Value of duplex doppler ultrasonography in non-invasive assessment of children with chronic liver disease

AIM:To investigate the value of duplex Doppler ultrasonography (US) in the assessment of the hemodynamics of the portal and hepatic veins in a cohort of children with chronic liver disease (CLD) and to detect any relationship between the US changes,etiology and severity (or stage) of CLD. METHODS:We prospectively enrolled 25 children with biopsy-proven CLD. Thirteen had cirrhosis (aged 8.9 ± 2.0 years) and 12 had chronic hepatitis (aged 9.3 ± 2.3 years). Gray scale and color-coded duplex Doppler US were performed for all,as well as 30 healthy age and sex-matched controls. Findings were correlated with clinical,laboratory and histopathological characteristics. RESULTS:Prominent caudate lobe was detected in 100% of cirrhotics,but none of the chronic hepatitis or controls. Thickened lesser omentum and loss of the triphasic waveform of the hepatic vein were present in 69.2% and 53.8% of cirrhotics vs 33.3% and 8.3% of chronic hepatitis respectively. Portal vein flow velocity was significantly lower (P elation with both abnormal hepatic vein waveform and direction of portal blood flow; and a negative correlation with both hepatic and portal vein flow velocities. No correlation with the etiology of CLD could be detected. CONCLUSION:Duplex Doppler added to grayscale US can detect significant morphologic and portal hemodynamic changes that correlate with the severity (stage) of CLD,but not with etiology.

AIM： We designed two synthetic-core-specific peptides core 1 （C1） and core 2 （C2）, and an E1-specific peptide （el）. We produced specific polyclonal antibodies against these peptides and used the antibodies for detection of HCV antigens on surface and within infected peripheral blood leukocytes. METHODS： Peripheral blood from a healthy individual who tested negative for HCV RNA was incubated with HCV type 4 infected serum for i h and 24 h at 37 ℃. Cells were stained by direct and indirect immunofluorescence and measured by flow cytometry. RESULTS： After 1 h of incubation, antibodies against C1, C2, and el detected HCV antigens on the surface of 27%, 26% and 73% of monocytes respectively, while 10%, 5% and 9% of lymphocytes were positive with anti-C1, anti-C2 and anti-E1 respectively. Only 1-3% of granulocytes showed positive staining with anti-C1, anti-C2 and anti E1 antibodies. After 24 h of incubation, we found no surface staining with anti-C1, anti-C2 or anti-E1. Direct immunostaining using anti-C2 could not detect intracellular HCV antigens, after 1 h of incubation with the virus, while after 24 h of incubation, 28% of infected cells showed positive staining. Only plus strand RNA was detectable intracellularly as early as 1 h after incubation, and remained detectable throughout 48 h post-infection. Interestingly, minus RNA strand could not be detected after 1 h, but became strongly detectable intracellularly after 24 h post-infection. CONCLUSION： Monocytes and lymphocytes are the preferred target cells for HCV infection in peripheral blood leukocytes. Our specific anti-core and anti-E1 antibodies are valuable reagents for demonstration of HCV cell cycle. Also, HCV is capable of infecting and replicating in peripheral blood mononuclear cells as confirmed by detection of minus strand HCV RNA as well as intracellular staining of core HCV antigen.

AIM： To observe the imbalance between T helper cell Th1 and Th2 cytokines in several chronic hepatitis disease at different stages of disease progression. METHODS： We measured the cytokine levels of Thl （IL-2 and IL-2R）, Th2 （IL-10） and the pro-inflammatory cytokines （IL-6 and IL-6R and TNF and TNF-RI and II） by the elISA technique in the sera of 33 hepatocellular carcinoma （HCC） patients and 20 chronic liver disease （CLD） patients. In addition, 20 asymptomatic hepatitis C virus carriers and 20 healthy subjects negative for hepatitis C virus（HCV） markers served as controls. RESULTS： Anti-HCV antibodies were found to be positive in 94% of HCC cases and 75% of CLD cases. On the other hand, HCV viremia was detected using RT- PCR in 67% of HCC cases and 65% of CLD cases. HBsAg was positive in 9% of HCC cases and 30% of CLD cases. Also bilharziaI-Ab was positive in 55% of HCC cases, 65% of CLD cases and in 70% of asymptomatic carriers （ASC）. HCC patients had significantly higher values of IL- 2R, TNF-RII （P〈0.001）, and TNF-RI （P〉0.05）, but lower TNFα （P〈0.001） and IL-6 （P = 0.032） in comparison to ASC. But, in comparison to non-cancer controls, HCC patients had higher values of IL-2R, IL-6R, TNF-RI and TNF-RII, but lower TNF-α （P〈0.001）. CLD patients had higher IL-2R, TNF-RI, and TNF-RII （P〈0.001） than ASC. But, in comparison to non-cancer controls, CLD patients had higher values of IL-2R, TNF-RI and TNF-RII, but lower TNF-α （P〈0.001）. IL-10 was higher （though not significantly） in HCC and CLD patients than in symptomatic carriers and non-cancer controls. CONCLUSION： Liver disease progression from CLD to HCC due to HCV genotype-4 infection is associated with an imbalance between Th1 and Th2 cytokines. IL-2R, TNF-RI, and TNF-RII could be used as potential markers.

BACKGROUND Hepatitis C virus(HCV)infection and its consequent complications are undeniably a public health burden worldwide,particularly in *** evidence suggests that many lncRNAs have relevant roles in viral infections and antiviral *** To investigate the expression profiles of circulating lncRNAGAS5,lncRNAHEIH,lncRNABISPR and mRNABST2 in naïve,treated and relapsed HCV Egyptian patients,to elucidate relation to HCV infection and their efficacy as innovative biomarkers for the diagnosis and prognosis of HCV *** One hundred and thirty HCV-infected Egyptian patients and 20 healthy controls were included in this *** lncRNAs and mRNABST2 were measured using quantitative real-time polymerase chain reaction(qRT-PCR).RESULTS Our results indicated that serum lncRNAGAS5 and LncRNABISPR were upregulated,whereas mRNA BST2 and LncRNA HEIH were downregulated in naïve *** contrast,HCV patients treated with sofosbuvir and simeprevir;with sofosbuvir and daclatasvir;or with sofosbuvir,daclatasvir and ribavirin exhibited lower levels of lncRNAGAS5 and lncRNABISPR with higher mRNABST2 compared to naïve ***,patients relapsed from sofosbuvir and simeprevir showed higher levels of these lncRNAs with lower mRNABST2 compared to treated ***5 and lncRNABISPR were positively correlated with viral load and ALT at Pely correlated with viral load at Pelation between lncRNA HEIH and AFP was observed at P<*** Differential expression of these RNAs suggests their involvement in HCV pathogenesis or antiviral response and highlights their promising roles in diagnosis and prognosis of HCV.

Introduction:la qualitéde vie des patients suivis pour une hémopathie maligne dépend non seulement du type de la maladie,de ses caractéristiques cliniques,biologiques et thérapeutiques,mais aussi de la santémentale du patient.L’objectif de notreétude est d’évaluer l’état psychologique des patients au diagnostic,de rechercher les facteurs associésàson altération et de préciser son impact sur la qualitéde vie des patients.Méthode:nous avons menéuneétude monocentrique transversale quiévalue la dépression,l’anxiétéet la qualitéde vie d’une population de patients atteints de syndromes lymphoprolifératifs sur une période de *** questionnaires ontétéutilisés au moment du diagnostic:le HADS pour rechercher la dépression et l’anxiétéet le SF12échelle pourévaluer la qualitéde vie liéeàla santé.Résultat:Les cent patients ont acceptéde répondre aux *** patients sont suivis pour un lymphome malin non hodgkinien,une maladie de Hodgkin,et une leucémie lymphoïde *** sex-ratio(H/F)est de 1,*** prévalence des symptômes anxieux est de 70%avec 95%CI(61,01%;78,99%)et des symptômes dépressifs de 76%avec 95%CI(49,4%;82,6%)des *** analyse univariée,le caractère féminin est associéaux symptômes anxieux(p=0,008),etàl’altération de qualitéde vie mentale(p=0,02).Alors qu’en analyse multivariée,Les symptômes anxio-dépressifs sont significativement associésàun score de qualitéde vie faible aussi bien mentale que *** plus les symptômes anxieux sont corrélés au sexe féminin(ORA=20,50;95%CI[1,58–264,48]);et la douleur(ORA=12,33;95%CI[1,07–141,77]).Conclusion:la recherche de comorbidités psychologiques doit faire partie du diagnostic initial de cette population vulnérable pour une meilleure qualitéde vie.

AIM： TO establish a cell culture system with longterm replication of hepatitis C virus （HCV） genome and expression of viral antigens in vitro. METHODS： HepG2 cell line was tested for its susceptibility to HCV by incubation with a serum from a patient with chronic hepatitis C. Cells and supernatant were harvested at various time points during the culture. Culture supernatant was tested for its ability to infect na＇ive cells. The presence of minus （antisense） RNA strand, and the detection of core and E1 antigens in cells were examined by RT-PCR and immunological techniques （flow cytometry and Western blot） respectively. RESULTS： The intracellular HCV RNA was first detected on d 3 after infection and then could be consistently detected in both cells and supernatant over a period of at least three months. The fresh cells could be infected with supernatant from cultured infected cells. Flow cytometric analysis showed surface and intracellular HCV antigen expression using in house made polyclonal antibodies （anti-core, and anti-E1）. Western blot analysis showed the expression of a cluster of immunogenic peptides at molecular weights extended between 31 and 45 kDa in an one month old culture of infected cells whereas this cluster was undetectable in uninfected HepG2 cells. CONCLUSION： HepG2 cell line is not only susceptible to HCV infection but also supports its replication in vitro. Expression of HCV structural proteins can be detected in infected HepG2 cells. These cells are also capable of shedding viral particles into culture media which in turn become infectious to uninfected cells.

A new simple, sensitive and precise green analytical procedure using an automated packed-reactor derivatization technique coupled with on-line solid-phase enrichment （SPEn） has been developed and evaluated to determine trace levels of methotrexate （MTX）. The method was based on injection of MTX into a flowing stream of phosphate buffer （0.04 M, ptt 3.4）, carried through the packed oxidant reactor of Cerium （IV） trihydroxyhydroperoxide for oxidative cleavage of the drug into highly fluorescent product, 2,4-diaminopteridine- 6-carboxylic acid, followed by SPEn on a head of short ODS column （10mm x 4.6 mm i.d., 5 I＋tm particle size）. The flow rate was 0.25 mL/min and packed reactor temperaturc was 40 ~C. The trapped product was back-flush eluted from the ODS column to the detector by column-switching with an environmentally friendly mobile phase consisting of ethanol and phosphate buffer （0.04 M, pH 3.4） in the ratio of 5：95 （v/v）. The eluent was monitored at emission and excitation wavelengths of 460 and was linear over the concentration range of 1.25-50 360 nm, respectively. The calibration curve ng/mL with a detection limit of 0.08 ug/ml..The method was successfully applied to determine MTX in pharmaceutical formulations with mean percentage recovery ranging from 99.48 to 99.60.

Melatonin and schistosomal antigens ameliorate the anti-oxidative and biochemical response to Schistosoma mansoni infection in hamster

The present study was designed to investigate the potential protective effect of melatonin as an antioxidant separately or in combination with antigens （cerearial; CAP or soluble worm; SWAP） against Schistosoma mansoni infection in hamsters. Each hamster was sensitized with an initial immunization of 0.6 ml of the extracted antigen （30μg protein/mL）. After four days, a second injection of 0.4 mL was given （20μg protein/mL）. Then, each hamster was exposed to 260 ± 20 S. mansoni cercariae followed with melatonin treatment （3.5 mg/kg） for thirty days from the 1st day of post infection. Levels of lipid peroxidation （LPO） products, eatalase （CAT） activity, hepatic glutathione （GSH） and biochemical changes in the liver and kidneys functions were investigated. The results revealed a high significant increasing of LPO and decreasing of CAT and GSH in liver of infected hamsters. Biochemical observations showed severe damage in the liver enzyme activities and increasing cholesterol level in infected animals. Melatonin co-treatment with antigen to the infeeted-hamster attenuated the increase of LPO and restored the activity of CAT and levels of hepatic GSH. Also, the biochemical damages in the liver and kidneys funetions were reduced. The present study suggests that melatonin may be useful in combating free radical-induced damage due to infection toxicity. The immunization with previous antigens resulted in a remarkable improvement on the liver enzyme activities, which were increased after infection. Thus, vaccination of hamsters with antigens （both CAP and SWAP） and melatonin treatment has more potent effect on the enhancement of antioxidant and biochemical of S. mansoni infected-hamster than each treatment separately. Immunization of the hamster with SWAP followed by melatonin was the best way among the other regime treatments to improve the biochemical and antioxidant parameters of the infected-hamsters.
