SPEAr3 is a third-generation synchrotron light source storage ring, about 234 meters in *** meet the beam stability requirement, our goal is to ultimately achieve an orbit variation（relative to the photon beam lines） of less than 10% of the beam size, which is about 1 micron in the vertical plane. Hydrostatic leveling system（HLS） measurements show that the height of the SPEAr3 tunnel floor can vary by tens of microns daily without thermal insulation improvements. We present an analysis of the HLS data that shows that adding thermal insulation to the concrete walls of the storage ring tunnel dramatically decreased diurnal tunnel floor motion.

Background To investigate the long-term results of global coronary and peripheral interventional treatment of diabetic foot patients. Methods We retrospectively included 220 diabetic patients （78.5 ±15.8 years, 107 females, all with Fontaine Ⅲor Ⅳclass） who were referred to our centre for diabetic foot syndrome and severe limb ischemia from January 2006 to December 2010. Patients were evaluated by a team of interventional cardiologists and diabetologists in order to assess presence of concomitant coronary artery disease （CAD） and eventual need for coronary revascularization. Stress-echo was performed in all patients before diagnostic peripheral angiography. Patients with indications for coronary angiography were submitted to combined diagnostic angiography and then to eventual staged peripheral and coronary interventions. Doppler ultrasonography and foot transcutaneous oximetry of transcutaneous oxygen pressure （TcPO2） before and after the procedure were performed as well as stress-echocardiography and combined cardiologic and diabetic examination at 1 and 6 month and yearly. results Stress-echocardiography was performed in 94/220 patients and resulted positive in 56 patients who underwent combined coronary and peripheral angiography. In the rest of 126 patients, combined coronary and peripheral angiography was performed directly for concomitant signs and symptoms of coronary heart disease in 35 patients. Coronary revascularization was judged necessary in 85/129 patients and was performed percutaneously after peripheral interventions in 72 patients and surgically in 13 patients. For Diabetic foot interventions the preferred approach was ipsilateral femoral antegrade in 170/220 patients （77.7%） and contralateral cross-over in 40/220 patients （18.8%） and popliteal retrograde ＋ femoral antegrade in 10/220 patients （4.5%）. The broke of a tailings dam from Vale S.A. destroyed a gigantic area killing 270 persons in Brumadinho/Brazil. Organizing activities after a mass disaster is a complex process that requires the involvement of many people and resources in the laboratory. To DNA identification of the victims, a daunting work had to be made by DNA laboratory staff in a collaborative effort with many partners. Efforts to implement good practice guidelines in Disaster Victim Identification (DVI) have revealed several important aspects that need to change in the forensic DNA laboratory. This article highlights the challenges of implementing DVI best practice guidelines in resource-poor settings, but with professionals from different sectors engaged in the same goal of briefly identifying victims and helping families and society.

BACKGrOUND Problematic Internet use(PIU)or Internet addiction has been recognized to be a behavioral addiction characterized by excessive or poorly controlled preoccupations,urges,or behaviors regarding computer use and Internet access that leads to impairment or distress resembling substance *** To investigate the prevalence and characteristics of Internet use and abuse in a group of drug addicts from Southern Italy,by means of a specific questionnaire[“Questionario sull’Utilizzo delle Nuove Tecnologie”(QUNT)].METHODS All subjects(183)were heavy smokers,almost 50%of them used heroin and/or opioid compounds,30%alcohol,10%cannabis,8%cocaine,and 5%were polydrug *** 10%of the individuals were also suffering from gambling *** The time spent online was more than 4 hours a day in the total sample,with a slight prevalence in male *** and cannabis users spent more than 6 hours online,significantly more than opioid and alcohol *** of the QUNT factors was not different in both *** users showed higher scores at the“loss of control”,“pornography addiction”,and“addiction to social networks”factors,for the stimulant effect of this ***,15 out of the total 17 cocaine users were pathological *** and statistically significant relationships were observed between some QUNT factors and body mass *** These findings indicate that PIU is less severe in subjects taking sedative substances,such as heroin/opioids and alcohol,than in subjects taking ***,it may be used as a“stimulant”trigger in cocaine and cannabis *** effect of abuse drugs was noted on possible sex-related differences in QUNT *** observed a sort of“protective”effect of a love relationship and/or living together with a partner,as those engaged subjects showed lower scores on different items than single subjects or those living *** relationship between time spent online(and re

Objectives To assess the impact on stent implantation rate and mid-term outcomes of prolonged high pressure angioplasty of femoropopliteal lesions. Methods We retrospectively enrolled 620 consecutive patients from January 2011 to December 2011 （75.6 ±12.3 years, 355 males, 76.5%in rutherford class 5-6）, referred for critical limb ischemia and submitted to prolonged high-pressure angioplasty of femoropopliteal lesions. The definition of prolonged high-pressure angioplasty includes dilation to at least 18 atm for at least 120 s. Proce-dural data, and clinical and instrumental follow-up were analyzed to assess stent implantation rate and mid-term outcomes. results The preferred approach was ipsilateral femoral antegrade in 433/620 patients （69.7%） and contralateral cross-over in 164/620 （26.4%） and pop-liteal retrograde＋femoral antegrade in 23/620 （3.7%）. Techniques included subintimal angioplasty in 427/620 patients （68.8%） and endolu-minal angioplasty in 193/620 patients （31.2%）. The prolonged high pressure balloon angioplasty procedure was successful in 86.2%（minor intra-procedural complications rate 15.7%）, stent implantation was performed in 74 patients （11.9%）, with a significant improvement of ankle-brachial index （0.29 ±0.6 vs. 0.88 ±0.3, P〈0.01） and rutherford class （5.3 ±0.8 vs. 0.7 ±1.9, P〈0.01）, a primary patency rate of 86.7%, restenosis of 18.6%on Doppler ultrasound and a target lesion revascularization of 14.8%at a mean follow-up of 18.1 ±6.4 months （range 1-24 months）. Secondary patency rate was 87.7%. Conclusions Prolonged high pressure angioplasty of femoropopliteal lesions appears to be safe and effective allowing for an acceptable patency and restenosis rates on mid-term.

Amine-basedcarbon-capture technologies have been shown to be energetically expensive and to cause significant environmental and epidemiological impacts due to their *** formation from carbon dioxide's reaction with water has been suggested as an effective alternative for capturing CO_2;however,the thermodynamics of this reaction are not well *** study experimentally determined the equilibrium constant of sodium bicarbonate(Na HCO_3)decomposition to sodium,water,and carbon dioxide;the study also compared the equilibrium constant to theoretical *** a combination of experimentation and thermodynamic relationships,the unitless equilibrium constants of the forward and reverse reactions were calculated accurately(errorrespectively).Equilibrium data were calculated using enthalpy and entropy values of each component of Na HCO_3decomposition at temperatures ranging from 25 to 155°C *** results offer more data essential to optimizing Na HCO_3use in environmentally friendly nextgeneration CO_2-capture technologies.

Left atrial dysfunction in elderly patients with patent foramen ovale and atrial septal aneurysm

Objective recently it has been suggested that, in patients with large patent foramen ovale （PFO） and atrial septal aneurysms （ASA）, a certain amount of left atrial （LA） dysfunction may be active as an alternate mechanism promoting arterial embolism. Following this hypothesis, elderly patients, being more susceptible to atrial chambers stiffness, should present a more severe LA dysfunction profile. We sought to evaluate the grade of LA dysfunction in elderly patients submitted to transcatheter PFO closure. Methods We retrospectively enrolled 28 consecutive patients with previous stroke （mean age 674-12.5 years, 18 females） referred to our centre for catheter-based PFO closure after recurrent stroke. Baseline values of LA passive and active emptying, LA conduit function, LA ejection fraction, and spontaneous echocontrast （SEC） in the LA and LA appendage were compared with those of 50 atrial fibrillation patients, as well as a sex/age/cardiac risk matched population of 70 healthy controls. results Pre-closure elderly subjects demonstrated significantly greater reservoir function as well as passive and active emptying, with reduced conduit function and LA ejection fraction, when compared to healthy and younger patients. After closure in elderly patients, LAparameters did not return completely to the levels of healthy patients, whereas LA dysfunction in younger subjects returned normal. Conclusions This study suggests that elderly patients have more severe LA dysfunction than younger patients, which affects the LA remodelling after closure.

Acute-on-chronic liver failure(ACLF) results from an acute insult that occurs in patients with chronic liver disease,ultimately leading to liver *** underlying chronic liver dysfunction can be characterized into three categories:chronic liver disease with no cirrhosis;compensated liver cirrhosis;decompensated *** underlying chronic liver diseases are numerous and include alcoholic liver disease,viral hepatitis(types B and C),nonalcoholic fatty liver disease,and other underlying etiologies.1 Acute liver injury can arise from viral hepatitis infections(types A,B,and E),acetaminophen toxicity,drug-induced liver injury,toxin exposure,and numerous other hepatic and extrahepatic causes including heat stroke.2 Due to the numerous aspects of pathology that can generate both the chronic and acute components of ACLF,these patients can have significantly different pathology *** makes ACLF difficult to clas-sify,and this is evident by the different ACLF definitions being used by the Asian Pacific Association for the Study of the Liver,the European Association for the Study of the Liver–Chronic Liver Failure Consortium,and the North American Consortium for the Study of End-Stage Liver Disease with regards to mor-tality,presence of organ failure,inclusion/exclusion criteria,and other disease characteristics.3,4 In addition,these differ-ent definitions of ACLF,depending on which definition is used,could potentially influence the outcomes of studies depending upon the metric investigated.
