Dry fish is the low cost dietary protein source in Bangladesh. Sometimes dry fishes are kept for a longer period that is the key factor of the deterioration of nutritional value of dry fishes, as they absorb moisture from the surrounding air. The general purpose of this study is to determine the proximate composition of three available marine dry fishes (Harpodon nehereus, Johnius dussumieri and Lepturacanthus savala) and to investigate the changes of nutritional value of these selected dry fishes with the increasing of storing period. The result of this study shows that the mean percentage of moisture content increased at a significant level with the increasing of storing period. For 2 years storing period the moisture content is increased by 12.77%, 11.89% and 6.69% for the three dry fishes (Harpodon nehereus, Johnius dussumieri and Lepturacanthus savala respectively). The result of this study also shows that the mean percentage of protein (6.35, 7.93 and 4.68), lipid (1.92, 0.67 and 1.13), and carbohydrate (1.70, 1.81 and 0.66) of three dry fishes (Harpodon nehereus, Johnius dussumieri and Lepturacanthus savala respectively) decreased greatly for 2 years storing period. The findings of this study showed that nutritional value of dry fishes deteriorate with the increasing of storage period.
