A 16 year-old girl was admitted to hospital because of fatigue and somnolence,nausea,epistaxis and jaund *** examination revealed jaundice,an enlarged liver and tenderness of upper right abdom *** tests revealed an increased level of acute liver failure,bilirubin,bile acids,GGTP and a decreased prot hrombin ratio,with elevated gamma-globulin and IgG levels,and the presence of anti-mitochondrial M2 antibodies and HBV infection *** patient was diagnosed with liver failure resulting from chronic hepatitis B with an autoimmune *** treatment consisted of steroids,azathioprine,vitam in K,low-protein diet and lactulose *** undergoing a molecular test(HBV DNA 3.23 × 105 IU/mL and mutations 204 and 80),the treatment was mo tient was discharged in good clinical condition,with the recommendation of continued entecavir,prednisone and *** subsequent months,no clinical deter ioration or abnormal biochemical liver function test results were found,despite the discontinuation of immunos up pressive therapy after 10 *** patient cont inues entecavir therapy.

Sediment constitutes an interesting and valuable material for observation of human impact on the *** on the sediment core samples in two Tatra Mountains lakes,determination of the selected elements（including cesium 137 radionuclide activity and heavy metals' concentrations） and dating of the obtained layers by the lead 210 method,chemometric analysis of the resulting data set was *** was established,as a result of Cluster Analysis,that the variability of the determined parameters was distinct in the lakes with much higher concentrations of heavy metals in Smreczynski staw ***,the influence of human activity（long distance transport of contaminants） is notable on the north slopes of the Tatras（Smreczynski staw lake） comparing to Popradske pleso *** examined elements can be divided into two groups-natural to the site origin（iron,manganese,and magnesium）,and elements whose accumulation in the sediments can be related（at least in part） to human activity(137Cs,zinc,copper,cadmium,lead,and chromium).The results confirm the existence of the so called "screen effect",meaning that the Tatra Mountains' crest delimits to a considerable extent further transport of pollutants to the south side of the Tatra Mountains slopes.

Efficiency and safety of lamivudine therapy in patients with chronic HBV infection, dialysis or after kidney transplantation

AIM: To analyze the effectiveness and safety of lamivudine treatment in patients with chronic HBV infection undergoing hemodialysis or after kidney transplantation, and to study the frequency of tyrosine - methionine - aspartate - aspartate (YMDD) mutation occurrence after lamivudine treatment. METHODS: We analyzed 91 patients with chronic hepatitis B, among whom, 16 patients underwent hemodialysis, 7 patients had kidney transplantation and 68 patients had normal function of kidney. The hemodialysis patients were treated by lamivudine 300 mg/wk. patients after kidney transplantation and patiente with normal function of kidney were treated with lamivudine 100 mg/d. Therapy lasted for 12 mo. HBV-DNA, HBsAg, HBeAg and anti-HBe, and anti-HCV antibodies were assessed in sera of patients. The analysis was performed before and 6 mo after the end of lamivudine treatment. Before, during and after the lamivudine therapy, the number of erythrocytes, leukocytes, platelets and hemoglobin concentration, ALT and AST activity, as well as bilirubin, urea and creatinine concentrations were analyzed in sera from patients. RESULTS: After the 12-mo lamivudine treatment, elimination of HBV - DNA was observed in 56% patients undergoing hemodialysis and in 53% patients with normal kidney function. Only 1 from 7 (14%) kidney-transplanted patients eliminated HBV-DNA. Furthermore, HBeAg elimination was observed in 36% hemodialysis patients, in 51% patients with normal function of kidneys and in 43% kidney transplanted patients. Among the patients undergoing dialysis, no YMDD mutation was found after 12 mo of therapy, while it was detected in 9 patients (13%) with normal function of kidney and in 2 kidney-transplanted patients (29%, Pmal function of kidney. Liver biopsy is a standard method used for obtaining liver tissue for histopathological evaluation. Since reliable serological and virological tests are currently available, liver biopsy is no longer needed for the etiological diagnosis of chronic hepatitis B and C. However, liver histology remains the gold standard as a prognostic tool, providing information about the liver disease progression(grading of necroinflammatory activity and staging of fibrosis) and serving clinicians in the management and therapeutic decisions. In general, histopathological evaluation is indicated before starting the antiviral treatment. main limitations of the liver biopsy include its invasive and painful procedure, sampling errors and the inter- and intra-observer variability. In addition, indications for the liver biopsy in pediatric patients with chronic viral hepatitis were questioned recently, and efforts have been made toward the development of non-invasive methods as an alternative to the liver biopsy. The most commonly used methods are novel imaging studies(elastography) and combinations of biomarkers. However, to date, none of these tests was validated in children with chronic viral hepatitis. In this review, we present the current status of the liver biopsy in the management of chronic viral hepatitis B and C in pediatric population, including specific indications, complications, contraindications, problems, limitations, and alternative non-invasive methods.
