Is Primary Repair of Tracheobronchial Rupture Curative?

Background: Tracheobronchial disruption as a result of blunt thoracic trauma is a rare entity and only clinically serious lesions come to our notice, which can be life-threatening and need prompt recognition and treatment. Objectives: To review the authors’ experience with tracheobronchial injuries to emphasize the need for prompt diagnosis and treatment to avoid lethal complications including severe hypoxic organ failure, sepsis, mediastinitis and bronchopleural fistula. Patients and methods: A retrospective study of total 32 patients with tracheobronchial injury from 2001 to 2011. This study limited to patients with thoracic tracheal or bronchial injury, excluding those with cervical injuries. The study includes collected information about mechanism of injury, presentation, time until diagnosis and treatment, anatomical site of injury, type of treatment, diagnostic methods, duration of follow up and outcome. Results: Twenty-four patients were male (75%) and eight were females (25%). Patient’s ages ranged from 7 - 53 years. Majority of cases was referred because of blunt trauma in 15 cases (46.8%), 6 (18.75) motor vehicle accident, 5 (15.6%) fall from a height and 4 (12.5%) with trauma by heavy object, while 8 cases (25%) were referred due to penetrating injury and 2 cases (6.25%) due to iatrogenic injury. In initially diagnosed group, the predominant clinical signs that give a suspicion of tracheobronchial disruption were increased subcutaneous surgical emphysema, shortness of breath, hemoptysis. After the admission to emergency unit, all of them were examined radiologically by chest X-ray film. Longitudinal tear of right upper lobe bronchus was found in 8 cases (32%), complete cut of right upper lobe bronchus in 4 cases (16%), tear of right intermediate bronchus in 4 cases (16%), 3 cases with clear cut left upper lobe (12%), longitudinal tear of distal lateral tracheal wall extend to right upper lobe in 2 cases (8%), 2 cases (8%) showed complex disruption of dist

Preoperative serum vascular endothelial growth factor correlated to three dimensional power Doppler indices in ovarian masses

Objective: To assess the preoperative serum levels of vascular endothelial growth factor (VEGF) in ovarian masses compared to normal controls and to find a correlation between VEGF and Doppler indices in ovarian masses. Methods: The study was conducted from December 2009 to September 2012 in the oncology and ultrasound units in department of obstetrics and gynecology, Ain Shams University on 150 patients with ovarian masses. During preoperative workup, serum Ca-125 levels and serum VEGF levels were obtained. Ultrasonographic examination included two and three dimensional power Doppler ultrasound (3DPD). Laparotomic approaches were undertaken to obtain the final pathologic results. VEGF was measured in one hundred normal cases as controls. Results: Final ovarian pathology revealed seventy three malignant ovarian masses and seventy seven were benign. Serum Ca-125 levels in malignant cases were higher compared to those in benign cases (p < 0.001). Preoperative serum VEGF revealed higher levels in malignant ovarian masses than benign conditions and normal controls (p < 0.001). Three dimensional power Doppler indices, vascularization index (VI), flow index (FI) and vascularization flow index (VFI), correlated positively with serum VEGF levels. Conclusion: Preoperative serum VEGF revealed higher levels in malignant ovarian masses than benign conditions and normal controls. These levels positively correlated with the Doppler vascular indices of the masses.

Beef Meat Allergy in Cow’s Milk Allergic Adults

Milk has been recognized as a leading cause of food allergy in children;however, studies on cow’s milk allergy (CMA) in adults are scanty. The known cross-reactivity of bovine serum albumin and bovine γ-globulin which are present in both milk and beef is recognized, so that, patients with allergy to cow’s milk are often instructed to avoid beef. Our Objective was to determine the prevalence of allergy to beef meat in adult patients with allergy to cow’s milk. Thirty adults with CMA were included in the study. The diagnosis of cow’s milk and beef allergy was based on a thorough history concerning type of allergic symptoms and its link to the ingestion of milk and beef supported by skin prick test (SPT) with fresh milk and raw beef extracts and by determination of their serum specific IgE by enzyme linked immuosorbant assay (ELISA). Finally, cross reactivity test between milk and meat was performed. Three of 30 patients (10%) evaluated for CMA were found to have symptomatic sensitivity to beef, 2 gave urticarial symptoms and one gave gastrointestinal symptoms. Six patients (20%) had a positive SPT response to beef. On the other hand, beef specific IgE were positive in 18 CMA patients (60%). Concordance of positive specific IgE and SPT to meat was found in only 5 patients (17%). Mean cross reactivity between milk and beef was 31.6 ± 13.1% among studied patients. Patients allergic to milk protein are not necessarily to be allergic to beef, so, elimination of beef from the diet of adults with CMA should not be done except after investigations so as not to lose its nutritional value without benefit.

Extraction Methods of Cyanotoxins Aqueous Media and Sediments

Cyanotoxins are chemical compounds produced by cyanobacterial mats grown in aquatic ecosystems. These may threaten human health and aquatic organisms. Extraction of these toxins is usually associated with many difficulties due to their concentration in aquatic ecosystems. This study is designed to provide suitable and effective extraction procedures that can effectively extract low concentration cyanotoxin from water and bacterial cells. The methodology is based on collecting raw material of cyanobacterial mats from naturally growing sites such as Wadi Gaza along with 16 liters of aquatic surrounding media. The materials were left in the Lab for 24 - 48 h for stabilization of the mats. The floating mats were collected using special funnel and allowed to air drying. The aqueous phase was extracted by liquid/liquid extraction using solvent mixture (hexane + ethylacetate 10% w:w), and by liquid solid extraction using several types of organoclays complexes. The solid phase was extracted by acetone and ultrasonic device. Results showed some difficulties were associated with liquid/liquid extraction whereas effective and easy extraction procedures were obtained by liquid solid extraction using either organoclay complex or activated charcoal. In contrast combination of both solid materials did not show improvement in the extracted cyanotoxin. Thus we recommend the use of organoclays or activated charcoal separately for extracting cyanotoxin. Further improvement of extraction can be tailored by using a specific organoclay complex that has some similarity in the chemical structure between the pre-adsorbed organic cation to the clay mineral and the chemical structure of cyanotoxin.

Detecting Accuracy of Three Dimensional Power Doppler (3DPD) Vascular Indices for Prenatal Diagnosis of Morbidly Adherent Placenta in Patients with Placenta Previa

Objective:?The study’s objective was to assess the accuracy of using prenatal 3-dimensional power Doppler analysis of vascular placental indices to accurately diagnose morbidly adherent placenta objectively. Background:?Traditionally, 2D ultrasound was used for the diagnosis of a suspected morbidly adherent placenta (MAP) previa. More objective techniques like 3D power Doppler haven’t been well studied. Study Design:?A prospective cohort study?is?designed for women with gestational age between 28 and?32 weeks with suspected placenta previa. Patients were examined by 2D ultrasound which was used in management decisions.?3D Power Doppler’s VI, FI and VFI were measured during the same examination after manual tracing of placenta;data were blinded to obstetricians. Histopathology was performed to confirm MAP. Results: Our results showed that the 3D power Doppler VI ≥ 16 predicted the diagnosis of MAP with 100% sensitivity, 100% specificity which is better than those of 2D ultrasound. While VI > 33.1 measured by 3D Doppler predicted severe MAP with a sensitivity of 73.9% and specificity of 86.4%, which was superior to 2D ultrasound. Conclusion:?In patients with placenta previa, the 3D Doppler’s vascular index accurately predicts MAP. Furthermore, vascular and vascular flow indices of 3D Doppler were more predictive of severe cases of MAP compared to 2D ultrasound.

Investigation of a Modified Cobalt-Free Alloy for Nuclear Application

Cobalt-free alloy of constituent’s “0.045%C-12.73%Ni-6.53%Cr-3.2%Mo-0.02%Ti-0.01%V” has been prepared by electro slag remelting technique. Mass attenuation coefficient, half value layer and effective atomic number have been determined for the prepared sample at photon energies 235 - 2700 keV. The results are compared with the corresponding theoretical calculations based on XCOM program and a fair agreement is obtained.

The Impact of Pretransplant Disease Characteristics on the Outcome of Autologous Stem Cell Transplantation for Neuroblastoma with High-Risk Features: A Retrospective Model from a Limited Resources Country

High-risk neuroblastoma still has poor survival outcome. Improvement of outcome is attributed to the consolidation of chemotherapy by autologous bone marrow transplant. Further improvement of the outcome by tandem autologous transplant is followed by immune therapy. We aimed with this study to correlate initial disease characteristics with the outcome of transplanted high-risk neuroblastoma. A retrospective analysis was done for 73 transplanted patients. Patients were treated in Children’s Cancer Hospital Egypt from July 2012 to July 2015. Seventy patients received Busulphan/Melphalan conditioning. The 3-year overall survival (OS) and event-free survival (EFS) was 63.3% and 51.3%, respectively. Disease stage did not impact the OS and EFS, P = 0.54 and 0.62 respectively. Status of MYCN did not reflect statistically on outcome for tumors with amplified compared to nonamplified (EFS, 49% and 63.1%, respectively). Response after induction chemotherapy pointed that patients who had objective response (complete response, very good partial response and partial response) were better compared to those with less response with EFS and OS of 53.3% and 64.2% compared to 49.3% and 63.5%, respectively, which may indicate that chemo-sensitive tumors have better outcome. By the end of the study, twenty-seven patients relapsed, out of them 25 patients died. Pretransplant risk features for neuroblastoma was nullified by autologous stem cell transplant. The modest outcome observed, highlights some limitations that need to be sorted out in countries with limited resources. The introduction of immune therapy and tandem transplant is needed to achieve a better outcome, yet it adds to more financial burden.

AIM:To identify blood donors with occult hepatitis B virus(HBV) infection(OBI) to promote safe blood ***:Descriptive cross sectional study was conducted on 3167 blood donors negative for hepatitis B surface antigen(HBsAg),hepatitis C antibody(HCV Ab) and human immunodeficiency virus *** were subjected to the detection of alanine aminotransferase(ALT) and aspartate transaminase(AST) and screening for anti-HBV core antibodies(total) by two different techniques;[Monoliza antibodies to hepatitis B core(Anti-HBc) Plus-Bio-Rad] and(ARC-HBc total-ABBOT).Positive samples were subjected to quantitative detection of antibodies to hepatitis B surface(anti-HBs)(ETI-AB-AUK-3,Dia Sorin-Italy).Serum anti-HBs titers > 10 IU/L was considered *** HBV DNA by real time polymerase chain reaction(PCR)(QIAGEN-Germany) with 3.8 IU/mL detection limit was estimated for blood units with negative serum anti-HBs and also for 32 whose anti-HBs serum titers were > 1000 IU/***,265 recipients were included,34 of whom were followed up for 3-6 *** were investigated for ALT and AST,HBV serological markers:HBsAg(ETI-MAK-4,Dia Sorin-Italy),anti-HBc,quantitative detection of anti-HBs and ***:525/3167(16.6%) of blood units were positive for total anti-HBc,64% of those were antiHBs *** by ARCHITECT anti-HBc assay were carried out for 498/525 anti-HBc positive samples,where 451(90.6%) confirmed *** for anti-HBc was considered confirmed only if two positive results were obtained for each sample,giving an overall prevalence of 451/3167(14.2%) for total *** DNA was quantified by real time PCR in 52/303(17.2%) of anti-HBc positive blood donors(viral load range:5 to 3.5 x 105 IU/mL) with a median of 200 IU/mL(mean:1.8 x 104 ± 5.1 x 104 IU/mL).AntiHBc was the only marker in 68.6% of *** and multivariate logistic analysis for identifying risk factors associated with anti-HBc and HBV-DNA positivity a
