Effect of tuberosity repair on functional outcome of reverse shoulder arthroplasty in proximal humerus fractures

Purpose:Unsatisfactory results of hemiarthroplasty in Neer’’s 3- and 4-part proximal humerus fractures in elderly,have led to the shift towards reverse shoulder arthroplasty (RSA).The objective of our study was to repair the tuberosities that are generally overlooked during RSA and observe its impact on the functional outcome and shoulder ***:We include elderly patients with acutely displaced or dislocated 3- or 4-part proximal humerus fractures from July 2013 to November 2019 who were treated with RSA along with tuberosity repair by non-absorbable sutures and bone grafting harvested from the humeral *** injuries and cases with neuro-muscular involvement of the deltoid muscle were *** to the tuberosity healing on radiographs of the shoulder at 9th postoperative month,the patients were divided into 2 groups,as the group with successful tuberosity repair and the other with failed tuberosity *** analysis of the functional outcome and shoulder scores between the 2 groups were done by independentt-test for normally distributed parameters and Mann-Whitney test for the parameters,where data was not normally ***:Of 41 patients,tuberosity healing was achieved in 28 (68.3%) and failed in 13 (31.7%) *** of the tuberosity occurred in 5 patients,tuberosity displacement in 2,and nonunion in *** age was 70.4 years (range 65 – 79 years) and mean follow-up was 58.7 months (range 18 – 93 months).There were no major *** with successful tuberosity repair showed improvement in mean active range of movements,like anterior elevation (165.1°± 4.9°vs.144.6°± 9.4°,p < 0.000),lateral elevation (158.9°± 7.2°vs.138.4°± 9.6°,p < 0.000),external rotation (30.5°± 6.9°vs.35.0°± 6.3°,p = 0.367),internal rotation (33.7°± 7.5°vs.32.6°± 6.9°,p = 0.671) and in mean shoulder scores including Constant score (70.7 ± 4.1vs.55.5 ± 5.7,p < 0.000),American shoulder and elbow surgeons score (90.3 ± 2.4vs.69.0 ± 5.7,p < 0.0

Stability Analysis of an Underactuated Autonomous Underwater Vehicle Using Extended-Routh's Stability Method

In this paper, a new approach to stability analysis of nonlinear dynamics of an underactuated autonomous underwater vehicle（AUV） is presented. AUV is a highly nonlinear robotic system whose dynamic model includes coupled terms due to the hydrodynamic damping factors. It is difficult to analyze the stability of a nonlinear dynamical system through Routh＇s stability approach because it contains nonlinear dynamic parameters owing to hydrodynamic damping coefficients. It is also difficult to analyze the stability of AUVs using Lyapunov＇s criterion and LaSalle＇s invariance principle. In this paper, we proposed the extended-Routh＇s stability approach to verify the stability of such nonlinear dynamic systems. This extended-Routh＇s stability approach is much easier as compared to the other existing methods. Numerical simulations are presented to demonstrate the efficacy of the proposed stability verification of the nonlinear dynamic systems, e.g., an AUV system dynamics.

This paper deals with de the fiasco created by the Tehri Uttarakhand, India, particularly resettlement and rehabilitation tailed analysis of High Dam in in terms of of the local inhabitants. Aspects pertaining to the environmental issues are also discussed. Currently, the river valleys in Uttarakhand state of India are the targets of increasing hydroelectric projects. Virtually all rivers are being exploited for generating environmental friendly power. Having being learned the hard lesson from Tehri Dam, it has been decided to opt for such schemes in which comparatively little submergence hnd tempering with the fragile eco-systems is involved However, our observations suggest that even in such schemes if due care is not taken they may turn out to be a failure.

Effect of Frey's procedure on islet cell function in patients with chronic calcific pancreatitis

Background: Frey’s procedure involves both drainage and resection of the pancreas in subjects with chronic calcific pancreatitis(CCP). The procedure may affect the pancreatic endocrine function after surgery. The present study was to evaluate the effect of Frey’s procedure on both beta and alpha cell function in CCP ***: Thirty CCP patients who underwent Frey’s procedure were included. According to the glycemic status, patients were divided into the diabetes mellitus(DM), prediabetes, and normal glucose tolerance(NGT) groups. Islet cell function was assessed before and 3 months after ***: At baseline, there was a significant difference in beta cell function among the three groups [NGT group 1.71(1.64–2.07) vs prediabetes group1.50(0.83–1.61) vs DM group 0.33(0.12–0.55), P < 0.0001], but the insulin resistance was not different among them. Post glucose hyperglucagonemia representing alphacell dysfunction during oral glucose tolerance test was present in all of them, but showed no significant difference [NGT group 0.15(0.06–0.31) vs prediabetes group 0.32(0.05–0.70) vs DM group 0.07(0.02–0.18), P = 0.20]. Frey’s procedure did not change beta cell function and insulin resistance. However, alphacell dysfunction deteriorated after surgery [0.10(0.03–0.27) vs 0.33(0.09–0.68), P = 0.004].Conclusions: Although Frey’s procedure does not affect the beta cell function and insulin resistance in CCP patients, the alpha-cell dysfunction deteriorates after surgery.

Full-disk synoptic observations of the chromosphere using Hα telescope at the Kodaikanal Observatory

This paper reports on the installation and observations of a new solar telescope installed on 2014 October 7 at the Kodaikanal Observatory. The telescope is a refractive type equipped with a tunable Lyot Hα filter. A CCD camera with size 2k ×2k acquires images of the Sun and has a pixel size of 1.21″ pixel^-1 and a full field-of-view of 41＇. The telescope is equipped with a guiding system which keeps the image of the Sun within a few pixels throughout the observations. The FWHM of the Lyot filter is 0.4A and the filter is motorized, capable of scanning the Hα line profile at a smaller step size of 0.01 A. Partial-disk imaging covering about 10＇ is also possible with the help of a relay lens kept in front of the CCD camera. In this paper, we report the detailed specifications of the telescope, filter unit, the installation, observations and the procedures we have followed to calibrate and align the data. We also present preliminary results with this new full-disk telescope.

Current evidence of concept analyses recommending nanotechnology for biomedical uses abounds in recent research. The area of biotechnology interfaces with nanostructures, reconfigures their composition, and alters their characteristics;which influences the dispersion of the particles, the biotransformation they cause, and their potential toxic effect. It is vital to link the idea of the lifecycle of nanostructures to the biological impacts and use methodologies to identify, estimate, and track the gradual bioprocessing of nanostructures in vivo, from a body-wide level to a nanoscopic size. This is necessary because understanding how nanostructures processing, degradation, persistence, and recycling predict potential exposure risks. The safe implementation of nanotechnology-based products in biomedical applications necessitates an extensive understanding of the recycling and transformations of nanomaterials in a living organism. Long-term fate in the body is crucial, as it governs potential environmental risks to human health. Strategies may be used to manage the long-term outcome of nanostructures in an organism since, in addition to composition, their design also affects how long they last and how easily they degrade. The lifespan of nanoparticles, a flexible and biocompatible category of nanostructures that have made it into clinical trials, is the subject of this article. Strategies may be used to manage the long-term outcome of nanoparticles in an organism since, in addition to composition, their design also affects how long they last and how easily they degrade. This review explained the safety of nanoscale materials, biotransformation, and the multifunctional recycling mechanism of nanostructures.
