Cardiorespiratory fitness measured with cardiopulmonary exercise testing and mortality in patients with cardiovascular disease:A systematic review and meta-analysis

Background Cardiorespiratory fitness(CRF)is inversely associated with mortality in apparently healthy subjects and in some clinical populations,but evidence for the association between CRF and all-cause and/or cardiovascular disease(CVD)mortality in patients with established CVD is *** study aimed to quantify this *** We searched for prospective cohort studies that measured CRF with cardiopulmonary exercise testing in patients with CVD and that examined all-cause and CVD mortality with at least 6 months of *** hazard ratios(HRs)were calculated using random-effect inverse-variance *** Data were obtained from 21 studies and included 159,352 patients diagnosed with CVD(38.1%female).Pooled HRs for all-cause and CVD mortality comparing the highest *** category of CRF were 0.42(95%confidence interval(95%CI):0.28–0.61)and 0.27(95%CI:0.16–0.48),*** HRs per 1 metabolic equivalent(1-MET)increment were significant for all-cause mortality(HR=0.81;95%CI:0.74–0.88)but not for CVD mortality(HR=0.75;95%CI:0.48–1.18).Coronary artery disease patients with high CRF had a lower risk of all-cause mortality(HR=0.32;95%CI:0.26–0.41)than did their unfit *** 1-MET increase was associated with lower all-cause mortality risk among coronary artery disease patients(HR=0.83;95%CI:0.76–0.91)but not lower among those with heart failure(HR=0.69;95%CI:0.36–1.32).Conclusion A better CRF was associated with lower risk of all-cause mortality and *** study supports the use of CRF as a powerful predictor of mortality in this population.

Clinical heterogeneity in autoimmune acute liver failure

AIM: To describe the outcome and prognosis in a cohort of patients with acute liver failure due to autoimmune hepatitis without liver transplantation. METHODS: A retrospective trial was conducted in 11 patients with acute liver failure due to autoimmune hepatitis who attended the Instituto Nacional de Ciencias Médicas y Nutrición Salvador Zubiran. Demographic, biochemical and severity indexes, and treatment and outcome were assessed. RESULTS: Among the 11 patients, with a median age of 31 years, 72% had inflammatory response syndrome, and six patients received corticosteroids. The mortality rate within four weeks was 56%, and the one-year survival was 27%. In the survivors, severity indexes were lower and 83% received corticosteroids. CONCLUSION: We observed a relatively high survival rate in patients with acute liver failure due to autoimmune hepatitis. This survival rate could be influenced by severity of the disease and/or use of corticosteroids.

A Scientometric Approach to Analyze Scientific Development on Renewable Energy Sources

Purpose: This paper aims to point out the scientific development and research density of renewable energy sources such as photovoltaic, wind, and biomass, using a mix of computational tools. Based on this, it was possible to verify the existence of new research trends and opportunities in a macro view regarding management, performance evaluation, and decision-making in renewable energy generation systems and ***/methodology/approach: A scientometric approach was used based on a research protocol to retrieve papers from the Scopus database, and through four scientometric questions, to analyze each area. Safety of the Patient from the Medication

Currently, more than 7% of admissions to acute care hospitals am related with AEMs （adverse events to medications）. AEMs are the sixth cause of death, causing a cost of over $5.6 million dollars （USD） per hospital per year. There is an estimate that between 19% and 23% of hospitalized patients will have an adverse effect within the first 30 days after being discharged, 14.3% will be re-admitted and 70% of these events will be related to a medication prescription. Fortunately, at least 58% of these AEMs are preventable, since they result from a lack of information on the medication, prescription and dosage errors and from the abuse and underuse of the same. Polymedicated patients, especially the elderly with multiple pathologies and/or chronic patients that need to be admitted into the hospital more frequently, usually to internal medicine, neurology, psychiatry, rehabilitation and intensive care, are precisely the most liable to suffer from medication errors. It must be the objective to aim for the increase in the patient safety standards when it comes to medications.

Surface-plasmonic sensor using a columnar thin film in the grating-coupled configuration [Invited]

The excitation of a surface-plasmon-polariton(SPP) wave guided by a columnar thin film(CTF) deposited on a one-dimensional metallic surface-relief grating was investigated for sensing the refractive index of a fluid infiltrating that CTF. The Bruggemann homogenization formalism was used to determine the relative permittivity scalars of the CTF infiltrated by the fluid. The change in the refractive index of the fluid was sensed by determining the change in the incidence angle for which an SPP wave was excited on illumination by a p-polarized plane wave, when the plane of incidence was taken to coincide with the grating plane but not with the morphologically significant plane of the CTF. Multiple excitations of the same SPP wave were found to be possible, depending on the refractive index of the fluid, which can help increase the reliability of results by sensing the same fluid with more than one excitation of the SPP wave.

Assessing the Influence of Global Climate and Anthropogenic Activities on the Water Balance of an Andean Lake

Tropical regions along the Andean Cordillera face an uncertain future as mountain lakes and snow peaks exhibit receding trends associated with factors such as climatic precursors and local anthropogenic activities. Tota, the largest mountain lake in the Colombian Andes, exemplifies the role played by these factors on the lake's hydrologic evolution. A monthly water balance in Tota Lake was performed using available hydrological information from 1958 to 2007 to address interannual and multiannual level fluctuations associated with human activities and climatic precursors. The balance shows that net water uses fluctuated around 2 m3/s during this period with a pattern that, although constrained during years of severe decline in lake levels, is able to explain most of the multiannual decaying trend of 1.5 cm/year in the last 50 years. The lake’s naturalized levels were used to determine the influence of climate precursors on the lake evolution. Using Multichannel Singular Spectrum Analysis (M-SSA), significant five-year ENSO and 20-year PDO related quasi-oscillations were detected, explaining 54% of the variance associated with mean annual naturalized level fluctuations. ENSO is markedly in-phase with lake levels, with critical declines associated with low precipitation and high evaporation rates during El Ni?o years, whereas the PDO signal exhibits a phase opposition with lake levels, with low naturalized levels during a positive PDO phase and high levels during a negative PDO phase (an important result to consider given the current cooling trend of the PDO signal).

The thin layer activation technique(TLA)is very effective in wear analysis for tribological systems where disassembly is costly or *** its development in the 1970s,TLA has been applied through direct irradiation of solid targets using ionizing particle beams to obtain an activation with a specific *** profile is then used to measure wear in a variety of *** experiments rely heavily on how deep the normalized activation *** of 30μm to 50μm are obtained for constant activation by the standard method,which for many experiments is ***,many others would benefit greatly from a 300μm constant activation layer,such as high-performance engines,softer alloys(brakes),and oil transport *** this aim was the goal of this ***,we needed to find an adequate irradiation line for the TLA ***,another technique/system was developed(the Attenuation Wheel),which would bring the depth of constant activation from 50μm to 300μm.

Uncontrolled hypertension in older patients： markers and associated factors to masked and white-coat effect

Background Hypertension is the main risk factor for cardiovascular diseases, affecting more than half the elderly population. It is essential to know if they have proper control of hypertension. The aim of this study was to identify the associated factors to masked uncon- trolled hypertension and false uncontrolled hypertension in older patients. Methods Two-hundred seventy-three individuals （70.1±6.7 years-old） had blood pressure （BP） measured at the office and by ambulatory BP monitoring （ABPM）, with the definition of controlled group （C）, individuals with high office BP and adequate ABPM, called white-coat effect group （WCE）, uncontrolled （UC）, and subjects with ap- propriate office BP and elevated ABPM denominated masked effect group （ME）. Age, body mass index, diabetes, pulse pressure （PP） and BP dipping during sleep were evaluated （Kruskal-Wallis test and logistic regression models）. Results Age was higher in UC than in C and ME （P 〈 0.01）, and 24-h ABPM PP was lower in C （48± 7 mmHg） and WCE （51±6 mmHg） than in UC （67±12 mmHg） and ME （59±8 mmHg）（P 〈 0.01）. Sleep systolic BP dipping was lower in ME than in C （P = 0.03）. Female gender was associated with a greater chance of being of ME group, which showed a higher PP and lower BP dipping during sleep. Conclusions In older individuals, office BP measure- ments did not allow the detection of associated factors that would permit to differentiate WCE from UC group and C from ME group. ABPM favored the identification of a higher PP and a lower BP dipping during sleep in the masked effect and uncontrolled groups.
