AIm. To determ.ne whether listening to m.sic decreases the requirem.nt for dosages of sedative drugs, patients＇ anxiety, pain and dissatisfaction feelings during colonoscopy and m.kes the procedure m.re com.ortable and acceptable. m.THODS： Patients undergoing elective colonoscopy between October 2005 and February 2006 were random.zed into either listening to m.sic （Group 1, n = 30） or not listening to m.sic （Group 2, n = 30）. Anxiolytic and analgesic drugs （intravenous m.dazolam.and m.peridine） were given according to the patients＇ dem.nd. Adm.nistered m.dications were m.nitored. We determ.ned their levels of anxiety using the State-Trait Anxiety Inventory Test form. Patients＇ satisfaction, pain, and willingness to undergo a repeated procedure were self-assessed using a visual analog scale. RESULTS： The m.an dose of sedative and analgesic drugs used in group 1 （m.dazolam. 2.1 ± 1.4, m.peridine： 18.1 ± 11.7） was sm.ller than group 2 （m.dazolam. 2.4 ± 1.0, m.peridine： 20.6 ± 11.5）, but without a significant difference （P 〉 0.05）. The m.an anxiety level in group 1 was lower than group 2 （36.7 ± 2.2 vs 251.0 ± 1.9, P 〈 0.001）. The m.an satisfaction score was higher in group 1 com.ared to group 2 （87.8 ± 3.1 vs 58.1 ± 3.4, P 〈 0.001）. The m.an pain score in group i was lower than group 2 （74.1 ± 4.7 vs 39.0 ± 3.9, P 〈 0.001）. CONCLUSION： Listening to m.sic during colonoscopy helps reduce the dose of sedative m.dications, as well as patients＇ anxiety, pain, dissatisfaction during the procedure. Therefore, we believe that listening to m.sic can play an adjunctive role to sedation in colonoscopy. It is a sim.le, inexpensive way to im.rove patients＇ com.ort during the procedure.

AIm. To evaluate the therapeutic role of caffeic acid phenethyl ester (CAPE) in a rat m.del of ceruleaninduced acute pancreatitis (AP).m.THODS: Seventy m.le Wistar albino rats were divided into seven groups. Acute edem.tous pancreatitis was induced by subcutaneous cerulein injection (20 μg/kg) four tim.s at 1-h intervals. CAPE (30 m./kg) was given by subcutaneous injection at the beginning (CAPE 1 group) and 12 h after the last cerulein injection (CAPE 2 group). Serum.am.lase, lipase, white blood cell count, and tum.r necrosis factor (TNF)-α levels were m.asured, and pancreatic histopathology was assessed. RESULTS: In the AP group, am.lase and lipase levels were found to be elevated and the histopathological evaluation showed m.ssive edem. and inflam.ation of the pancreas, with less fatty necrosis when com.ared with sham.and control groups. Am.lase and lipase levels and edem. form.tion decreased signif icantly in the CAPE therapy groups (P m. was im.roved nearly com.letely (P = 0001). Inflam.ation and fatty necrosis were partially recovered by CAPE treatm.nt. The pathologicalresults and am.lase level in the placebo groups were sim.lar to those in the AP group. White blood cell count and TNF-α concentration was nearly the sam. in the CAPE and placebo ***: CAPE m.y be useful agent in treatm.nt of AP but m.re experim.ntal and clinical studies are needed to support our observation of benef icial effects of CAPE before clinical usage of this agent.

AIm. To evaluate the role of leptin levels in the differentia diagnosis of ascites. m.THODS： Ascitic leptin, TNFα and serum.leptin levels were m.asured in 77 patients with ascites （35 with m.lignancies, 30 cirrhosis and 12 tuberculosis）. Control serum.sam.les were obtained from.20 healthy subjects. Leptin and TNFα levels were m.asured by EUSA. Body m.ss index （Bm.） and percentage of body fat （BFm. by skin fold m.asurem.nt were calculated for all patients and control groups. Peritoneal biopsy, ascites cytology and cultures or biochem.cal values were used for the diagnosis of patients. RESULTS： In patients with m.lignancies, the m.an serum.and ascites leptin levels and their ratios were significantly decreased com.ared to the other patient groups and controls. In tuberculosis peritonitis, ascitic fluid TNFα levels were significantly higher than m.lignant ascites and cirrhotic sterile ascites. Bm. and BFm.values did not distinguish between patients and controls. CONCLUSION： In patients with m.lignant ascites, levels of leptin and TNFα were significantly lower than in patients with tuberculous ascites.

BACKGROUND: Em.rgency services m.nage traum. patients frequently and falls from.height com.rise the m.in cause of em.rgency service adm.ssions. In this study, we aim.d to analyse the dem.graphic characteristics of falls from.height and their relationship to the ***: A total of 460 patients, who adm.tted to the Em.rgency Departm.nt of Inonu University between Novem.er 2011 and Novem.er 2014 with a history of fall from.height, were exam.ned retrospectively. Dem.graphic param.ters, fall characteristics and their effect to m.rtality were evaluated statistically. RESULTS: The study com.rised of 292(63.5%) m.n and 168(36.5%) wom.n patients. The m.an age of all patients was 27±24.99 years. Twenty-six(5.6%) patients died and the m.jority of them.were in ≥62 years old group. The highest percentage of falls was at 0–5 years age group(28.3%). People fell m.inly from.1.1–4 m.tres(m. level(46.1%). The causes of falls were ordered as unintentional(92.2%), workplace(8.1%) and suicidal(1.7%). Skin and soft tissue injuries(37.4%) were the m.in traum.tic lesions. CONCLUSION: Age, fall height, fall place, linear skull fracture, subarachnoidal hem.rrhage, cervical fracture, thoracic vertebra fracture and traum. scores had statistically significant effect on m.rtality. The casualties died because of subarachnoid hem.rrhage m.stly.

Pulm.nary em.olism.(PE) is a clinical entity with high m.rtality rate and therefore rapid diagnosis is necessary. For this purpose m.ny diagnostic strategies have been developed for avoiding or necessitating further investigations. The hallm.rk of these strategies is assessing the pretest clinic probability of PE. In this study, the effectiveness of Wells, Geneva and Kline m.thods were investigated in a university hospital em.rgency departm.nt. 74 patients were enrolled in this study. The inter-group differences in scoring system. were significant for Wells and Kline but non-significant for Geneva m.thod. The diagnosis of PE was correlated with Wells and Kline system.but Geneva system.lacked it. The Receiver Operating Characteristic analyses was perform.d for com.aring the pretest clinical probability scoring system. and the greatest area under the curve was found in Wells system. Wells m.thod seem. m.re useful com.ared to Geneva and Kline m.thods particularly in em.rgency departm.nt.

Poisoning is the disruption of functions of a living organism.by any agent. The aim.of this study is to identify the clinical and dem.graphic characteristics of intoxication cases adm.tted to the Num.ne Training and Research Hospital em.rgency departm.nt. Characteristics such as age and gender of the patients, intoxication agents, and the results (discharge, forwarding, hospitalization, death etc.) were recorded to the prepared form. Data were assessed with the SPSS 15.0 software package and expressed. Quantitative variables were sum.arised as m.an ± standard deviation. Frequency and percent distribution were used for the evaluation of data. The chi-squared test was used for com.arison of the quailtative variables. p 0.05 was accepted significant. The m.an age of the receiving 651 poisoned patients was 32.04 ± 12.24 years. Of them. 67.7% were fem.le. The m.st causes of poisoning agents were a pharm.ceuticals (79.4%), carbonm.noxide (8.9%) and alcohols (7.1%). Gastric lavage was perform.d in 76% patients. Activated charcoal was adm.nistered in 81% patients. Antidotes treatm.nt was applied in 7.4% patients. m.st patients were evaluated and then treated and discharged from.the ED (91.9%).
