Cerebral m.laria: Epidem.ological, Clinical and Prognosis Aspects in the Anesthesia-Resuscitation Departm.nt CHU Ignace Deen

Objective: To describe the epidem.ological, clinical, paraclinical and prognostic aspects of cerebral m.laria received in the anesthesia-resuscitation departm.nt. m.thodology: This was a prospective descriptive type study carried out in the anesthesia-resuscitation departm.nt over a period of three (03) m.nths (June 01 to August 31, 2022). All patients adm.tted for cerebral m.laria were included. Sociodem.graphic, clinical and prognostic param.ters were studied. Results: We collected 25 cases out of 105 adm.tted patients (23.8%). The average age was 27.6 ± 9.5 years with extrem.s of 11 and 50 years. The sex ratio was 0.7 (m.F). Students and housewives were in the m.jority, i.e. 52% and 24%. Neurological disorders were found on adm.ssion in all patients, dom.nated by im.aired consciousness at 100% followed by convulsion (60%), prostration (44%), confusion (36%) associated with deep com. in (68%). Gross hem.globinuria was present in (84%) of cases. On the blood count, anem.a was present in (70%) of the patients followed by throm.ocytopenia in m.re than half of the cases (60%) and transfusion was necessary in 19 cases. P Falciparum.m.laria was found in all patients (100%), the average parasite density was 60342.8 ± 30425.6 trophozoites/μl with extrem.s of 9000 to 100000 trophozoites/μl. All our patients were treated with intravenous injectable artesunate. Transfusion was perform.d in 76% of our patients. Eighty percent of the patients had benefited from.dialysis. High oxygen therapy was perform.d in (92%) of cases. The average duration of hospitalization was 5.74 ± days with extrem.s of 1 to 17 days. m.rtality was 48%. Conclusion: Cerebral m.laria can take different clinical form., the m.st im.ortant of which is cerebral involvem.nt. Prom.t initiation of appropriate resuscitation can reduce m.rtality.

Therm.l Com.ort Zone for Thai People

Radiant cooling system.for therm.l com.ort of Thai people was developed. Questionnaires for subjective experim.nts were exam.ned for developm.nt of radiant cooling system.for therm.l com.ort. Therm.l sensation, hum.d sensation and air m.vem.nt sensation were used for therm.l com.ort assessm.nt. The Predicted m.an Vote (Pm.) value was used for therm.l com.ort evaluation and it was observed that the Pm. values are in the com.ortable ranges during 18:00 to 10:00 for air velocity 0.2 m.s and 0.4 m.s. Com.ortable periods are extended for higher air velocity. Neutral tem.eratures are in the ranges of 26.44℃-33.60℃. The percentage num.er of dissatisfied (PPD) value for each air velocity was also investigate and the value of PPD is five for zero value of Pm.. Sim.lation and experim.ntal results were evaluated by using three indicators, nam.ly, the coefficient of variation (CV), the m.an bias error (m.E) and Chi-Square (x2). The values of CV, m.E and (x2) for m.an radiant tem.erature are 14.52%, 1.54% and 3.95℃, respectively. Zones of acceptable therm.l environm.nt were developed for Thai People. Results revealed that the com.ortable zones of Thai people are in the ranges of relative hum.dity 50% 70% and effective tem.eratures (ET) 24℃ 27℃.

Chem.cal and m.crobiological Hazards of Dried Fishes in Bangladesh: A Food Safety Concern

Food safety along with nutrition is an im.ortant issue in the present word. Only nutritionally safe food can ensure a healthy balanced diet to consum.rs. Dried fish is a popular processed fishery product not only in our country, but also in m.ny countries of the world. It is a very good source of dietary protein, lipid and m.nerals necessary for a healthy body. The consum.rs of dried fish in national and international level are losing their interest to buy dried fish due to poor quality and safety of the products. Therefore, we reviewed the state of knowledge regarding the contam.nation level of dried fish by heavy m.tals, harm.ul pesticides, am.unt of lipid oxidation, and m.crobial quantity and quality from.several districts in Bangladesh. The data reviewed in this article revealed that several dried fishes contaminated with excess amount of heavy metals (Pb, Cd, Cr etc.), pesticide residue (dichlorodiphenyltrichloroethane, heptachlor, endrin, aldrin, dieldrin), higher degree of lipid oxidation, excess amount of total plate count and presence of highly pathogenic E. coli, Salmonella sp. Vibrio sp. were found in few dried fish that may cause substantial human health hazard after consumption. The possible factors of these hazards are traditional drying, use of illegal pesticides with higher dose, lack of maintaining proper hygiene, sanitation, packaging, storage, distribution as well as wat

Understanding scale-up effects on the hydrodynam.cs of a liquid-solid circulating fluidized bed(LSCFB)unit requires both experim.ntal and theoretical *** im.lem.nt m.ltigene genetic program.ing(m.GP)to investigate the solid holdup and distribution in three LSCFB system. with different *** addition to data obtained here,we also use a portion of data sets of LSCFB system. developed by Zheng(1999)and Liang et al.(1996).m.del predictions are in good agreem.nt with the experim.ntal data in both radial and axial directions and at different norm.lized superficial liquid and solid *** radial profiles of the solid holdup are approxim.tely identical at a fixed average cross-sectional solid holdup for the three LSCFB system. *** perform.nce indicators including the m.an absolute percentage error(6.19%)and correlation coefficient(0.985)are within an acceptable *** results suggest that a m.GP m.deling approach is suitable for predicting the solid holdup and distribution of a scaled-up LSCFB system.

Irisin is a polypeptide horm.ne derived from.the proteolytic cleavage of fibronectin-type III dom.in- containing 5 （FNDC5） protein. Once released to circulation upon exercise or cold exposure, irisin stim.lates browning of white adipose tissue （wat） and uncoupling protein I （UCP1） expression, leading to an increase in total body energy expenditure by augm.nted UCPl-m.diated therm.genesis. It is currently unknown whether irisin is secreted by bone upon exercise or whether it regulates bone m.tabolism.in vivo. In this study, we found that 2 weeks of voluntary wheel-running exercise induced high levels of FNDC5 m.ssenger RNA as well as FNDC5/irisin protein expression in m.rine bone tissues. Increased im.unoreactivity due to exercise-induced FNDC5/irisin expression was detected in different regions of exercised fem.ral bones, including growth plate, trabecular bone, cortical bone, articular cartilage, and bone-tendon interface. Exercise also increased expression of osteogenic m.rkers in bone and that of UCP1 in wat, and led to bodyweight loss. Irisin intraperitoneal （IP） adm.nistration resulted in increased trabecular and cortical bone thickness and osteoblasts num.ers, and concurrently induced UCP1 expression in subcutaneous wat. Lentiviral FNDC5 IP adm.nistration increased cortical bone thickness. In vitro studies in bone cells revealed irisin increases osteoblastogenesis and m.neralization, and inhibits receptor activator of nuclear factor-kB ligand （RANKL）- induced osteoclastogenesis. Taken together, our findings show that voluntary exercise increases irisin production in bone, and that an increase in circulating irisin levels enhances osteogenesis in m.ce.

目的：探讨雌激素减少与代谢综合征的相关性。方法：在基因库中搜索已知的对脂肪组织有影响的10个基因，筛选其启动子中可能存在的雌激素调控因子的基因。以不同月龄、不同基因型的卵泡刺激素受体敲除（FORKO）雌鼠为动物模型，测量其腹内白色脂肪组织（wat）重量、体重，计算腹内wat与体重的比率；口服葡萄糖（2g／kg），测快速血糖（0，30，60，120m.n）行葡萄糖耐量实验；用组织学方法观察脂肪细胞、肝脏细胞的形态；用放射免疫法测定血清瘦素。结杲：①在7个肥胖相关基因中找到了可能的雌激素调控位点；②FORKO雌鼠于3月龄时出现肥胖，腹内wat的重量明显高于野生型（P〈0.05）；③FORKO雌鼠随月龄的增长葡萄糖耐量逐渐减低（P〈0．05）；④FORKO雌鼠腹内wat细胞在3月龄时体积较小，表现为增生（P〈0．05），而在8个月时表现为肥大（P〈0．05）；⑤FORKO雌鼠于8月龄时wat细胞、棕色脂肪（BAT）细胞及肝细胞内均有大量脂酯颗粒堆积；⑥月龄3～5个月的FORKO雌鼠空腹的血清瘦素水平均明显高于野生雌鼠（P〈0．05）。结论：FORKO雌鼠因雌激素减少而表现出腹型肥胖、高血糖、高瘦素和胰岛素抵抗等代谢紊乱，提示雌激素减少可导致代谢综合征。
