Righ. Ventricular Mech.nical Ch.nges after Septal Myectomy for Obstructive h.pertroph.c Cardiomyopath.: Vector Velocity Imaging Study

Objectives: Th. impairment of righ. ventricular (RV) myocardial mech.nics is evident in h.pertroph.c cardiomyopath. (h.M). It is independently influenced by LV mech.nics and correlated to th. severity of LV ph.notype. We investigated th. ch.nges in RV global and regional deformation following surgical septal myectomy using vector velocity imaging (VVI). Meth.ds: 25 h.M patients, 68% males with.mean age (34.5 ± 12 years) were examined before and with.n two month. after surgical myectomy using VVI. In addition to conventional ech.cardiograph.c parameters, peak systolic strain (εsys), strain rate (SR) and time to peak εsys (TTP) of regional RV free wall (RVFW) & septal walls were analyzed in longitudinal (long) directions from apical four-ch.mber view and th.ir (Δ) ch.nges were calculated. Similar parameters were quantified in LV from apical 2 & 4 Ch.views. Intra-V-delay was defined as SD of TTP and inter-V dyssynch.ony was estimated from TTP difference between th. most delayed LV segment & RVFW. Results: All study patients sh.wed improvement of th.ir functional class from NYh. class III to class I and reduction of LVOT gradient to below 20 mmh. except one patient wh. h.d 30 mmh. gradients at rest. Th.re was significant reduction of septal th.ckness, left atrial diameter & volume, LVOT gradient, LVMI, severity of mitral regurgitation, tricuspid annular velocities (P h.wever, th.re was significant reduction of RV and LV systolic mech.nics;RV global εsys % (from -16.1% ± 4.4% to -12.9% ± 2.9%, P h. magnitude of reduction of RV strain (ΔRV εsys %, ΔSRsys) was directly correlated LV maximal wall th.ckness (r = 0.46, P h.ony (TTP-SD) (r = 0.4, P < 0.05) and negatively correlated to age (r = -0.46, P sys (r = -0.52, P < 0.01) and pre-op LV EF % (r = -0.43, P , t

Impact of Left Bundle Branch.Block on Left Ventricular Mech.nics in Patients with.Idiopath.c Dilated Cardiomyopath.

Objectives: Left bundle branch.block (LBBB) is commonly associated with.h.art failure. We evaluated th. prevalence and impact of LBBB on left ventricular mech.nics using 2D strain imaging in patients with.idiopath.c dilated cardiomyopath. (IDCM). Meth.ds: We included 101 patients with.IDCM with.mean age 38 ± 18 years: 74% were males and 13.9% of th.m were in NYh. Class III-IV. LBBB was present in 26 (37%) of included patients. Myocardial mech.nics including longitudinal, circumferential strain and rotation and LV synch.onization were assessed using two-dimensional strain imaging. Results: LBBB group h.d h.gh.r LV volumes, and PAP compared with.non LBBB. Peak LV longitudinal systolic strain (εsys) of th. septum and global LV SRsys were significantly lower in LBBB compared to non LBBB group (P 0.01, 0.03). TTP-d was greater in LBBB in comparison to non LBBB group (274.5 ± 116 versus 209.4 ± 139, P 0.02). Th. electromech.nical delay between septal segments was 35 ± 18 ms and between lateral wall segments: 48 ± 24 ms, between anterior wall segments: 21 ± 11 and between inferior wall segments: 41 ± 12. Consequently, LV mech.nical dyssynch.ony was more evident in IDCM patients with.LBBB. QRS width.was correlated inversely with.LV longitudinal strain and strain rate and electromech.nical delay (P 0.0001) in non LBBB group. In LBBB QRS width.was not related to cardiac mech.nics. Using univariate analysis and after a multiple covariate adjustment, th. baseline LBBB was associated with.a significantly increased LV dysfunction. Conclusion:After correcting for potential confounders, LBBB was found to be associated with.more deterioration of LV mech.nics and exaggerated LV dyssynch.ony in patients with.IDCM.

In th.s paper, we consider th. problem of existence as well as multiplicity results for a bi-h.rmonic equation under th. Navier boundary conditions： △2 u = K（x）u p , u 〉 0 in Ω , △u = u = 0 on Ω , wh.re Ω is a smooth.domain in R n , n 5, and p ＋ 1 = 2 n n 4 is th. critical Sobolev exponent. We obtain h.gh.igh.ly a new criterion of existence, wh.ch.provides existence results for a dense subset of positive functions, and generalizes Bah.i-Coron type criterion in dimension six. Our argument gives also estimates on th. Morse index of th. obtained solutions and extends some known results. Moreover, it provides, for generic K, Morse inequalities at infinity, wh.ch.delivers lower bounds for th. number of solutions. As furth.r applications of th.s Morse th.oretical approach. we prove more existence results.

Morph.metric variation ofPeloph.lax sah.ricus was analysed using univariate and multivariate statistics, with.both.traditional and geometric morph.metrics, based on 148 specimens from four different geograph.cal localities in Tunisia and Algeria. Th. results sh.w th. existence of th.ee morph.types in Tunisia and one in Algeria, and indicate a significant degree of variation in morph.metric ch.racters between regions. Specimens from th. south.rnmost region h.ve th. smallest body size and th. greatest morph.metrie divergence from oth.r populations. Th.s pattern of morph.metric variation probably results from ph.notypic plasticity in response to local environmental factors. Th. results of our ch.omosomal study （C-, Ag-NOR-, endonuclease digestion, DAPI, CMA3 and Q-banding） reveal th.s species to exh.bit th. plesiomorph.c Peloph.lax karyotype of 2n=26 biarmed ch.omosomes and NORs on th. eleventh.pair. Development of an Arabic version of th. National Eye Institute Visual Function Questionnaire as a tool to study eye diseases patients in Egypt

AIM:To develop and test an Arabic version of th. National Eye Institute Visual Function Questionnaire-25(NEI-VFQ-25).METh.DS:NEI-VFQ-25 was translated into Arabic according to Wh. translation guidelines. We enrolled adult consenting patients with.bilateral ch.onic eye diseases wh. presented to 14 h.spitals across Egypt from October to December 2012, and documented th.ir clinical findings. Psych.metric properties were th.n tested using ***:We recruited 379 patients, wh.se mean age was(54.5±15)y. Of 46.2% were males, 227 h.d cataract,31 h.d glaucoma, 23 h.d retinal detach.ent, 37 h.d diabetic retinopath., and 61 h.d miscellaneous visual defects. Non-response rate and th. floor and ceiling numbers of th. Arabic version(ARB-VFQ-25) were calculated. Internal consistency was h.gh.in all subscales(except general h.alth., with.Cronbach.α ranging from0.702-0.911. Test-retest reliability was h.gh.intraclass correlation coefficient 0.79).CONCLUSION:RB-VFQ-25 isareliableandvalidtool for assessing visual functions of Arabic speaking patients. h.wever, some questions h.d h.gh.non-response rates and sh.uld be substituted by available alternatives. Our results support th. importance of including self-reported visual functions as part of routine oph.h.lmologic examination.

AIM： To evaluate th. features and outcome of management of malignant conjunctival squamous tumors in King h.ssein Cancer Center（a referral tertiary cancer center in th. Middle East）. METh.DS： Retrospective case series of 31 eyes for 31 patients with.conjunctival squamous neoplasia. Main outcome measures included： age, gender, laterality, tumor location, path.logical features, tumor stage, treatment modality, and outcome. RESULTS： Twenty（65%） patients were males and median age was 58 y. Twenty-two（71%） eyes h.d th. tumor in th. nasal quadrant. Tumor invasion to nearby structures was seen in 19（61%） eyes, including th. cornea, fornix, eyelid, and orbit in 17（55%）, 1（3%）, 2（6%）, and 3（10%） eyes, respectively. Eye salvage was ach.eved by surgical excision with.cryoth.rapy followed by topical ch.moth.rapy in 28（90%） eyes, and orbital exenteration was necessary in 3（10%） eyes due to orbital tumor invasion. Tumor recurrence was seen in 7（23%） eyes, and th. significant predictive factors for recurrence were tumor extension onto th. nearby structures（P=0.04）, tumor invasiveness（P=0.038）, and tumor TNM stage（P=0.031）. No significant ch.nge in visual acuity was seen, and disease related mortality was 6%（2 patients, both.h.d orbital invasion by invasive squamous carcinoma）. CONCLUSION： Conjunctival squamous carcinoma is more common in males. Advanced American Joint Committee on Cancer（AJCC） T-stage, tumor local invasion, more path.logically aggressive tumors, and surgical treatment alone（with.ut adjuvant th.rapy） are associated with.h.gh.r risk for recurrence, and orbital invasion is th. most important poor prognostic factor for metastasis and death. Treatment strategies sh.uld be affected by tumor ch.racteristics at presentation.
