Probable benign paroxysmal positional vertigo, spontaneously resolved:Incidence in medical practice, patients’ characteristics and the natural course

Background: Probable benign paroxysmal positional vertigo, spontaneously resolved (pBPPVsr), is a variant of benign paroxysmal positional vertigo (BPPV) in which there is no observable nystagmus and no vertigo with any positional maneuver. Objectives: To calculate the incidence pBPPVsr, compare the characteristics of the patients with pBPPVsr and BPPV not spontaneously resolved and describe the spontaneous resolution in the natural course of BPPV. methods: multicenter prospective descriptive study. During a one-year period, all patients with suspected BPPV that presented to the Neurotology Units of five participating centers were recruited. The incidence of pBPPVsr was calculated as a percentage of the total number of patients with BPPV. The prevalence of several variables was compared between pBPPVsr and BPPV not spontaneously resolved. The timing of spontaneous resolution was estimated using Kaplan-meier curves. Results: 457 patients met the inclusion criteria. The incidence of pBPPVsr was 33.5%. It was significantly higher in males, in patients with normal bone mass and in patients who were not taking sulpiride. A rate of 18% of spontaneous resolution after the first month and 51% after the first year was found. This percentage did not change in a significant way after this moment. The curves for males, patients under 50 and patients with normal blood pressure decreased significantly faster. Conclusions: In our serie, BPPV spontaneously resolved in half of the patients with BPPV during the first year. This seemed to occur more commonly in males and could have been hindered by sulpiride intake, osteoporosis, advanced age and high blood pressure.

A review of enhanced oil recovery(EOR)methods applied in Kazakhstan

Application of enhanced oil recovery methods in Kazakhstan has been ongoing for decades alongside the continued discovery of new oil and gas fields in the Pre-Caspian *** objective of this review is to provide an overview of the hydrocarbon reserves and production,and the status of the petroleum industry in Kazakhstan,with a focus on the EOR methods and projects being applied to recover those reserves.A summary of the specific EOR methods in use was prepared,and existing enhanced recovery projects in Kazakhstan were reviewed and their successes and challenges were *** performance of these projects in the context of EOR performance indicators such as capillary number and mobility ratio,as well as operational and environmental issues,were *** for current and potential applications of EOR in Kazakhstan were also *** widespread application of thermal EOR methods,in use for decades in Kazakhstan’s older fields,was found to be successful,with very favorable impacts on mobility ratio from the addition of thermal energy to the *** EOR methods in Kazakhstan have had more limited success,with some significant challenges due to high concentration of hydrogen sulfide in the injected *** injection started in the late 1960s,achieving good results.A recent polymer injection pilot project has shown some promise,with a favorable impact on mobility ratio and oil production,although the project has not yet been expanded beyond two polymer *** results indicate the huge potential of existing and future EOR *** review is the first compilation of Kazakhstan’s existing oil and gas reserves,production,and EOR project performance,and should be seen as a guide to the existing applications of EOR methods in Kazakhstan.

Ultrasound shear wave elastography and liver fibrosis: A Prospective multicenter Study

AIm To assess the accuracy of shear wave elastography(SWE) alone and in combination with aminotransferase platelet ratio index(APRI) score in the staging of liver *** A multicenter prospective study was conducted to assess the accuracy of SWE(medians) and APRI to predict biopsy results. The analysis focused on distinguishing the different stages of liver disease, namely, F0 from F1-4, F0-1 from F2-4, F0-2 from F3-4 and F0-3 from F4; F0-F1 from F2-F4 being of primary interest. The area under the receiver operating characteristic(AUROC) curve was computed using logistic regression model. The role of age, gender and steatosis was also *** SWE alone accurately distinguished F0-1 from F2-4 with a high probability. The AUROC using SWE alone was 0.91 compared to 0.78 for using the APRI score *** APRI score, when used in conjunction with SWE, did not make a significant contribution to the AUROC. SWE and steatosis were the only significant predictors that differentiated F0-1 from F2-4 with an AUROC of *** Our study validates the use of SWE in the diagnosis and staging of liver fibrosis. Furthermore, the probability of a correct diagnosis is significantly enhanced with the addition of steatosis as a prognostic factor.

Impact of a colorectal cancer screening program implantation on delays and prognosis of non-screening detected colorectal cancer

Experience of Patients Undergoing mini-Arthroscopy Compared to mRI in the Earliest Phases of Arthritis

Objective: To evaluate the expectations and experience of patients undergoing mini-arthroscopy compared to contrast enhanced mRI for research purposes. methods: Seventeen patients with early, active arthritis (group A) and 21 autoantibody-positive individuals without any evidence of arthritis upon physical examination (group B) were included. All subjects underwent both contrast enhanced mRI and synovial biopsy sampling by mini-arthroscopy of the same joint within one week. At inclusion and after both procedures, subjects filled in questionnaires with items about expectations and experience with regard to the procedures. Results: Before procedures, subjects in group B had a higher fear of and reluctance to undergo mini-arthroscopy compared to mRI (p mRI, 11% of the subjects preferred mini-arthroscopy and 47% had no preference for either procedure. After both procedures, subjects preferences changed to 39% for mRI, 32% for mini-arthroscopy and 29% for no preference for one or the other procedure. When comparing group A with group B, there were no significant differences in preference before and after the procedures. Conclusion: Synovial biopsy sampling by mini-arthroscopy for analysis of synovial inflammation is a well-experienced procedure when compared to contrast enhanced mRI. These results support the use of mini-arthroscopy in a research setting from a patient perspective.

Cutaneous Vasculitis with Renal Impairment in Octogenarian

Henoch-Schönlein purpura is a vasculitis of small vessels, characterized by tissue deposition of IgA, affecting predominantly the skin, intestine, joints and kidneys. It mainly affects the pediatric age group, especially from 2 to 11 years old. most often, it is preceded by episode of upper airway infection from days to weeks. The most common clinical manifestations are purpuric cutaneous lesions located in the lower limbs and buttocks, abdominal pain in colic, nausea, vomiting, diarrhea, in addition to arthralgia/arthritis, usually with spontaneous resolution. Renal impairment usually manifests with hematuria and proteinuria, and renal function may also be impaired. The case described refers to a female patient, 86 years old, with a previous history of infection of the upper airways, followed by purpura in the lower limbs and renal impairment: serum creatinine 3.1 mg/dL (N 0.5 - 1.2 mg/dL);glomerular filtration rate 15 ml/min (N > 60 ml/min);urea 118 mg/dL (N önlein purpura in an atypical age group, with significant renal impairment, improvement only with conservative treatment, thus avoiding the use of immunosuppressants and the risks of immunosuppression and toxicity related to such drugs.
