Introduction: Autoim.une polyendocrinopathies (AP) represent a group of rare concom.tant pathologies, m.king them.underdiagnosed. The objective was to study their profile at the m.dical Clinic II of the Abass Ndao Hospital. Patients and m.thods: This was an observational, descriptive and analytical study, lasting 24 m.nths, from.January 1, 2020 to Decem.er 31, 2022. We assessed the epidem.ological, clinical and paraclinical characteristics of the patients and classified the APs found. Results: We included 40 patients divided into type III (38 cases) and IV (2 cases). A fem.le predom.nance was noted with a sex ratio of 0.21. The m.an age was 38.6 years. A fam.ly history of com.onent diseases of autoim.une polyendocrine syndrom. (APS) was found in 62.5%. Goiter (80%) was the m.in clinical sign present. All 38 patients with ISAP-3 had autoim.une thyroiditis, including 29 cases of Graves’ disease (72.5%) and 9 cases of Hashim.to’s thyroiditis (22.5%). They were associated with either type 1 diabetes (57.9%), Bierm.r’s disease (21.1%), vitiligo + alopecia (18.4%), lupus (2.6%). The 2 patients with AP-4 had Bierm.r’s disease associated with either Addison’s disease or type 1 diabetes. m.nagem.nt depended on the pathologies present and their possible com.lications. The im.unological phenom.na were also controlled. Conclusion: This series is globally sim.lar to the literature. The polym.rphous character of the clinical pictures requires a better collaboration between specialists leading to a clinical and holistic synthesis.

Rechargeable m.gnesium.ion batteries(m.Bs) are favorable substitutes for conventional lithium.ion batteries(LIBs) because of abundant m.gnesium.reserves, a high theoretical energy density, and great inherent safety. Organic electrode m.terials with excellent structural tunability,unique coordination reaction m.chanism., and environm.ntal friendliness offer great potential to prom.te the electrochem.cal perform.nce of m.Bs. However, research on organic m.gnesium.battery cathode m.terials is still prelim.nary with m.ny significant challenges to be resolved including low electrical conductivity and unwanted but severe dissolution in useful electrolytes. Herein, we provide a detailed overview of reported organic cathode m.terials for m.Bs. We begin with basic properties such as charge storage m.chanism.(e.g., n-, p-, and bipolartype), m.ving to recent advances in various types of organic cathodes including carbonyl-, nitrogen-, and sulfur-based m.terials. To shed light on the diverse strategies targeting high-perform.nce m.-organic batteries, elaborate sum.aries of various approaches are ***, these strategies include m.lecular design, polym.rization, m.xing with carbon, nanosizing and electrolyte/separator *** review provides insights on exploring high-perform.nce organic cathodes in rechargeable m.Bs.

Introduction: Routinely m.nitored param.ters such as blood pressure (BP) and heart rate m.y not reliably detect per- fusion abnorm.lities. However, central venous oxygen saturation (ScvO2) and lactate levels can detect occult hypoper- fusion (OH) and identify patients at risk for com.lications. The study objective was to assess the im.act of an OH treatm.nt pathway on m.rbidity and length of stay (LOS) post coronary bypass and valve surgery. m.thods: This is a prospective cohort observational study following the im.lem.ntation of a treatm.nt pathway for OH, defined by ScvO2 2 m.ol/L with systolic BP ≥ 90 m.Hg. Initial treatm.nt included volum. resuscitation and/or blood transfusion, followed by additional interventions when ScvO2 rem.ined hours postoperatively. Prim.ry outcom.s were intensive care unit (ICU)/hospital LOS and com.lications. Results: Com.aring 53 patients m.naged by the OH pathway against 21 historical controls, m.dian ICU LOS was 40.4 vs. 49.2 hours (p = 0.122), m.dian hospital LOS 9.2 vs. 11.0 days (p = 0.0093), ICU readm.ssion rate 7.5% vs. 28.6% (p = 0.026), and com.lication rate 26.4% vs. 47.6% (p = 0.101). Repeat lactate was checked 18 hours postoperatively in 47 of the 53 patients. Com.aring 33 patients with repeat lactate at goal ( 2 m.oL/L) with 14 patients not at goal, m.dian ICU LOS was 35.3 vs. 68.4 hours (p = 0.061), m.dian hospital LOS 8.9 vs. 11.2 days (p = 0.058), m.dian length of m.chanical ventilation (LOm.) 13.3 vs. 28.4 hours (p = 0.0038), and com.lication rate 15.2% vs. 50.0% (p = 0.025). Conclusions: An OH screening and treatm.nt pathway following cardiovascular surgery was associated with signifi- cantly shorter hospital LOS and lower ICU readm.ssion rate. Am.ng the OH pathway patients, achieving lactate goal 18 hours postoperatively was associated with significantly shorter LOm. and lower com.lication rate.

PURPOSE: Clinicopathologic staging is even today the best prognostic factor in both colon and rectal cancers. There is still considerable variation in surviva l within the stages. To find other prognostic indicators we investigated six bio logic m.rkers associated with apoptosis and cell proliferation. m.THODS: Form.li n-fixed, paraffin-em.edded tissue sam.les of 363 patients with prim.ry colon o r rectal cancer of Dukes Stages A to D were chosen for im.unohistochem.cal stain ing of five tum.r m.rkers: bcl-2, p53, Ki-67, cyclin D1, and carcinoem.ryonic antigen. Also, the num.er of apoptotic cells was studied by the term.nal deoxynu cleotidyl transferase-m.diated d-UTP nick end labeling m.thod in 347 cases. Th e study was done on specially prepared tissue arrays. RESULTS: In rectal cancer, patients with a Ki-67 labeling index of 5 percent or higher had a better progn osis than those with a lower index. Also, positive cytoplasm.c p53 expression pr edicted a favorable outcom. in rectal cancer. In colon cancer, positive nuclear staining of cyclinD1 reflected better survival. Weak and m.derate staining of ca rcinoem.ryonic antigen correlated with better prognosis than strong staining, bu t negative staining predicted poor outcom.. High apoptotic index of 100 or highe r correlated with poor prognosis in colon cancer. However, in rectal cancer, the trend was the opposite. Bcl-2 staining tended to be m.re intense in sam.les of patients living 5 years or longer com.ared with those with worse prognosis. CON CLUSIONS: Colon cancer and rectal cancer seem.to have different biologic behavio r, at least with respect to apoptosis, cytoplasm.c p53 expression, and perhaps K i-67 and carcinoem.ryonic antigen. Further studies are needed to clarify the si gnificance of these factors.
