Diagnostic accuracy of serum biochemical fibrosis markers in children with chronic hepatitis B evaluated by receiver operating characteristics analysis

METHODS： We determined the serum level of apolipoprotein A-I （APO A-I）, haptoglobin （HPT） and a-2 macroglobulin （A2I） with an automatic nephelometer in 63 children （age range 4-17 years, mean 10 years） with biopsy-verified chronic HBeAg-positive hepatitis B. Fibrosis stage and inflammation grade were assessed in a blinded fashion according to Batts and Ludwig. We defined mild liver fibrosis as a score ≤2 and advanced fibrosis as a score equal to 3. ROC analysis was used to calculate the power of the assays to detect advanced liver fibrosis （AccuROC, Canada）. RESULTS： Serum concentrations of APO A-I, HPT and A2M were not significantly different in patients with chronic hepatitis B compared to controls. However, APO A-I level of 1.19 ng/L had a sensitivity of 85.7% and a specificity of 60.7% （AUC = 0.7117, P = 0.035） to predict advanced fibrosis. All other serum biochemical markers and their combination did not allow a useful prediction. None of these markers was a good predictor of histologic inflammation. CONCLUSION： Apolipoprotein A-I may be a suitable serum marker to predict advanced liver fibrosis in children with chronic hepatitis B.

Objective:Epidermal growth factor receptor(EGFR)activation was reported to upregulate programmed death-ligand 1(PD-L1)expression in lung cancer cells and subsequently contribute to immune escape,indicating its critical role in EGFR-driven lung *** study characterized PD-L1 expression in patients with surgically resected EGFR-mutant non-small cell lung cancer(NSCLC).The effect of PD-L1 expression on clinical outcomes was also investigated in advanced EGFR-mutant NSCLC treated with EGFR-tyrosine kinase inhibitors(TKIs).Methods:In total,73 patients with surgically resected NSCLC and EGFR mutations were ***-L1 expression and CD8+tumor-infiltrating lymphocyte(TIL)density were assessed by immunohistochemistry.A literature review of publications that assessed the predictive and prognostic value of PD-L1 expression in advanced EGFR-mutant NSCLC patients treated with EGFR-TKIs was ***:Nineteen(26.0%)patients were positive for PD-L1 expression,which was significantly associated with concomitant KRAS mutation(P=0.020)and marginally associated with higher CD8+TILs density(P=0.056).Positive PD-L1 expression was associated with markedly inferior overall survival(OS)in multivariate analysis(P=0.032).The combination of PD-L1 and CD8+TILs expression could be used to stratify the population into three groups with distinct prognoses.A meta-analysis of six publications showed that positive PD-L1 expression was not associated with OS[hazard ratio(HR)=0.90;95%confidence interval(CI),0.42–1.38]or progression-free survival(HR=1.03;95 CI,0.73–1.33)in advanced EGFR-mutant NSCLC patients receiving ***:PD-L1 expression tended to correlate with CD8+TIL expression,concomitant KRAS mutation,and poor survival in surgically resected EGFR-mutant ***-L1 expression was neither the predictive nor the prognostic factor in advanced EGFR-mutant NSCLC patients treated with EGFR-TKIs.

A new model of the modified Newtonian gravity called Compacted & Collapsing Gravity (CCG) is proposed. Similar to the Milgrom’s MOND, it allows explaining the flattening of rotation curve in spiral galaxies, thus eliminates the need for dark matter at this level. However, in contrast to MOND, it puts a distinct limit on effective gravity;thereby constraints the sizes of single galaxies in connection to their masses, which complies with observations. In the bigger than single galaxies structures such as galaxy clusters, CCG rather complements than replaces interpretations of the observational data based on dark matter. Besides, the new model provides a plausible explanation to the hierarchical structure of the universe.

Gravitational Time Dilation inside the Solid Sphere

Gravitational time dilation directly reflects the difference between gravitational potentials at different altitudes in the gravitational field. At the same time this phenomenon is expected to obey the Einstein’s equivalence principle, one of two pillars (apart from general covariance) of general relativity. The experiments aimed at detecting the gravitational time dilation are therefore described as the tests of general relativity or, alternatively, the tests of equivalence principle. When applied to the exterior of a solid sphere, these two interpretations are fully compatible both theoretically and experimentally. However, when applied to the interior of a solid sphere (e.g., to the interior of Earth), they seem to contradict each other. Namely, a strict dependence of the gravitational time dilation on the gravitational potential inside the sphere proves to be at odds with the equivalence principle. This paper reveals this problem and provides solution to it. As a consequence, it is concluded that, contrary to the current belief, the Earth’s center is older, not younger, than the Earth’s surface. Since all the previous experiments have been performed either on or above the Earth’s surface, an experiment performed below the Earth’s surface is proposed.

We present a combination of machine learning and high throughput calculations to predict the points defects behavior in binary intermetallic(A–B)compounds,using as an example systems with the cubic B2 crystal structure(with equiatomic AB stoichiometry).To the best of our knowledge,this work is the first application of machine learning-models for point defect *** throughput first principles density functional calculations have been employed to compute intrinsic point defect energies in 100 B2 intermetallic *** systems are classified into two groups:(i)those for which the intrinsic defects are antisites for both A and B rich compositions,and(ii)those for which vacancies are the dominant defect for either or both composition *** data was analyzed by machine learning-techniques using decision tree,and full and reduced multiple additive regression tree(MART)*** these three schemes,a reduced MART(r-MART)model using six descriptors(formation energy,minimum and difference of electron densities at the Wigner–Seitz cell boundary,atomic radius difference,maximal atomic number and maximal electronegativity)presents the highest fit(98%)and predictive(75%)*** model is used to predict the defect behavior of other B2 compounds,and it is found that 45%of the compounds considered feature vacancies as dominant defects for either A or B rich compositions(or both).The ability to predict dominant defect types is important for the modeling of thermodynamic and kinetic properties of intermetallic compounds,and the present results illustrate how this information can be derived using modern tools combining high throughput calculations and data analytics.
