The main objective of this study is the control of the agricultural greenhouse in view of the economic interest generated by such an activity. A simulation model is developed, gathering all the external and internal climatic conditions that influence the microclimate of the greenhouse to predict the temporal evolution of the state variables characterizing this microclimate. The fuzzy control is an alternative to the approaches proposed by the automatic for the control of complex systems. The perform.n.e objectives of the looped systems and the corresponding actions are summarized in the form of rules of expertise, which are spelled out in plain language. This technique thus makes it possible to dispense with the use of mathematical models which are sometimes difficult to obtain. Our objective is the multivariable strategy synthesis and the fuzzy application to a multivariate system （mImO ~ such as the agricultural greenhouse.） First, the principles of fuzzy logic and fuzzy control are recalled. The origins of non-Linearitys of the comm.n. are explained. One of the practical problems of this technique is the combinatorial explosion of the rule base when the number of variables involved becomes large. A solution to simplify the complexity of the system is presented together with an optimization algorithm to automatically adjust the parameters of the fuzzy controller. The last part is devoted to the synthesis of an optimal control of the greenhouse in order to compare it to the fuzzy control implem.n.ed.

Background and Study Aims: Several endoscopic antireflux therapies have been m arketed, but long-term data on their objective and clinical efficacy are sparse . This report presents prospective 1-year follow-up results, including technic al, clinical,and functional success rates, for the first of these treatm.n.s to be developed, endoscopic gastroplication (EGP). Patients and methods: A total of 43 EGP procedures were carried out in 38 patients with gastroesophageal reflux disease (GERD).Two or three EndoCinch gastroplications were constructed at the l evel of the gastric cardia in each patient; five patients were treated twice wit hin 6-12 m.n.hs. Each endoscopic suture joined two gastric folds to each other as a double fold, known as a “gastroplication", in order to narrow the esophago gastric junction. Postprocedure data after 2 m.n.hs and after 1 year were compar ed with preoperative data, focusing on symptoms,medication requirem.n.s, endosco pic findings, and pH-metry results. Results: In contrast to the findings at 2 m onths (which showed that 72 %of the sutures were present and that there was a r eduction in the percentage of time when the esophageal pH was m 15.4%to 8.7%), the results 1 year after EGP were considered to indicate failure of the treatm.n. in all 38 patients because none of them still had all of the initially placed gastroplications in situ (90%of gastroplications werelost). The percent age of patients who did not require proton pump inhibitor medication decreased f rom 52%at 2 m.n.hs to only 20%at 1 year and even more patients had evidence of re-flux esophagitis at 1 year (56%) than had initially dem.n.trated signs of this (41%). Conclusions: EGP has some short-term beneficial effects on clinica l symptoms and pH-metry. However,mainly due to the loss of the endoscopically p laced sutures,these effects were not maintained at the 1-year follow-up. EGP c an not therefore be recomm.n.ed for routine clinical use. Objective: To exam.n. the evidence for efficacy of surgery in the treatm.n. of intractable epilepsy. method: meta analysis of randomized clinical trials. Res ults: One randomized clinical trial (n=80) and 3 observational studies are avail able. Interpretation: Compared to medical treatm.n., the surgery increases the p roportion of patients who were free of seizure imparing awareness at 1 year (rel ative risk: 7.67, IC 95%2.5 `a 23.51) Conclusion: The available evidence abou t the clinical efficacy of surgery for patient with intractable epilepsies is li mited to an unique open randomized clinical trial, that enrolled patient in a si ngle centre, without concealm.n. of the randomization and with a partially subje ctive primary edpoint. However, this trial shows a substantial effect correspond ing to a 8 fold incrase in the number of patient without seizure at 1 year. The informational situation will be better whent the results of a currently ongoing trial will be available.

In patients with anti-Yo associated paraneoplastic cerebellar degeneration (P CD) neurological symptoms precede the diagnosis of the underlying cancer in abou t 60%. Ovarian carcinoma, breast cancer and other gynaecological malignancies a re most frequently found as causative malignancies. Antitumour treatm.n. should be applied in an early stage of disease. The identification of the tumour is a d iagnostic challenge in m.n. of these patients. In the first of two patients repo rted here a pelvic tumour was suggested after detection of a pathological lymph node and elevated tumour markers. The intraoperative findings appeared macroscop ically normal during ovariectomy with adnexectomy. not until microscopic exam.n. tion of the resected tissue was performed was a tubal adenocarcinoma found. If i ntrapelvic gynaecological tumours are suspected a deliberate surgical exploratio n seems to be justified, but only after an intensive diagnostic investigation. T o search for the underlying cancer in patients with paraneoplastic neurological disorders successive CT and[18F]-FDG-PET are widely recomm.n.ed. Instead of th is in the second reported patient whole-body dualmodality PET/CT was performed revealing enhanced uptake in three regions of the left thorax. By combining func tion and anatomy PET/CT was able to localise the lesions and characterise them a s lymph node metastases of breast cancer. Diagnosis could be confirmed by subseq uently executed needle biopsy. PET/CT seems to be highly applicable in the inves tigation of paraneoplastic disorders with unknown primary cancer. It may help in guidance of needle biopsy or to optimise the results of deliberate surgery and it provides whole-body tumour staging in a single session with higher diagnosti c accuracy than PET alone.
