培美曲塞加铂类联合或不联合帕博利珠单抗对初治转移性非鳞状NSCLC患者的疗效与安全性分析：KEYNOTE-189研究方案指定的最终分析

背景在KEYNOTE-189此项Ⅲ期研究(NCT02578680)中,与安慰剂联合培美曲塞加铂类相比,帕博利珠单抗联合培美曲塞加铂类化疗(培美曲塞-铂类)显著改善了初治转移性非鳞状非小细胞肺癌(non-small cell lung cancer,NSCLC)患者的总生存期(overall survival,OS)和无进展生存期(progression-free survival,PFS)。本研究报告了研究方案指定的最终分析的疗效结局,包括从培美曲塞-铂类交叉至帕博利珠单抗治疗的患者,以及完成35个周期(约2年)帕博利珠单抗治疗组的疗效。方法符合条件的患者以2:1随机分配至帕博利珠单抗,每3周一次,200 mg (n=410)或安慰剂(n=206)(共35周期,约2年),加上4个周期的培美曲塞(500 mg/m~2)和由研究者选择的每3周顺铂(75 mg/m~2)或卡铂(曲线下面积5 mg/min/mL)治疗,随后使用培美曲塞直至疾病进展。若符合纳入标准,分配至安慰剂联合培美曲塞加铂类治疗的患者可在疾病进展时转向帕博利珠单抗治疗。主要研究终点为OS和PFS。结果中位随访31.0个月,无论PD-L1的表达水平如何,与安慰剂联合培美曲塞-铂类治疗相比,帕博利珠单抗联合培美曲塞-铂类治疗可使患者的OS[风险比(hazard ratio,HR)为0.56;95%可信区间(confidence interval,CI) 0.46-0.69]和PFS (HR=0.49;95%CI:0.41-0.59)显著获益。接受帕博利珠单抗联合培美曲塞-铂类治疗的患者客观缓解率(objective response rate,ORR)(48.3%vs 19.9%)和第二次/后续无进展生存期(PFS2;HR=0.50;95%CI:0.41-0.61)均得到改善。安慰剂联合培美曲塞-铂类组的84例患者(40.8%)交叉至帕博利珠单抗治疗组。72.1%的接受帕博利珠单抗联合培美曲塞-铂类的患者和66.8%的接受安慰剂联合培美曲塞-铂类的患者发生了3级-5级不良事件。56例患者完成了35个周期(约2年)的帕博利珠单抗治疗;ORR为85.7%,53例(94.6%)患者存活至数据截止时间。结论与安慰剂联合培美曲塞-铂类治疗相比,帕博利珠单抗联合培美曲塞-铂类可持续提高疗效,且安全性可控。该研究结果支持将帕博利珠单抗联合培美曲塞-铂类作为初治转移性非鳞状NSCLC患者的一线治疗方案。

过去十年内，转移性非小细胞肺癌（metastatic non-small-cell lung cancer, m NSCLC）的治疗取得了关键性进展，大大提高了患者的生存率。尤其是一线/二线治疗策略中针对程序性死亡受体1(programmed cell death protein 1, PD-1)、程序性死亡配体1(programmed death ligand-1, PD-L1)和细胞毒性T淋巴细胞相关蛋白4受体（cytotoxic T-lymphocyteassociated protein 4, CTLA-4）等的特异性抗体惊艳亮相，使部分患者生存期明显延长。尽管抗PD-1和PD-L1疗法的免疫检查点抑制剂（immune checkpoint inhibitor, ICI）起初主要是作为二线治疗的单药治疗，但最近已转向作为晚期一线治疗的联合疗法，早期肺癌也已将免疫治疗纳入临床范式。如今，除了具有靶向基因突变的m NSCLC外，几乎所有的m NSCLC患者都在一线接受PD-1或PD-L1治疗。在此，我们报告了m NSCLC一线免疫治疗的现状以及当前为患者选择最佳免疫治疗方案所面临的挑战。

对人流感病毒A/Udorn/72（H3N2）株与禽类流感病毒A/mallard/NY/78/（H2N2）重组后的重组株分析表明,仅含禽类病毒的核蛋白（NP）或膜蛋白（m）的RNA片段的重组株,在松鼠猴的呼吸道繁殖是受限制的。另外。仅有禽类的RNAl和NS基因的重组株（Clone 12）在松鼠猴的气管内的繁殖也明显受限制,而只具有其中一个基因的Clone 9, Clone 2, 则限制就不明显。由此表明,禽类流感病毒的NP和m基因在宿主范围的繁殖限制中起主要作用,而RNAI和SN基因的结合,同样起着繁殖受限制作用。

Development of a novel in vivo lung perfusion(IVLP)procedure allows localized delivery of high-dose doxorubicin(DOX)for targeting residual micrometastatic disease in the ***,DOX delivery via IVLP requires careful monitoring of drug level to ensure tissue concentrations of this agent remain in the therapeutic window.A small dimension nitinol wire coated with a sorbent of biocompatible morphology(Bio-SPmE)has been clinically evaluated for in vivo lung tissue extraction and determination of DOX and its key *** in vivo Bio-SPmE-IVLP experiments were performed on pig model over various(150 and 225 mg/m^(2))drug doses,and during human clinical *** patients with metastatic osteosarcoma were treated with a single 5 and 7 μg/mL(respectively)dose of DOX during a 3-h *** both pig and human cases,DOX tissue levels presented similar trends during *** lung tissue concentrations of drug ranged between 15 and 293 μg/g over the course of the IVLP *** addition to DOX levels,Bio-SPmE followed by liquid chromatography-mass spectrometry analysis generated 64 metabolic features during endogenous metabolite screening,providing information about lung status during drug ***-time monitoring of DOX levels in the lungs can be performed effectively throughout the IVLP procedure by in vivo Bio-SPmE chemical biopsy ***-SPmE also extracted various endogenous molecules,thus providing a real-time snapshot of the physiology of the cells,which might assist in the tailoring of personalized treatment strategy.

This article presents and analyses the modular architecture and capabilities of CODE-DE(Copernicus Data and Exploitation Platform–Deutschland,***),the integrated German operational environment for accessing and processing Copernicus data and products,as well as the methodology to establish and operate the *** march 2017,CODE-DE has been online with access to Sentinel-1 and Sentinel-2 data,to Sentinel-3 data shortly after this time,and since march 2019 with access to Sentinel-5P *** products are available and accessed by 1,682 registered users as of march *** this period 654,895 products were downloaded and a global catalogue was continuously updated,featuring a data volume of 814 TByte based on a rolling archive concept supported by a reload mechanism from a long-term *** November 2017,the element for big data processing has been operational,where registered users can process and analyse data themselves specifically assisted by methods for value-added product *** 195,467 core and 696,406 memory hours,982,948 products of different applications were fully automatically generated in the cloud environment and made available as of march *** features include an improved visualization of available Sentinel-2 products,which are presented within the catalogue client at full 10 m resolution.
