Addition of statins to the standard treatment in patients with cirrhosis:Safety and efficacy

This review summarizes the safety and efficacy of statins in patients with cirrhosis.Due to concerns about the safety of statins in patients with impaired liver function,they have recently been investigated as a potential treatment option in cirrhosis.The most clinically significant adverse event is statin-related myopathy,and this may be related to the high serum statin concentrations in the setting of severely impaired liver function.Rhabdomyolysis is the most serious and potentially life-threatening manifestation.It has recently been demonstrated that the recommended dose of simvastatin in patients with decompensated cirrhosis would be 20 mg/d because higher values,such as 40 mg/d,are associated with many adverse events,especially muscle injury.Likewise,simvastatin should not be administered to patients with model for End-stage Liver Disease score>12 and/or Child-Pugh class C because of the high risk of severe muscle injury.Due to the pleiotropic effects,the focus on statins has shifted from being considered harmful to something useful.Through these effects,statins could prevent liver-related morbidity and mortality in cirrhotic patients.Observational studies in large populations of patients with cirrhosis have shown that treatment with statins to decrease high cholesterol levels was associated with a reduced risk of hepatic decompensation,hepatocellular carcinoma development and death.The few randomized controlled trials in patients with cirrhosis and portal hypertension showed that statins lower portal pressure,quite likely through a reduction in hepatic resistance.Another large randomized controlled trial in patients with variceal bleeding showed that simvastatin in addition to standard of care did not prevent rebleeding but improved survival rate.Despite these encouraging outcomes,the quality of the evidence regarding the use of statins is low or very low due to the observational characteristics of most of the studies involved.Therefore,it is advisable to perform further randomized controlled trials on a l

Peyronie’s disease(PD)is a condition of penile connective tissue affecting up to 10%of men worldwide.In the complexity ofits management,nonsurgical treatments,such as intraplaque injections,are gaining attention.The current literature shows dataon the efficacy of intraplaque injections of hyaluronic acid(HA)mainly in acute-phase PD.However,data on injections of HA instable-phase PD are lacking.Data for this retrospective study were derived from a prospectively maintained database of privatepatients presenting at a private medical practice affiliated to the University of Naples“Federico II”(Naples,Italy)with stable-phasePD between January 2020 and march 2023.Patients underwent a standard protocol of three injections,each administered at atwo-week interval.During the intervals,patients performed vacuum device therapy,penile stretching,and modeling exercises.Allpatients compiled the Peyronie’s Disease Questionnaire(PDQ)and Global Assessment of Peyronie’s Disease(GAPD)at baselineand 2 weeks after the third injection.A penile Doppler ultrasound was performed 2 weeks after the last injection to record thefinal curvature.Overall,we recruited 62 patients with stable-phase PD and a mean(±standard deviation[s.d.])curvature of52.7°(±9.7°).After 6 weeks,eight(12.9%)patients did not experience any curvature improvement.The remaining 54 patientshad a final mean(±s.d.)curvature of 40.3°(±9.1°)with Pmpared to that before treatment.We found improvement inall PDQ domains(all P≤0.01),and 50(80.6%)patients reported subjective improvement of the penile curvature according to theGAPD.In conclusion,we demonstrated that after three injections of HA administered according to the adopted protocol,patientswith stable-phase PD could experience significant improvements in penile curvature,and physical and psychological consequencesof the disease without significantly relevant side effects.

Nitric oxide(NO)is an essential reactive oxygen species and a signal molecule in plants.Although several studies have proposed the occurrence of oxidative NO production,only reductive routes for NO production,such as the nitrate(NO_(3)^(-))-upper-reductase pathway,have been evidenced to date in land plants.However,plants grown axenically with ammonium as the sole source of nitrogen exhibit contents of nitrite and NO3−,evidencing the existence of a metabolic pathway for oxidative production of NO.We hypothesized that oximes,such as indole-3-acetaldoxime(IAOx),a precursor to indole-3-acetic acid,are intermediate oxidation products in NO synthesis.We detected the production of NO from IAOx and other oximes catalyzed by peroxidase(POD)enzyme using both 4-amino-5-methylamino-2′,7′-difluorescein fluorescence and chemiluminescence.Flavins stimulated the reaction,while superoxide dismutase inhibited it.Interestingly,mouse NO synthase can also use IAOx to produce NO at a lower rate than POD.We provided a full mechanism for POD-dependent NO production from IAOx consistent with the experimental data and supported by density functional theory calculations.We showed that the addition of IAOx to extracts from medicago truncatula increased the in vitro production of NO,while in vivo supplementation of IAOx and other oximes increased the number of lateral roots,as shown for NO donors,and a more than 10-fold increase in IAOx dehydratase expression.Furthermore,we found that in vivo supplementation of IAOx increased NO production in Arabidopsis thaliana wild-type plants,while prx33-34 mutant plants,defective in POD33-34,had reduced production.Our data show that the release of NO by IAOx,as well as its auxinic effect,explain the superroot phenotype.Collectively,our study reveals that plants produce NO utilizing diverse molecules such as oximes,POD,and flavins,which are widely distributed in the plant kingdom,thus introducing a long-awaited oxidative pathway to NO production in plants.This knowledge has essential implications for und

Retinal dysfunction is the most common cause of vision loss in several retinal disorders.It has been estimated a great increase in these pathologies that are becoming more globally widespread and numerous over time,also supported by the life expectancy increment.Among different types of retinopathies,we can account some that share causes,symptoms,and treatment including diabetic retinopathy,age-related macular degeneration,glaucoma,and retinitis pigmentosa.molecular changes,environmental factors,and genetic predisposition might be some of the main causes that drive retinal tissue to chronic inflammation and neurodegeneration in these retinopathies.The treatments available on the market contain compounds that efficiently ameliorate some of the important clinical features of these pathologies like stabilization of the intraocular pressure,reduction of eye inflammation,control of eye oxidative stress which are considered the major molecular mechanisms related to retinal dysfunction.Indeed,the most commonly used drugs are antiinflammatories,such as corticosteroids,antioxidant,hypotonic molecules and natural neuroprotective compounds.Unfortunately,these drugs,which are fundamental to treating disease symptoms,are not capable to cure the pathologies and so they are not life-changing for patients.This review provided an overview of current treatments on the market,but more interestingly,wants to be a quick window on the new treatments that are now in clinical trials.Additionally,it has been here highlighted that the recent technical enhancement of the investigation methods to identify the various retinopathies causes might be used as a sort of“precise medicine”approach to tailor the identification of molecular pathways involved and potentially study a dedicated treatment for each patient.This approach includes the use of cutting-edge technologies like gene therapy and metabolomics.

Survival to adult life in patients born with“functionally”single ventricle,without any surgical treatment,as natural history,is extremely rarely reported[1,2,3,4,5,6,7].In general,these group of patients with“functionally”single ventricle are symptomatic from the first weeks of life,and require two to three staged operations to obtain that the single ventricle pumps the oxygenated blood to the systemic circulation,while the less oxygenated blood is deviated by gravity from the superior and inferior vena cava directly to the lungs[1,8,9].The result of this artificial“Fontan circulation”is high systemic venous pressure,and the chronic elevation of the systemic venous pressure may results in liver failure,renal failure.

Evaluation of wave retrieval for Chinese Gaofen-3 synthetic aperture radar

The goal of this study was to investigate the performance of a spectral-transformation wave retrieval algorithm and confirm the accuracy of wave retrieval from C-band Chinese Gaofen-3(GF-3)Synthetic Aperture Radar(SAR)images.more than 200 GF-3 SAR images of the coastal China Sea and the Japan Sea for dates from January to July 2020 were acquired in the Quad-Polarization Strip(QPS)mode.The images had a swath of 30 km and a spatial resolution of 8 m pixel size.They were processed to retrieve Significant Wave Height(SWH),which is simulated from a numerical wave model called Simulating WAves Nearshore(SWAN).The first-guess spectrum is essential to the accuracy of Synthetic Aperture Radar(SAR)wave spectrum retrieval.Therefore,we proposed a wave retrieval scheme combining the theocratic-based max Planck Institute Algorithm(mPI),a Semi-Parametric Retrieval Algorithm(SPRA),and the Parameterized First-guess Spectrum method(PFSm),in which a full wave-number spectrum and a non-empirical ocean spectrum proposed by Elfouhaily are applied.The PFSm can be driven using the wind speed without calculating the dominant wave phase speed.Wind speeds were retrieved using a Vertical-Vertical(VV)polarized geophysical model function C-SARmOD2.The proposed algorithm was implemented for all collected SAR images.A comparison of SAR-derived wind speeds with European Center for medium-Range Weather Forecasts(ECmWF)ERA-5 data showed a 1.95 m/s Root-mean-Squared Error(RmSE).The comparison of retrieved SWH with SWAN-simulated results demonstrated a 0.47 m RmSE,which is less than the 0.68 m RmSE of SWH when using the PFSm algorithm.

The Application of model-Based Systems Engineering to Rural Healthcare System Disaster Planning:A Scoping Review

Disasters and other emergency events have complex effects on human systems,particularly if the events are severe or prolonged.When these types of events happen in rural communities,the resources of the local public health,healthcare,and emergency response organizations can be quickly depleted or overwhelmed.Planning for emergencies can help to mitigate their impact.model-based systems engineering(mBSE)methods,including computer simulations,can provide insight on how best to prepare for these events and to explore the effects of varying approaches and resource utilization.To best apply these methods for improving disaster management in rural settings,a synthesis of the current body of evidence in this field is needed.The objective of this scoping review was to provide a descriptive overview of the application of computer simulation based on mBSE approaches to disaster preparedness and response for rural healthcare systems.Six studies met inclusion criteria,and varied in terms of mBSE method used,healthcare setting,and disaster type and context considered.We identified a gap in the research regarding the application of mBSE approaches to support rural healthcare disaster preparedness planning efforts.model-based systems engineering and systems thinking,therefore,represent novel methods for developing tools and computational simulations that could assist rural communities better prepare for disasters.

Xenoestrogens challenge 17β-estradiol protective effects in colon cancer

Several epidemiological,cellular,and molecular studies demonstrate the role of environmental chemicals with endocrine disrupting activities,typical of Westernized societies,in the pathogenesis of numerous diseases including cancer.Nonetheless this information,the design and execution of studies on endocrine disruptors are not yet cognizant that the specific actions of individual hormones often change with development and ageing,they may be different in males and females and may be mediated by different receptors isoforms expressed in different tissues or at different life stages.These statements are particularly true when assessing the hazard of endocrine disruptors against 17β-estradiol(E2)actions in that this hormone is crucial determinant of sexrelated differences in anatomical,physiological,and behavioral traits which characterize male and female physiology.moreover,E2 is also involved in carcinogenesis.The oncogenic effects of E2 have been investigated extensively in breast and ovarian cancers where hormone-receptor modulators are now an integral part of targeted treatment.Little is known about the E2preventive signalling in colorectal cancer,although this disease is more common in men than women,the difference being more striking amongst pre-menopausal women and age-matched men.This review aims to dissect the role and action mechanisms of E2 in colorectal cancer evaluating the ability of estrogen disruptors(i.e.,xenoestrogens)in impair these E2 actions.Data discussed here lead to define the possible role of xenoestrogens in the impairment and/or activation of E2signals important for colorectal cancer prevention.
