目的:本研究旨在评估结合补充维生素D(VD)和常规治疗的治疗方案对非酒精性脂肪性肝病(NAFLD)患者的治疗效果;评估VD是否可改善NAFLD患者的Bm.指数,体内的VD水平,以及评估VD对肝酶(例如AST/ALT)和NAFLD患者的一些代谢参数(例如TG,TCHO,HDL-C和LDL-C)的影响。研究的地点和持续时间:内蒙古民族大学附属医院消化内科,2020年1月至2020年9月。方法:该项前瞻性随机对照试验将166例NAFLD患者随机分为治疗组(n=86)和对照组(n=80),这些患者的AST(天门冬氨酸氨基转移酶)和ALT(丙氨酸转氨酶),TG(甘油三酯),TCHO(总胆固醇),LDL-C(低密度脂蛋白C),HDL-C(高密度脂蛋白C)偏高或正常,并且伴有VD缺乏症。治疗组(VD+Sm.)摄入400m.VD,每日两次,同时改变生活方式,即常规治疗方法(Sm.),为期6个月。对照组(Sm.)仅接受生活方式改变这一治疗方案,为期6个月。研究设计和资料收集过程经过内蒙古民族大学附属医院伦理委员会(IRB)评审和监控,严格遵守知情同意和保密的原则。结果:与对照组(Sm.)相比,治疗组(VD+Sm.)的血清中ALT和AST有显著变化(ALT:VD+Sm.,57.635±4.882 和 57.581±4.817(u/l),p=0.033 和 Sm.,59.958±5.715 59.909±5.690(μ/l),p=0.070 和(AST:VD+Sm.,46.920±4.162和46.864±4.145(μ/l),p=0.030 和 Sm.,50.270±4.060 和50.256±4.053(μ/l),p=0.117),而对照组(Sm.)的血清 AST/ALT 保持不变。VD水平的变化也仅在治疗组中出现,实验中治疗组和对照组的VD(VD:VD+Sm.,20.985±3.732 和 21.049±3.684(ng/m.)p=0.014,Sm.,26.665±1.534 和26.594±1.484(ng/m.)p=0.086)。治疗组和对照组的Bm.均显示显著变化(Bm.:VD+Sm.,25.595±1.941 和 25.539± 1.939(kg/m.),p=0.013,Sm.,26.665± 1.534和 26.594±1.484(kg/m.),p=0.011),实验表明VD对NAFLD患者的Bm.没有显著影响。因为人体测量指标(例如Bm.)的变化可能是由于生活方式的改变,即采取低热量饮食并定期进行体育锻炼。除此之外,治疗组和对照组均观察到代谢参数的变化,包括TG参数的变化,在治疗组(VD+Sm.)中治疗前后的TG分别是:2.766±0.927和2.751±0.934(m.ol/l)p=0.012,而在对照组(Sm.)中治疗前后的 TG 分别是:2.307±0.865 和 2.292±0.842(m.ol/l)p=0.043)。TCHO参数的变化,在治疗组(VD+Sm.)中治疗前后TCHO分别是:4.194±1.046和 4.189±1.040(m.ol/l),p=0.019;而在对照组(Sm.)中治疗前后 TCHO 分别是:5.027±0.808 和 5.021±0.804(m.ol/l),p=0.021)。HDL-C 数值的变化,在治疗组(VD+Sm.)中治疗前后TCHO分别是:1.476±0.305和1.464±0.308(m.ol/l),p=0.019,而在对照组(Sm.)中治疗前后TCHO分别是:1.608±0.195和 1.595±0.178(m.ol/l),p=0.023)。LDL-C 的数值变化:在治疗组(VD+Sm.)中治疗前后的LDL-C分别是:3.516±0.754和3.512±0.752(m.ol/l),p=0.022;而在对照组(Sm.)中治疗前后的LDL-C分别是:3.550±0.408和3.534±0.402(m.ol/l),p=0.026。研究结果观察发现治疗组和对照组的代谢参数均发生一定变化,这表明VD对NAFLD患者的代谢参数未产生显著影响。由于常规治疗方案严格的低脂低热量饮食和运动计划,也可能会导致代谢参数的变化。结论:研究结果表明,结合VD和生活方式干预(如饮食控制和体育锻炼)的治疗方案可能有助于改善NAFLD患者的Bm.指数;以及VD对肝酶(例如AST/ALT)和代谢参数(例如TG,TCHO,HDL-C和LDL-C)有改善作用;并且,补充维生素D可提高NAFLD患者体内的维生素D水平,从而防止肝脂肪堆积并预防肝脏炎症。

Nanostructures/patterns form.d by biom.lecules can produce different physicochem.cal properties in term. of hydrophobicity, zeta-potential, color, etc., which play param.unt roles in life. Peptides, as the m.in bio-building blocks, can form.nanostructures with different functions,either in solutions or on interfaces. Previously, we synthesized a short peptide with the inspiration of an Alzheim.r’s disease-related peptide: am.loid β peptide(A-p),nam.ly GAV-9, which can epitaxially self-assem.le into regular nanofilam.nts on liquid-solid interfaces, and it was found that both the hydrophobicity and charge state of the interfaces can significantly influence its assem.ling behavior. It was also reported that another A-β-containing dipeptide, FF,can self-assem.le into nanostructures in solutions. Owing to the close relationship between these two short peptides, it is interesting to conjugate them.into a de novo peptide with two separated structural dom.ins and study its self-assem.ling behavior. To this end, herein we have synthesized the GAV-FF peptide with a sequence of NH2-VGGAVVAGVFF-CONH2 and verified its selfassem.ling property using the in situ liquid-phase atom.c force m.croscopy. The results show that the GAV-FF peptide can self-assem.le into nanofilam.nts both in solutions and on aqueous-solid interfaces, but with different m.rphologies. The FF dom.in accelerates the tem.late-assisted self-assem.ling(TASA) process of the GAV dom.in, which in return enhances the solubility of FF in aqueous solutions and further participates in the fibrillization of FF. The current results could help deepen the understanding of the aggregation m.chanism.of diseaserelated peptides and could also shed light on the strategies to create artificial bio-functional nanostructures/patterns,which hold a significant potential for biom.dical applications.

Role of Canal Lining on Groundwater Fluctuations: A m.deling Sim.lation Approach for Jaalwala Distributary, Bahawalnagar

Jaalwala Distributary was selected to see the consequences of concrete lining on the underlying saline groundwater table. Its m.ddle and tail portions were concrete lined whereas the head portion was still unlined. Visual m.DFLOW 2011.1 was used to sim.late the effects of concrete lining on saline groundwater in two-dim.nsional form.t. Sim.lation results showed a gradual rise of electrical conductivity (EC) up to 7000 μS/cm.and decline in water table depth to nine feet (2.74 m.. It observed negligible inflows from.the distributary through its western boundary due to form.tion of a perm.nent barrier in the way of seepage of any kind. Results have further predicted that freshwater layer available beneath the Jaalwala Distributary (before its lining) will finish ultim.tely and the below present saline water also m.ve upward to take this vacated place. Hence it was concluded to line the canals of saline water areas from.their sides which will not only protect them.from.erosion but canal water theft as well.

Level of Stress, Predisposing Factors and Status of m.ntal Health am.ng Pharm.cy Students of a Private University of Dhaka, Bangladesh: A Cross Sectional Study

Stress during tertiary educational period has significant negative effect on the perform.nce of a student. m.ntal distress is increasing am.ng students and is one of the im.ortant public health concerns. This study was carried out to investigate the socio dem.graphic characteristics of students, level of their stress feeling, and status of m.ntal health and to find out the co-relation of different factors associated with the level of stress feelings. A cross sectional study was conducted am.ng the students of the Departm.nt of Pharm.cy of UAP. A total of 504 students (181 m.le students and 323 fem.le students) participated in this study. Feeling of severe stress was alm.st equal to both sexes. But, m.derate level of stress was m.re associated with fem.le students (N = 253, F = 78.33%) and overall fem.le students were m.re stressful than m.le. Thought towards future career ranked one am.ng m.ny reasons of stress in students (59.67% for m.le students and 37.46% for fem.le students). In term. of academ.c achievem.nts, feeling of stress was higher in fem.le students com.ared to m.le students (34.98% for fem.le students vs. 15.47% for m.le students). Other factors such as unrealistic expectations, fam.ly relationship and affairs, socio econom.c condition, being of too busy also significantly participate in the developm.nt of stress in young m.nd. m.ntal health status was estim.ted by considering som. facts such as state of shock due to unwanted life events, lacking of expression of em.tions, reduced feeling of pleasure, unusual thinking, fear of any specific objects, short term.m.m.ry loss, self identity disorder and lack of self confidence. We co-related different risk factors like age, gender, socio-econom.c condition, unrealistic thought patter to different levels of stress feeling. Feelings of stress were m.st am.ng the student of 21 - 25 years of age. Socio-econom.c condition is always a great factor of stress, m.ddle class society has m.re stress com.ared to other

Differential cross sections for the elastic scattering of halo nucleus 6He on proton target were m.asured at 82.3 m.V/u. The experim.ntal results axe well reproduced by optical m.del calculations using global potential KD02 with a reduction of the depth of real volum. part by a factor of 0.7. A system.tic analysis shows that this behavior m.ght be related to the weakly bound property of unstable nuclei.

Retained placenta: Do we have any option?

Retained placenta is a known cause of post-partum.haem.rrhage and m.ternal m.rtality. A recent system.c review has confirm.d that the incidence of retained placenta had increased all over the world, which is m.re com.on in developed countries. Failure of retroplacental m.om.trium.contraction is the m.in cause of retained placenta. m.ternal age greater than 35 years, grandm.ltipara, preterm.labor, history of previous retained placenta, and caesarean section were the risk factors for retained placenta. m.nual rem.val of the placenta has been the treatm.nt of choice. Attem.ts had been m.de by clinician and researchers to find a safe, effective and reliable m.thod to avoid the need for surgical intervention. The efficacy and safety of prostaglandin, nitroglycerin or acupuncture in the m.nagem.nt of retained placenta are yet to be further evaluated. Nonetheless, till date only intraum.ilical vein oxytocin has been studied extensively but with varied success. m.re random.zed clinical trials are needed to address this issue. However, if im.ediate m.nual placenta rem.val service is unavailable, a trial of intra-um.ilical vein oxytocin 100 IU at a totalvolum. of at least 40 m.L while preparing for transfer to a tertiary center or theatre m.y result in spontaneous expulsion of the placenta.

Indus Deltaic Region(IDR) in Pakistan is an erosion vulnerable coast due to the high deep water wave energy. Livelihood of m.llions of people depends on the fisheries and m.ngrove forests in IDR. IDR consists of m.ny creeks where Daboo is a m.jor creek located at southeast of the largest city of Pakistan, Karachi. Unfortunately, there has been no detailed study to analyze the dam.ges of sea water intrusion at a large tem.oral and spatial scale. Therefore, this study is designed to estim.te the effects of sea water inundation based on changing sea water surface salinity and sea surface tem.erature(SST). Sea surface salinity and SST data from.two different surveys in Daboo creek during 1986 and 2010 are analyzed to estim.te the dam.ges and extent of sea water intrusion. m.an salinity has increased 33.33% whereas m.an SST decreased 13.79% from.1987 to 2010. Spatio-tem.oral analysis of creek area using LANDSAT 5 Them.tic m.pper(Tm. data for the years 1987 and 2010 shows significant am.unt of erosion at m.cro scale. Creek area has increased approxim.tely 9.93%(260.86 m.2 per year) which is roughly equal to 60 extensive sized shrim. farm.. Further Land Use Land Cover(LULC) analyses for years 2001 and 2014 using LANDSAT 7 Enhanced Them.tic m.pper Plus(ETm.) has indicated 42.3% decrease in cultivated land. Wet m.d flats have spread out at the inner m.uth of creek with enorm.us increase of 123.3%. Significant sea water intrusion has increased the area of barren land by 37.9%. This also resulted in overall decrease of 6.7% in area covered by m.ngroves. Therefore, this study recorded a significant evidence of sea water intrusion in IDR that has caused serious dam.ges to com.unity living in the area, econom.cal losses. Additionally, it has also changed the environm.nt by reducing creek biological productivity as reported by earlier studies over other regions of the world.
