Relationship between the severity of hepatitis C virus-related liver disease and the presence of Helicobacter species in the liver: A prospective study

AIm: To determine the presence of Helicobacter species DNA in the liver of chronic hepatitis C (CHC) patients with and without cirrhosis as compared to controls, and to identify the bacterial species involved. mETHODS: Seventy-nine consecutive patients (HBV and HIV negative) with a liver sample obtained after liver biopsy or hepatic resection were studied: 41 with CHC without cirrhosis, 12 with CHC and cirrhosis, and 26 controls (HCV negative). Polymerase chain reactions (PCRs) targeting Helicobacter 16S rDNA and species- specific were performed on DNA extracted from the liver. A gastric infection with H pylori was determined by serology and confirmed by 13C-urea breath test. RESULTS: Overall, Helicobacter 16S rDNA was found in 16 patients (20.2%). Although positive cases tended to be higher in CHC patients with cirrhosis (41.6%) than in those without cirrhosis (17.0%) or in controls (15.4%), the difference was not statistically significant (P =0.08). H pylori-like DNA was identified in 12 cases and H. pullorum DNA in 2, while 2 cases remained unidenti- fied. Gastric infection with H pylori was found in only 2 of these patients. CONCLUSION: Our results do not confirm the associ- ation of Helicobacter species DNA in the liver of CHC patients with advanced liver disease. The lack of correlation between positive H pylori serology and the presence of H pylori-like DNA in the liver may indicate the presence of a variant of this species.

研究背景:大剂量马法兰（high-dose melphalan,HDm）是多发性骨髓瘤（multiple myeloma,mm）自体造血干细胞移植（autologous hematopoietic stem cell transplantation,autoHSCT）前的标准预处理方案。尽管患者的反应率和存活率有所提高,但复发率和与方案相关的安全性仍然是一个挑战。目的:本研究主要目的是比较接受白消安、环磷酰胺和依托泊苷（BVC）和HDm作为自体造血干细胞移植预处理方案的mm患者的疗效和预处理毒副作用的差异。方法:对2015年8月至2020年12月期间来自山东省3个主要移植中心（山东大学齐鲁医院、山东省立医院和山东省千佛山医院）的29-65岁（中位年龄52岁）接受自体造血干细胞的78例mm患者进行回顾性分析。37名患者接受BVC预处理方案,而41名患者接受HDm预处理方案。多发性骨髓瘤的诊断使用ImWG标准进行,并使用国际分期系统进行分期。纳入标准包括肌酐低于***/dL、无活动性感染或经肺功能检查确定的严重阻塞性和/或限制性肺疾病（即DLCO＜50%和/或FEV1＜50%和/或FVC＜50%）且心脏射血分数（EF）大于40%的成人（年龄在18-65岁之间）。患者首先接受若干个周期的诱导化疗（通常为3-6个周期）,对疗效达部分缓解以上的患者进行干细胞动员和收集。自体外周血造血干细胞（PBHSCs）用作自体干细胞移植细胞。76例患者接受大剂量环磷酰胺联合粒细胞集落刺激因子（G-CSF）化疗动员采集外周血造血干细胞。2名患者使用plerixafor联合G-CSF稳态动员采集干细胞。CD34+细胞平均采集量为4.61 × 106/kg。CD34+细胞最小采集量为2×1 06个/kg。37名mm患者接受BVC预处理方案:白消安在-7天、-6天和-5天以0.8mg/kg/天静脉注射。依托泊苷在-5天和-4天以400mg/m2/天静脉给药,环磷酰胺在-3天和-2天以50mg/m2/天静脉内给药。41名mm患者接受HDm预处理方案:马法兰总剂量为200mg/m2,分成-3天和-2天静脉注射,然后进行自身HSCT。预处理方案对待评估的移植受者中发生的毒副作用毒性的影响如下:a.方案相关的毒性;包括血液学毒性和非血液学毒性。b.中性粒细胞和血小板植入的时间。c.住院时间。要分析的结果包括:a.移植后疾病的反应状态。b.移植1年时复发率。c.移植后100天内的移植相关死亡率。d.移植 1 年时无进展生存期（progression-free survival,PFS）。e.移植1年时的总生存期（overall survival,OS）。使用SPSS版本26对两组之间的数据进行分析和比较。描述性统计用于报告结果,包括人口统计学,疾病相关因素和移植相关因素。使用Kaplan-meier方法估计PFS和OS的概率,并使用Log-rank检验对组进行比较。使用Cox比例风险回归模型对OS,PFS和累积复发发生率进行多变量分析,该模型用于估计与HDm相比BVC的风险比（HR）。结果:本研究共纳入mm患者78例,其中37名患者在接受BVC 预处理方案后接受autoHSCT,而41名患者在接受HDm预处理方案后接受autoHSCT。两组的总缓解率（ORR）、PFS、OS和复发率在统计学上相似。BVC组的中位随访时间为540天,HDm组为436天。HDm组肺毒性发生率显著增加（BVC为36.6%对5.4%,P=0.002）。BVC组患者的住院时间明显长于HDm组（中位数20天,范围:BVC为18-27;与中位18天范围:HDm为13-27,P＜0.0001）。BVC组（7天,范围:4-12）中性粒细胞减少的中位持续时间明显长于HDm组(6天,范围:（2-12）;P=0.002。有两例TRm病例,每组一例。在多变量亚组分析中,BVC调理在PFS、OS和复发方面与HDm调理有统计学相似。但是,低风险疾病分期显示可以预测PFS和复发的累积发生率,ISSⅠ和ISSⅡ疾病阶段的患者PFS明显更长;（ISS Ⅰ:HR=0.186,P=0.006,ISS Ⅱ:HR=0.240,P=0.022）;并延迟复发时间;（ISSⅠ:HR=0.220,P=0.013,ISS Ⅱ:HR=0.262,P=0.036）。β-2 微球蛋白预测 OS,β-2微球蛋白水平升高显示显著恶化OS（HR=1.356,P=0.041）。年龄,性别,mm亚型,KPS,移植前使用的化疗方案/周期数或移植时的疾病状况与两组的PFS,OS或复发率无关。结论:尽管两种方案的反应率和生存率似乎相似,但与HDm方案相比,BVC方案在非血液学毒性（肺毒性）方面似乎更优越,但在血液学毒性（血细胞减少）方面较差。本研究显示BV

The metabolic syndrome(mS), which includes obesity,dyslipidaemia, hypertension and hyperglycaemia according to the most widely accepted definitions now used, is one of the most common post-transplant complications, with a prevalence of 44%-58%. The mS, together with the immunosuppression, is considered the main risk factor for the development of cardiovascular disease(CVD) in transplant recipients, which in turn accounts for 19%-42% of all deaths unrelated to the graft. The presence of mS represents a relative risk for the development of CVD and death of 1.78. On the other hand, non-alcoholic fatty liver disease(NAFLD), considered as the manifestation of the mS in the liver, is now the second leading reason for liver transplantation in the United States after hepatitis C and alcohol. NAFLD has a high rate of recurrence in the liver graft and a direct relation with the worsening of other metabolic disorders, such as insulin resistance or diabetes mellitus. Consequently, it is vitally important to identify and treat as soon as possible such modifiable factors as hypertension, overweight, hyperlipidaemia or diabetes in transplanted patients to thus minimise the impact on patient survival. Additionally, steroid-free regimens are favoured, with minimal immunosuppression to limit the possible effects on the development of the mS.

Effect of Pectin Lyase Enzyme on Fermentation and Drying of Cocoa (Theobroma cacao L.): An Alternative to Improve Raw material in the Industry of Chocolate

Cocoa (Theobroma cacao), in all its presentations, is consumed all over the world and is one of the main drivers of the economic in several countries. The world’s Cocoa tendency is focused on developing special beans. This category is subject to postharvest processes of utmost importance such as the fermentation and dry, which are currently carried out with traditional and poorly effective devices, which need to be improved to obtain a high quality product. The aim of this study was to evaluate the influence of the pectin lyase enzyme (E.C.4.***.2.10) on the postharvest cocoa process. We evaluated the enzyme dosage (1.0% and 0.5%) in fermentation and its effect on the variables temperature, acidity and drying time by convection at 60°C. The Pectin lyase activity during fermentation does not cause a significant effect on the variables of temperature and acidity;however, the drying process time required to achieve 7.0% moisture was reduced. The enzyme dosage of 1.0% was the best result, the amount of exudate obtained (115 ml) during fermentation and the best degree of fermentation (77% ± 3.8) were increased and further shows a change in porosity facilitating the scale surface and internal moisture diffusion. The drying rate (Nw) expressed in kgwater/m2 * min was determined based on the empirical model of Newton, where the higher speed was obtained during the falling period. In conclusion, enzyme dosage 1% was the best concentration evaluated because weaken grain husk, which allowed an adequate fermentation,and subsequent time drying reduction until 10.8 h.

目的钙摄入水平低以及体内维生素D不足是导致亚洲女性罹患骨质疏松的危险因素。此项研究的目的是评估高钙维生素D强化牛奶(HCm)对健康的中国绝经后妇女的体内维生素D状态和骨吸收标志物进行干预的影响。方法本试验对象为55岁以上,绝经至少5年的女性。共有124名女性进行了总体健康状况和骨密度检查筛选。63名女性被随机分为对照组和试验组,试验组在12w内每日饮用两次高钙/维生素D强化奶(含900 mg钙、96 mg镁、2.4 mg锌和6.4μg维生素D)。2组分别在试验开始、试验2 w、8 w和12 w时测量血矿物质、25(OH)维生素D3和Ⅰ型胶原C端肽(CTX)水平。结果对照组女性平均年龄为63±4.6岁,高钙强化奶(HCm)组女性平均年龄为62±3.8岁。所测得的每日钙摄入量的基线值为HCm组260～482 mg,对照组为252～692 mg。12 w后HCm组的平均身体质量指数(BmI)显著降低(Pm组的血25(OH)维生素D3水平显著提高(33.13～39.49 nmol/L),而对照组则维持不变(29.27～28.21 nmol/L)。HCm组的血CTX水平在0 w和2 w之间下降了25%,并保持不变直至第12w。从第2w开始HCm组和对照组相比血CTX水平就出现了显著差异(Pm),在2至4w内能显著提高中国绝经妇女体内维生素D水平,并减少骨吸收破坏。如果长期持续地饮用含高钙和维生素D的强化奶(HCm),能减少骨吸收和骨量丢失的风险,从而减少患骨质疏松和由此引起骨折的风险。

Design and Characterization of a Horizontal Double Impinging Jet Cell: Determination of Flow modes at the Surface of a Flat Electrode

An electrochemical cell consisting of a double horizontal Impinging Jet Cell (IJC) has been conceived and characterized. The purpose of this system is the simultaneous electrodeposition of a composite metal/particle coating on both surfaces of a metal sheet. The silica particles imprint in the nickel matrix has allowed to distinguish four different flow areas onto the electrode namely the stagnation area, the radial flow area characterized by a higher flow speed, the return flow area that involves gravity effect, and the drainage area with a constant draining speed. Based on the limiting current evolution as a function of the Reynolds number, three flow modes were extracted: the Laminar Low Flow (LLF), the Laminar High Flow (LHF) and the Disturbance. The IJC investigated ensures a laminar flow for a large range of flow rate from a nozzle-to-sample distance of 19 mm and creates an laminar flow ovoid plan merged with the sample for the high flows.

Assessment and Impact of Leachate Generated by the Landfill City in Abidjan on the Quality of Ground Water and Surface Water (m’Badon Bay, Côte d’Ivoire)

The municipal Akouedo landfill of the city of Abidjan (Cote d’Ivoire) is composed of more than 60% of household waste. It lacks a leachate collecting and treatment device. To highlight the impact of the leachate on the receiving environments, some sampling campaigns of leachate, groundwater and surface waters were conducted from June 2013 to January 2014. Analyses of leachate samples revealed relatively high concentrations of trace metals: Fe (6450 ± 8690 μg/L), Cu (400 ± 272 μg/L), Zn (520 ± 240 μg/L), Cd (113 ± 105 μg/L), Pb (550 ± 237 μg/L), Ni (312 ± 97 μg/L) and Co (77 ± 56 μg/L). Fe is the most abundant chemical element. The analysis of groundwater and surface indicates that the levels of trace metals in these waters are higher than WHO standards except Zn.
