In recent years,research on facial expression recognition(FER)under mask is *** a mask for protection from Covid 19 has become a compulsion and it hides the facial expressions that is why FER under the mask is a difﬁcult *** prevailing unimodal techniques for facial recognition are not up to the mark in terms of good results for the masked face,however,a multi-modal technique can be employed to generate better *** proposed a multi-modal methodology based on deep learning for facial recognition under a masked face using facial and vocal *** multimodal has been trained on a facial and vocal *** have used two standard datasets,m-LFW for the masked dataset and CREmA-D and TESS dataset for vocal *** vocal expressions are in the form of audio while the faces data is in image form that is why the data is *** order to make the data homogeneous,the voice data is converted into images by taking spectrogram.A spectrogram embeds important features of the voice and it converts the audio format into the ***,the dataset is passed to the multimodal for *** network and the experimental results demonstrate that the proposed multimodal algorithm outsets unimodal methods and other state-of-the-art deep neural network models.

middle East respiratory syndrome coronavirus （mERS-CoV） is a betacoronavirus which can cause acute respiratory distress in humans and is associated with a relatively high mortality rate. Since it was first identified in a patient who died in a Jeddah hospital in 2012, the World Health Organization has been notified of 1735 laboratory-confirmed cases from 27 countries, including 628 deaths. most cases have occurred in Saudi Arabia. mERS-CoV ancestors may be found in Old World bats of the Vespertilionidae family. After a proposed bat to camel switching event, transmission of mERS-CoV to humans is likely to have been the result of multiple zoonotic transfers from dromedary camels. Human-to-human transmission appears to require close contact with infected persons, with outbreaks mainly occurring in hospital environments. Outbreaks have been associated with inadequate infection prevention and control implementation, resulting in recommendations on basic and more advanced infection prevention and control measures by the World Health Organization, and issuing of government guidelines based on these recommendations in affected countries including Saudi Arabia. Evolutionary changes in the virus, particularly in the viral spike protein which mediates virus-host cell contact may potentially increase transmission of this virus. Efforts are on-going to identify specific evidence-based therapies or vaccines. The broad-spectrum antiviral nitazoxanide has been shown to have in vitro activity against mERS-CoV. Synthetic peptides and candidate vaccines based on regions of the spike protein have shown promise in rodent and non-human primate models. GLS-5300, a prophylactic DNA-plasmid vaccine encoding S protein, is the first mERS-CoV vaccine to be tested in humans, while monoclonal antibody, m336 has given promising results in animal models and has potential for use in outbreak situations.

背景:高血压是一个全球流行的疾病。在撒哈拉以南的非洲,高血压患者就诊时,常常己经处于有并发症的晚期。另外,由于高血压早期没有明显症状,患者坚持服药的依从性较差,从而导致其继续进展损害靶器官。血压的慢性增加导致了心肌重构,进而促使左心室肥大,增加了心血管事件的风险,甚至心衰。左心室肥大可以作为高血压相关的靶器官损害的标记物。在高血压患者中,左心室肥大具有临床相关性,因为它是心血管事件风险的综合标志物,反映了高血压中血液动力学和非血液动力学因素对心脏的长期影响。心血管事件的风险随着左心室肥大的进展而增加。抗高血压治疗通过改善左心室肥大的而降低心血管事件风险。因此,如果通过常规程序评估,检测高血压患者的左心室肥大程度,结合肥大心室几何结构信息,将其分为低等或中等心血管事件风险。这对于分级总心血管事件风险和分级相应的治疗措施至关重要。在桑给巴尔-坦桑尼亚,尚无对左心室肥大负荷及其进展相关研究。目的:本研究旨在调查桑给巴尔的mnazimmoja医院的患者中左心室肥大的患病率,模式和相关因素。具体目标具体目标如下:1.确定接受mmH治疗的LVH高血压患者的人口学特征2.确定接受mmH护理的高血压患者的LVH患病率。3.描述接受mmH治疗的LVH高血压患者的临床表现4.确定接受mmH护理的高血压患者的LVH相关危险因素。5.描述接受mmH护理的高血压患者的LVH心血管病变模式。方法:2018年6月至2018年12月,我们连续登记了 389名参加mnazimmoja医院门诊的高血压患者。我们采用了连续方便的采样方法。心电图上左心室肥大的评估基于电压标准。此外,ECHO上的LVH分别基于男性和女性的左心室质量指数(LVmI),截止点分别是>115g/m2和95g/m2。使用卡方或Fisher精确检验来检测组间分类特征的频率差异。通过Studentt检验分析连续变量,并使用逻辑回归分析技术评估关联因子。P值mI和尿素水平是LVH的预测因子。女性(AOR 1.7,95%CI 1.1-2.7,P = 0.02),失业率(AOR 3.2,95%CI 1.6-6.4,P2年(AOR.1.6,95%CI 1.0-2.5,P=0.04),肥胖(AOR 2.0,95%CI 1.1-3.4,P=0.02)和尿素升高(AOR 4.3,95%CI 1.9-9.5,P0.001)是多变量分析中LVH的独立预测因子。结论:本研究中左室几何结构异常的患病率达62%。女性,失业,农村居民,高血压持续时间>2年,肥胖和高血压患者的尿素水平升高,会增加LVH的患病风险。

In the present study,the base pressure variations induced by the presence of a cavity,known to have a strong influence of the behaviour of supersonic projectiles,are investigated through numerical solution of the balance equations for mass,momentum,and *** area ratio of four is considered and numerical simulations are carried out at mach m=1.2,1.4,1.6,and 1.8 assuming no cavity or cavity locations 0.5D,1D,1.5D,and *** inlet pressure of the nozzle is considered as a flow *** Taguchi method is also used,and the considered cases are then analyzed using a full factorial experimental *** results show that the cavity is effective in increasing the base pressure for the conditions *** other nozzle pressure ratios,cavities do not lead to passive control due the change in the reattachment *** distribution of wall pressure reveals that,in general,a cavity used to implement passive control of the base pressure does not adversely influence the flow pattern in the domain.

BACKGROUND Supracondylar humerus fractures account for more than 60%of all elbow fractures and about 1/5 of all pediatric ***,these fractures can be associated with risk of complications including neurovascular injuries,malunions and limb *** exists regarding the effect of time of surgical intervention and/or level of surgeon performing the surgery on outcome of these *** To determine whether time of surgical intervention and/or surgeon level influence the outcomes of surgically managed pediatric supracondylar humerus *** We retrospectively studied 155 pediatric patients presenting with a supracondylar humerus fracture in a level 1 trauma center from January 2006 to December *** data extracted included demographic data,fracture characteristics,surgical data,and follow-up *** collected data was analyzed and P values ofmented post-operative complications,of which the majority occurred in surgeries performed after day time working hours and in fractures requiring open *** the lowest complication rate was found in surgeries performed by pediatric orthopaedic surgeons,this did not reach statistical *** In pediatric patients undergoing surgery for supracondylar fractures,we found a higher complication rate when surgeries were not performed during working *** level and training had no significant effect on the risk of post-operative complications.

We prepared a humanized alternative B7-H3 CAR(B7-H3-haCAR-T)targeting B7-H3 which contained the humanized singl-chain varinlde fragment(scFv)from humanized 8H9 monoclonal Antibody(hu8H9m Ab).The antitumor effects of B7-H3-haCAR-T cells were evaluated in B7-H3 overexpressed tumor cells(in vitro)and B7-H3 xenograft models(in vivo).The specific tumour killing ability of B7-H3-haCAR-T in overexpressing B7-H3 tumour cells was verified by cytotoxicity and ELISA *** addition,B7-H3 haCAR-T cells were also noted to suppress the tumour growth remarkably well in the xenograft murine models,and the survival time in the haCAR-T cell treatment group was appreciably longer than that in the control *** specific recognition and highly efficient tumoricidal behavior of B7-H3 haCAR-T provide a basis for future clinical studies with humanized scFv-transduced CAR-T cells targeting solid tumors.

Coronary artery injuries are uncommon. Acute injuries of major coronary vessels, particularly the left anterior descending artery (LAD), are life-threatening because of the significant cardiac dysfunction that ensues and usually requires repair or bypass. In contrast, late presentation of missile coronary artery injuries has been sporadically reported in the literature. Herein we present a 32-year-old lady from Sulaimania, Iraq with multiple shrapnel injuries 16 years earlier presented recently with severe anterolateral myocardial ischemia unresponsive to medical treatment. Coronary angiography and surgical exploration revealed shrapnel compressing the LAD. The patient responded very well to shrapnel extraction and coronary artery revascularization (off pump left internal mammary artery—LImA—to LAD anastomosis). The management of this patient is discussed with review of relevant literature. It is concluded that long-standing shrapnel close to a major coronary artery may incur an external trauma initiating a localized arteriosclerosis with an extremely long time interval from injury to the onset of symptoms of myocardial ischemia.
