Influence of age on outcome of total laparoscopic fundoplication for gastroesophageal reflux disease

AIm: To demonstrate that age does not influence the choice of treatment for gastroesophageal reflux disease (GERD). We hypothesized that the outcome of total fun-doplication in patients > 65 years is similar to that of patients aged ≤ 65 ***: Four hundred and twenty consecu-tive patients underwent total laparoscopic fun-doplication for GERD. Three hundred and fifty-five patients were younger than 65 years (group Y), and 65 patients were 65 years or older (group E). The following elements were considered: pres-ence, duration, and severity of GERD symptoms; presence of a hiatal hernia; manometric evalu- ation, 24 h pH-monitoring data, duration of operation; incidence of complications; and length of hospital stay. RESULTS: Elderly patients more often had atypical symptoms of GERD and at manometric evaluation had a higher rate of impaired esophageal peristalsis in compari-son with younger patients. A mild intensity of heartburn often leads physicians to underestimate the severity of erosive esophagitis. The duration of the operation was similar between the two groups. The incidence of intra-operative and postoperative complications was low and the difference was not statistically significant between the two groups. An excellent outcome was observed in 92.9% young patients and 91.9% elderly patients. CONCLUSION: Laparoscopic antireflux surgery is a safe and effective treatment for GERD even in elderly pa-tients, warranting low morbidity and mortality rates and a significant improvement of symptoms comparable to younger patients.

目的前瞻性比较超声(US)、多层螺旋CT(mDCT)和对比增强mRI在肝硬化肝移植病人中诊断肝细胞肝癌的能力。方法共对140例病人的163个肝细胞肝癌结节进行了US、mRI和mDCT检查。通过病理或者随访12个月病灶明显增大来确诊肝癌的诊断。研究对US、mDCT、mRI平扫加动态扫描、mRI平扫、动态扫描加肝胆相扫描等4种不同的成像技术数据进行分析。确定诊断准确性、敏感性、特异性、阳性预测值(PPV)和阴性预测值(NPV)及相应的95%置信区间。统计分析针对所有病灶及3个不同的亚组(m,1～2cm,>2cm)进行。结果平扫、动态扫描加肝胆相mRI扫描的准确度、敏感度及NPV显著高于US、mDCT和单独动态增强mRI。US的特异度及PPV显著低于mDCT、动态增强mRI和动态增强加肝胆相扫描mRI。所有的亚组分析所得到的结果相似,其中对1～2cm之间的小病灶具有特别的诊断价值。结论动态增强加肝胆相扫描mRI检查对于肝硬化病人肝癌具有更高的检出力和定性诊断价值。其最有利于诊断大小在1～2cm之间的病灶。要点①US、CT和mRI都能够识别肝硬化病人的肝细胞肝癌。②US具有良好的敏感度,但存在假阳性结果。③在肝细胞肝癌的诊断上,动态CT及mRI具有相似的诊断性能。④动态增强加肝胆相扫描mRI检查能显著提高肝癌的检出及定性诊断。⑤最有利于诊断大小在1～2cm之间的病灶。

Prevalence of bile reflux in gastroesophageal reflux disease patients not responsive to proton pump inhibitors

Laparoscopic approach to meckel's diverticulum

AIm: To retrospective review the laparoscopic management of meckel diverticulum(mD) in two Italian Pediatric Surgery ***: Between January 2002 and December 2012, 19 trans-umbilical laparoscopic-assisted(TULA) procedures were performed for suspected mD. The children were hospitalized for gastrointestinal bleeding and/or recurrent abdominal pain. median age at diagnosis was 5.4 years(range 6 mo-15 years). The study included 15 boys and 4 girls. All patients underwent clinical examination, routine laboratory tests, abdominal ultrasound and technetium-99m pertechnetate scan, and patients with bleeding underwent gastrointestinal endoscopy. The abdominal exploration was performed with a 10 mm operative laparoscope. Pneumoperitoneum was establishedbased on the body weight. Systematic overview of the peritoneal cavity allowed the ileum to be grasped with an atraumatic instrument. The complete exploration and surgical treatment of mD were performed extracorporeally, after intestinal exteriorization through the umbilicus. All patients' demographics, main clinical features, diagnostic investigations, operative time, histopathology reports, conversion rate, hospital stay and complications were registered and ***: mD was identified in 17 patients, while 1 had an ileal duplication and 1 a jejunal hemangioma. Fifteen patients had painless intestinal bleeding, while 4 had recurrent abdominal pain and exhibited cyst like structures in an ultrasound study. Eleven patients had a positive technetium-99m pertechnetate scan. In the patients with bleeding, gastrointestinal endoscopy did not name the source of hemorrhage. All patients were subjected to a TULA surgical procedure. An intestinal resection/anastomosis was performed in 14 patients, while 4 had a wedge resection of the diverticulum and 1 underwent stapling diverticulectomy. Langerin-mediated internalization of a modified peptide routes antigens to early endosomes and enhances cross-presentation by human Langerhans cells

The potential of the skin immune system to generate immune responses is well established, and the skin is actively exploited as a vaccination site. Human skin contains several antigen-presenting cell subsets with specialized functions. In particular, the capacity to cross-present exogenous antigens to CD8^＋ T cells is of interest for the design of effective immunotherapies against viruses or cancer. Here, we show that primary human Langerhans cells （LCs） were able to cross-present a synthetic long peptide （SLP） to CD8^＋ T cells. In addition, modification of this SLP using antibodies against the receptor langerin, but not dectin-1, further enhanced the cross-presenting capacity of LCs through routing of internalized antigens to less proteolytic early endosome antigen 1 ^＋ early endosomes. The potency of LCs to enhance CD8^＋ T-cell responses could be further increased through activation of LCs with the toll-like receptor 3 ligand polyinosinic：polycytidylic acid （phC）. Altogether, the data provide evidence that human LCs are able to cross-present antigens after langerin-mediated internalization. Furthermore, the potential for antigen modification to target LCs specifically provides a rationale for generating effective anti-tumor or anti-viral cytotoxic T lymphocyte responses.

Loss-induced control of light propagation direction in passive linear coupled optical cavities

Redirecting the flow of light on the basis of the absorption/gain properties of optical systems is of great interest in many research fields, ranging from optical routing to optical cloaking. In this paper we investigate the control of the direction of the light propagation through loss-induced absorption in passive linear coupled optical systems. The considered optical system consists of a mode-splitting resonant cavity formed by coupling a Fabry-Perot （FP） cavity with a ring resonator. The coalescence of the asymmetric resonances, generated through mode-splitting dynamics, is the spectral result of the parity time symmetry breaking at FP resonance wavelengths. For specific values of the FP overaU loss, a predominant backward propagation in the FP ring resonator occurs. In fiber optics technology, this device shows an ability to invert the sense of propagation of the light, quantified through the contrast ratio, in the order of 20 dB. This value can be obtained by externally varying the FP loss coefficient for a fixed set of the other physical parameters of the FP ring resonator. Our results can open a new way toward novel high-performance optical modulation and routing schemes.

地震灾害倒塌房屋的识别是地震灾害损失评价的重要环节,也是判断地震强度的指标之一.5.12汶川大地震发生后,中欧地震灾害评估项目组使用自动识别技术处理0.5m分辨率的ADS40航空遥感数据,需要克服航空遥感数据中未倒塌混凝土桥梁、滑坡体滚石与倒塌房屋反射光谱的相似性.经过多次实验改进后的技术,一次性识别的倒塌房屋面积覆盖率优于95%.介绍了这次实验过程和结果,实验表明,该方法也可以用于其他灾害造成的倒塌房屋的自动识别.
