This.article analys.s.numerically the s.multaneous.influence of the compres.ion rate, fuel nature and the advanced injection of fuel on maximum cylinder pres.ure during the combus.ion phras. with the help of the Python s.yder calculation code. Indeed, s.veral authors.have s.own that the combus.ion of biofuels.which make it pos.ible to compens.te for fos.il and exhaus.ible res.urces. pres.nts.a cylinder pres.ure higher by about 3.5% compared to that of conventional dies.l D100. This.increas. in pres.ure can be reduced by the means.of controlling parameters.making it pos.ible to pres.rve the life of the engine and als. reduce nitrogen oxides.(NOx) and particular matter (PM). This.article has.two objectives.which are: putting in place a numerical tool for the evaluation and s.mulation of thermal engines.and the influence of control parameters.on cylinder pres.ure. The s.ngle zone 0D combus.ion model which cons.ders.only the phys.cal phenomena and cons.ders.the mixed fuel as.a perfect gas.is.us.d. The fuel us.d is.the Neem biofuel produced by Doctor Merlin Ayis.i of the Univers.ty of Douala and the D100 dies.l fuel. The res.lts.are obtained from three fuel injection angles.of 20°, 13° and 10° before the TDC (Top Dead Centre) and three values.of the engine compres.ion rates.of 15, 20 and 25. The delay in combus.ion is.characteris.ic of the fuel us.d as.illus.rated by the cetane number. The res.lts.s.ow that the cylinder pres.ure increas.s.with increas.ng compres.ion rate and a very high advanced injection. It als. s.ows.that the pres.ure is.high when dies.l D100 is.us.d ins.ead of D100 biodies.l.

Background: Data on emergency diges.ive oncology s.rgery are limited in Cameroon. The aim of this.work was.to give the s.ort-term res.lts.of emergency diges.ive carcinological s.rgery in our context. Patients.and Methods. We conducted a des.riptive and analytical obs.rvational s.udy with retros.ective data collection in four reference hos.itals.in the city of Yaoundé. Files.of patients.who had emergency diges.ive oncological s.rgery, for an acute complication, from January 1, 2016 to December 31, 2020, were included. The outcomes.of the patients.in the 30 days.following the s.rgery had to be known. Res.lts. We collected 41 patients. repres.nting 20% of the diges.ive oncological s.rgery activity. Their average age was.51.76 ± 16.59 years.with a male predominance (63.4%). The cancer complication was.inaugural in 27 patients. The main tumor s.tes.were colic (56.1%), rectal (19.5%), and gas.ric (9.7%). The indications.for s.rgery were: acute bowel obs.ruction (60.9%), acute generalized peritonitis.(29.3%), and gas.rointes.inal bleeding (4.9%). The tumor was.diagnos.d intraoperatively in 10 patients.(24.4%). The main operative procedures.were left colectomy (21.9%) and Hartmann’s.intervention (19.5%). The morbidity and mortality rates.were 60.9% and 43.9%, res.ectively. Preoperative anemia (p = 0.019), peritonitis.as.indication for s.rgery (p = 0.039) and TNM s.age 4 (p = 0.015) were identified as.as.ociated with an increas.d ris. of death. Conclus.on: In our context, one-fifth of diges.ive oncological s.rgery is.done urgently in front of an acute complication which is.inaugural for cancer in nearly two-thirds.of patients. Pos.operative morbidity and mortality are s.gnificant.

Quality of Life Following Major Limb Amputations.in a Rural Community in Cameroon

Background: Limb amputation is.cons.dered the las. res.rt when the limb is.no longer s.lvageable or when the limb is.dead or dying, viable but nonfunctional or endangering the patient’s.life. It is.as.ociated with profound economic, s.cial, and ps.chological effects.on the patients. The aim of this.s.udy is.to evaluate the quality of life of major limb amputees.in a rural s.tting in wes.ern Cameroon. Methods. This.was.a cros.-s.ctional des.riptive and analytical s.udy carried out at the BATs.NGLA-Ds.HANG community in the Wes. Region of Cameroon. Participants.were interviewed and data collected us.ng a pre-defined accredited ques.ionnaire of the WHOQOL-BREF to as.es. the quality of life. Res.lts. There were 63 participants. and a majority (60.32%) reported trauma as.the caus. of amputation. Participants.with pros.hes.s.had a better quality of life. Conclus.on: The age range of the s.udy participants.was.18 to 85 years.with a mean of 46.73 ± 18.31 years. The majority were males.(74.6%). Mos. of them (41.27%) had attained at leas. a s.condary level of education, a majority (80.95%) were unemployed and more than half (55.56%) have les. than the guaranteed inter-profes.ional minimum wage. Major limb amputations.were mos.ly due to traumatic caus.s.(72%) and involved the lower limbs. Only a few (12.70%) us.d pros.hes.s. Almos. all of them (90.48%) had s.mptoms.cons.s.ent with a phantom limb. The quality of life after major limb amputation in this.s.udy was.generally fair according to the WHO quality of life tool.

Introduction: Hyperuricemia is.defined as.a level of s.rum uric acid greater than or equal to 70 mg/l (420 μmol/l) in men and 60 mg/l (360 μmol/l) in women. s.veral s.udies.have s.own that it is.a ris. factor or a factor of progres.ion of chronic kidney dis.as.. Recent experimental and epidemiological data correlate the as.ociation of hyperuricemia with chronic kidney dis.as. (CKD), arterial hypertens.on and cardiovas.ular dis.as.s. thus.rais.ng the ques.ion of the us.fulnes. of therapeutics.in the prevention of renal dis.as.s. The objective of this.s.udy is.to s.ek a link between chronic kidney dis.as. and hyperuricemia. Materials.and Methods. This.is.a des.riptive and analytical s.udy conducted at hemodialys.s.unit and cardiology s.rvice of General Hos.ital of National reference of N’Djamena (Chad) from 1th January to 1th October 2013 (10 months.. We included all chronic kidney dis.as. patients.hos.italized in hemodialys.s.unit and cardiology s.rvice who pres.nted as.ociated hyperuricemia. Res.lts. There were 712 CKD patients.who were hos.italized. Among them, there were 108 patients.who were included in the s.udy and who had hyperuricemia as.a prevalence of 15.20%. The average age of patients.was.35.5 years.and the s.x ratio was.3/1. The age group between 40 to 60 years.repres.nted 54.6%. There were 41.7% of traders. Hypertens.ve patients.accounted for 49.1%;as.ociation of diabetes.and hypertens.on was.noted in 12.90%. Renal ins.fficiency was.moderate in 43.5% of patients. Hyperuricemia was.pres.nt in more than 90% of patients. Profes.ion, age, hematuria, proteinuria and hypertens.on were s.atis.ically pos.tively related to hyperuricemia. Treatment cons.s.ed of pres.ribing allopurinol in 84% of patients. In more than 11% of patients.the progres.ion was.unfavorable.
