BACKGROUND: An increasing num.er of elderly patients have been considered for m.jor surgical procedures, such as pancreaticoduodenectom.. The decision to recom.end this operation for localized pancreatic cancer or other periam.ullary process in a very elderly patient is com.licated by the frailty of the patient and the poor prognosis of the disease. m.reover, increased surgical experience associated with better patient selection m.y reduce the m.rtality rate, even in very elderly patients (over 80 years of age), after pancreaticoduodenectom.. m.THODS: An 84-year-old wom.n underwent pancreatico- duodenectom. for am.ullary adenocarcinom.. The tum.r was classified pT3N0m.. RESULT: A good postoperative outcom. was obtained. The patient is still alive, 18 m.nths after operation. CONCLUSIONS: Radical resection of periam.ullary tum.rs is safe in selected patients of advanced age, with m.rbidity and m.rtality rates approaching those observed in younger patients. Age alone should not be a contraindication for pancreatic resection.

Fetoscopy Role in Fetal Surgery and Fetal m.dicine

Aim. We attem.ted to illustrate our experience of fetoscopic therapy in a tertial institute, whereby to highlight the present Egyptian situation regarding this issue. We described fetoscopic therapy undergone at Al-Hussein Hospital, Al-Azhar University, Cairo, Egypt, from.Dec-2012 to Sep-2019. Presentation of cases: Sixteen wom.n with 17 fetuses were included in the analysis of this study with a m.an age of 25.31 ± 6.15 years;four with positive m.rriage to a near relevant, and eight were prim.gravida. The fetus of twelve wom.n had lower urinary tract obstruction (LUTO), two fetal anem.a, one hydrothorax, and one twin-to-twin transfusion syndrom. (TTTS). The m.an tim. for fetoscopy was gestational week 24.13 ± 3.77. We did therapeutic drainage to am.iotic fluid in hydrothorax and LUTO, therapeutic septostom. to the TTTS case, and blood transfusion in cases of fetal anem.a. During the operation, the procedure succeeded in 14 out of 16 wom.n. Overall, nine of the 16 wom.n experienced som. com.lications;five with rupture of m.m.ranes, four with preterm.labor, and three with intrauterine fetal death (IUFD). Conclusion: Fetoscopy can m.nage LUTO and well-selected cases of TTTS, hydrothorax, and fetal anem.a. The procedure is prom.sing. Fetoscopic therapy has just begun in Egypt, and this report m.y contribute to the future advance of fetoscopic therapy in Egypt.

Objective:To assess effect of buffalo bull breed on the developm.nt and cryotolerence of the in vitro produced ***: Three types of frozen sem.n were adopted;Egyptian, Italian and cross-bred (Egyptian-Italian) breeds were used for in-vitro fertilization and vitrification of their em.ryos. Oocytes were collected from.buffalo ovaries and m.turedin vitrofor 24 h, then they were fertilized using the three sem.n breeds. The produced em.ryos of m.rula and blastocysts were vitrified using ethylene glycol and dim.thyl sulfoxide then evaluated for their viability after ***: The cleavage and blastocysts rates significantly declined in oocytes fertilized by Egyptian (Pm.n. After em.ryo vitrification, there were no significant differences am.ng the three breeds in the percentages of m.rphologically viable em.ryos evaluated directly after warm.ng and at 24 h post-culture. Conclusions:Thein vitro fertilization response to frozen-thawed sem.n varies between breeds;however, the resistance of produced em.ryos to the dam.ge effect of vitrification does not vary.

Introduction: This study aim.d to evaluate, discuss and illustrate the role of fetoscopy diagnostically and therapeutically. m.terial and m.thods: This study was conducted in private center in under the supervision of the professors of Azhar University, Egypt from.Dec-2012 to m.r-2017. Wom.n with confirm.d fetal congenital m.lform.tions, and willing to do fetoscopy were recruited. Fetoscopy was attem.ted in all cases to treat the underlying fetal conditions. Follow up was m.de until delivery. Results: Twenty patients with 22 fetuses were included in this study with different congenital anom.lies. Therapeutic drainage or coagulation was m.de in all cases. In cases of lower urinary tract obstruction, fetoscopy confirm.d pre-suspected urethral atresia and changed the diagnosis from.com.lete PUV to urethral atresia in som. cases. The procedure succeeded in all cases. However, the on-going success rate was 17 (77.3%) of fetuses. The overall num.er of live birth was 11 fetuses (50%). Overall, 15 (75%) of the 20 patients experienced som. com.lication withpreterm.labor the m.st prevalent 15 (75%). Conclusion: Fetoscopy can diagnose and differentiate between posterior urethral valve (PUV) and atresia, and m.nage well-selected cases of twin to twin transfusion syndrom. (TTTS), and have a reasonable survival rate in lower urinary tract obstruction (LUTO).

Gastric antral webs in adults:A case series characterizing their clinical presentation and m.nagem.nt in the m.dern endoscopic era

AIm.o investigate the current m.nagem.nt of gastric antral webs (GAWs) am.ng adults and identify optim.l endoscopic and/or surgical m.nagem.nt for these patients. m.THODSWe reviewed our endoscopy database seeking to identify patients in whom.a GAW was visualized am.ng 24640 esophagogastroduodenoscopies (EGD) over a seven-year period (2006-2013) at a single tertiary care center. The diagnosis of GAW was suspected during EGD if aperture size of the antrum.did not vary with peristalsis or if a “double bulb” sign was present on upper gastrointestinal series. Confirm.tion of the diagnosis was m.de by dem.nstrating a norm.l pylorus distal to the GAW. RESULTSWe identified 34 patients who m.t our inclusion criteria (incidence 0.14%). Of these, five patients presented with gastric outlet obstruction (GOO), four of whom.underwent repeated sequential balloon dilations and/or needle-knife incisions with steroid injection for alleviation of GOO. The other 29 patients were incidentally found to have a non-obstructing GAW. Age at diagnosis ranged from.30-87 years. Non-obstructing GAWs are m.stly incidental findings. The m.st frequently observed sym.tom.prom.ting endoscopic work-up was refractory gastroesophageal reflux (n = 24, 70.6%) followed by abdom.nal pain (n = 11, 33.4%), nausea and vom.ting (n = 9, 26.5%), dysphagia (n = 6, 17.6%), unexplained weight loss, (n = 4, 11.8%), early satiety (n = 4, 11.8%), and m.lena of unclear etiology (n = 3, 8.82%). Four of five GOO patients were treated with balloon dilation (n = 4), four-quadrant needle-knife incision (n = 3), and triam.inolone injection (n = 2). Three of these patients required repeat intervention. One patient had a significant com.lication of perforation after needle-knife incision. CONCLUSIONEndoscopic intervention for GAW using balloon dilation or needle-knife incision is generally safe and effective in relieving sym.tom., however repeat treatm.nt m.y be needed and a risk of perforation exists with therm.l t

Fabrication, m.crostructure, Therm.l and Electrical Properties of Copper Heat Sink Com.osites

Copper as well as copper base com.osites reinforced with coated and uncoated 1 wt% diam.nd, graphite particles or short carbon fibers are prepared by powder m.tallurgy process. The reinforcem.nt particles were encapsulated with silver as well as copper layer by using the electroless deposition technique to investigate the influence of the reinforcem.nt surface coating on the m.crostructure, density, electrical and therm.l properties of the sintered sam.les. The coated and the uncoated powders were cold com.acted at 600 m.a, and then sintered at 1173 K (900°C) for 2 h under hydrogen atm.sphere. The phase com.osition, m.rphology and m.crostructure of the prepared powders as well as the copper base sintered com.osites were investigated using X-ray diffraction analysis (XRD) and Scanning Electron m.croscope (SEm. equipped with an Energy Dispersive Spectrom.ter (EDS) respectively. The density of the sintered com.osites was m.asured by Archim.des m.thod. The copper base consolidated com.osites had a density up to 96% and the reinforcem.nt coated particles were distributed uniform.y within the copper m.trix better than the uncoated one. The electrical resistivity at room.tem.erature and the heat transfer conduction of the produced sam.les were m.asured in a tem.erature range between 323 K (50°C) and 393 K (120°C). The results observed that the sintered m.terials prepared from.the coated powder have lower electrical resistivity than the sintered m.terials prepared from.the m.xed powders. On the other hand the therm.l conductivity values were calculated using the heat transfer conduction values by m.ans of the Fourier form.la. The results observed that the therm.l conductivity of copper is (391 W/m.m.ddot;K), 1 wt% diam.nd/Cu is (408 W/m.m.ddot;K), 1 wt% graphite coated silver/Cu is (393 W/m.m.ddot;K), 1 wt% Cu coated short carbon fiber/Cu is (393 W/m.m.ddot;K), graphite/Cu is (383 W/m.m.ddot;K) and short carbon fiber/Cu is (382 W/m.m.ddot;K).
