肥胖儿童胰岛素抵抗综合征的发生率

Aims: To assess prevalence of the insulin resistance syndrome (IRS: obesity, a bnormal glucose homoeostasis, dyslipidaemia, and hypertension) in obese UK child ren and adolescents of different ethnicities and to assess whether fasting data is sufficient to identify IRS in childhood obesity. methods: A total of 103 obes e (BmI > 95th centile) children and adolescents 2-18 years of age referred for assessment underwent an oral glucose tolerance test measurement of fasting lipid s, and blood pressure determination. main outcome measures were prevalence of co mponents of IRS by modified WHO criteria, with IRS defined as ≥3 components (in cluding obesity). Results: There were 67 girls (65%). BmI z-score ranged from 1.65 to 6.15, with 72%having a z-score ≥*** glucose homoeostasis was identified in 46%(hyperinsulinism in 40%, impaired fasting glucose in 0.8%, impaired glucose tolerance in 11 %). No subjects had silent type 2 diabetes. Dyslipidaemia was identified in 30 %and hypertension in 32%. Thirty one per cent had obesity alone, 36%had two c omponents, 28%had three, and 5%had all four components. Birth weight, BmI, and family history of IRS were not associated with risk of IRS. Higher age increase d the risk of IRS; however the syndrome was seen in 30%of children under 12 yea rs. The use of fasting glucose and insulin data for identifying IRS had a sensit ivity of 88%and specificity of 100%. Conclusions: One third of obese children and adolescents have the IRS; however type 2 diabetes is rare. Obese children wi th the IRS may form a high risk group to whom scarce intervention resources shou ld be targeted. Further work is needed to develop appropriate screening programm es for IRS components in significantly obese children.

Objective: To test the hypothesis that pregnancy rates are low if grade z1 pre-embryos are not available for transfer and to determine if pronuclear morphology is a better predictor of pregnancy than traditional embryo morphology. Design: Prospective clinical study. Setting: Academic human reproduction laboratory. Patient(s): One hundred couples undergoing IVF with conventional insemination or ICSI. Intervention(s): Embryo quality was assessed using both pre-embryo pronuclear morphology (zygote scoring or z-scoring) at the time of fertilization evaluation and standard day 2 and day 3 embryo morphology (number of blastomeres and grading based on degree of fragmentation and blastomere size). main Outcome measure(s): We tested two decision models, one based on z scores and another on morphology, to determine which grading system better predicted pregnancy outcomes in assisted reproductive technique. zygote score and embryo morphology were measured for all embryos and the transferred embryo pool. Implantation and pregnancy rates resulting from the embryo transfers of all cycles were calculated. Result(s): The z-score distribution of 552 embryos was 27%z1, 8%z2, 50%z3, and 15%z4. z1 and z3 embryos had significantly (P～.03) higher quality over z2 and z4 embryos. Using the z-score decision model with z1 embryos having highest priority for transfer, pregnancy rates were similar between z1 and z3 embryos. Using embryo morphology as a decision model, pregnancy rates were highest in transfers containing one or two “best”-quality embryos. Conclusion(s): z1 and z3 embryos had similar morphology and pregnancy rates. The decision model based on the z-score model was not better than standard embryo morphology in predicting pregnancy outcome.

Suicide is the second leading cause of death in youth globally; however, there is uncertainty about how best to intervene. Suicide rates are typically higher in males than females, while the converse is true for suicide attempts. We review this "gender paradox" in youth, and in particular, the age-dependency of these sex/gender differences and the developmental mechanisms that may explain them. Epidemiologic, genetic,neurodevelopmental and psychopathological research have identified suicidal behaviour risks arising from genetic vulnerabilities and sex/gender differences in early adverse environments, neurodevelopment, mental disorder and their complex interconnections. Further, evolving sex-/gender-defined social expectations and norms have been thought to influence suicide risk. In particular, how youth perceive and cope with threats and losses(including conforming to others' or one's own expectations of sex/gender identity) and adapt to pain(through substance use and helpseeking behaviours). Taken together, considering brain plasticity over the lifespan, these proposed antecedents to youth suicide highlight the importance of interventions that alter early environment(s)(e.g., childhood maltreatment) and/or one's ability to adapt to them. Further, such interventions may have more enduring protective effects, for the individual and for future generations, if implemented in youth.

Objectives: To identify the effect of vertical hepatitis C virus (HCV) infecti on or exposure on growth in childhood. Study design: Children (n = 1203) born to HCV-infected mothers were followed up from birth prospectively in centers of t he European Paediatric Hepatitis C virus Network. z-scores compared height-and weight-for-age in HCV-infected and -uninfected children, adjusting for othe r factors using linear regression. We also quantified the effect of maternal chr onic infection with HCV on childhood growth. Results: There was no significant e ffect of vertical HCV infection on growth (height P = .223, weight P = .095) nor a significant effect of maternal chronic infection with HCV (height P = .733, w eight P = .274). Prematurity and maternal injecting drug use were associated wit h a significant reduction in height (P maternal injecting drug use and prematurity than standard population s and so there are implications for growth of these children, but this is not a direct result of HCV infection or exposure to chronic maternal HCV infection.

The article reports a study of the influence of regional land subsidence on the 6.05 km long Su-tong Yangtze River *** characteristics of the major compressive geological layers and their potential to settlement are discussed in the light of the past and continuing groundwater exploitation in the area.A numerical model for land subsidence quantification was developed and used to predict the differential settlements along the bridge axis to *** is a significant difference in the settlement occurring in the north and south areas of the bridge,in part due to the variation in the Quaternary alluvial sediments. The case study highlights the importance of considering both site specific and regional subsidence conditions when designing large structures in such an area.
