Does Right Coronary Artery Stenosis Increase m.rbi-m.rtality in Patients Undergoing Coronary Artery Bypass Graft for Left m.in Coronary Artery Disease?

Background: The association of right coronary artery (RCA) stenosis in patients undergoing m.ocardial revascularization for left m.in coronary artery (Lm.A) stenosis affects the prognosis. This study aim.d to com.are im.ediate results of patients with isolated Lm.A stenosis and those with com.ined RCA stenosis. Patients and m.thods: We retrospectively evaluated 107 consecutive patients with Lm.A disease who underwent coronary artery bypass grafts. The patients were divided into two groups: isolated Lm.A stenosis (n = 36) and Lm.A stenosis + RCA stenosis (n = 71). Different variables (preoperative, intra operative and post operative) were com.ared. Results: Patients with Lm.A stenosis + RCA stenosis experienced higher prevalence of diabetes m.llitus (p = 0.024) and sm.ker (p = 0.032). Also left ventricular EF was reduced (p = 0.004). m.ocardial revascularization was m.re com.lete in patients with Lm.A stenosis + RCA stenosis (p = 0.033), but in-hospital m.rtality rate was higher (12.6% vs 5.5%) in isolated LLm.A stenosis, but it did not reach statistical significance (p = 0.32). Except low output syndrom. (LOS) that was frequent in presence of RCA stenosis (p = 0.026), no significant difference was found between groups for other com.lications. Conclusion: The presence of RCA stenosis in patients undergoing CABG for Lm.A disease increases 30 day m.rtality but without significant im.act on overall m.rbidities.

Background: The aim.of this study was to quantify the effect of obesity on early results after coronary artery bypass grafting (CABG). m.thods: A retrospective cohort study of patients underwent isolated CABG from.January 2000 through Decem.er 2012 was conducted. 586 patients were classified into two groups: Obese (n = 100) Bm. ≥ 30 kg/m. and non-obese (n = 486) Bm. ≤ 30kg/m.. Results: The obese patients included m.re wom.n (p 0.01), hypertensives (p = 0.01) and dyslipidem.cs (p = 0001). The CPB, aortic cham. tim.s and num.er of bypass graft were sim.lar between the groups (p = 0.35, p = 0.51 and p = 0.59 respectively). Also the com.osite of in-hospital m.rtality and postoperative com.lication didn’t differ between the groups. The incidence of perioperative m.ocardial infarction, and need for inotropic drugs or IABP were significantly less in obese patients (p = 0.028, p = 0.031 and p 0.01 respectively). Conclusions: The current study showed that obesity is not a risk factor of adverse events after CABG and continuous to give another aspect of the “obesity paradox”.
