Outcomes of Thyroid Surgery at Niamey National Hospital

Introduction: Goiter is the most common endocrine disease in the world. The objective of our study is to assess and compare the morbidity linked to the surgical management of goiters and thyroid tumors in the general surgery department of the Niamey National Hospital. Result: We collected data on a prospective basis on 112 cases having undergone a thyroidectomy between March 2019 and April 2020. The female sex is predominant with 96.4%, i.e. a sex ratio of 0.037. The average age is 41.2 years with extremes ranging from 22 to 72 years old. The average duration of evolution of goiter is 7.5 years with extremes ranging from 5 months to 20 years old. The first reason for consultation is anterior cervical swelling. TSH testing confirmed euthyroidism in 107 patients (95.5%). Therapeutically, loboisthmectomy is the most performed procedure with 43.7%. The postoperative hormonal assessment revealed 32% hypothyroidism at 6 months, with a significant difference for cases of total thyroidectomy. The occurrence of postoperative hypocalcemia was significantly greater after total and subtotal thyroidectomy. There is no significant difference in terms of recurrent complications. Conclusion: Thyroidectomies must be performed after adequate preparation and information to the patient on possible postoperative complications, hence the need for close medical monitoring immediately and remotely.

Homogeneous Finite-Time Consensus Control for Higher-Order Multi-Agent Systems by Full Order Sliding Mode

This paper investigates the distributed finite-time consensus tracking problem for higher- order nonlinear multi-agent systems （MASs）. The distributed finite-time consensus protocol is based on full order sliding surface and super twisting algorithm. The nominal consensus control for the MASs is designed based on the geometric homogeneous finite time control technique. The chattering is avoided by designing a full order sliding surface. The switching control is constructed by integrating super twisting algorithm, hence a chattering alleviation protocol is obtained to maintain a smooth control input. The finite time convergence analysis for the leader follower network is presented by using strict Lyapunov function. Finally, the numerical simulations validate the proposed homogeneous full-order sliding mode control for higher-order MASs.

An Adaptive Full Order Sliding Mode Controller for Mismatched Uncertain Systems

In this paper, an adaptive full order sliding mode （FOSM） controller is proposed for strict feedback nonlinear systems with mismatched uncertainties. The design objective of the controller is to track a specified trajectory in presence of significant mismatched uncertainties. In the first step the dynamic model for the first state is considered by the desired tracking signal. After the first step the desired dynamic model for each state is defined by the previous one. An adaptive tuning law is developed for the FOSM controller to deal with the bounded system uncertainty. The major advantages offered by this adaptive FOSM controller are that advanced knowledge about the upper bound of the system uncertainties is not a necessary requirement and the proposed method is an effective solution for the chattering elimination from the control signal. The controller is designed considering the full-order sliding surface. System robustness and the stability of the controller are proved by using the Lyapunov technique. A systematic adaptive step by step design method using the full order sliding surface for mismatched nonlinear systems is presented, Simulation results validate the effectiveness of the proposed control law.

This study addresses two issues about the interaction of the upper limb rehabilitation robot with individuals who have *** first step is to estimate the human's target position(also known as TPH).The second step is to develop a robust adaptive impedance control mechanism.A novel Non-singular Terminal Sliding Mode Control combined with an adaptive super-twisting controller is being developed to achieve this *** combination's purpose is to provide high reliability,continuous performance tracking of the system's *** proposed adaptive control strategy reduces matched dynamic uncertainty while also lowering chattering,which is the sliding mode's most glaring *** proposed TPH is coupled with adaptive impedance control with the use of a Radial Basis Function Neural Network,which allows a robotic exoskeleton to simply track the desired impedance *** validate the approach in real-time,an exoskeleton robot was deployed in controlled experimental circumstances.A comparison study has been set up to show how the adaptive impedance approach proposed is better than other traditional controllers.

This paper investigates the active fault tolerant cooperative control problem for a team of wheeled mobile robots whose actuators are subjected to partial or severe faults during the team *** cooperative robots network only requires the interaction between local neighbors over the undirected graph and does not assume the existence of leaders in the *** assume that the communication exists all the time during the *** avoid the system's deterioration in the event of a fault,a set of extended Kalman filters(EKFs)are employed to monitor the actuators'behavior for each ***,based on the online information given by the EKFs,a reconfigurable sliding mode control is proposed to take an appropriate action to accommodate that *** this research study,two types of faults are *** first type is a partial actuator fault in which the faulty actuator responds to a partial of its control input,but still has the capability to continue the mission when the control law is *** addition,the controllers of the remaining healthy robots are reconfigured simultaneously to move within the same capability of the faulty *** second type is a severe actuator fault in which the faulty actuator is subjected to a large loss of its control input,and that lead the exclusion of that faulty robot from the team ***,the remaining healthy robots update their reference trajectories and form a new formation shape to achieve the rest of the team mission.

Polychaetes are one of the most important groups ofmacrobenthic organisms in coastal and marine environments,in terms of diversity and abundance,and play an important role in the functioning of *** study aims to describe the structure and spatial distribution ofpolychaete communities along Essaouira's rocky shoreline(Atlantic coast of Morocco)in relation to the major environmental variables such as granulometry,salinity,temperature,pH,and intertidal *** sites from these intertidal rocky shores were sampled using a quadrat method during the summer 2016.A total of 4435 individuals belonging to 34 taxa were *** the families,Sabellaridae(24%)and Nereididae(23%)were ranked first in terms of abundance of *** alveolata(24%)and Perinereis cultrifera(13%)were the dominant ***(H')values varied from 2.28 to ***'s index(J')varied between 0.62 and *** was essentially due to the low dominance of few *** analysis was used to characterise Essaouira's rocky shores on the basis of benthic polychaete *** analysis confirmed the presence of three distinct *** to canonical correspondence analysis,the structure and distribution of faunal assemblages of benthic polychaetes was mainly related to topographical complexity,water temperature,hydrodynamic conditions and *** conclude that these rocky shores show typically high benthic polychaete diversity compared to the taxonomic list of other rocky *** addition,these results constitute a baseline data for the development of a sustainable network for long-term monitoring of benthic polychaete community changes due to ecological factor influences and anthropogenic activity impacts.

Eigenstructure Assignment Method and Its Applications to the Constrained Problem

A partial eigenstructure assignment method that keeps the open-loop stable eigenvalues and the corresponding eigenspace unchanged is presented. This method generalizes a large class of systems previous methods and can be applied to solve the constrained control problem for linear invariant continuous-time systems. Besides, it can be also applied to make a total eigenstructure assignment. Indeed, the problem of finding a stabilizing regulator matrix gain taking into account the asymmetrical control constraints is transformed to a Sylvester equation resolution. Examples are given to illustrate the obtained results.

Investigation of Electrical Stability of Nonwovens with Conductive Circuits Using Printed Conductive Inks

In this work, we study the stability of a class of materials obtained by printing a textile with conductive inks using a method called screen printing. Under the action of a certain external factors, the printed circuit suffers deterioration and the conductivity decreases considerably. In this work, we propose to model the overall damage of the textile sheet in terms of the partial damages of the conductive lines. We also apply this approach to evaluate the damage of a system being made of transmission lines printed into nonwoven substrates using different conductive inks.

Clinical significance of main pancreatic duct dilation on computed tomography: Single and double duct dilation

AIM: To study the patients with main pancreatic duct dilation on computed tomography (CT) and thereby to provide the predictive criteria to identify patients at high risk of significant diseases, such as pancreatic cancer, and to avoid unnecessary work up for patients at low risk of such diseases. METHODS: Patients with dilation of the main pancreatic duct on CT at Emory University Hospital in 2002 were identified by computer search. Clinical course and ultimate diagnosis were obtained in all the identified patients by abstraction of their computer database records. RESULTS: Seventy-seven patients were identified in this study. Chronic pancreatitis and pancreatic cancer were the most common causes of the main pancreatic duct dilation on CT. Although the majority of patients with isolated dilation of the main pancreatic duct (single duct dilation) had chronic pancreatitis, one-third of patients with single duct dilation but without chronic pancreatitis had pancreatic malignancies, whereas most of patients with concomitant biliary duct dilation (double duct dilation) had pancreatic cancer. CONCLUSION: Patients with pancreatic double duct dilation need extensive work up and careful followup since a majority of these patients are ultimately diagnosed with pancreatic cancer. Patients with single duct dilation, especially such patients without any evidence of chronic pancreatitis, also need careful follow-up since the possibility of pancreatic malignancy, including adenocarcinoma and intraductal papillary mucinous tumors, is still high.

Model for end-stage liver disease score versus Child score in predicting the outcome of surgical procedures in patients with cirrhosis

AIM： To determine factors affecting the outcome of patients with cirrhosis undergoing surgery and to compare the capacities of the Child-Turcotte-Pugh （CTP） and model for end-stage liver disease （MELD） score to predict that outcome. METHODS： We reviewed the charts of 195 patients with cirrhosis who underwent surgery at two teaching hospitals over a five-year period. The combined endpoint of death or hepatic decompensation was considered to be the primary endpoint. RESULTS： Patients who reached the endpoint had a higher MELD score, a higher CTP score and were more likely to have undergone an urgent procedure. Among patients undergoing elective surgical procedures, no statistically significant difference was noted in the mean MELD （12.8 ＋ 3.9 vs 12.6 ＋ 4.7, P = 0.9） or in the mean CTP （7.6 ± 1.2 vs 7.7 ± 1.7, P = 0.8） between patients who reached the endpoint and those who did not. Both mean scores were higher in the patients reaching the endpoint in the case of urgent procedures （MELD： 22.4 ± 8.7 vs 15.2 ± 6.4, P = 0.0007; CTP： 9.9 ± 1.8 vs 8.5 ± 1.8, P = 0.008）. The performances of the MELD and CTP scores in predicting the outcome of urgent surgery were only fair, without a significant difference between them （AUC = 0.755 ± 0.066 for MELD vs AUC = 0.696 ± 0.070 for CTP, P = 0.3）. CONCLUSION： The CTP and MELD scores performed equally, but only fairly in predicting the outcome of urgent surgical procedures. Larger studies are needed to better define the factors capable of predicting the outcome of elective surgical procedures in patients with ***, but only fairly in predicting the outcome of urgent surgical procedures.
