Probable benign paroxysmal positional vertigo, spontaneously resolved:Incidence in medical practice, patients’ characteristics and the natural course

Background: Probable benign paroxysmal positional vertigo, spontaneously resolved (pBPPVsr), is a variant of benign paroxysmal positional vertigo (BPPV) in which there is no observable nystagmus and no vertigo with any positional maneuver. Objectives: To calculate the incidence pBPPVsr, compare the characteristics of the patients with pBPPVsr and BPPV not spontaneously resolved and describe the spontaneous resolution in the natural course of BPPV. methods: multicenter prospective descriptive study. During a one-year period, all patients with suspected BPPV that presented to the Neurotology Units of five participating centers were recruited. The incidence of pBPPVsr was calculated as a percentage of the total number of patients with BPPV. The prevalence of several variables was compared between pBPPVsr and BPPV not spontaneously resolved. The timing of spontaneous resolution was estimated using Kaplan-meier curves. Results: 457 patients met the inclusion criteria. The incidence of pBPPVsr was 33.5%. It was significantly higher in males, in patients with normal bone mass and in patients who were not taking sulpiride. A rate of 18% of spontaneous resolution after the first month and 51% after the first year was found. This percentage did not change in a significant way after this moment. The curves for males, patients under 50 and patients with normal blood pressure decreased significantly faster. Conclusions: In our serie, BPPV spontaneously resolved in half of the patients with BPPV during the first year. This seemed to occur more commonly in males and could have been hindered by sulpiride intake, osteoporosis, advanced age and high blood pressure.

Background:Ocular surface disease in glaucoma patients is a significant ocular co-morbidity that can affect 40%to 59%of these patients worldwide.The current study was aimed at evaluating the potential clinical benefit of an intense pulsed light(IPL)-based treatment in glaucomatous patients with ocular surface disease due to prolonged hypotensive eyedrop treatments.To our knowledge,this is the first series analyzing the therapeutic effect of this treatment option in this type of patients.methods:This non-comparative prospective case series study enrolled a total of 30 glaucoma patients ranging in age from 57 to 94 years old and treated with hypotensive eyedrops for years with dry eye symptomatology.All patients received four sessions of IPL treatment using the Optima IPL system(Lumenis,Yokneam,Israel)adjusted to the official optimized Lumenis setting.Changes in symptomatology,corneal staining,conjunctival hyperemia,non-invasive break-up time(NIBUT),tear osmolarity,tear meniscus height(TmH),meiboscore and meibomian gland expressibility was analyzed after treatment.Results:Statistically significant reductions were observed after IPL treatment in the symptomatology scores measured with different questionnaires[ocular surface disease index(OSDI),standard patient evaluation of eye dryness(SPEED)and symptom assessment questionnaire in dry eye(sande)]as well as with the visual analogue scale(Pmean change in OSDI was−15.0±11.3.A significant reduction was found after treatment in the corneal staining score(Pgnificant reduction was found in tear film meniscus height(P=0.012),as well as in tear film osmolarity(P=0.001).A significant reduction was also found in meibomian gland expressibility(P=0.003),changing the percentage of grade 3 eyes from 44.4%before IPL to 17.2%after treatment.Conclusions:IPL therapy combined with meibomian gland expression(mgX)seems to be an effective option to improve symptomatology in glaucomatous patients with ocular surface disease due to prolonged hypotensive eyedrop treatment

Fourier continuation(FC)is an approach used to create periodic extensions of non-periodic functions to obtain highly-accurate Fourier expansions.These methods have been used in partial differential equation(Pde)-solvers and have demonstrated high-order convergence and spectrally accurate dispersion relations in numerical experiments.Discontinuous galerkin(Dg)methods are increasingly used for solving Pdes and,as all galerkin formulations,come with a strong framework for proving the stability and the convergence.Here we propose the use of FC in forming a new basis for the Dg framework.

目的:探讨藏药脑神宝丸方中应用培育牛黄与天然牛黄对高原性心脏病(HAHD)患者症状表现、心功能、凝血功能及血清氧化应激水平的影响。方法:按照单盲随机数字表法将2019年9月至2022年8月该院收治的80例HAHD患者分为天然组、培育组,各40例。两组患者在常规治疗基础上给予藏药脑神宝丸方治疗,天然组患者方中应用天然牛黄,培育组患者方中应用培育牛黄。比较两组患者治疗前后临床症状变化、心率、心功能[右心室射血分数(RVEF)、右心房收缩波(As)和肺动脉收缩压(SPAP)]、凝血功能[活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)、纤维蛋白原降解产物(FDP)和凝血酶原时间(PT)]、氧化应激指标[超氧化物歧化酶(SOD)、脂质过氧化物(LPO)、谷胱甘肽过氧化物酶(gSH-Px)和丙二醛(mDA)]及安全性。结果:治疗后,两组患者疲乏、呼吸困难、胸闷、头晕/头痛和烦躁不安评分均显著低于治疗前,差异均有统计学意义(P0.05)。治疗后,两组患者心率、SPAP和As水平较治疗前降低,RVEF水平较治疗前升高,差异均有统计学意义(P0.05)。治疗后,两组患者PT、APTT均长于治疗前,FDP、FIB水平均低于治疗前,差异均有统计学意义(P0.05)。与治疗前比较,两组患者治疗后SOD、gSH-Px水平升高,LPO、mDA水平降低,差异均有统计学意义(P0.05)。天然组患者的不良反应发生率为7.50%(3/40),与培育组(2.50%,1/40)比较,差异无统计学意义(P>0.05)。结论:藏药脑神宝丸方中应用天然牛黄与培育牛黄效果一致,联合常规治疗均可缓解HAHD患者的氧化应激损伤,改善凝血指标,促进临床病症好转,且安全性高。

背景随着疾病谱的转变和现代医学模式的发展,中医在疾病预防、保健、康复方面起着重要作用,需要进一步推动开展中医“治未病”服务,大力发扬中医药在人民健康维护和促进上独特作用,而目前社区居民中医“治未病”理念普及教育情况尚不明确。目的了解广州市社区居民对中医“治未病”的总体知晓情况、信任度及采用行为情况,探讨社区居民对中医“治未病”服务信任度的影响因素。方法2022年4—8月,采用分层整群抽样法,结合地理位置(广州中心城区、城乡结合部、郊区)、经济等因素选择广州市4个行政区域12个街道的652位社区居民进行访谈式问卷调查。调查内容包括居民基本信息、对中医“治未病”知晓情况、获取中医“治未病”知识的途径、对中医“治未病”的信任度、服务需求和利用情况。采用二元Logistic回归分析对广州市社区居民对中医“治未病”服务信任度的影响因素进行分析。结果652位社区居民中,67.79%(442/652)了解中医“治未病”,77.91%(508/652)信任中医“治未病”服务,69.63%(454/652)的社区居民曾接受过中医传统理疗,仅有6.59%(43/652)的社区居民过去一年接受中医“治未病”的服务次数超过4次。单因素相关分析显示,广州市社区居民中医“治未病”服务信任度的影响因素主要有年龄(χ^(2)=9.218)、户口性质(χ^(2)=19.356)、婚姻状况(χ^(2)=2.490)、职业(χ^(2)=17.889)、医疗保险付费方式(χ^(2)=13.516);二元Logistic回归分析显示,户口性质(非农业、农业转居民、非农业转居民)是社区居民对中医“治未病”服务信任度的影响因素(OR=2.646、5.593、10.502)。结论广州市社区居民对中医“治未病”的总体认知程度(67.79%)及信任度(77.91%)相对较高,但实际服务的接受次数不足,存在“知、信、行”分离现象。非农业转居民户口的社区居民信任度较高,农业户口的信任度较低。应在开展中医“治未病”理念普及和服务工作中,充分利用居民偏好的形式,并针对特定群体加大宣传力度,以促进社区居民培养中医“治未病”养生保健习惯。

Irregular fine protrusions formed on the surface of a mechanical part through biomimetic technology can enhance the part’s properties,including tribology,self-cleaning,and light absorption.However,underlying principles for the formation of fine protrusions according to the requirements of their shapes,sizes,and material distributions have not been studied sufficiently.This paper presents the software development for modeling irregular fine protrusions,which is essential for the simulation,experimentation,and analysis of fine protrusions formed by sputter etching.

glioblastoma remains as the most common and aggressive malignant brain tumor,standing with a poor prognosis and treatment prospective.despite the aggressive standard care,such as surgical resection and chemoradiation,median survival rates are low.In this regard,immunotherapeutic strategies aim to become more attractive for glioblastoma,considering its recent advances and approaches.In this review,we provide an overview of the current status and progress in immunotherapy for glioblastoma,going through the fundamental knowledge on immune targeting to promising strategies,such as Chimeric antigen receptor T-Cell therapy,immune checkpoint inhibitors,cytokine-based treatment,oncolytic virus and vaccine-based techniques.At last,it is discussed innovative methods to overcome diverse challenges,and future perspectives in this area.

Ultrasound shear wave elastography and liver fibrosis: A Prospective multicenter Study

AIm To assess the accuracy of shear wave elastography(SWE) alone and in combination with aminotransferase platelet ratio index(APRI) score in the staging of liver fibrosis.mETHODS A multicenter prospective study was conducted to assess the accuracy of SWE(medians) and APRI to predict biopsy results. The analysis focused on distinguishing the different stages of liver disease, namely, F0 from F1-4, F0-1 from F2-4, F0-2 from F3-4 and F0-3 from F4; F0-F1 from F2-F4 being of primary interest. The area under the receiver operating characteristic(AUROC) curve was computed using logistic regression model. The role of age, gender and steatosis was also assessed.RESULTS SWE alone accurately distinguished F0-1 from F2-4 with a high probability. The AUROC using SWE alone was 0.91 compared to 0.78 for using the APRI score alone.The APRI score, when used in conjunction with SWE, did not make a significant contribution to the AUROC. SWE and steatosis were the only significant predictors that differentiated F0-1 from F2-4 with an AUROC of 0.944.CONCLUSION Our study validates the use of SWE in the diagnosis and staging of liver fibrosis. Furthermore, the probability of a correct diagnosis is significantly enhanced with the addition of steatosis as a prognostic factor.

Keeping in School Shape (KiSS): A Program for Rehearsing math Skills over Breaks from School

If you don’t use it,you lose it.School breaks,during which students do not regularly participate in instruction,can therefore have negative consequences on learning.This is especially true for mathematics learning since skills build progressively on earlier materials.How can we bridge these gaps in formal instruction?The Keeping in School Shape(KisSS)program is a mobile,engaging,innovative,and cost-effective way of using technology to help students who have time off between related math courses stay fresh on prerequisite knowledge and skills.Founded on learning theory and designed on a model of behavioral change,the KiSS program embodies retrieval practice and nudges by sending students a daily multiple-choice review problem via text messaging over school break.After rating their confidence in solving the daily problem students receive feedback and a solution.This study explores measures of participation,accuracy,and confidence in an implementation of the KiSS program over winter break between two sequential introductory engineering courses at a large state university in the Southwest United States.Results indicate that careful attention should be paid to the construction of the first few days of the program,and that encouragement,problem difficulty may improve participation.

A review of enhanced oil recovery(EOR)methods applied in Kazakhstan

Application of enhanced oil recovery methods in Kazakhstan has been ongoing for decades alongside the continued discovery of new oil and gas fields in the Pre-Caspian Basin.The objective of this review is to provide an overview of the hydrocarbon reserves and production,and the status of the petroleum industry in Kazakhstan,with a focus on the EOR methods and projects being applied to recover those reserves.A summary of the specific EOR methods in use was prepared,and existing enhanced recovery projects in Kazakhstan were reviewed and their successes and challenges were investigated.The performance of these projects in the context of EOR performance indicators such as capillary number and mobility ratio,as well as operational and environmental issues,were examined.Recommendations for current and potential applications of EOR in Kazakhstan were also discussed.The widespread application of thermal EOR methods,in use for decades in Kazakhstan’s older fields,was found to be successful,with very favorable impacts on mobility ratio from the addition of thermal energy to the reservoirs.miscible EOR methods in Kazakhstan have had more limited success,with some significant challenges due to high concentration of hydrogen sulfide in the injected gas.Polymer injection started in the late 1960s,achieving good results.A recent polymer injection pilot project has shown some promise,with a favorable impact on mobility ratio and oil production,although the project has not yet been expanded beyond two polymer injectors.These results indicate the huge potential of existing and future EOR projects.This review is the first compilation of Kazakhstan’s existing oil and gas reserves,production,and EOR project performance,and should be seen as a guide to the existing applications of EOR methods in Kazakhstan.
