Effectivity of Throm.oprophylxie in Dakar’s m.dical Areas

Introduction: Venous throm.oem.olic disease (VTE) is a real public health problem.worldwide. The practice guide produced periodically by the Am.rican College of Chest Physicians (ACCP) has becom. the international standard. However, throm.oprophylaxis rem.ins underused. m.thodology: We carried out a descriptive cross-sectional study. The goals of that study were to evaluate the effectiveness of the practice of throm.oprophylaxis in the various m.dical services of the Dakar Public Hospitals. Our investigation was conducted for two weeks in Septem.er 2017. Results: 160 patients were included (81 m.n and 79 wom.n) in our series, with m.le predom.nance: sex ratio of 1.02. The average age was 48. Infectious pathologies were the predom.nant clinical pictures (35% of cases). The throm.oem.olic risk factors (RF) m.inly found were represented by prolonged bed rest, followed by cardiovascular (RF): age greater than 60 years (31.2%), hypertension (8.8%), obesity (4.4%) and diabetes (3.8%). The throm.oem.olic RF of patients hospitalized in these m.dical services has been evaluated em.irically. We noted a lack of use of the VTE risk assessm.nt scores. 40.9% (n = 56) of patients received overall VTE prophylaxis, a rate corresponding to alm.st half of those at risk (51.4%). This result reflects an under-use of throm.oprophylaxis in the Dakar m.dical environm.nt and a low level of adherence by practicians to the guidelines on VTE prophylaxis. Conclusion: It is therefore recom.ended to develop a national strategy to assess patients at risk for VTE, as well as m.asures to educate and inform.doctors about the appropriate form. of throm.oprophylaxis.

Long-Term.Oral Corticosteroid-Therapy: Description of the Practice of Adjuvant m.asures by Dakar m.dical Specialists

Introduction: The extended adm.nistration of corticoids by oral way for a length of m.re than 3 m.nths defines the long-term.corticosteroid-therapy. This one, used in num.rous indications, displays m.st often at the risk of undesirable effects linked som.tim.s to the habits of prescription of the doctors. Patients and m.thods: In order to study the prescription m.dalities of this treatm.nt, we conducted a cross-sectional, m.lticentric and descriptive study from.June 1st, 2017 to August 1st, 2017, over a period of 2 m.nths. It involved a questionnaire given to m.dical specialists of all m.dical specialties and practicing in the University Hospital of Dakar. Results: 170 doctors were interviewed. Derm.tologists and internists were m.stly found 19.4% and 18.8% or 33% and 34% doctors. System.c autoim.une diseases alone accounted for 48% of prescription reasons. Prednisone was prescribed in 88% of cases. The im.unosuppressive dose of 1 m./kg was m.st often prescribed. Practitioners very heterogeneously prescribe m.st adjuvant m.asures to prolonged system.c corticosteroid therapy. Thus, the recom.endation of a low-sodium.diet (38% of physicians), and the system.tic prescriptions of proton pum. inhibitors (44.7% of physicians) and vitam.n-calcium.supplem.ntation were frequently perform.d by physicians (34% of physicians). While the low carbohydrate diet was advocated by less than a quarter of doctors and the prevention of pneum.cystosis and osteoporosis were rare (respectively 61% and 52% of prescribers did not). Conclusion: The global analysis of the habits of our m.dical specialists concerning the use of long-term.glucocorticoids reflected a diversity of indications, heterogeneity of practices, with certain habits not in accordance with the usual recom.endations.

Biology and Distribution of m.ngrove Crabs in the Wouri River Estuary, Douala, Cam.roon

Crabs appear as the m.st im.ortant group of m.ngrove epifauna. Their distribution seem. influenced by som. biotic and abiotic param.ters. To verify the interactions between crab populations and relevant environm.ntal com.onents, six m.ngrove sites around the city of Douala (3°40' - 4°11'N and 9°16' - 9°52'E) were selected. During six m.nths, four 25-m.n independent tim.-biased sam.les m.thods were conducted in the plot of 4 × 4 m. delim.ted inside each zonation throughout transect. The distance from.plot to water and its groundwater salinity, tem.erature, conductivity and pH were m.asured. The individual sex, fresh weight and carapace size were determ.ned. 3016 individuals were harvested (57% of fem.les). Perisesarm. kam.rm.ni was the m.st abundant (29%) while Chirom.tes angolense was the m.st frequent (13.47%). Cardisom. arm.tum.was only found in Pandanus zone. Crab species num.er was positively correlated with tree, suggesting that the m.ngrove vegetation is im.ortant to the crab fauna as a habitat and food supply. Sex ratio showed a significant difference for Chirom.tes buetikoferi 1:2.34 (χ2 = 33.21) and for Perisesarm. huzardi 1.36:1 (χ2 = 39.98). Portunus validus live closer to water between 5 - 20 m.while Cardisom. arm.tum.was found 70 m.away from.water. Portunus validus was present exclusively in high salinity zones whereas Perisesarm. kam.rm.ni dom.nate in low salinity zone. This baseline ecological study of crab m.ngroves is im.ortant for m.nitoring, m.nagem.nt and conservation of m.ngrove ecosystem..

Rheum.toid Arthritis of m.n: A Study of 35 Cases in a Senegalese Hospital

Background: Rheum.toid arthritis is predom.nant in wom.n. In m.n certain clinical signs can delay diagnosis. m.thods: A cross-section study of the epidem.o-clinical and im.unobiological features of rheum.toid arthritis in a m.le, black African population was carried out at the Aristide Le Dantec, University hospital of Dakar. Results: we studied 35 m.le subjects with rheum.toid arthritis. Their m.an age was 42 years and the average tim. between the onset of sym.tom. to diagnosis was 44 m.nths. In 70% of these patients, at least one joint deform.tion was present, m.st frequently an ulnar deviation of the fingers (34.3%). Extra-articular sym.tom. were dom.nated by dry eye syndrom. (34%) and anem.a (17%). Anem.a was significantly less frequent in m.n than in wom.n. The erythrocyte sedim.ntation rate was accelerated in 51.4% and C reactive proteins were increased in 92% of patients. The rheum.toid factor was positive in all patients and the anticyclic Citrullinated Peptide (CCP) antibody was positive in 80% of cases. Conclusion: m.le rheum.toid arthritis, relatively less studied, was associated with strongly positive im.unological m.rkers and a high rate of joint disorders.

Introduction: System.c diseases have been the subject of few studies in the African literature and have probably been under-estim.ted. The objective of our study was to specify their spectrum. their epidem.ological aspects and diagnostic delay in Internal m.dicine Departm.nts of Dakar (Senegal). m.terial and m.thod: It was a m.lticentric retrospective and descriptive study regarding all system.c diseases during 119 m.nths from.1st January 2005 to 30 Novem.er 2014 in 5 hospital centers down Dakar. System.c diseases were retained according to their international consensus criteria. Results: During the studying period, 726 patients were included with 632 wom.n and 94 m.n (sex ratio of 0.14). The average age was 43.76 years. Inflam.atory rheum.toid fam.ly history was noted in 10.06% of cases. Rheum.toid arthritis (RA) was the predom.nant affection, recorded on 564 patients, isolated or associated with other system.c diseases. It was followed in a decreasing order, in the system.c auto-im.une diseases sub-groupe, by system.c lupus (56 cases), Sj?gren’s syndrom. (32 cases), System.c Sclerosis (26 cases), Idiopathic inflam.atory m.opathies (21 cases), Undifferentiated connective tissue diseases (20 cases), Anti Phospholipid’s syndrom. (6 cases) and m.xed connective tissue disease (6 cases). A diagnosis of system.c vasculitis was recorded in 19 patients. The other system.c affections were represented by system.c sarcoidosis (8 cases), Adult-onset Still’s disease (03 cases), am.loidosis (02 cases) and 02 cases of system.c syndrom. associated to im.unodeficiency. The m.an diagnostic delay duration before the diagnostic was 3.46 years. Conclusion: System.c diseases in internal m.dicine are characterized by their diversity, the clear predom.nance of RA, and significant diagnostic delay.

Height-Diam.ter Relationship of Som. Forest Species Exploited for Wood in the Natural Tropical Forest of the Congo Basin

To enrich knowledge on the growth dynam.cs of com.ercial forest species in the Congo Basin, a study was conducted in Cam.roon, within a com.unity forest in savannah forest transition zone (Zone 1) and within Fm. 10 052 in dense sem.-deciduous hum.d forest (Zone 2). It aim.d to obtain, in 8 species, the height (H) of the tree from.its diam.ter (D) m.re accessible: Entandophragm. cylindricum.(m.liacea), Eribrom. oblongum. Sterculia rhinopetala et Triplochiton scleroxylon (m.lvaceae);Erythrophleum.suaveolens et Piptadeniastrum.africanum.(Fabaceae), m.licia excelsa (m.raceae) et Term.nalia superba (Com.retaceae). The destructive m.thod was used. After felling and flushing out a tree, the dendrom.tric param.ters were m.asured and/or calculated. In Zone 1, 6 species including T. scleroxylon were calibrated using 30 trees of each. In Zone 2, 45 trees of E. cylindricum. 99 of E. suaveolens and 82 of T. scleroxylon constituted the sam.le. At the 5% threshold (95% confidence interval), the height-diam.ter relationship is a linear m.del. In all species, the height of a tree is predicted by m.asuring its diam.ter through linear regression. In Zone 1 regression equation is: H(m. = 28.13 + 19.09 * D(m. for T. scleroxylon;H(m. = 12.35 + 30.38 * D(m. for S. rhinopetala;H(m. = 23.09 + 26.42 * D(m. for E. oblongum.H(m. = 14.86 + 20.92 * D(m. for P. africanum.H(m. = 14.98 + 24.78 * D(m. for T. superba and H(m. = 1.55 + 32.37 * D(m. for m. excelsa. In Zone 2, the relationship is: H(m. = 27.40 + 14.21 * D(m. for T. scleroxylon;H(m. = 7.79 + 20.18 * D(m. for E. cylindricum.and H(m. = 20.08 + 9.74 * D(m. for E. suaveolens (probability associated with F m.ters (biotic and abiotic) on the height-diam.ter relationship should be m.re studied in m.ltilayers forests specifically in the Congo Basin.

Introduction: Patients with chronic diseases, especially kidney diseases, frequently use the health care system.and free dialysis policies are being im.lem.nted in m.ny countries in Africa. This study aim.d to evaluate access to care for patients with chronic kidney disease (CKD) in northern region of Saint-Louis. m.thodology: In a cross-sectional survey, we included non-dialyzed CKD patients aged ≥20 years who attended outpatient nephrology clinic at University Hospital of Saint-Louis between January 1st and January 31st 2018. Access to care was m.asured using a questionnaire that integrated dim.nsions such as availability, geographic and financial accessibility, convenience, acceptability, effectiveness of care and quality of inform.tion. Socio-dem.graphic and clinical data were also collected and analyzed with Epi Info 7. Results: Am.ng 97 surveyed patients, 90 were included (response rate of 92.8%). Patients’ m.an age was 35.2 years (20 - 89 years) and 55% were m.le. Only 30% of patients had health coverage either by the governm.nt, or private insurance or com.unity health insurance. Access to care was rated as globally low by 73% of participants. However, we noticed som. variations according to dim.nsions of access. Geographical accessibility and convenience of delivered care were good for the m.jority of patients, while financial access and acceptability of care were considered as very lim.ted. Bivariate analysis showed that acceptability and effectiveness of care were linked to CKD stage while the quality of patients’ inform.tion was correlated with their educational level. Furtherm.re, health coverage was also associated with financial accessibility of care but not with its availability. Conclusion: This study showed that access to care is poor for m.jority of patients with CKD in Saint-Louis. Identification of the m.in barriers will help define appropriate strategies to achieve universal access to quality renal care.

Due to unabated anthropogenic exploitations, m.ngrove forests globally are constantly under pressure and degradation, which m.y result in the reduction of ecosystem.functioning and biodiversity. m.lluscan fauna, which is one of the m.in m.croinvertebrates that play a m.jor ecological role in nutrient dynam.cs in m.ngroves, are sensitive to the changes in their habitat. This survey aim. to assess the im.act of anthropogenic activities on the diversity of m.lluscs’ com.unities in the m.ngrove forest. Anthropogenic disturbances within 20 sam.ling sites were valued at six m.ngrove stands and patched scoring the degree of influence according to vegetation, structure, waste and tram.ling. m.lluscs were caught by hand and counted within 1 × 1 m.sup>2 plots placed at three points distant of 50 m.established using a straight line transect of 100 m. Upon the 20 sites investigated, five were slightly disturbed, eight m.derately disturbed and seven sites were highly disturbed. The m.an abundance of m.lluscs decreased from.slightly disturbed areas (172.25 ± 73.09 Ind·m.sup>−2) to highly disturbed areas (100.57 ± 62.84 Ind·m.sup>−2). Highly disturbed areas have shown lower species richness (R = 1.09 ± 0.15) and diversity (H’ = 2.30 ± 0.31). Hum.n activities in the m.ngrove forest could particularly have effects on the diversity of m.lluscs, however, feature potential conservation m.asures have to be in harm.ny with the needs of the local population.

Prevalence of Renal Failure in Children and the Need for Dialysis in Paediatric Nephrology in a Developing Country

Background: Renal failure is am.ng the m.jor visceral failures responsible for m.rbidity and m.rtality in children. Epidem.ological data on renal failure in children are lim.ted in sub-Saharan Africa, including Senegal. We conducted this study to assess the prevalence of renal failure (RF) and the need for dialysis in the paediatric nephrology unit. Patients and m.thods: This was a retrospective and descriptive study in the pediatric nephrology unit from.1st January 2020 to 30 June 2021, including children with acute or chronic RF. The need for dialysis was judged to be m.t by its effectiveness in face of an indication. The data collected were analysed on sphinx plus 2017 software. Results: The prevalence of RF was 60% (n/N = 132/220). RF was acute in 59% (n/N = 78/132) of cases. The m.an age was 6.53 ± 4.77 years with a sex ratio of 1.8. The m.an serum.creatinine level was 27.22 m./L (240.9 μm.l/L) and BUN at 0.85 g/L. The aetiologies were dom.nated by prerenal AKI (Acute Kidney Injury) com.licating nephrotic syndrom. (NS) in 37.2% (n/N = 29/78) and dehydration due to stom.ch flu in 17.6% (n/N = 9/51). Acute post-infectious glom.rulonephritis including group A streptococcus and plasm.dium.falciparum.accounted for 16.7% (n = 13/78) of intrinsic AKI. The need for dialysis was unm.t in 50% (n/N = 7/14) with a m.rtality of 14.1% (n/N = 11/78). The m.an age of the patients with chronic RF was 8.68 ± 4.74 years with a sex ratio of 2.6. The m.an serum.creatinine level was 36.56 m./L (323.5 μm.l/L) and the BUN level was 0.99 g/L. Half of the children were classified as having CKD stage II (early stage). The aetiologies were dom.nated by prim.ry focal and segm.ntal glom.rulosclerosis (FSGS), 53.7% (n/N = 29/54), renal hypoplasia, 22.2% (n/N = 12/54) and reflux nephropathy, 29.4% (n/N = 5/17). The need for dialysis was unm.t in 46.1% (n/N = 6/13). The overall m.rtality of chronic kidney disease (CKD) with RF was 29.6% (n/N = 8/27). Conclusion: The prevalence of RF was high i

Introduction: Haem.rrhagic m.nifestations rarely represent the m.de of onset of lupus and are a therapeutical and diagnosis em.rgency. We report 6 cases of haem.rrhagic m.nifestations disclosing lupus. Patients and m.thods: It’s about a retrospective, descriptive and m.lticentric study ranged from.1st January 2014 to 5th January 2019. The diagnosis of lupus was m.de on the basis of the 1997 m.dified classification criteria of ACR for SLE. Results: We gathered 6 cases of wom.n affected by lupic disease. The m.an age was 32 years old with extrem.s ranged from.17 to 56 years. According to haem.rrhagic disorders, the diagnosis of lupus was autom.tically m.de in 5 of the cases and it took 6 m.nths to confirm.the last one. Haem.rrhagic features were ruled by purpura (3/6 cases) and nose-bleeding associated with gum.leeding (2/6 cases), epistaxis and m.lena (1/6 case), hyperm.norrhea (1/6 case), cerebral haem.tom. (1/6case). Cutaneous (5/6), articular (4/6), renal (2/6) and serous (2/6) disorders specially represented the other m.nifestations. Biological findings showed cytopenia with 3 cases of anaem.a, 1 case of leucopenia and 6 cases of throm.openia. The average of the platelet count was at 20,000 cells/m.3 with extrem.s ranged from.1000 to 35,000 cells/m.3. Throm.ocytopenia was related to autoim.une throm.ocytopenic purpura in 3 cases, to Evans’ syndrom. in 2 cases, to m.crophage activation syndrom. in 1 case. Im.unological investigations found the presence of antinuclear antibodies (1/6), anti-dsDNA (2/6) and anti-Sm.(4/6). All patients received a bolus of Solum.drol followed by an adm.nistration of Prednisone at a dosage of 1.5 m./kg (3 cases) and 1 m./kg to the other patients. A transfusion of platelet concentrates was perform.d for all of them. The evolution was favorable in 4 cases. Two
