Social health insurance and drug spending a mong cancer inpatients in China

A social health insurance(ShI) program has been established in China to ensure that people can obtain health care economically and equitably. Our analysis indicates that in 2005-06, 66.5 percent of Chinese citizens were non-ShI inpatients. We also found that drug spending for ShI inpatients was significantly higher than that for non-ShI inpatients. After adjusting for other variables, we found that the ShI coverage was also associated positively with higher drug costs. We present evidence to show that drug spending differences are attributable at least in part to differences in insurance courage.

Three thicknesses of TiO2 films, 174, 195, and 229 nm, were deposited onto quartz substrates by sol–gel spin coating method. The as-deposited thin films were characterized by nano-crystallite with different sizes (19–46 nm) and relatively high porous structure. Optical constants were determined and showed the lowest refractive index of 1.66 for the as-prepared films that ever reported till now. Obtained results were discussed through current theoretical ideas.

Corticotropin-releasing factor(CRF), which activates the hypothalamic-pituitary-adrenal axis under stress, also has proinflammatory peripheral effects possibly through mast cells. The purpose of this study was to investigate the effect of urocortin (UCN), a 40-amino-acid CRF family peptide, on degranulation and intracellular calcium of rat lung mast cells. The activation and degranulation of mast cells were observed by Toluidine blue staining and transmission electron microscope. The intracellular calcium was investigated using confocal laser scanning microscopy and flow cytometry. The results indicated that all the three different concentrations of UCN(0.1, 1 and 10 mu M) significantly induced the activation and degranulation of rat lung mast cells in vitro. This effect was markedly blocked by selective CRF receptor 1(CRF-R1) antagonist antalarmin, but not by specific CRF receptor 2(CRF-R2) antagonist antisauvagine-30(anti-Svg-30). The results also showed that UCN caused a rapid peak increase inCa2+(i) at point of 300s after UCN treatment, followed by a decrease to a sustained plateau phase. The peak increase inCa2+(i) induced by UCN was significantly inhibited by antalarmin, but not by anti-Svg-30. This effect of UCN onCa2+(i) in rat lung mast cells was also found by flow cytometry. Regression analysis revealed a positive correlation between mast cells degranulation extent and the maximum value ofCa2+(i)(P her, our present study suggested that UCN induced the increase of Ca2+(i) and degranulation of rat lung mast cells through CRF-R1. These findings may have implications for the pathophysiology of allergic and inflammatory lung disorders such as asthma, which is closely associated with mast cell activation and degranulation. AIMTo review the evidence on the association between specific colon adenoma features and the risk of future colonic neoplasia [adenomas and colorectal cancer (CRC)]. METhODSWe performed a literature search using the National Library of medicine through Pubmed from 1/1/2003 to 5/30/2015. Specific medical Subject headings terms (colon, colon polyps, adenomatous polyps, epidemiology, natural history, growth, cancer screening, colonoscopy, CRC) were used in conjunction with subject headings/key words (surveillance, adenoma surveillance, polypectomy surveillance, and serrated adenoma). We defined non-advanced adenomas as 1-2 adenomas each 25% villous histology or high-grade dysplasia. A combined endpoint of advanced neoplasia included advanced adenomas and invasive CRC. RESULTSOur search strategy identified 592 candidate articles of which 8 met inclusion criteria and were relevant for assessment of histology (low grade vs high grade dysplasia, villous features) and adenoma size. Six of these studies met the accepted quality indicator threshold for overall adenoma detection rate > 25% among study patients. We found 254 articles of which 7 met inclusion criteria for the evaluation of multiple adenomas. Lastly, our search revealed 222 candidate articles of which 6 met inclusion criteria for evaluation of serrated polyps. Our review found that villous features, high grade dysplasia, larger adenoma size, and having ≥ 3 adenomas at baseline are associated with an increased risk of future colonic neoplasia in some but not all studies. Serrated polyps in the proximal colon are associated with an increased risk of future colonic neoplasia, comparable to having a baseline advanced adenoma. CONCLUSIONData on adenoma features and risk of future adenomas and CRC are compelling yet modest in absolute effect size. Future research should refine this risk stratification.

The ERCC1(excision repair cross complementation group 1) and ERCC2(excision repair cross complementation group 2) genes are important in repairing DNA damage and genomic instability in germ cells, and are essential for normal spermatogenesis. It has been verified that polymorphisms of these two genes could alter DNA repair capacity in some phenotypic studies. however, little information is available on these polymorphisms in male infertility. This study was designed to examine whether ERCC1 polymor- phisms 3 UTR(C8092A), Asn118Asn(G19007A) and ERCC2 polymorphisms Asp312Asn(G -> A), Lys751Gln (A -> C), which have been reported to contribute to some cancers, are associated with idiopathic azoospermia in the Chinese population. The four polymorphisms were genotyped using polymerase chain reaction-restriction fragment length polymorphism assay in a hospital-based case-control study, comprising 202 infertile patients with idiopathic azoospermia and 187 fertile controls. It was found that the ER- CC1 8092 CA + AA genotypes were significantly associated with an increased risk of idiopathic azoospermia(OR = 1.750, 95% CI = 1.170-2.618), while other polymorphisms appeared to show no significant differences between cases and controls. Further studies are needed to confirm the roles of these polymorphisms in idiopathic azoospermia.

Confocal Raman microscopy is important for characterizing 2 D materials, but its low throughput significantly hinders its applications. For metastable materials such as graphene oxide(GO), the low throughput is aggravated by the requirement of extremely low laser dose to avoid sample damage. here we introduce algorithm-improved confocal Raman microscopy(ai-CRM), which increases the Raman scanning rate by one to two orders of magnitude with respect to state-of-the-art works for a variety of 2 D materials. Meanwhile, GO can be imaged at a laser dose that is two to three orders of magnitude lower than previously reported, such that laser-induced variations of the material properties can be avoided. ai-CRM also enables fast and spatially resolved quantitative analysis, and is readily extended to 3 D mapping of composite materials. Since ai-CRM is based on general mathematical principles, it is cost-effective, facile to implement and universally applicable to other hyperspectral imaging methods.

Inhibition of microglia-mediated neuroinflammation has been regarded as a prospective strategy for treating neurodegenerative disorders,such as Parkinson's disease(PD). In the present study,we demonstrated that systematic administration with iptakalim(IPT),an adenosine triphosphate(ATP)-sensitive potassium channel(K-ATP) opener,could alleviate rotenone-induced degeneration of dopaminergic neurons in rat substantia nigra along with the downregulation of microglial activation and mRNA levels of tumor necrosis factor-alpha(TNF-alpha) and cyclooxygenase-2(COX-2). In rat primary cultured microglia,pretreatment with IPT suppressed rotenone-induced microglial activation evidenced by inhibition of microglial amoeboid morphological alteration,declined expression of EDl(a marker for activated microglia),and decreased production of TNF-alpha and prostaglandin E2(PGE(2)). These inhibitory effects of IPT could be reversed by selective mitochondrial KATP(mitoKATP) channel blocker 5-hydroxydecanoate(5-hD). Furthermore,pretreatment with IPT prevented rotenone-induced mitochondrial membrane potential loss and p38/c-jun N-terminal kinase(JNK) mitogen-activated protein kinase(MAPK) activation in microglia,which might in turn regulate microglial activation and subsequent production of TNF-alpha and PGE(2). These data strongly suggest that the K-ATP opener IPT may be a novel and promising neuroprotective drug via inhibiting microgliamediated neuroinflammation.
