Patients with decompensated hepatic cirrhosis may present systemic alterations and dysfunction of multiple organs. Ascites, portal hypertension, esophageal varices, and hepatic encephalopathy are common complications arising from cirrhosis. The aim of this paper is to report a case of a patient with liver cirrhosis and the complications of a transjugular intrahepatic portosystemic shunt. Male, elderly, and ex-alcoholic, diagnosed with liver cirrhosis, ascites, and esophageal varices. He underwent transjugular intrahepatic portosystemic shunt due to portal hypertension and returned to the hospital after 24 hours with agitation and mental confusion. He had a bowel movement stop, neurological worsening, loss of renal function, hepatic hydrothorax, hepatic encephalopathy, hypernatremia, hypocalcemia, and hypophosphatemia. He underwent a new procedure to occlude the transjugular intrahepatic portosystemic shunt, showing improvement of the mental status and ascites. However, continued with decompensation and hydro-electrolytic disorders. He evolved with worsening of the ventilatory pattern, and neurological and renal function, with a fatal outcome. mily:Verdana;">Esophageal varices due to portal hypertension can be corrected with the transjugular intrahepatic portosystemic shunt. However, complications such as hypernatremia, hepatic hydrothorax, and hepatic encephalopathy may occur. Therefore, there is a need for reintervention to shunt or reduce its caliber. Thus, for patients with advanced age and decompensated cirrhosis, the potential risks and benefits of this procedure should be carefully evaluated due to the risk of complications and death.

Lymphomas are neoplastic transformations that affect lymphoid cells. Diffuse large B-cell non-Hodgkin’s lymphoma has a high degree of cell proliferation, accounting for 30% of all lymphomas. Lung cancer is the leading cause of death worldwide and the recommended treatment is chemotherapy. Among the main complications resulting from non-Hodgkin’s lymphoma, lung cancer and chemotherapy used in their treatment, we can mention sepsis, acute kidney injury and febrile neutropenia. Febrile neutropenia can occur by suppressing the production of neutrophils. Sepsis, a widespread infection, is the main cause of acute kidney injury, which can also be caused by hydroelectrolytic complications or by nephrotoxicity. This is a report of a smoking patient with metastatic lung cancer who sought care due to progressive dysphagia, cough with chest pain, fever, and lower airways critical obstruction due to mediastinal lymphadenopathy, being diagnosed with diffuse large B-cell non-Hodgkin’s lymphoma. The patient evolved to death because of a significant worsening of the ventilatory pattern of multifactorial cause, mainly due to sepsis, acute kidney injury, and febrile neutropenia. The patient had mostly classic characteristics of her comorbidities, however, the overlapping of interrelated comorbidities led to the outcome of death. What is unusual about the present case report is that the patient’s characteristics, such as age, sex, and ethnicity, are opposite to those described as risk factors for diffuse large B-cell non-Hodgkin’s lymphoma.

AIm: To increase inspiratory muscle strength and improve the quality of life of candidates for liver ***: Twenty-three candidates for liver transplantation participated in the control group and 14 made up the intervention group. The control group consisted of 18 men and 5 women, body mass index(BmI) 27.3 ± 4.5 kg/m2 and model for End-Stage Liver Disease(mELD) 18.2 ± 6.1. The intervention group consisted of 11 men and 3 women, BmI 28.6 ± 5.4 kg/m2 and mELD 18 ± 4.5. The presence or absence of ascites was identified in the first patient evaluation and after three months. We evaluated maximal inspiratory pressure(mIP) and maximal expiratory pressure, spirometry, root mean square(RmS) of diaphragm and rectus abdominis, and the quality of life. The exercises were performed daily by patients at home for three months and were supervised at distance monthly. The manual consisted of diaphragmatic breathing exercises, diaphragmatic isometric exercise, Threshold ImT®, lifting upper limbs with a bat and strengthening the abdomen. RESULTS: There was significant difference(P = 0.01) between the first(initial) and the third month(final) mIP in the control group and in the intervention group, but there was no difference(P = 0.45) between the *** RmS of the diaphragm was lower(P = 0.001) and the functional capacity was higher(P = 0.006) in the intervention group compared to the control. The general health and mental health domains received higher scores after three months in the control group(P = 0.01) and the intervention group(P = 0.004), but there was no significant difference between them. The comparison between the presence of initial ascites with the presence of ascites was performed after three months in the control group(P = 0.083) and intervention group(P = 0.31). New and compiled detrital zircon U-Pb ages from the southern Neoproterozoic-Cambrian Ribeira Belt,SE Brazil,demonstrate Laurentian affinity of the Embu Terrane which is statistically distinct from the adjoining Apiaíand São Roque terranes with cratonic affinity(e.g.,São Francisco Craton).Zircon provenance results indicate that the type-area of the Embu Terrane is dominated by detrital zircon age modes at ca.1200 ma,1400 ma,and 1800 ma,with maximum depositional age of ca.1000 *** contrast,the Apiaíand São Roque terranes are dominated by Paleoproterozoic detrital zircon ages(ca.2200-2000 ma age dominant component),with maximum depositional ages of ca.1400 ma and 1750 ma,*** scaling(mDS)analysis of non-parametric similarity measurements on zircon age populations indicates for the first time that the Embu Terrane encompass two statistically distinct detrital zircon age spectra,which is also reflected in the metamorphic zircon age *** statistical characterization of the Embu Terrane through populational metrics allow a quantitative comparison with surrounding tectonic domains and rock samples classified such as *** results clearly highlight the distinction between the statistically differentiated Embu Terrane from the Apiaíand São Roque terranes,supporting an allochthonous *** addition,we demonstrate that rocks samples previously classified as Embu-type are significantly dissimilar to the definition of Embu Terrane,failing to support alternative tectonic models(e.g.,intracontinental evolution).Detrital zircon age spectra reveal that the Apiaíand São Roque terranes have similar zircon provenance to domains sourced from the São Francisco Craton,whereas detrital zircon populations from the Embu Terrane have greater affinity with SW Laurentia basins(and their inferred sediment sources),consistent with previous ***,we interpret the Embu Terrane as a Rodinia descendant developed along the active margin o

Purpose: To evaluate the influence of age, sex, race, refractive error and opt ic disc topography on the sensitivity and specificity of scanning laser polarime try (SLP) in the diagnosis of glaucoma. methods: A total of 88 normal individual s and 95 glaucoma patients were included in this study. Glaucoma was defined on the basis of both optic nerve damage and visual field defects. Scanning laser po larimetry, optic disc topography, automated perimetry and refractometry were per formed in all subjects. The sensitivity and specificity of SLP were assessed app lying a previously calculated cut-off to a previously described linear discrimi nant function (LDF). Results: The sensitivity and specificity of SLP in the stud y population were 82%and 83%, respectively. Sensitivity and specificity were n ot affected by age, sex, race, average disc diameter or disc area. The sensitivi ty of SLP tended to be higher in myopes (93%) than in emmetropes (80%) and hyp eropes (71%) (P=0.08). Sensitivities were higher in individuals with cup areas >0.96 mm2(89%), rim areas ≤1.36 mm2 (92%), and cup area/disc area ratios >0.4 5 (89%) (P 0.96 mm2 and a rim areamm2 significantly increased the sensitivity of the LDF, whereas a cup area/disc area ratio ≤0.45 significa ntly increased the specificity of the LDF. Conclusion: The sensitivity and speci ficity of SLP may be influenced by refractive error and optic disc parameters th at are affected by glaucomatous damage (cup area, rim area and cup area/disc are a ratio). These parameters must be considered in studies evaluating the sensitiv ity and specificity of optic nerve/retinal nerve fibre layer imaging technologie s.
