Diagnosis and Management of Childhood Tuberculosis: A Cross-Sectional Study of Practices in Gilgit Baltistan, Pakistan

Background: Tuberculosis (TB) is one of the major killer diseases among infectious diseases. The success of TB control depends on the capability of the health care system to detect and accurately manage TB cases. Tuberculosis in children remained a low public health priority with limited epidemiologic studies. Struggles for TB control in children need to be enhanced as children are providing the reservoir for future cases to develop. Objectives: The study evaluated diagnostic and treatment practices related to childhood pulmonary tuberculosis in Gilgit Baltistan (GB), Pakistan. Methods: A descriptive, cross-sectional study based on retrospective record review of childhood Pulmonary Tuberculosis patients registered in the year 2020 with self-administered questionnaire. Results: Data of 557 childhood cases were collected. Most childhood cases were in age group 1 - 4 years (54%) with male predominance. More than 90% were diagnosed and treated at public sector facilities. 99% of the cases were clinically diagnosed with passive case finding. Cough was considered as a symptom of childhood Tuberculosis (TB) by 94% of physicians. Other important features included failure to thrive (13%), contact with a family history of TB (15%), Malnutrition(24%) and respiratory signs (21%). 99% physicians advised chest X-ray, Complete blood count (CBC) (95%) and Erythrocytes sedimentation rate (ESR) (72%) for diagnosis and fewer physicians (2%) used sputum smear microscopy and induced sputum (0.1%). Combining data on dosage, frequency and duration for drugs, 99% of the cases were found receiving correct regimen. The treatment outcomes of the cases were cured 4 (0.8%), treatment completed 551 (99.3%) and died 2 (0.4%) with no lost to follow up. Conclusions: The study highlights inappropriate diagnostic and treatment practices for managing childhood pulmonary TB among physicians in public and private sectors of Gilgit Baltistan. The problem of Tuberculosis (TB) and Human Immune Deficiency Virus (HIV) co-infection becomes vital when it is seen in the context of under developed countries like Pakistan. Pakistan ranks 5th high burden countries for drug-susceptible and 6th among drug-resistant TB patients [1]. Objectives of the study were to assess the prevalence of TB-HIV Co-infection at the designated Sentinel Sites across Pakistan. A cross-sectional study is based on retrospective record review of routinely maintained TB program data at all 17 designated sentinel sites of TB Control Program from 2013-15. Among the screened TB patients 145 (0.66%) were found HIV reactive. The prevalence of HIV was higher (1.02%) in extra-pulmonary and male TB patients (1.23 %) as compared to pulmonary (0.55%) and female patients (0.09%). Scale up TB surveillance activities, integrating TB-HIV care services, active case finding among key affected populations will have a positive impact on TB-HIV co-infection and disease control.

Does Cash Incentive Effect TB Case Notification by Public Private Mix-General Practitioners Model in Pakistan?

Setting: Private healthcare providers including solo practitioners and laboratories enrolled with Public Private Mix General Practitioners model in Pakistan who were enrolled with the program and maintained participation during 2014 to 2016. Objective: To assess the change in case notification by private healthcare providers as well as effect on sputum smear positivity rate at private laboratories after introduction of incentives. Design: A comparative cross-sectional study based on routinely maintained National TB Control Program data collected before and after the introduction of incentives (2014 and 2016). Results: In 2016, TB case notification increased by 71% after cash incentives was introduced. Among the 618 private healthcare providers who enrolled and maintained participation in the Public Private Mix program, only 1% of the GPs contributed to 19% increase in case notification, whereas 27% GPs had reported a reduced number of cases compared to the pre-intervention period. After incentives, slide positivity rate for diagnostic declined by 1.9% and follow-up increased by 0.6%. Number of slides for diagnostic and follow-up testing increased by 82% and 72% respectively. Conclusion: Engaging the private healthcare sector, by providing cash incentives to private health care Provider’s is an effective strategy for providing a substantial increase in National TB case notification.

To improve childhood Tuberculosis management, the National Tuberculosis Program implemented a package of interventions Hospital DOTS linkages (HDL) in 2015 in 144 tertiary and secondary care hospitals across Pakistan. This included systematic engagement of hospital administration and all specialist doctors, staff training and regular facility-based review meetings. HDL was associated with 35% increase in childhood TB notifications in 2015 (versus 2014) in HDL sites as compared to 16% increase in non-HDL sites. The increase was seen across provinces, age-groups and sexes, but did not correlate with presence of Xpert MTB/RIF®?or “screeners” (health workers deployed to screen children for TB).

Software project outcomes heavily depend on natural language requirements,often causing diverse interpretations and issues like ambiguities and incomplete or faulty requirements.Researchers are exploring machine learning to predict software bugs,but a more precise and general approach is needed.Accurate bug prediction is crucial for software evolution and user training,prompting an investigation into deep and ensemble learning methods.However,these studies are not generalized and efficient when extended to other datasets.Therefore,this paper proposed a hybrid approach combining multiple techniques to explore their effectiveness on bug identification problems.The methods involved feature selection,which is used to reduce the dimensionality and redundancy of features and select only the relevant ones;transfer learning is used to train and test the model on different datasets to analyze how much of the learning is passed to other datasets,and ensemble method is utilized to explore the increase in performance upon combining multiple classifiers in a model.Four National Aeronautics and Space Administration(NASA)and four Promise datasets are used in the study,showing an increase in the model’s performance by providing better Area Under the Receiver Operating Characteristic Curve(AUC-ROC)values when different classifiers were combined.It reveals that using an amalgam of techniques such as those used in this study,feature selection,transfer learning,and ensemble methods prove helpful in optimizing the software bug prediction models and providing high-performing,useful end mode.

Impact of numerous larval diets on the biology of pink bollworm Pectinophora gossypiella(Saunders)(Lepidoptera:Gelechiidae)under laboratory conditions

Background Pink bollworm,Pectinophora gossypiella(Saunders)(Lepidoptera:Gelechiidae)has become a poten-tial pest of cotton by causing substantial yield losses around the world including Pakistan.Keeping in view the facts like limited research investigations,unavailability,and high cost of artificial diet’s constituents and their premixes,the present research investigations on the dietary aspect of P.gossypiella were conducted.The larvae of P.gossypiella were reared on different diets that were prepared using indigenous elements.The standard/laboratory diet com-prised of wheat germ meal 34.5 g,casein 30.0 g,agar–agar 20.0 g,sucrose 10.0 g,brewer’s yeast 5.0 g,α-cellulose 1.0 g,potassium-sorbate1.5 g,niplagin 0.5 g,decavitamin 0.01 g,choline-chloride 0.06 g,maize-oil 3.30 g,honey 2.0 g,and water 730.0 mL.Alternatives to cotton bolls and wheat germ meal were okra seed sprouts,okra fruit,cottonseed meal,and okra seed meals,which were included in the study to introduce an efficient and economic mass-rearing system.Results The larval development completed in 19.68d±0.05 d with a weight of 20.18mg±0.20 mg at the fourth instar fed on the cottonseed meal-based diet instead of wheat germ meal based diet.On the same diet,84.00%±4.00%,17.24 mg±0.03 mg,and 7.76d±0.06 d were recorded as pupae formation,pupal weight,and pupal duration,respectively.Adult emergence,76.00%±1.00%was recorded from pupae collected from larvae raised on cottonseed meal-based diet.These male and female moths lived for 40.25d±0.10 d,and 44.34d±0.11 d,respectively.Females deposited 21.28±0.04 eggs per day with the viability of 65.78%±0.14%.The larval mortal-ity at the fourth instar was 37.20%±1.36%and malformed pupation of 12.00%±1.41%was recorded.Replacement of wheat germ meal with that of local meals(cottonseed and okra seed)in the standard laboratory diet has saved 463.80 to 467.10 PKR with 1.62 to 1.63 cost economic returns,respectively.Conclusion This research is of novel nature as it provides a concise and workable system for the economic and suc-c

Face recognition (FR) technology has numerous applications in artificial intelligence including biometrics, security,authentication, law enforcement, and surveillance. Deep learning (DL) models, notably convolutional neuralnetworks (CNNs), have shown promising results in the field of FR. However CNNs are easily fooled since theydo not encode position and orientation correlations between features. Hinton et al. envisioned Capsule Networksas a more robust design capable of retaining pose information and spatial correlations to recognize objects morelike the brain does. Lower-level capsules hold 8-dimensional vectors of attributes like position, hue, texture, andso on, which are routed to higher-level capsules via a new routing by agreement algorithm. This provides capsulenetworks with viewpoint invariance, which has previously evaded CNNs. This research presents a FR model basedon capsule networks that was tested using the LFW dataset, COMSATS face dataset, and own acquired photos usingcameras measuring 128 × 128 pixels, 40 × 40 pixels, and 30 × 30 pixels. The trained model outperforms state-ofthe-art algorithms, achieving 95.82% test accuracy and performing well on unseen faces that have been blurred orrotated. Additionally, the suggested model outperformed the recently released approaches on the COMSATS facedataset, achieving a high accuracy of 92.47%. Based on the results of this research as well as previous results, capsulenetworks perform better than deeper CNNs on unobserved altered data because of their special equivarianceproperties.

Transcription factor CabHLH035 promotes cold resistance and homeostasis of reactive oxygen species in pepper

Plant basic helix-loop-helix(bHLH)transcription factors(TFs)play central roles in various abiotic stresses.However,its role in plant cold resistance is largely unknown.Previously,we characterised CaNAC035 in pepper,which positively regulates tolerance to cold,salt and drought stresses tolerance.Here,we identified CabHLH035,a CaNAC035-interacting protein in pepper.To explore its functions in cold stress tolerance,we silenced the gene in pepper via virus-induced gene silencing(VIGS)and overexpressed the gene in Arabidopsis.The results showed that CabHLH035 expression was induced by cold treatment,and silencing of CabHLH035 decreased cold stress tolerance.Conversely,overexpression of CabHLH035 in Arabidopsis increased cold stress tolerance.To investigate homologs genes of C-repeat binding factor(CBF)pathway proteins and reactive oxygen species(ROS)marker gene expression blocking by CabHLH035,we performed yeast one-hybrid(Y1H),dual luciferase and electrophoretic mobility shift assay experiments.The results showed that CabHLH035 bound to the region upstream of the CaCBF1A and CaAPX promoters.Additionally,CaCBF1A bound to the CaDHN4 promoter.Taken together,our results showed that CabHLH035 plays a crucial role in cold stress tolerance and its potential as a target for breeding cold-resistant crops.The findings provide a basis for studying the functions and regulatory network of cold stress tolerance in pepper.

Deep Learning Based Efficient Crowd Counting System

Estimation of crowd count is becoming crucial nowadays,as it can help in security surveillance,crowd monitoring,and management for different events.It is challenging to determine the approximate crowd size from an image of the crowd’s density.Therefore in this research study,we proposed a multi-headed convolutional neural network architecture-based model for crowd counting,where we divided our proposed model into two main components:(i)the convolutional neural network,which extracts the feature across the whole image that is given to it as an input,and(ii)the multi-headed layers,which make it easier to evaluate density maps to estimate the number of people in the input image and determine their number in the crowd.We employed the available public benchmark crowd-counting datasets UCF CC 50 and ShanghaiTech parts A and B for model training and testing to validate the model’s performance.To analyze the results,we used two metrics Mean Absolute Error(MAE)and Mean Square Error(MSE),and compared the results of the proposed systems with the state-of-art models of crowd counting.The results show the superiority of the proposed system.

BACKGROUND In severe cases of coronary artery disease,percutaneous coronary intervention provide promising results.The stent used could be a drug-eluting stent(DES)or a titanium-nitride-oxide coated stent(TiNOS).AIM To compare the 5-year effectiveness and safety of the two stent types.METHODS The following systematic review and meta-analysis was conducted in accordance with the preferred reporting items for systematic reviews and meta-analysis guidelines,and PubMed/MEDLINE,Scopus,and Cochrane Central were searched from inception till August 2023.Primary outcomes were major adverse cardiac events(MACE),cardiac death,myocardial infarction(MI),cardiac death or MI,and ischemia-driven total lesion revascularization(ID-TLR).RESulTS Four randomized controlled trials(RCT),which analyzed a sum total of 3045 patients with acute coronary syndrome(ACS)after a median follow-up time of 5 years were included.Though statistically insignificant,an increase in the ID-TLR was observed in patients receiving TiNOSs vs DESs.In addition,MI,cardiac death and MI,and definite stent thrombosis(DST)were significantly decreased in the TiNOS arm.Baseline analysis revealed no significant results with meta-regression presenting non-ST elevated MI(NSTEMI)as a statistically significant covariate in the outcome of MACE.CONCLUSION TiNOS was found to be superior to DES in terms of MI,cardiac death or MI,and DST outcomes,however,the effect of the two stent types on ID-TLR and MACE was not significant.A greater number of studies are required to establish an accurate comparison of patient outcomes in TiNOS and DES.
