Geochemistry of lamprophyres associated with uranium mineralization, Southeastern Desert, Egypt

Two brecciated shear zones (NNW-SSE) are found crosscutting cataclastic rocks. The cataclastic rocks (3.0 km2) occupy the core of the granitic pluton and enclose a roof pendant of mafic-ultramafic rocks. The NNW-SSE-extending lamprophyre dykes vary in thickness from 0.5 m to 1 m and up to 800 m long, cutting the cataclastic rocks and are composed mainly of plagioclases, amphiboles, relics of pyroxenes and K-feldspar phenocrysts embedded in fine-grained groundmass. They are characterized as being peraluminous, calc-alkaline in composition (chemical trap) and enriched in calcite, sulfide and P2O5. The lamprophyres were affected by hydrothermal alteration (chlorite-carbonate alteration) while the cataclastic rocks were affected by diagenetic alteration (K-feldspar-albite alteration). Uranium mineralization is the product of hydrothermal events and has been investigated by X-ray diffraction (XRD) and environmental scanning electron microscopy (ESEM), involving primary uranium minerals (U3O8) and secondary uranium minerals (uranophane and beta-uranophane, kasolite, torbernite, autonite and meta-autonite) in addition to U- bearing minerals (astrocyanite, betafite and fergusonite). The presence of different mineral parageneses associated with clay minerals indicates that the lamprophyres were subjected to acidic and alkaline mineralizing solutions. Moreover, the U-Zr/U, U-Ce/U values show negative correlations, confirming U-enrichment in both cataclastic rocks and shear zones while the Th-eU/eTh, Th-Zr/Th and Th-Ce/Th values show negative correlations, indicating that the U-bearing solutions are rich in Th in the cataclastic rocks only.

Behavior of circulating epithelial tumor cells(CETC)and FISH(fluorescence in situ hybridisation)of epidermal growth factor receptor(EGFR)-gene amplification in lung cancer patients during the course of therapy

Introduction: Monitoring the response of CETC to therapy in lung cancer allows early detection of patients at risk of progression. Analysis of the EGFR-gene amplification in these cells may help to characterize patients who might benefit from tyrosine kinase inhibitors. Methods: CETCs were quantified at least twice during treatment from blood of 52 patients with advanced non small cell lung cancer (NSCLC) using fluorescence labelled anti-EpCAM. EGFR-gene amplification was analysed in these cells with double probe (EGFR/CEP7) using FISH analysis. Results: Progression of the tumor was observed in 30 of the 52 patients (58%). With respect to changes in CETCs during therapy and progression free survival 31 patients showed a decrease in CETCs, 2 developing a single brain metastasis and 12 progressive disease;20 patients showed an increase in CETC more than twofold 16 of which developed progressive disease. The difference was highly significant (p=0.007 Fisher’s exact test) irrespective of age, sex, tumor size, pathological type and therapy. Kaplan-Meier progression free survival was significantly different between patients with decreasing and increaseing CETC (p=0.038). 5/20 patients tested were positive for EGFR amplification with 85-100% of EpCAM positive cells showing this chromosomal abnormality. One patient could be followed during therapy with increasing CETC during therapy with bevacizumab followed by relapse. He subsequently received erlotinib resulting in a decrease in CETC and is still free of progress after 516 days. Conclusions: These results show that peripherally circulating tumor cells in patients with advanced NSCLC are influenced by systemic chemotherapy and an increase in spite of therapy is a marker of aggressiveness of the tumor cells. Determination of the EGFR amplification might help to better treat part of these patients.

Postoperative Radiotherapy in Bladder Cancer Patients: 5-Year Institutional Experience of National Cancer Institute, Cairo University

Purpose: Adjuvant radiation therapy could reduce loco regional failure, but currently has no defined role because of previously reported morbidity. NCI-Cairo routine work is to give adjuvant PORT for locally advanced bladder carcinoma patients. The aim of this work is to re-evaluate this protocol regarding its effect on prognosis and complications. Patients and Method: A retrospective study included 208 patients with pathologically proven bladder cancer who presented to the NCI, Cairo University from 2007-2011. All of them underwent RC with bilateral PLND followed by conventional post-operative radiotherapy in 2 - 6 weeks after surgery for 5000 cGy in 25 fractions, over 5 weeks using 2D technique. Analysis of data from their files was done for the treatment results, prognostic factors and complications. Results: Three years overall survival (OS) and disease free survival (DFS) for the whole group was ~60%, and 54% respectively in favour of the female gender, non-smokers, Squamous cell carcinoma patients, low grade tumours (grade 1 and 2) negative margins, N0, pT2b and early stage group showed the best prognoses. The 3 years metastases free survival (MFS) was ~71%. Only four factors showed a significant relation with the MFS which were the grade, LN status, T-stage and group staging. The local recurrence rate (LRC) at 2 years for the whole group was ~95% and 94% at 3 years. Only surgical margin status and extent of LN dissection had a significant impact on the LRC. Conclusions: Adjuvant radiotherapy shows sustained improvement in the loco regional control, and should be recommended for patients with locally advanced disease especially those with less than 10 dissected lymph nodes and those with positive margins.

Hypoglycemic and antioxidant activities of Caesalpinia ferrea Martius leaf extract in streptozotocininduced diabetic rats

Objective: To evaluate the antidiabetic and antioxidant ef ects of aqueous ethanolic extract of Caesalpinia ferrea(C. ferrea) leaf in normal and streptozotocin(STZ) induced diabetic ***: Male Sprague-Dawley rats divided into 6 groups of 6 rats each were assigned into diabetic and non-diabetic groups. Diabetes was induced in rats by single intraperitoneal administration of STZ(65 mg/kg body weight). C. ferrea extract at the doses of 250 and 500 mg/kg body weight was orally administered to both diabetic and non-diabetic animals for a period of 30 days. After completion of experimental duration serum, liver and pancreas were used for evaluating biochemical and histopathological ***: Oral administration of C. ferrea leaf extract significantly reduced elevated serum glucose, α-amylase, liver function levels and signii cantly increased serum insulin, total protein and body weight as well as improved lipid proi le due to diabetes. Furthermore, the treatment resulted in a marked increase in glutathione peroxidase, superoxide dismutase, catalase and reduced glutathione, and diminished levels of lipid peroxidation in liver and pancreas of diabetic rats. Histopathological studies demonstrated the reduction in the pancreas and liver damage and coni rmed the biochemical i ***: From the present study, it can be concluded that the C. ferrea leaf extract ef ectively improved hyperglycaemia while inhibiting the progression of oxidative stress in STZ-induced diabetic rats. Hence, it can be used in the management of diabetes mellitus.

Asthma Patient Care: The Pharmacist’s Perspective

Aim of the Study: To compare effect of asthma care by pharmacist intervention versus routine care on asthma control. Patients and Methods: A 2-month randomised, controlled trial was conducted in outpatient clinics of Ain Shams University Hospitals, Cairo, Egypt. Patients were randomly assigned to receive routine care or a pre-defined pharmacist intervention. This intervention was mainly focused on patient education, improving inhalation technique and medication assessment. Primary outcome was the level of asthma control, as assessed by the Asthma Control Questionnaire (ACQ). Results: By the end of the study, intervention patients who received a written action plan significantly improved their ACQ results than routine care group who did not receive a plan (p < 0.0001). Inhalation technique and adherence to controller medication were significantly better in the intervention group. Conclusion: The present study results provide supportive evidence concerning pharmacists’ favourable effects on asthma patient care and support pharmacists as valuable members of the health care team.

Evaluation of Management of Pre-Eclamptic Cases Admitted to elshatby Maternity University Hospital

Hypertensive diseases of pregnancy, mainly PET, consist of several diseases which are associated with increased morbidity and mortality rates both maternal and fetal wise. Its incidence among pregnant females is about 3% - 10%. This study included 500 pregnant PET women, and they categorized to mild and severe forms, after exclusion of cases with essential hypertension, renal disease and SLE. All pregnant ladies underwent history taking, abdominal examination (general and local), obstetric ultrasound, Doppler ultrasound, laboratory investigations;treatment received (antihypertensive medications, magnesium sulphate (MgSO4) and steroids) and finally follow up till delivery. Regarding complications, PET can result in serious fetal/neonatal or maternal complications. Integrated Geophysical Investigations to Detect the Shallow Subsurface Settings at New Sohag City, Egypt

New Sohag City has an area of approximately 1300 acres (546 hectares) and is located within Sohag Governorate limits, about 18 km away from old Sohag City. It is established to reduce the population within the old Sohag city and to create better economic conditions in a new appropriate living environment. The main target of this study is to apply the relevant surface geophysical methods to image the shallow subsurface setting, to define the shallow lithological layers and to locate any geological structures that may be cause cracks in buildings, and any unstable materials, which may decrease the bearing capacity or human activities, hence increasing hazards in the study area. This study embraces, electrical Resistivity Tomography (ERT), Shallow Seismic Refraction (SSR), and Ground Penetrating Radar surveys (GPR). The integrated interpretation of all techniques classified the subsurface layers into two layers. A strong reflector separating the upper gravel and calcareous sandy layer from the lower clayey layer. They gave also interesting information about the geotechnical competent of the ground and some structural features in the study area.

Parameters Affecting the Production of High Carbon Ferromanganese in Closed Submerged Arc Furnace

This study has been performed to investigate the different parameters affecting on the production of high carbon ferromanganese in closed submerged arc furnace. The analysis of industrial data revealed that using manganese ores with low Mn/Fe ratio necessitates higher amount of Mn-sinter in the charge. Using Mn-blend with higher Mn/Fe ratio reduces the coke consumption and this leads to reducing the electrodes consumption. The recovery of Mn ranges between 70 and 80 %. Much higher basic slag has slight effect on Mn- recovery. However, as slag basicity increases, the MnO- content of slag decreases. The manganese content of produced HCFeMn depends mainly on Mn/Fe ratio of Mn-blend. For obtaining HCFeMn alloy containing minimum 75%Mn, it is necessary to use Mn-blend with Mn/Fe ratio of higher than 6. A model for determination of the amount and composition of off-gases has been derived based on the chemical composition and material balance of the input raw materials and the produced alloy and slag. By using this model, the amount of off-gases was found to increase by increasing both Mn-blend and coke consumption.

Microwave Assisted Liberation of High Phosphorus Oolitic Iron Ore

The influence of microwave treatment on the liberation of iron ore from the high phosphorus oolitic iron ore from Aswan region, Egypt was studied. The effect of microwave power, exposure time and grain size on the liberation of iron ore was investigated. The microfractures and cracks of the samples were characterized before and after microwave treatments. The heating rate of high phosphorus oolitic iron ore was studied. Crystallinity of hematite was characterized before and after microwave pretreatment. The results indicated that intergranular fractures formed between the gangues (fluorapatite and chamosite) and hematite after microwave treatment, leading to improved liberation of iron ore and a significant reduction in comminution energy. Percentages of fraction ≤ -0.125 mm increased from 46.6% to 59.76% with increased exposure time from 0 to 60 seconds. The heating rate of iron ore showed that microwave treatment was less efficient at smaller particle sizes for a fixed applied power density. Crystallinity of hematite increased with the microwave exposure time.
