Simulated Microbe Removal around Finger Rings Using Different Hand Sanitation Methods

Aim It is our opinion that the CDC and the WHO have underestimated cross-contamination under examination gloves in dental clinics while wearing j.welry, such as finger rings. These agencies only ＂recommend＂ removing j.welry, and only washing hands for 15 seconds with soap and warm water before donning gloves. This study examined several washing procedures and finger rings using simulated microbes. Methodology A gloved robber hand manikin was made and fitted with a flesh disposable vinyl glove. Four fingers were fitted with rings or no ring, dusted with simulated microbes, and washed with a scrub brush for 5, 15, and 25 seconds under 20℃ and 40℃ water alone, or with liquid hand soap. Light levels （in lux） of fluorescent powder before and after washing were measured and delta scores calculated for changes in light levels, equivalent to effectiveness of hand washing procedures. A full-factorial, 3-factor analysis of variance （ANOVA） was used to test for differences among levels of the three study factors-time, temperature, and soap use. Tukey＇s post hoc honestly significant difference （HSD） test was applied to significant factors to examine pair-wise differences between factor levels. Results It was found that the longer the hands with rings were washed with a scrub brush under flowing water, the more simulated microbes were removed. By 25 seconds, all methods were essentially the same. Simulated microbes were more difficult to remove from the palm compared to the back of the hand. The liquid hand soap used in this study was more effective with warm water than cold. When given a choice of washing with cold water up to 15 seconds, it would be preferable not to use soap to remove simulated microbes. Qualitatively, the outer surface of finger rings were more effectively cleaned than the crevice below the ring, and the ring with a stone setting appeared to accumulate and retain simulated microbes more than other rings. Strategies for CADD vary depending on the extent of structural and other information available regarding the target (enzyme/receptor) and the ligands. Computer-aided drug design (CADD) is an exciting and diverse discipline where various aspects of applied and basic research merge and stimulate each other. In the early stage of a drug discovery process, researchers may be faced with little or no structure activity relationship (SAR) information. The process by which a new drug is brought to market stage is referred to by a number of names most commonly as the development chain or “pipeline” and consists of a number of distinct stages. To design a rational drug, we must firstly find out which proteins can be the drug targets in pathogenesis. In present review we reported a brief history of CADD, DNA as target, receptor theory, structure optimization, structure-based drug design, virtual high-throughput screening (vHTS), graph machines.

Osteoma of Temporal Bone—A Rare Case Report

Osteoma is a slow-growing tumor formed by mature bone tissue. In the temporal bone, intracanalicular osteomas are more frequent while extracanalicular ostemoas are rare. They very rarely occur over the mastoid region. They are treated mainly for cosmetic purposes. These are readily excised and recurrence is rare after complete excision. In this case report, we want to revisit this rare mastoid osteoma and discuss the differential diagnosis and treatment. We believe that this report will be of interest to otolaryngologists when dealing with temporal bone osteomas.

Triple-c Cartilage Tympanoplasty: Case Series

The triple-c cartilage tympanoplasty i.e. (composite chondroperichondrial clip) technique was devised by Fernandes in 2003. Obj.ctives: The obj.ctive of our case series was to assess the success rate and efficacy of the triple-c cartilage tympanoplasty by transcanal approach. Study Design: A retrospective analysis of patients subj.cted to the technique was conducted. Methods: 20 cases who met the inclusion criteria were assessed by otomicroscopy and pure tone audiometry before and 2 months after the surgery. Results: All patients had complete take-up of the graft and a hearing improvement which was statistically significant. Conclusions: Thus the triple-c technique provides an effective method of closing nonmarginal perforations of the tympanic membrane.

Post-Stapedectomy Granuloma: A Rare Case Report

Reparative granuloma in the oval window region is an uncommon complication of stapes surgery, which usually develops within one to eight weeks after operation and causes a sudden hearing loss and disturbance of balance. It may also cause otalgia. Because of its rarity, no single centre would be able to give conclusive evidence regarding this complication. Hence we would like to report our experience and hope to get a better understanding regarding the stapes surgery induced granulation.

CSF Rhinorrhoea with Encephalocele through Sternberg’s Canal: Our Experience

Spontaneous cerebrospinal fluid rhinorrhoea with encephalocele restricted to the sphenoid sinus is rare clinical finding. As of today, only 17 cases encephalocele protruding through the Sternberg’s canal and extending into the lateral recess of sphenoid sinus, have been described in literature. Patients presenting with this special clinical entity usually do not have any history of trauma, tumour or iatrogenic inj.ry. Thus the lesions are considered to originate from a congenital bony defect in the lateral wall of the sphenoid sinus, first described by Sternberg in 1888 as the lateral craniopharyngeal canal (Sternberg’s canal). In our experience each patient of spontaneous CSF rhinorrhea should have suspicion of intrasphenoid encephalocele though cribriform plate is a common site. Endoscopic tranasnasal approach is one of the best modalities for such cases.

Klestadt’s Cyst: Case Report

Klestadt’s cyst, more commonly known as the nasolabial cyst, is an uncommon, non-odontogenic, and soft tissue cyst. It is classified as a fissural cyst, found outside the bone, and on the region corresponding to the nasolabial furrow and alar nose. Following its description first by Zukuerkandl in 1882, only 267 cases have been found in English literature. In spite of the low occurrence of nasolabial cysts, it is important to recognize the clinical characteristics of this lesion. The purpose of this report is to review the literature and discuss the histomorphology and etiology of this condition, and also its management by surgical excision. As per our experience, sublabial approach is the best for complete and scarless excision of this cyst.

Intratympanic Inj.ctions: An Unsolved Mystery

Aims: The aim of this study was to test the effectiveness of intratympanic dexamethasone inj.ctions as a treatment for severe disabling tinnitus and also observe its effect on hearing loss if any. Materials and Methods: Thirty patients with severe disabling tinnitus in the age group 20 to 60 years were selected and randomly assigned to receive intratympanic inj.ctions of a dexamethasone solution 4 mg/ml (0.5 ml) or isotonic saline (0.5 ml) solution under topical anaesthesia, once per week for 4 weeks using a zero degree endoscope. Improvement in tinnitus was assessed using a visual analog scale, considering 2-point improvement as significant and alteration in hearing if any was noted by pure tone audiometery before and after the therapy. Results: The improvement in tinnitus was not significant, with no alteration in audiometery reports. Conclusions: Intratympanic therapy is an attractive mode of treatment because of its highly targeted delivery, low concentration of the drugs required and a very good patient tolerance. Although there has been no breakthrough in intratympanic therapy for tinnitus or other otological conditions, accessibility to the inner ear through the semipermeable round window membrane holds many promises in the near future.

Prolonged Middle Ear Ventilation by Cartilage-Grommet Tympanoplasty

In 1994, the American Otological Society reported favourable experience with composite cartilage shield tympanoplasty. The tragal cartilage with a grommet inserted in it was used for tympanoplasty in our patient with unilateral CSOM, supposedly because of severe chronic eustachian tube dysfunction. The marriage of cartilage tympanoplasty with grommet insertion was aimed to add the advantages and abolish the disadvantages of both the procedures. In 1990 Lary Hall first introduced the “long term ventilation of the middle ear” with a T-tube placed in the tragal cartilage perichondrium composite island graft. T-tube insertion in the cartilage has been described. But insertion of the Indian Sheperds grommet (ventilation tube) in the cartilage graft as described in this case and its technique are possibly the first of its kinds in literature. The report is aimed to ignite innovation of newer and better techniques of cartilage tympanoplasty.

The composite materials based on nanopowder dispersed liquid crystals are important both from fundamental research and device applications due to their unique properties such as improvement in various properties like electrical,optical,thermal,energy storage and spontaneous polarization *** proper selection of nanoparticle and its size which to be dispersed in particular liquid crystals is very important for a particular *** the present study,a ferroelectric nanopowder of Barium Titanate(BaTiO3)was dispersed in cholesteric liquid crystal and the same was confirmed by Fourier Transform Infrared(FTIR)*** various acoustical properties like ultrasonic velocity,density,Adiabatic Compressibility,Rao Constant,Wada Constant and Acoustic Impedance were investigated by ultrasonic interferometer at room temperature at fixed *** dielectric constant was determined by Precision Impedance *** addition to these investigations,particle size and surface area were also *** investigation shows enhanced in dielectric and acoustical properties which may be useful for device applications extensively in microelectronics,low cost-photovoltaic devices,and custom-shaped containers possibly applied as a coating.
