Objectives:Treatment of metastatic colorectal cancer(mCRC)includes resection of liver metastases(LM),however,no validated biomarker identifies patients most likely to benefit from this *** meta-analysis aimed to assess the impact of the most relevant molecular alterations in cancer-related genes of CRC(i.e.,RAS,BRAF,SMAD4,PIK3CA)as prognostic markers of survival and disease recurrence in patients with mCRC surgically treated by LM ***:A systematic literature review was performed to identify studies reporting data regarding survival and/or recurrence in patients that underwent complete liver resection for CRC LM,stratified according to RAS,BRAF,PIK3CA,and SMAD4 mutational *** ratios(HRs)from multivariate analyses were pooled in the meta-analysis and various adjustment strategies for confounding factors were *** search was conducted in numerous databases,including MEDLINE(PubMed),Embase,Cumulative Index to Nursing and Allied Health Literature(CINAHL)(EBSCO host),and WHO Global Index Medicus,through March 18th,***-analyses,editorials,letters to the editor,case reports,studies on other primary cancers,studies with primary metastatic sites other than the liver,studies lacking specific oncological outcome variables or genetic data,non-English language studies,and studies omitting residual disease data from liver metastasectomy were *** remaining 47 studies were summarized in a descriptive table which outlines the key characteristics of each study and final results were graphically ***:RAS mutation status was negatively associated with overall survival(OS)(HR,1.68;95%CI,1.54–1.84)and recurrence free survival(RFS)(HR,1.46;95%CI,1.33–1.61).A negative association was also found for BRAF regarding OS(HR,2.64;95%CI,2.15–3.24)and RFS(HR,1.89;95%CI,1.32–2.73)and SMAD4 regarding OS(HR,1.93;95%CI,1.56–2.38)and RFS(HR,1.95;95%CI,1.31–2.91).For PIK3CA only three studies were eligible and no significant associati

Adaptive Pinpoint and Fuel Efficient Mars Landing Using Reinforcement Learning

Future unconstrained and science-driven missions to Mars will require advanced guidance algorithms that are able to adapt to more demanding mission requirements, e.g. landing on selected locales with pinpoint accuracy while autonomously flying fuel-efficient trajectories. In this paper, a novel guidance algorithm designed by applying the principles of reinforcement learning(RL) theory is presented. The goal is to devise an adaptive guidance algorithm that enables robust, fuel efficient,and accurate landing without the need for off line trajectory generation and real-time tracking. Results from a Monte Carlo simulation campaign show that the algorithm is capable of autonomously following trajectories that are close to the optimal minimum-fuel solutions with an accuracy that surpasses that of past and future Mars missions. The proposed RL-based guidance algorithm exhibits a high degree of flexibility and can easily accommodate autonomous retargeting while maintaining accuracy and fuel efficiency. Although reinforcement learning and other similar machine learning techniques have been previously applied to aerospace guidance and control problems(e.g., autonomous helicopter control), this appears, to the best of the authors knowledge, to be the first application of reinforcement learning to the problem of autonomous planetary landing.

Subpicosecond pulses in a self-starting mode-locked semiconductor-based figure-of-eight fiber laser

We have experimentally studied the mode-locking dynamics of a polarization-maintaining figure-of-eight laser which has a semiconductor optical amplifier as gain medium. Self-starting mode-locking at the fundamental repetition rate of 18 MHz is obtained at the lasing threshold, and further increasing the bias current leads to the progressive emission of additional optical pulses in each round trip and eventually to mode-locked emission at increasingly high harmonics of the fundamental repetition rate, up to 2.45 GHz. The intensity autocorrelation of the amplified mode-locked pulses has a full width at half-maximum duration of 382 fs, which corresponds to a pulse duration of 247 fs.

This work aims to describe a well standardized therapeutic path in reference to the article “Osteopathy and Emergency: A Model of Osteopathic Treatment Aimed at Managing the Post-Traumatic Stress—Part 1 in the form of a practical guide for manual therapist, Open Journal of Therapy and Rehabilitation, Vol. 10, No. 7, July 2018, https://***/10.4236/health.2018.107074”. The osteopathic manipulative treatment (OMT) approach we propose can be complementary to the psychological therapy in use today, and consists of ten standardized techniques directed to the anatomical structures more involved in the stress answer. This protocol was designed to improve and sustain the management of subjects exposed to extraordinary exogenous stress or post-traumatic stress disorder patients (PTSD).

BACKGROUND Intestinal ischemia has been described in case reports of patients with severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)disease(coronavirus disease 19,COVID-19).AIM To define the clinical and histological,characteristics,as well as the outcome of ischemic gastrointestinal manifestations of SARS-CoV-2 *** A structured retrospective collection was promoted among three tertiary referral centres during the first wave of the pandemic in northern ***,radiological,endoscopic and histological data of patients hospitalized for COVID-19 between March 1st and May 30th were *** diagnosis was established by consecutive analysis of all abdominal computed tomography(CT)scans *** Among 2929 patients,21(0.7%)showed gastrointestinal ischemic manifestations either as presenting symptom or during *** CT showed bowel distention in 6 patients while signs of colitis/enteritis in *** patients presented thrombosis of main abdominal ***,when feasible,confirmed the diagnosis(6 patients).Surgical resection was necessary in 4/21 *** tissue examination showed distinctive features of endothelial inflammation in the small bowel and *** hospital stay was 9 d with a mortality rate of 39%.CONCLUSION Gastrointestinal ischemia represents a rare manifestation of COVID-19.A high index of suspicion should lead to investigate this complication by CT scan,in the attempt to reduce its high mortality *** shows atypical feature of ischemia with important endotheliitis,probably linked to thrombotic microangiopathies.
