Progressive Characteristics of HIV Infection in the Elderly in the Cohort of the Internal medicine Department of the Points G University Hospital, Bamako. mali

Few data are available on HIV infection in the elderly and they are an important population in our different cohorts in view of the improvement in the quality of care over the last ten years. Objective: to determine the clinical and immunovirological characteristics as well as the acceptance of antiretroviral drugs in this patient population. This is a cohort study of the files of PLHIV (People living with HIV) aged 50 years or more, followed in the Internal medicine Department of the Point G University Hospital between January 2007 and December 2012. Results: Out of 161 HIV-infected patients in the cohort, 38 were aged 50 years or older, of whom 11 met the criteria and were included. The median age was 55 years, 90.9% of whom were in the [50 - 59 years] age group, with 63.6% being women (sex ratio = 0.57). At admission, 3 patients (27.3%) had prurigo and 2 (18.2%) had oral-pharyngeal candidiasis. At inclusion, 45.5% of patients were classified as WHO stage 2 and one as WHO stage 4. All were on ARVs, including 10 on 1st line HIV-1 and 1 on HIV-2. On triple therapy, the average weight gain was 5.1 kg at D15 and 6.84 kg at m6. However, at m12 there was a weight loss of 1.04 kg. The mean CD4 T cell gain was +102/mm3 at m6 and +188/mm3 at 12 months. At D0, mean viral load = 565024.75 copies/mm3 [99 - 1100000] in 4/11. At m12, two patients had undetectable viral loads. Conclusion: The prevalence of HIV in elderly subjects is certainly underestimated. Thanks to triple antiretroviral therapy, PLHIV are aging with HIV but screening is not systematically proposed during consultations in elderly subjects. It is necessary to reinforce communication about HIV at all ages.

Investigation on the Use of Local Sand in Burkina Faso as Standard Sand for Testing Compressive Strength of Cement

The aim of this work is to identify a substitute for imported standardized sand for determining the strength class of cements using local sands. To this end, three (03) local sands (manga, Kaya and Dori) were characterized and then used to formulate sand mixtures fitting into the granular spindle of standardized sand noted Sm1, Sm2 and Sm3. Sm1 is composed of 80% manga sand plus 20% Dori sand. Sm2 is a mixture of 80% of the refusal and 20% of the passing through the 0.25 mm sieve of the manga sand. Sm3 is composed of 77% manga sand 0.25 mm sieve refusals plus 23% Kaya sand. These sands were used to characterize a CEm II/B-m (P-L) 42.5 R cement by comparing it with the results of a standardized sand noted SS. Characterization of the local sand showed them to be less dense and more absorbent than the standardized sand. mortars made with Sm1, Sm2 and Sm3 thus have lower workability and bulk densities in the fresh and hardened states than the SS-based mortar, with higher porosities accessible to water after 28 days of curing. mechanical characterization of the cement gave lower compressive strength values with local sands at 2 and 28 days cure. At 2 days cure, local sands gave 17.42 mPa, 14.68 mPa and 17.13 mPa respectively for Sm1, Sm2 and Sm3, compared with 21.12 mPa for standard sand. At 28 days, mortars based on Sm1 and Sm3 sands give higher strength values than the 42.5 mPa required by standard NF EN 197-1, with 44.21 mPa and 43.17 mPa respectively, compared with 49.84 mPa for standardized sand. On this basis, correction factors have been proposed for the use of local sands in cement characterization. They nevertheless require repeatability studies before validation.

management of Urological Emergencies at the Nianankoro Fomba Hospital in Segou: A Case Report of 72 Patients

PURPOSE: Our aim was to present the diagnostic and therapeutic aspects of urological emergencies in a regional hospital. mATERIAL AND mETHODS: We conducted a prospective study over a period of 6 months (April 2021 to September 2021) collecting all the emergencies received by the on-call urology team at the Nianankoro Fomba Hospital in Segou. This team was led by a DES in urology under the supervision of a urological surgeon. We were interested in age, sex of patients, diagnosis, number of patients hospitalised and type of surgery performed in emergency. RESULTS: We registered 72 patients. The mean age of our patients was 58.8 years with extremes of 6 and 90 years. Acute bladder retention was represented in 61% and total haematuria in 24%. Renal colic was reported in 8%. Acute prostatitis was reported in 4% of cases, and acute pyelonephritis in 1%. Urogenital trauma accounted for 6%. CONCLUSION: Although underestimated, in our context, the management of urological emergencies remains a regular activity of the urology department in view of the number of patients managed. Bladder drainage remains the most frequent procedure.

In vehicle dynamics, there are wide applications concerning the simulation of vehicles on roads. These simulation applications relate to vehicle driving, ride comfort and durability. An accurate prediction of simulation results requires reliability and efficiency of road representations. The mATLAB graphical user interface module, called mATLAB GUI, is used to develop virtual simulation laboratories that allow the user to interact with a computer program using graphical objects. In this context, the aim of this article is to use the mATLAB OpenCRG suite of tools and the mATLAB GUI to develop a virtual environment for simulating a 3D road profile. A three-dimensional model of a pothole with variable parameters is developed and integrated into the 3D road profile.

Scrotal Trauma in a motorcycle Accident Leading to a Partial Orchidectomy: Case Report

Scrotal trauma in motorcycle accidents (STmA) is a rare entity in the literature. In general, trauma to the genitals is not life-threatening. However, STmA can cause rupture of the testis with consequences that can lead to alteration of sexuality and reproduction. We present an STmA case managed in a rural hospital where various types of injuries arise from motorcycle accidents. A 20-year-old man without any previous pathology presented in our hospital with a swollen painful scrotum. One week prior to presentation, the patient with a speeding motorbike drove over a speed bump, and the frame of his motorcycle broke into two halves, violently striking his genitals and causing scrotal trauma. After examination and investigation, patient has consented to scrotal exploration which was carried out. A partial left orchidectomy was performed and the right testicle was healthy. The evolution was favorable and the patient was discharged on the 5th postoperative day. Prognosis at 6 months was good. It is possible that genital trauma related to motorcycle accidents will become more frequent in the future. This underscores the importance of educating motorcyclists to respect the Highway Code;and making practitioners aware of the risk of serious injury in the event of acute scrotal trauma.

Africa is the world region that is most affected by malaria. Among the therapies used, injectable quinine is considered to be one of the effective antimalarial drug, however non-quality assured antimalarials clearly have a strong market penetration across Africa. To overcome this problem, it becomes more and more necessary to set up quantitative and qualitative analysis system for antimalarial quality control. The objective of the present investigation is an attempt to use customized multispectral microscope equipped with UV-Visible lasers for injectable quinine quality assurance control routinely. For that, we have established the calibration curve of quinine solution concentration as a function of area under light intensity histogram crossing the solution. From this calibration curve, we can check the conformity of any injectable quinine according to the pharmacopoeia involved. The proposed technique is a promising alternative for drug quality control routinely.

Introduction: Acute otitis media is an acute inflammation of the mucosa of the middle ear cavities. It is often secondary to nasopharyngitis, which favors the passage of infection through the Eustachian tube to the middle ear. The aim of our study was to improve the management of AOm in the Paediatric Department of the Hospital National Ignace Deen (Conakry). Patients and methods: This was a prospective descriptive study lasting 6 months from 01 July to 31 December 2011;the study covered 525 cases out of a total of 6276 children, i.e. a frequency of 8.36%. Results: The most affected age group was 6 to 11 months. males predominated (69.71%). 82.29% had a history of recurrent rhinopharyngitis. The most frequent reason for consultation was incessant crying (66.29%). Rhinopharyngitis and malaria were the most commonly associated pathologies (87.62% and 39.62% respectively). 72.19% of our patients were admitted with congestive AOm and received medical treatment. We recorded one case of otomastoiditis which was treated surgically. Conclusion: AOm is more common in children aged between 6 and 24 months. Good collaboration between paediatricians and ENT specialists is essential to reduce the morbidity of AOm.

The uterus bicorne is a uterine malformation whose pathophysiology is known but the etiology remains obscure. Few studies have been interested in this subject. This is the clinical observation of mrs. X 26-year-old housewife nulliparous who consulted for repeated late spontaneous abortions. The clinical examination was normal. The ultrasound revealed a double uterine cavity in Y, in favor of a unicervical bicorne uterus. At hysterosalpingography, the tubes were permeable and the uterus doubled with a single cervix. The treatment consisted of a uterine plasty. The postoperative course was simple. The patient gave birth to a newborn male 20 months after surgery.

Groundwater is an essential resource of drinking water in Fu Xian and of whole Yan An. Recently in Fu County;we have seen an increase in the population（Density（/km2）=38） resulting the increased demand for drinking water and can make some environmental problems such surface waters pollution which can also contaminate groundwater. In addition, groundwater once polluted it is difficult to clean. For forestalling groundwater pollution and making respect the existent narrow line between human and its environment, as said by Rio Declaration on Environment and Development 1992, we carried out a series of research in Fu County. A research has taken around 2 years, from 2013 to *** main purpose of this research is to prevention and assessment of the groundwater, and this study will achieve to the following objectives: Providing critical analysis and interpretation of existing data, estimating the risks of contamination, characterize the aquifer system in the aspect of the distribution and evolution of their pollution, and recommendation for improvement of groundwater in Fu *** achieve our goals, we approached the research by using the following scheme which mainly includes three（3） aspects:? Problem definition and data collection? modelling? Objective: to report our experience in the management of adult kidney cancer. Patients and methods: This was a prospective and descriptive study, carried out in the Urology Department of Gabriel Touré University Hospital, during the period from January 1, 2001 to December 31, 2017 for a period of six years, interesting all the medical records of 24 adult patients diagnosed with kidney cancer. Results: During the study period, 24 patients met our inclusion criteria;the mean age was 48.23 ± 8.02 years with extreme ranging from 17 to 82 years. most of our patients were female (58.3%), and were referred or consulted for lumbar mass in 66.7% of cases. The average tumor size was 13.6 ± 5.8 cm with extremes of 7.8 to 21.1 cm. management included total nephrectomy in 50% and total nephrectomy expanded in 50% of cases. The histological type found was renal cell carcinoma in 23 patients and papillary carcinoma in one patient. Conclusion: Kidney cancer is rare in our context, its clinical and histological characteristics remain the same for two decades. Its management is largely based on enlarged total nephrectomy.
