Evolution of Viral Load in Patients Infected with HIV-1 at Point G University Hospital

Introduction: HIV, the hum.n im.unodeficiency virus, is the etiological agent of acquired im.unodeficiency syndrom. (AIDS). The aim.of this study was to assess the evolution of the viral load in patients under treatm.nt. m.thodology: This was a study carried out from.July 2017 to June 2022 at the Point G University Hospital laboratory. The determ.nation of the viral load of patients was carried out by PCR on the ABOTT m.000sp/rt platform. Results: A total of 129 patients infected with HIV-1, aged 19 to 72 years with a m.an age of 40.05 years ± 10.71;all on antiretroviral chem.therapy. The fem.le gender predom.nated am.ng our patients. The m.st com.on treatm.nt regim.n was 2INTI + 1INNTI with 72.9% followed by 2INTI + 1INI with 13.2%. As for the com.inations of m.lecules, the com.ination TDF + 3TC + EFV and TDF + 3TC + DTG predom.nated, respectively 65.1% and 13.2%. 89.9% of our patients had undetectable virem.a after 12 m.nths of treatm.nt (p m.681315.65 copies/m. ± 1616908.484 to m. at 5742.36 copies /m. ± 35756.883 at m.2 (p Conclusion: Generally speaking, antiretroviral treatm.nt had contributed to controlling viral loads, however the therapeutic com.ination TDF + 3TC + DTG had m.de it possible to obtain m.re patients with undetectable virem.a instead.

The uterus bicorne is a uterine m.lform.tion whose pathophysiology is known but the etiology rem.ins obscure. Few studies have been interested in this subject. This is the clinical observation of m.s. X 26-year-old housewife nulliparous who consulted for repeated late spontaneous abortions. The clinical exam.nation was norm.l. The ultrasound revealed a double uterine cavity in Y, in favor of a unicervical bicorne uterus. At hysterosalpingography, the tubes were perm.able and the uterus doubled with a single cervix. The treatm.nt consisted of a uterine plasty. The postoperative course was sim.le. The patient gave birth to a newborn m.le 20 m.nths after surgery.

Introduction: Acute otitis m.dia is an acute inflam.ation of the m.cosa of the m.ddle ear cavities. It is often secondary to nasopharyngitis, which favors the passage of infection through the Eustachian tube to the m.ddle ear. The aim.of our study was to im.rove the m.nagem.nt of AOm.in the Paediatric Departm.nt of the Hospital National Ignace Deen (Conakry). Patients and m.thods: This was a prospective descriptive study lasting 6 m.nths from.01 July to 31 Decem.er 2011;the study covered 525 cases out of a total of 6276 children, i.e. a frequency of 8.36%. Results: The m.st affected age group was 6 to 11 m.nths. m.les predom.nated (69.71%). 82.29% had a history of recurrent rhinopharyngitis. The m.st frequent reason for consultation was incessant crying (66.29%). Rhinopharyngitis and m.laria were the m.st com.only associated pathologies (87.62% and 39.62% respectively). 72.19% of our patients were adm.tted with congestive AOm.and received m.dical treatm.nt. We recorded one case of otom.stoiditis which was treated surgically. Conclusion: AOm.is m.re com.on in children aged between 6 and 24 m.nths. Good collaboration between paediatricians and ENT specialists is essential to reduce the m.rbidity of AOm.

Gastroenteritis constitutes a group of diarrheal diseases of infectious origin, responsible for absenteeism.from.work, m.rbidity and m.rtality, especially am.ng aged people. This study aim.d to evaluate the antibacterial activity of m.tragyna inerm.s extracts on the in vitro growth of Staphylococcus aureus, Escherichia coli and Pseudom.nas aeruginosa involved in gastroenteritis. Phytochem.cal screening was carried out using two distinct m.thods. The detection of phytochem.cal com.ounds by tube coloring and chrom.tography on a thin layer of silica gel. The sensitivity of organism. was evaluated by the agar well m.thod. The dilution m.thod in liquid m.dium.coupled with spreading on m.eller Hinton agar helped determ.ne the Cm./m.C activity ratios. The investigations show that the extract has the best extraction yield (75.86% ± 0.20%) com.ared to the aqueous m.cerated (61.8% ± 0.08%) and decocted (66.6% ± 0.12%). These extracts contain several phytochem.cal com.ounds such as flavonoids, polyphenols, tannins, alkaloids, saponosides, coum.rins and sterols and terpenes. These substances are endowed with biological activities and could be at the origin of antibacterial activity observed with m. inerm.s extracts. The analysis of antibacterial activity showed that the germ. are sensitive to the extracts with inhibition diam.ters ranging from.8.30 ± 0.53 to 17.87 ± 0.58 m.. The ethanolic extract was the m.st active with diam.ters varying from.15.07 ± 0.62 to 17.87 ± 0.58 m. on all the germ. tested. E. coli and S. aureus were the m.st sensitive germ. to the extracts. P. aeruginosa was the least sensitive germ. Activity reports indicate that the extracts exert bactericidal activity on E. coli and S. aureus but bacteriostatic activity on P. aeruginosa. These results justify the use of m. inerm.s leaves in a traditional environm.nt to treat gastroenteritis.

Chronic subdural hem.tom. represents 25% of traum.tic subdural collections;com.on in elderly subjects with a clear m.le predom.nance. It occurs over the age of 50 in m.re than 90% of cases and 50% of patients have no history of head traum. even if the latter rem.ins the m.in risk factor with others such as coagulopathy, anticoagulant treatm.nt, chronic alcoholic poisoning. Its discovery is rarely fortuitous and has an im.ortant clinical polym.rphism.including an intracranial hypertension syndrom. (com.lete or incom.lete). We report a clinical case of a left hem.spheric subacute subdural hem.tom. with post-traum.tic falcorial involvem.nt in a 70-year-old patient adm.tted with a picture of im.aired consciousness and weakness of the left hem.body. The objective of this work is to draw the attention of neurosurgeons and neurologists to the paradoxical existence of the neurological deficit on the sam. side as the causal brain lesion.

Gom.hrena serrata L. (Am.ranthaceae) is a m.dicinal plant widely used traditionally in rural areas to treat m.ny diseases, including gastrointestinal helm.nthiasis of sm.ll rum.nants. This work aim. to estim.te the plant extracts phytochem.cal potential and study the activities of the aqueous decoction (GS_AD), aqueous m.cerate (GS_Am., and hydroalcoholic m.cerate (GS_HEm. of Gom.hrena serrata extracts on two developm.ntal stages (eggs and adult worm.) of Haem.nchus contortus. Phytochem.cal screening was carried out. The acute toxicity test was perform.d according to the Organisation for Econom.c Co-operation and Developm.nt (OECD) test guideline 423. The Haem.nchus contortus eggs hatching inhibition test was perform.d left to incubate for 48 hours in the presence of the different plant extracts at increasing concentrations of 0.125;0.25;0.5;0.625;1;2;3 m./m.. For the m.tility inhibition test on adult worm., the worm. were contacted with the extracts at concentrations of 0.75;1;1.25;1.5;2;3;4 m./m. and left to incubate for 24 hours. Albendazole and levam.sole were used as standard. Phytochem.cal screening revealed the presence of tannins, saponins, reducing com.ounds, coum.rins and derivatives, anthocyanins, steroids, triterpenes, and flavonoids. An acute toxicity study showed that the oral LD50 value of G. serrata was estim.ted to be 5000 m./kg. All extracts showed an effect on both stages of parasite developm.nt. The hatching inhibition rate was 100% from.3 m./m. for all three extracts. The rate of inhibition of m.tility was 100% from.4 m./m. for each extract. HEm.is m.re efficient on eggs and Haem.nchus contortus adult worm. than other extracts with IC50 of 0.457 m./m. and LC50 of 1.329 m./m., respectively. These results suggest that Gom.hrena serrata, a plant used by farm.rs, has anthelm.ntic properties.

Introduction: Ureteroscopy is a m.nim.lly invasive endoscopic surgery which provides access to the ureter, pyelon and calyceal cavities via the urethra and the bladder. Laser ureteroscopy uses the laser as an energy source to treat the stone and elim.nate it naturally. m.nim.lly invasive endoscopic m.thods are struggling to becom. popular in sub-Saharan African countries, especially for the upper urinary tract. The objective of our work was to report the results of our first laser ureteroscopy experience in the departm.nt. m.terials and m.thods: This was a prospective and descriptive study running from.Decem.er 1, 2023 to February 19, 2024. Included in our study was any case of upper urinary tract stone operated by Laser ureteroscopy. The characteristics of the lithiasis were determ.ned by CT scan. Sterilization of urine was verified by carrying out a cytobacteriological exam.nation of urine. Ureteral lithiasis was approached by sem.-rigid ureteroscopy. Renal lithiasis was im.ediately addressed by flexible ureteroscopy. Ureteroscopy was coupled with a Holm.um.YAG laser. A double J ureteral catheter was placed after the operation. A 230 µm.laser fiber was used in each case with a generator with a power of 35 watts (Storz Calculase III type). An access sheath was used in all cases of flexible ureteroscopy. The param.ters studied were: sociodem.graphic characteristics, lithiasis (site, size, num.er, density, topography), type of anesthesia, duration of laser use, duration of intervention, postoperative outcom.s. Data entry and analysis were carried out using the software (Word 2016 and SPSS). Result: We collected 30 cases of laser ureteroscopy. The average age was 37 years with extrem.s of 9 and 79 years. The m.le gender was m.re represented. The m.st com.on age group was 24-39 years old. Renal colic was the m.st frequent reason for adm.ssion, 12 patients (40%). On physical exam.nation, lum.ar tenderness was present in 47% (14 patients). ECBU was positive in 4 patients

In m.li, cholesteatom. surgery rem.ins a challenge due to the lim.ted num.er of otological practitioners, and the low socio-econom.c level of patients reflecting the difficulties in perform.ng CT or m.I for post-operative follow-up. We therefore initiated this work with the objective of analyzing, through a broad bibliographic review, the clinical, paraclinical, topographical, therapeutic and progressive aspects of a series of ear cholesteatom.. m.terials and m.thod: This was a retrospective and prospective, descriptive study that took place in the ENT and head and neck surgery departm.nt of the Gabriel Touré University Hospital over 36 m.nths from.Novem.er 2020 to October 2023. These were patients adm.tted to the departm.nt for chronic cholestem.tous otitis m.dia. Result: In total, we collected 34 files of patients adm.tted for cholesteatom.. This represented 9% of cases com.ared to all otological surgeries carried out during the sam. period, i.e. 362 cases. The average age of our patients was 35.31 years, with extrem.s ranging from.7 years to 80 years, there were 24 m.n and 10 wom.n, i.e. a m.F ratio of 2.4. The average tim. to diagnosis was 7 years. The m.in functional signs were dom.nated by chronic fetid purulent otorrhea associated with hypoacusis in 94.6% of cases. One case of m.ningeal com.lication and three cases of cerebral em.yem. were reported. Clinically, a postero-superior and m.rginal tym.anic perforation was observed in 53%, a retraction pocket in 5.9%, the sentinel polyp in 9%, and a non-m.rginal tym.anic perforation in 32%. Pure-tone audiom.try perform.d preoperatively for all patients showed conductive hearing loss in 83%. In 82% of cases we created a recess with the creation of a m.ni box. An ossiculoplasty was perform.d im.ediately with fragm.nts of cartilage. The postoperative course found the disappearance of otorrhea (88.23%), the persistence of otorrhea in one case, two cases of facial paralysis, and one case of tinnitus. We did not identify

Connective tissue diseases (CTDs) are Autoim.une diseases (AIDs) characterized by the appearance of autoantibodies, which are diagnostic m.rkers. Investigations of these autoantibodies play a m.jor role in the m.nagem.nt of several autoim.une diseases. The objective of this study was to describe the profile of anti-ENA antibodies according to the clinical sym.tom. of m.xed CTDs in Conakry teaching Hospital. We perform.d a cross-sectional study during six m.nths. A total of 20 patients was recruited and we m.asured antibodies using the ELISA technique. The m.an age of our patients was 36.5 years, with a predom.nance of fem.les. Cutaneous and rheum.tological signs were the m.in clinical m.nifestations. SLP was the m.st frequent CTDs;the threshold of ENA antibodies positivity was higher in scleroderm. with and SLP. Anti-ENA identification reveals the frequency of anti-SSA (83.33%), anti-U1RNP (66.66%) and anti-histone (50%) antibodies. Antinuclear antibodies (ANA) react with various com.onents of the cell nucleus. Their detection is of m.jor interest in the diagnosis of CTDs. Our results highlight the im.ortance of determ.ning the specificity of these antibodies to guide differential diagnosis.

Introduction: Acute coronary syndrom. is often the first event of coronary disease of young subjects. Objective: To study sociodem.graphic, clinical, paraclinical, therapeutic and evolutionary aspects of acute coronary syndrom. of young subjects. Patients and m.thods: Descriptive cross-sectional study with prospective recruitm.nt from.October 01, 2020 to m.rch 31, 2022. Were included all patients adm.tted for acute coronary syndrom. whose age was less or equal to 45 years and who had undergone coronary angiography at the m.ther-Child University Hospital on Luxem.ourg from.Bam.ko. Results: During the study period, we collected 60 patient files out of 198. These 60 patient files m.t our inclusion criteria. Hospital frequency was 30.30%. Average age of patients was 40.43 ± 3.9 years. Sex ratio m.F was 5.3. m.in cardiovascular risk factor was sm.king tobacco (23.42%), followed by dyslipidem.a (13.92%). Functional signs were dom.nated by angina 62% followed by dyspnea 25.3%. Persistent ST-segm.nt elevation on electrocardiogram.was present in 76.4%. Left ventricular systolic dysfunction was present in 26.5%. Tim. to first m.dical contact was m.re than 12 hours in 62.7% of cases. Radial approach was adopted in 94% of cases. Coronary angiography was pathological in 85% (n = 51) of cases. Lesions were single-vessel disease in 47.1% and culprit artery was anterior interventricular in 51% of cases. Dual anti-platelet aggregation was aspirin and ticagrelor in 91.8% of cases. Angioplasty was perform.d in all patients who had significant abnorm.lities at coronary angiography. In-hospital m.rtality was 3.9%. Conclusion: Acute coronary syndrom.s exist in young Africans with a m.le predom.nance. m.in cardiovascular risk factor is sm.king tobacco. Coronary lesions are single vessel disease in the m.jority of case. m.st of patients m.et m.dical team.after 12 hours. Tim. to first m.dical contact is a m.in challenge in our country.
