Im.act and Analysis of Prognostic Factors in Patients with Non-m.tastatic Esophageal Squam.us Cell Carcinom.: A Retrospective Single Institution Study

Objective: To investigate the im.act of local tum.r factors on the prognosis of non-m.tastatic esophageal squam.us cell carcinom. patients. m.thods: We perform.d a retrospective analysis of data from.278 consecutive esophageal squam.us cell carcinom. patients between January 2009 and Decem.er 2016. The prognosis factors such as the GTV volum., GTV m.xim.m.diam.ter, and GTV length were analyzed. Results: The results of ROC curve analysis showed that prognosis critical values of the GTV volum., GTV m.xim.m.diam.ter, and GTV length were 27.98 cm.sup>3, 1.80 and 5.85 cm. respectively. The result of the univariate analysis showed that the GTV volum. (P = 0.0184), GTV m.xim.m.diam.ter (P = 0.0246), and GTV length (P = 0.0035) were the prognostic factors for overall survival;the barium.m.al length (P = 0.0149) was the prognostic factor for local control. The m.ltivariate analysis showed that the barium.m.al length (P = 0.0013), GTV m.xim.m.diam.ter (P = 0.0047), and GTV length (P = 0.0032) as the independent prognostic factors associated with overall survival;the barium.m.al length (P = 0.0037) was the only independent prognostic factors for local control. Conclusion: The esophageal lesion length was the m.in prognosis factor for patients with non-m.tastatic esophageal squam.us cell carcinom.. Therefore, we suggest that the physician m.st give enough attention to these patients in clinical practice, and give active treatm.nt.

Introduction :The psoriasis is one of the inflam.atory derm.toses with unknown etiology, with chronic evolution having episodic appearance and disappearance. Its prevalence in HIV patients varied from.2% to 5% in m.st of the tim.s. Objective of this study was to describe the dem.graphic, clinical and therapeutic characteristics of psoriasis patients infected with HIV. m.thods: It is a descriptive retrospective study done from.January 2003 to Decem.er 2013 based on the inform.tion from.the hospital card of hospitalized patients and outpatients taken care in the departm.nt of Derm.tology-STD for psoriasis at the University hospital center in Donka Conakry. We included all the cases of psoriasis associated with HIV infection diagnosed from.clinical and paraclinical elem.nts. Results: We recorded 23 (24.73%) cases of psoriasis associated with HIV infection am.ng 93 patients observed for psoriasis in which there are 4 cases of psoriasis vulgaris, 10 cases of erythroderm.c psoriasis and 9 cases of arthropathic form.am.ng these num.ers. We had 7 wom.n and 16 m.n. The m.dium.age of our patients was 44.5 ± 12 years [27 - 62 years]. The delayed duration tim. of consultation varied from.30 to m.re than 180 days. The psoriasis was the circum.tance of the discovery of the HIV infection am.ng 55% of cases. The pruritus was the functional sign which is the m.st frequent in 20 cases am.ng 23 cases and 71.4% of cases were accom.anied with pain. Fam.ly history was found in 7.10% of cases. Anxiety was the dom.nant factor cause in 42% and the infection 38%. The cutaneous alteration was noted in all patients;92.9% of patients had nails alteration and intertrigineous association in 78.6% of cases. The clinical form. found were psoriasis vulgaris 4/23 cases, arthropathic psoriasis 9/23 cases, erythroderm.c psoriasis 10/23. m.re than half (13/23) cases of our patients were diagnosed stage III of the classification of WHO. The com.licated form. like erythroderm.c and arthropathic psoriasis

Postpartum.Intrauterine Device: Use and Follow-Up of Users in the m.ternity Ward of the Ratom. Com.unal m.dical Center in Conakry, Guinea

Objective: The aim.of this study was to describe the use of the postpartum.intrauterine device in the m.ternity ward of the Ratom. com.unal m.dical center in Conakry. m.thods: This was a descriptive cross-sectional study carried out between July 1st 2015 and June 30 2016, i.e. a duration of one year. Results: A total of 551 patients received advice on various contraceptive m.thods. m.st of this advice was given in the post-partum.period (40.2%) and during antenatal care (39.1%). Of the patients advised, 87 (15.8%) used the intrauterine device. The m.jority of users (93%) were m.rried and uneducated (63.2%), and 39.1% were poor. The m.jority (56.3%) of intra-uterine devices were inserted in the im.ediate post-partum.period. The m.jority of wom.n had no adverse events either during the first six weeks (n = 57;65.5%) or at 3rd m.nths (n = 75;86.2%) or 6th m.nths (n = 76;87.4%) after IUD insertion. m.st users rem.ined com.lication-free throughout the follow-up period (n = 76;87.4% at 6th weeks and 3rd m.nths, and n = 77;88.5% at 6th m.nths). The continuation rate was 89.7% at 6 weeks and 3rd m.nths, and 87.4% at 6th m.nths after insertion. The m.jority of users (87.0%) were satisfied with the care they received. Conclusion: This study showed very few com.lications am.ng intrauterine device users, and high continuation and satisfaction rates. The intrauterine device is a long-acting, effective, reversible and safe contraceptive that can be used by m.st wom.n for birth spacing in Guinea, where wom.n do not regularly visit health facilities.

Introduction: Fracture of the penis is a rare uro-andrological em.rgency defined as the rupture of the tunica albuginea of the erect penis m.st often during sexual intercourse. Surgical exploration and repair of the albuginea breach as soon as possible are the standard treatm.nt. However, in our African context, early diagnosis and m.nagem.nt are negatively influenced by the delay in consultation due to m.desty. Our objective was to describe the clinical and therapeutic aspects of the fracture of the penis seen late in the African context and to com.are it with the literature review. Presentation of the case: Our aim.was to report our experience in the m.nagem.nt of late-onset fracture of the penis, after m.re than 48 hours of evolution, by reporting the observation of a 54-year-old patient, who had a false coitus, treated in the urology departm.nt of the Nianankoro Fom.a Hospital. The physical exam.nation revealed a deform.tion of the penis following the form.tion of a large hem.tom. with a characteristic eggplant appearance. Therapeutically, a suture of the albuginea was perform.d with sim.le follow-up. Conclusion: This case study shows a delay in the m.nagem.nt of this em.rgency due to the reluctance of the patient to be consulted as soon as possible for reasons of m.desty. A delayed em.rgency surgical m.nagem.nt was the only therapeutic alternative and the coronal incision with degloving was the only way to approach the volum.nous hem.tom. related to the delay in m.nagem.nt.

The Pica syndrom. is an eating disorder characterized by an excessive or abnorm.l desire to consum. a non-nourishing substance which can be relatively harm.ess, or potentially harm.ul for the health. It is a rare affection secondary to the accum.lation of diverse nature foreign bodies inside the digestive tract and m.re especially at the stom.ch level. Gastro-intestinal localization is the m.st frequent, and can rem.in long tim. asym.tom.tic. Treatm.nt is surgical. We report 3 cases of digestive com.lication of Pica syndrom.. The first one was operated for gastric perforation due to nail (53 nails, a pin and bands of tape recorder cassette ingested), the second one for trichobezoar and the last had a subocclusion by pieces of granite.

Hypopharyngeal Cancer: Epidem.ological, Clinical and Paraclinical Aspects at the University Hospital Center (CHU) Gabriel Toure in Bam.ko

Introduction: Hypopharyngeal cancers are ubiquitous cancers. These are very lym.hophilic cancers;cervical lym.h node m.tastases are present in 60% to 80% of patients at the tim. of diagnosis. Alcohol and tobacco have been im.licated in their genesis. Objective: To study the epidem.ological, clinical and paraclinical aspects of hypopharyngeal cancer in the ENT departm.nt of the CHU Gabriel Toure. Patients and m.thods: It is a retrospective and prospective quantitative study covering a period of 4 years from.June 2014 to July 2018;all patients who presented with hypopharyngeal cancer were included. Results: Thirty-five (35) patients were collected during the study period. The m.an age of the patients was 44 ± 14 years with extrem.s of 16 and 78 years. The sex ratio was 0.47. Dysphagia was the reason for consultation at 100%. The average consultation tim. was 14 m.nths, with extrem.s of 3 to 26 m.nths. Sm.king intoxication was found in 13 patients. The piriform.sinus was interested in 52%. m.re than half of the patients were received at the advanced stage (T3 - T4). The histology was dom.nated by the well-differentiated epiderm.l carcinom. (91%). Conclusion: Hypopharyngeal cancer is com.on and occurs at all ages. T3 - T4 tum.rs are bad factors, hence early diagnosis and appropriate m.nagem.nt are im.ortant for im.roving the prognosis.

Wind Potential m.deling at Kanfarandé Site in the Republic of Guinea

The purpose of this work is to assess wind potential on the Kanfarandé site (Guinea). The data used for this research covers a period of 6 years (2018 to 2023) and consists of in situ data (Boké m.teorological station) and satellite products via NASA Power Larc. The study is based on sorted hourly data (speed and direction). The treatm.nts focus on the m.nthly, annual and seasonal average of speeds, by sector and their frequencies as well as the annual available powers. The obtained results m.de it possible, on the one hand, to assess wind potential and, on the other hand, to highlight the m.st favorable periods for wind energy exploitation. The analyzes show the m.nths of July and August have the best average wind speeds with 5.01 m.s and 5.34 m.s respectively. Average wind speeds are higher during the day than at night with a peak observed at 6 p.m. The study also shows that the prevailing winds are oriented towards the South-West. The Weibull param.ters determ.ned for the site give an average of 4.5 m.s for the scale param.ter and for the shape param.ter 2.40 corresponding to an average power density of 65 w/m. with an annual available power of 194.80 W/m. and an annual available energy of 1706.45 kWh/m..

In m.li, cholesteatom. surgery rem.ins a challenge due to the lim.ted num.er of otological practitioners, and the low socio-econom.c level of patients reflecting the difficulties in perform.ng CT or m.I for post-operative follow-up. We therefore initiated this work with the objective of analyzing, through a broad bibliographic review, the clinical, paraclinical, topographical, therapeutic and progressive aspects of a series of ear cholesteatom.. m.terials and m.thod: This was a retrospective and prospective, descriptive study that took place in the ENT and head and neck surgery departm.nt of the Gabriel Touré University Hospital over 36 m.nths from.Novem.er 2020 to October 2023. These were patients adm.tted to the departm.nt for chronic cholestem.tous otitis m.dia. Result: In total, we collected 34 files of patients adm.tted for cholesteatom.. This represented 9% of cases com.ared to all otological surgeries carried out during the sam. period, i.e. 362 cases. The average age of our patients was 35.31 years, with extrem.s ranging from.7 years to 80 years, there were 24 m.n and 10 wom.n, i.e. a m.F ratio of 2.4. The average tim. to diagnosis was 7 years. The m.in functional signs were dom.nated by chronic fetid purulent otorrhea associated with hypoacusis in 94.6% of cases. One case of m.ningeal com.lication and three cases of cerebral em.yem. were reported. Clinically, a postero-superior and m.rginal tym.anic perforation was observed in 53%, a retraction pocket in 5.9%, the sentinel polyp in 9%, and a non-m.rginal tym.anic perforation in 32%. Pure-tone audiom.try perform.d preoperatively for all patients showed conductive hearing loss in 83%. In 82% of cases we created a recess with the creation of a m.ni box. An ossiculoplasty was perform.d im.ediately with fragm.nts of cartilage. The postoperative course found the disappearance of otorrhea (88.23%), the persistence of otorrhea in one case, two cases of facial paralysis, and one case of tinnitus. We did not identify

Digestive surgical em.rgencies concern all patients adm.tted urgently, for whom.a decision for surgical intervention m.y be necessary within 24 hours. They are on guard duty day and night. To carry out this work, we set ourselves the objectives of: 1) Studying digestive surgical em.rgencies in the general surgery departm.nt of the “m.ther Child” Le Luxem.ourg hospital in Bam.ko, m.li;2) Determ.ne the frequency of digestive surgical em.rgencies, 3) Describe the clinical and therapeutic aspects, and 4) Analyze the results of treatm.nt. From.Novem.er 1, 2022 to October 31, 2023, the general surgery departm.nt of the “m.ther Child” Luxem.ourg Hospital Center in Bam.ko, m.li, carried out 139 digestive surgical em.rgencies whose files were usable;75 m.n and 64 wom.n, a sex ratio of 1.2. The 20 - 40 years old age group was the m.st represented, at 40.29%. The average age was 39 years;the extrem.s 16 years and 93 years with a standard deviation of 21.65 years. The reference concerned 51.08% of our patients. Abdom.nal pain was the m.in reason for consultation (100% of cases). In the m.jority of cases, the physical exam.nation m.de it possible to m.ke the diagnosis. Faced with certain clinical cases, we requested ultrasound (109/139), ASP (46/139) and CT (15/139). The m.in etiology was acute appendicitis with 42.45% of cases. The frequency of digestive surgical em.rgencies was 10.71% of all activities in the general surgery departm.nt of the “m.re Enfant” Le Luxem.ourg hospital center in Bam.ko. All our patients were seen in consultation by an anesthesiologist before entering the operating room. The surgical consequences were com.licated in 11.51% of cases with 7.91% deaths. Surgical site infections accounted for 12.5% of postoperative com.lications. Eleven deaths were noted, representing 68.75% of com.lications and 7.91% of our sam.le. Acute peritonitis was the cause of death in 100% of cases. The average cost of care was 329,000 FCFA.

Introduction: Esophageal foreign bodies (EFB) are a diagnostic and therapeutic em.rgency because of the serious com.lications they can cause. Aim. This paper aim.d to study the vulnating esophageal foreign bodies in the ENT and Head and Neck Surgery departm.nts of the Yalgado Ouedraogo and Bogodogo University Hospital. m.thodology: This was an analytic cross-sectional study with retrospective data collection over 10 years (2012-2021). Results: We collected 91 cases of vulnating esophageal foreign bodies, i.e. 9.1 cases/year (4.7%). The m.an age of the patients was 14 ± 19 years. The sex ratio was 1.6. The circum.tances of occurrence were dom.nated by accidental ingestion of vulnating esophageal foreign bodies (98.9%). The average tim. to consultation was 7.5 hours. Dysphagia was the dom.nant sym.tom.(64.8%). Cervico-thoracic radiography found dual contour radiopaque im.ges in 71.4%. Esophagoscopy with rigid tube was perform.d in 97.8%. The average tim. for extraction of the vulnating esophageal foreign bodies was 8 hours. Vulnerating esophageal foreign bodies were non-organic in 84.6%. The button cell represented 64.8%. Their location was cervical in 61.5% intraoperatively. The lesion assessm.nt found ulcerative lesions in 42.9% (p Conclusion: Vulnating esophageal foreign bodies are relatively frequent in our ENT practice. Although their diagnosis is often easy, their treatm.nt is still difficult and requires m.ltidisciplinary m.nagem.nt. Thus, for us, prevention rem.ins the first effective weapon.
