Background: Transurethral resection of prostate (TURP) is the gold standard in the surgical treatment of symptomatic benign prostatic hyperplasia (BPH). Blood loss is one of the most common complications of TURP. Objective: To evaluate the effect of preoperative dutasteride on bleeding related to TURP in patients with BPH. materials and methods: This prospective interventional study was done in the department of urology, Dhaka medical College Hospital, Dhaka, Bangladesh during the period of July 2016 to June 2017. A total of 70 cases of BPH planned for TURP were included in this study according to the statistical calculation. Patients were randomly allocated to control group A (TURP without dutasteride) and dutasteride group B (TURP with dutasteride). Each group consisted of 35 patients. Group B patients were treated with dutasteride 0.5 mg/day for 4 weeks before TURP. The main outcome of blood loss was evaluated in terms of reduction in serum hemoglobin (Hb) and hematocrit (Hct) levels, which were measured before and 24 hours after surgery. Data were analyzed and compared by statistical tests. Results: Comparison of outcome between groups shows that there was a significant difference in term of pre-post operative change of hemoglobin and hematocrit levels in the control group A compared to the dutasteride group B (Hb = 2.96 ± 0.80 gm/dl vs. 1.81 ± 0.71 gm/dl, respectively, p = 0.001;Hct = 11.20% ± 2.12% vs. 6.07% ± 2.02%, respectively, p = 0.02). A significant lower mean blood loss was observed in the dutasteride group compared to the control group. Conclusion: Preoperative dutasteride therapy reduces blood loss related to TURP in patients with BPH. This therapy can be practiced to reduce surgical bleeding associated with TURP.

Smartphone ownership among adolescents is getting common in this decade especially in malaysia;Adolescent are strongly devoted to their smartphone and this may lead to smartphone *** have reported that smartphone addiction has become an emerging social and health problem especially among the youth in many countries however there is lack of study among adolescents in *** study aimed to examine the prevalence and factors associated with smartphone addiction among adolescents in *** was a cross-sectional study involving adolescents from 15 primary care clinics throughout the *** were assessed on their smartphone activities using the malaysian short version of the Smartphone addiction scale(SAS-m-SV).multiple logistic regression was used to determine the predictors of smartphone addiction among *** study was conducted among 921 adolescents with 49.6%male(n=457).The mean age of adolescents was 16.4±2.4 *** ethnicity distribution were 74.6%malay,7.3%Chinese,4.7%Indian and 13.4%other *** prevalence of smartphone addiction was 37.1%(342/921);37.4%in male and 36.9%in *** on multiple logistic regression analysis,longer duration of smartphone use per week was associated with higher odds of smartphone addiction among adolescent(odd ratio=1.005%,95%confidence interval=1.000–1.009,p-value=0.039).Smartphone addiction is present in nearly four in ten adolescents in *** who spend longer duration in smartphone usage per week were associated with higher odds of having smartphone *** should be more alert and vigilant about this ***,parents should limit their children from spending too much of time with smartphone in order to prevent their children from getting smartphone addiction.

Epidemiological Profile of Chronic Kidney Disease at the General Hospital of National Reference of N’Djamena (Chad)

Introduction: Chronic renal failure is a disease that affects many patients worldwide and increasingly in Africa. At the end of 2003, about 1.1 million people were suffering from End-Stage Renal Disease (ESRD) and were treated with periodic dialysis [12]. In Africa, CKF represents 2% to 10% of hospital admissions and is responsible for 4% to 22% of deaths [14]. So, this study is conducted for the first time in Chad, with the aims to determine the prevalence of CKD. methods: This was a retrospective, descriptive and analytical study over a period of 12 months from April 29, 2011 to April 28, 2012. All patients with chronic renal failure regardless of etiology and stage of chronic kidney disease were included in the study. Chronic renal failure was defined as a glomerular filtration rate below 60 ml/min/1.73m (mDRD) for more than 3 months. This study was conducted in several departments of the National General Reference Hospital (NGRH) of N’Djamena. Result: Among 2039 inpatients, 195 patients had chronic renal failure, as a frequency of 9.6%. The average age of our patients was 51 ± 16.8 years, ranging from 11 to 85 years. male predominance was noted to be 59% of men against 41% of women. We noted that high blood pressure accounted for 66.2% (N = 129) of cases, diabetes in 48.2% (N = 94), alcoholism in 28.7% (N = 56), smoking in 14.9% (N = 29) and the association alcoholism-smoking in 19.5% (N = 38). Hypertension was the leading cause of chronic renal failure (66.2%). All patients had a serum creatinine and creatinine clearance was assessed. Among them, we noted 57 patients (29%) with end-stage renal failure. The average calcium and phosphate serum were 1.8 mmol/l and 1.6 mmol/l, respectively. We noted that 120 patients as 61.5%, currently took herbal medicine. 48 out of 57 of our patients with ESRD as 24.6% of patients in the study had received replacement therapy (hemodialysis) with 12.5% of deaths. Background: Since the outbreak of the COVID-19 pandemic caused massive case fatalities across the world, people have been left with significant physical and mental disabilities, which has had an adverse impact on their quality of life. The objective of this study is to measure the short-term and long-term outcomes among COVID-19 survivors. methods: This is a six-month-long multicentre prospective observational study, carried out in four specialized hospitals in the capital city Dhaka, where six hundred participants were enrolled by non-probability convenience sampling. Data were collected through three structured interviews, and follow-ups were done during discharge, at 1st month, and 6th month. An analytical study was done on demographic variables, socio-economic conditions, physical findings and outcomes. Data were analysed using Statistical Package for Social Sciences (SPSS). Results: A total of 600 participants were enrolled. The mean age was 49.83. many participants belong to the age range of 31 to 40 years. more than two-thirds (72.7%) of participants were male, whereas 27.30% were female. most patients admitted had mild and moderate COVID-19 symptoms (40% and 58%, respectively). Among short-term COVID complications, it was found that 43.3% of the participants complained of fatigue, 32.6% of depression, 24.7% of sleep disturbance, 19% of anxiety and, 5% memory loss. In long-term COVID complications the number reduced significantly: 5% of the participants complained of fatigue, 10% of depression, 2.7% of sleep disturbance, 7.3% of anxiety and, 2.7% memory loss. The severity of lung parenchymal disease also reduced in long term COVID symptoms. The study found a statistically significant relationship between age groups and CT severity index (χ2 = 9.458, p = 0.032). most patients (29.2%) in the under-30 age group had a CT Severity Index score of 2 & 3 (29.2%). 背景:马来西亚,沙巴,Kota Kinabalu及其周边社区。目的:了解影响抗结核化疗病人依从性的诸因素,以及他们的结核病知识水平及和DOTS管理的改进意见。设计:对2000年8月至9月期间到诊所DOTS治疗的合作及不合作病人进行家庭访视。结果:共访问了63例合作病人和23例不合作病人。对于不合作者,他们去治疗中心的花费(P<0.005)及旅途时间(P<0.005)均比合作者明显要多。交通费常常是不去治疗的主要原因。不合作病人中完成中等教育的多(P<0.05),并且有工作的也多(P<0.01)。更多的不合作者中其家庭成员曾患过本病(P<0.01)。有关结核病知识的得分,两组间无差别;但是,不合作病人大多认为,当症状消失后便可停止治疗(P<0.01)。很多病人(73％)认为,通过提供更多的结核病知识可改善DOTS管理体系。结论:赴治疗中心的交通费、占用的时间及家庭成员曾患过结核病等因素对沙巴,Kota Kinabalu地区病人的DOTS依从性产生影响。病人愿意获得更多关于结核病的知识。

Background: Urosepsis is one of the most common infections that require empirical broad spectrum antibiotics immediately after diagnosis. This has led to development of bacterial resistance by acquiring the capability to destroy the β-lactam ring. methodology: This is a cross-sectional hospital-based study. The study was conducted from 2019 to 2020 at Gezira Hospital for Renal diseases and surgery (GHRDS). A hundred patients were diagnosed clinically with urosepsis and the isolated organisms were Escherichia coli, Staphylococcus aureus, Proteus mirabilis, Klebsiella pneumonia and Pseudomonas aeruginosa. The susceptibility test was conducted by Kirby Bauer disc diffusion technique according to clinical laboratory standard institute (CLSI) guidelines. Seventy eight samples of bacterial genomic DNA were confirmed by 16srRNA and multiplex PCR, were performed for genotypic blaOXA-51 and blaOXA-23 gene characterization of isolated bacteria. Then gel electrophoresis was used to identify the presence or absence of (blaOXA-51 and blaOXA-23) genes. Results: 88.5% (69/78) in 16srRNA detected. Using multiplex PCR, the frequencies of blaOXA-51 and blaOXA-23 genes were 13% and 10.1%, respectively. The percentages of isolates which yielded both blaOXA-51 and blaOXA-23 among P. aeruginosa was 25% (1/4), among K. pneumonia was 17% (1/6), and among E. coli was 8% (3/37). Only blaOXA-51 was detected in P. mirabilis 10% (1/10) and only blaOXA-23 was detected in S. aureus 5% (1/18). Conclusion: In this study, the presence of blaOXA-51 and blaOXA-23 genes was increased in the isolated bacteria.

AST3-2(Antarctic Survey Telescopes)光学巡天望远镜位于南极大陆最高点冰穹A,其产生的大量观测数据对数据处理的效率提出了较高要求.同时南极通信不便,数据回传有诸多困难,有必要在南极本地实现自动处理AST3-2观测数据,进行变源和暂现源观测的数据处理,但是受到低功耗计算机的限制,数据的快速自动处理的实现存在诸多困难.将已有的图像相减方案同机器学习算法相结合,并利用AST3-22016年观测数据作为测试样本,发展一套的暂现源及变源的筛选方法成为可行的选择.该筛选方法使用图像相减法初步筛选出可能的变源,再用主成分分析法抽取候选源的特征,并选择随机森林作为机器学习分类器,在测试中对正样本的召回率达到了97%,验证了这种方法的可行性,并最终在2016年观测数据中探测出一批变星候选体.

由于环保和效率等原因混合电动车辆(HEVS)已经具有很大的吸引力。一个典型的HEV由两个动力系统:一个是普通的动力源如一台汽油机、一台柴油机或一组燃料电池和一个电动系统(包含一台电动机和一台发电机)组成,它可能产生比普通车辆高的燃油经济性。而且,这样的车辆不需要外部充电,而在其内部存在燃料基本设施。HEV的功率分流动力系统构造的独特的HEV动力系统结构串联和并联型式的优点。但是操纵功率分流HEV动力系统要求一个完善的控制系统。设计这样的控制系统要有适当精确的HEV装置的平面模型。对于串联和并联型式的大量研究已经开发了许多动力平面模型。但一个完整的和有效的功率分流HEV动力系统的动力模型尚处于初期阶段。本文提出了在不同传动条件下可实际地和重复所有静态和瞬态现象的功率分流动力系统HEV的动力模型。首次表明了数学推导和这种平面模型和构件的表达,其次是通过计算机仿真分析、验证及性能鉴定和试验车辆在Ford公司性能道路试验实测进行比较。该模拟和实验结果的极好的吻合证实所引导的平面模型的精确性和正确性。因为该平面模型是综合用型谱方法论的系统定向子系统建立的,它很容易改变子系统的功率。开发该平面模型是用来分析和了解动力系的控制动力学,子系统和构件间的干扰以及由于工况和波动的影响的变化的系统瞬变。该平面模型也可用于车辆系统控制器的开发,能量管理战略的估价,结果的解析,编码算法的证明,及其中其他许多目的。
